Late-onset hypothalamic-pituitary dysfunction in a childhood cancer survivor treated with cranial irradiation.

Hypothalamic-pituitary dysfunction is a well-recognized late complication in childhood cancer survivors (CCSs) treated with cranial irradiation. We report a 42-year-old male with a history of acute lymphoblastic leukemia diagnosed at 5 years of age and treated with chemotherapy and 26 Gray whole-brain irradiation. He presented with right upper extremity paresis due to bilateral chronic subdural hematomas. Following burr-hole drainage for the left chronic subdural hematoma, he developed anorexia, fatigue, and hyponatremia and was referred to the endocrinology department. Endocrine evaluation de

Aberrant splicing caused by a homozygous AK2 c.498 + 1G > A variant in a patient with reticular dysgenesis and concurrent B-cell acute lymphoblastic leukemia.

Reticular dysgenesis (RD) is a rare autosomal recessive severe combined immunodeficiency characterized by profound neutropenia, T-cell lymphopenia and sensorineural hearing loss, and is caused by biallelic loss-of-function variants in AK2. We identified a homozygous AK2 splice-site variant (c.498 + 1G > A) in a boy with RD and concurrent B-cell acute lymphoblastic leukemia (B-ALL), a clinical combination not previously reported in association with an AK2 variant. An in vitro minigene assay showed that c.498 + 1G > A disrupts the canonical 5' splice donor site and generates an aberrant transcri

Pediatric-inspired USC-ALL regimen vs hyper-CVAD in adults with Ph-negative and Ph-like acute lymphoblastic leukemia.

Adult acute lymphoblastic leukemia (ALL) outcomes remain inferior to pediatric results, motivating adoption of pediatric-inspired regimens (PIR) in adults. The optimal frontline regimen for high-risk subgroups, such as Philadelphia chromosome-like (Ph-like) ALL, has not been established. In this retrospective analysis, 160 adults with Philadelphia chromosome-negative B-cell or T-cell ALL treated between 2013 and 2024 received either the USC-ALL regimen, a pediatric-inspired protocol incorporating synchronized pegaspargase and high-dose methotrexate during two induction and six consolidation cy

Clinical relevance of the newly reclassified HLA-C*04:09L low-expression allele in allogeneic hematopoietic cell transplantation: a case report.

In the context of allogeneic hematopoietic cell transplantation (allo-HCT), misclassifying null- and low-expression alleles could result in an HLA mismatch with life-threatening clinical consequences. HLA-C*04:09N was considered one of the most frequent null alleles, but it was recently reclassified as a low-expression allele, HLA-C*04:09L, based on a recent work which described that could be detected at the cell surface and capable of inducing peptide-specific T-cell activation in vitro. The authors suggested that these findings could imply clinical consequences in the field of transplantatio

Combining the Telomerase Inhibitor Imetelstat with the BCL-2 Inhibitor Venetoclax Enhances Apoptosis and Improves Survival in Acute Myeloid Leukemia Cellular and Animal Models.

Background/Objectives: Acute myeloid leukemia (AML) is a heterogeneous disease with limited treatment options. New therapies with novel mechanisms of action are needed. Telomerase and BCL-2 are overexpressed in AML and correlate with disease severity and poor prognosis, suggesting dual targeting may be beneficial. Methods: We evaluated the effect of imetelstat, a telomerase inhibitor, in combination with venetoclax, a BCL-2 inhibitor, in AML cell lines, AML patient-derived samples, and in a MOLM-13 AML xenograft model. Results: In AML cell lines and patient samples, imetelstat plus venetoclax

Efficacy of Sorafenib Combined with Venetoclax and Azacitidine in Newly Diagnosed FLT3-ITD-Mutant Acute Myeloid Leukemia: Remission Efficacy and Prognostic Influence of Post-Remission Consolidation Therapy.

Background: Mutations of the FLT3 gene, particularly internal tandem duplication (FLT3-ITD), define a biologically aggressive AML subtype with historically poor outcomes. Here we retrospectively assessed the clinical performance and tolerability of sorafenib with venetoclax plus azacitidine (VA) as front-line therapy in patients with newly diagnosed FLT3-ITD-mutated AML. Methods: Forty-five consecutive patients who received sorafenib plus VA induction were retrospectively analyzed. Post-remission therapy comprised sorafenib-based chemotherapy consolidation, continued sorafenib-VA, or allogenei

Cell-free DNA fragmentomics and epigenomic profiling in hematologic malignancies: current evidence, translational limitations, and future directions.

Mutation-based liquid biopsy supports diagnosis and disease monitoring in hematologic malignancies, but interpretation of plasma variants may be complicated by clonal hematopoiesis, treatment-related changes, and nonmalignant blood-cell turnover. This structured narrative review evaluates whether cell-free DNA (cfDNA) fragmentomics, DNA methylation, and 5-hydroxymethylcytosine (5hmC) provide complementary information beyond mutation detection. Direct plasma evidence is most developed for selected fragmentomic and 5hmC applications, although most studies remain retrospective, internally validat

The prognostic impact of CEBPA mutation in acute myeloid leukemia patients undergoing allogeneic hematopoietic stem cell transplantation.

The presence of mutations in basic leucine zipper (bZIP) region of the CEBPA gene in patients with acute myeloid leukemia (AML) is defined as a favorable risk marker according to the guidelines. A total of 173 CEBPA-mutated AML patients receiving allogeneic hematopoietic stem cell transplantation (allo-HSCT) were retrospectively recruited, compared with 345 control patients selected by propensity score matching. Patients carrying CEBPAmut had a better 5-year PFS than CEBPAwt group (60.8% vs. 55.0%, P = 0.016). Failure to achieve complete remission after first induction chemotherapy, the presen

Beyond Eczema Control: Long-Term Outcomes of Dupilumab Therapy in Six Patients with Netherton Syndrome.

Netherton syndrome is a rare and severe genodermatosis caused by pathogenic variants in SPINK5, leading to epidermal barrier dysfunction, immune dysregulation, and prominent atopic manifestations. Therapeutic options remain limited, and long-term real-world data on biologic treatment are scarce. We report a retrospective two-center real-world case series of six patients with Netherton syndrome treated with dupilumab, with emphasis on long-term outcomes, safety, and clinical effects beyond eczema control. Five patients were managed at one dermatology referral center and one patient at a second

Outcomes of Allogeneic Stem Cell Transplantation in a North American Adult T Cell Leukemia/Lymphoma Cohort.

BACKGROUND: Adult T-cell leukemia/lymphoma (ATLL) is an and aggressive mature T-cell neoplasm driven by human T-cell leukemia virus type 1 (HTLV-1) with dismal prognosis. North American ATLL (NA-ATLL) disproportionately impacts individuals of Afro-Caribbean and Latin American descent who face significant disparities in identifying conventional 8/8 HLA-matched donors in international registries. While allogeneic hematopoietic cell transplantation (HCT) is established in East Asian cohorts, outcomes data in Western populations remain limited. We aimed to evaluate post-HCT survival, toxicities, a

Purpureocillium lilacinum Fungemia Masquerading as an Unidentified Yeast in a Heavily Immunosuppressed Patient With Relapsed Multiple Myeloma and Therapy-Related Mixed-Phenotype Acute Leukemia: A Case Report.

Purpureocillium lilacinum (formerly Paecilomyces lilacinus) is an environmental saprophytic mold and an increasingly recognized opportunistic pathogen in immunocompromised hosts, most often causing cutaneous, ocular, or sinopulmonary disease; bloodstream infection (fungemia) is rare. The initial microscopic morphology on Gram stain of a positive blood culture can simulate yeast, delaying correct identification and appropriate antifungal therapy. We describe a 67-year-old man with relapsed multiple myeloma and therapy-related mixed-phenotype acute leukemia, further complicated by a prior Mycoba

RUNX1-ETO oncoprotein levels determine the balance between endothelial reprogramming and pre-leukemic growth in a human ES cell model of AML.

In t(8;21) acute myeloid leukemia (AML), the RUNX1 DNA binding domain is fused to RUNX1T1 (ETO) producing the RUNX1-ETO fusion protein. We previously showed that t(8;21) leukemic stem cells aberrantly activate endothelial signaling pathways to initiate blast cell proliferation. Here, we employed a human embryonic stem cell (ESC) line carrying an inducible RUNX1-ETO transgene to determine whether endothelial signaling pathway activation depends on disease progression or is directly induced by RUNX1-ETO. Single-cell analyses demonstrate that RUNX1-ETO reprograms ESC-derived myeloid progenitors t

Transcriptome Analysis Identifies a Myeloid-Like Signature in Relapsed or Refractory B-Cell Acute Lymphoblastic Leukemia Patients With Poor Response to CD19 Chimeric Antigen Receptor T Cell Therapy.

BACKGROUND: Relapsed or refractory B-cell acute lymphoblastic leukemia (R/R B-ALL) patients exhibit heterogeneous responses to CD19 chimeric antigen receptor T-cell (CAR-T) therapy. However, current studies primarily focus on the CAR-T cells or the leukemia blasts, and the patient's intrinsic molecular characteristics before treatment remain poorly understood. METHODS: Fifteen R/R B-ALL patients enrolled in a phase II, single-arm clinical trial (NCT04684147) were stratified at a three-month landmark into 8 good responders and 7 poor responders. Clinical characteristics, genetic alterations an

A rare case of acute basophilic leukemia transformed from high risk myelodysplastic syndrome.

Acute basophilic leukemia (ABL) transformed from myelodysplastic syndrome is exceedingly rare. We report the case of a 73-year-old man with ABL which transformed from IPSS-M High risk MDS. The patient had a history of metastatic castration-sensitive prostate cancer. Further investigations demonstrated circulating blasts on peripheral blood smear. Upon bone marrow examination, a diagnosis of ABL arising from underlying MDS was established, characterized by cytogenetic analysis demonstrating trisomy 8 (+ 8), with the additional copy of chromosome 8 present in the form of a ring chromosome, and a

Addition of thiotepa to total body irradiation-based conditioning in allogeneic hematopoietic stem cell transplantation for relapsed/refractory acute leukemia and lymphoma: a propensity score-matched retrospective study.

BACKGROUND: Relapsed/refractory (R/R) acute leukemia (AL) carries a poor prognosis, with allogeneic hematopoietic stem cell transplantation (allo-HSCT) the primary curative option. Total body irradiation (TBI)-based conditioning is a cornerstone, but optimizing intensity remains challenging. Thiotepa, a bifunctional alkylating agent, may enhance disease control when added to TBI, but data in R/R AL are lacking. METHODS: This single-center retrospective study (2020-2025) included R/R AL and lymphoma patients receiving TBI-based conditioning with (TBI + TT) or without (TBI) thiotepa. Propensity

Genetically Defined Acute Myeloid Leukemia Induced Pluripotent Stem Cell Clones Reveal Selective Impairment of Hematopoietic Development.

Patient-derived induced pluripotent stem cells (iPSCs) provide a renewable model for studying acute myeloid leukemia (AML), but whether AML-iPSCs faithfully preserve the functional heterogeneity of genetically distinct AML clones remains unclear. Here, we functionally characterized a genetically defined AML-iPSC resource comprising 34 AML-iPSC lines from 11 AML patients, with complementary assays performed across partially overlapping subsets of lines. AML-iPSC lines were classified as aberration-positive (Ab+) or aberration-negative (Ab-) based on retention of the clinically defining patient-

Case Report: Myelodysplastic changes and 7q deletion in a case of vitamin B12 deficiency: a diagnostic pitfall.

Vitamin B12 plays a crucial role in DNA synthesis, and its deficiency can occasionally present with atypical hematological manifestations mimicking myelodysplastic syndrome (MDS). There is limited literature describing reversible chromosomal abnormalities associated with vitamin B12 deficiency. We report an 11-year-old girl who presented with generalized body aches for 1 month and fever, easy fatigability, and respiratory distress for 2 days. Examination revealed pallor and hepatomegaly, with no splenomegaly. Investigations showed severe anemia (hemoglobin 3.1 g/dL), thrombocytopenia, a low co

Hematologic Malignancies With RUNX1::ZFPM2 Translocation: A Case Report and Literature Review.

UNLABELLED: RUNX1 rearrangements are recurrent in myeloid neoplasms, but rare fusion partners remain poorly characterized. We report the first acute myeloid leukemia (AML) with a RUNX1::ZFPM2 fusion generated by t(8;21)(q23;q22). A 72-year-old woman presented with acute myelomonocytic leukemia carrying concomitant IDH2 and NRAS mutations. Azacitidine-venetoclax induced transient remission with a marked decrease in the fusion transcript. However, persistent cytopenias, molecular relapse, and treatment resistance led to death 1 year after diagnosis. This case expands the spectrum of RUNX1-rearra
