An integrated deep learning and molecular dynamics pipeline for the discovery of novel CDK8 inhibitors against acute myeloid leukaemia.

Cyclin-dependent kinase 8 (CDK8) has emerged as a promising therapeutic target for acute myeloid leukaemia (AML). We developed an integrated computational pipeline combining neural network-based potency prediction with molecular dynamics (MD) simulations for CDK8 inhibitor discovery. A curated dataset of 1,200 unique CDK8 inhibitors was assembled from ChEMBL. The optimal neural network architecture (two hidden layers, 512→128 units) with dropout regularization achieved a test set r2 of 0.47 and RMSE of 0.87. Y-randomization testing confirmed genuine structure-activity relationships. Using an e

Multiparameter flow cytometry versus droplet digital polymerase chain reaction for monitoring measurable residual disease in paediatric fusion gene-positive acute myeloid leukaemia.

Measurable residual disease (MRD) monitoring is crucial for prognostic stratification in paediatric acute myeloid leukaemia (AML). This study compared the performance of multiparameter flow cytometry (MFC) and droplet digital polymerase chain reaction (ddPCR) for MRD assessment in 116 paediatric AML patients at end of induction (EOI)-1, EOI-2 and end of consolidation (EOC)-1. Compared to MFC, ddPCR revealed a significantly slower rate of MRD negativity conversion, with moderate concordance between methods. Both MFC and ddPCR were significant predictors of 5-year event-free survival (EFS) at EO

Minor Myeloid Leukemic Populations in Pediatric Acute Lymphoblastic Leukemia: A Potentially Underrecognized Mixed-Phenotype Tumor?

In "bilineal" mixed-phenotype acute leukemia (MPAL), the proportions of lymphoid and myeloid leukemic cells may differ substantially. A myeloid component may constitute <10% of bone marrow (BM) cells and show subtle immunophenotypic abnormalities detectable by multicolor flow cytometry (MFC). The significance of minor myeloid leukemic populations in otherwise acute lymphoblastic leukemia (ALL), including their association with lineage switch, remains uncertain. We retrospectively reviewed diagnostic MFC data from 5094 children with B-cell precursor or T-cell ALL. An additional minor myeloid le

An Artificial Intelligence Framework for Detection of Leukemia with Comparison Between Segmented and Non-Segmented Images.

BACKGROUND: Accurate classification of Acute Lymphoblastic Leukemia (ALL) subtypes from peripheral blood smear (PBS) images using deep learning may support automated image-based analysis of this blood malignancy. METHODS: We developed a custom lightweight Convolutional Neural Network (CNN) to classify white blood cells into four subtypes: Benign, Early Pre-B ALL, Pre-B ALL, and Pro-B ALL. We assessed performance using accuracy, ROC curves, and explainability techniques, including upsampled feature map activations, occlusion sensitivity, and SHAP-style explanation overlays, comparing original

Association of XRCC6 -61 C > G and XRCC7 6721G > T polymorphisms with susceptibility to pediatric acute lymphoblastic leukemia.

BACKGROUND: Acute lymphoblastic leukemia (ALL) is the most frequent malignancy in children. The XRCC6 and XRCC7 genes play a key role in the nonhomologous end-joining (NHEJ) repair process and may influence susceptibility to ALL. The aim of this study is to evaluate the polymorphisms XRCC6 -61 C > G (rs2267437) and XRCC7 6721G > T (rs7003908) in children with ALL. MATERIALS AND METHODS: Genomic DNA was extracted from the buffy coat from whole blood samples of patients. Genotyping of XRCC6 -61 C > G and XRCC7 6721G > T polymorphisms was performed by PCR followed by restriction fragment length

Hospital length of stay and cost associated with routine-care documentation of probable or suspected fungal infection among patients hospitalized with acute myeloid leukemia: a propensity score analysis.

BACKGROUND: Fungal infection remains a recurring concern during inpatient care for acute myeloid leukemia (AML), but many hospitals lack local estimates of the bed-day and cost burden associated with fungal-infection documentation. We estimated excess length of stay (LOS) and hospitalization cost associated with routine-care documented probable or suspected fungal infection (PFI) among AML inpatient encounters. METHODS: We conducted a retrospective encounter-level study using routinely collected 2020-2025 AML inpatient data from the electronic medical record database of Ganzhou People's Hospi

Split intein-mediated reconstitution of regulatable CAR T-cells.

INTRODUCTION: Chimeric antigen receptor (CAR) T-cell therapy has transformed the treatment of B-cell malignancies. However, strategies that improve the modularity and controllability of CAR signaling could broaden its applicability and safety across additional clinical contexts. Here, we describe a regulable CAR T-cell platform based on split intein-mediated protein trans-splicing (PTS) to conditionally reconstitute functional receptors in primary T-cells. METHODS: As a proof of concept, we engineered a split CAR targeting CD1a (NCT05679895), a surface antigen expressed in T-cell acute lympho

Genetic and clinical evidence supports HLA-DR+ CD8+ T cells as a favorable immune biomarker in acute myeloid leukemia.

Immune dysregulation is central to acute myeloid leukemia (AML), but the contribution of specific immune subsets remains uncertain. We combined Mendelian randomization (MR), clinical flow cytometry, and transcriptomic profiling to investigate HLA-DR+ CD8+ T cells in AML. Two-sample MR using 731 immunophenotypes and AML data from UK Biobank and FinnGen identified four HLA-DR-related T-cell traits associated with lower AML susceptibility. In 85 non-APL patients, HLA-DR+ CD8+ T-cell proportions increased after complete remission, while among 46 newly diagnosed patients, a lower baseline HLA-DR+ C

Genomic and clinical landscape of RAS mutations in acute myeloid leukemia.

RAS mutations are recurrent in acute myeloid leukemia (AML); yet, their genomic and clinical landscape remain poorly characterized. We analyzed centralized molecular profiling data in 8,553 patients with newly diagnosed AML. Of these, 1,775 (21%) harbored RAS mutations: NRAS in 1,103 (13%), KRAS in 445 (5%), and both in 227 (3%). RAS mutations primarily clustered in P-loop hotspots (G12/G13, 70%), with NRAS showing higher Q61 mutation prevalence (24% vs. 9%) and KRAS showing additional K117/A146 hotspots. Multiple RAS mutations occurred in 23% of patients. Convergent evolution of RAS signaling

"Beyond the Genetic Blueprint: Disrupting the Epitranscriptomic Code in Myeloproliferative Neoplasms".

Philadelphia chromosome-negative myeloproliferative neoplasms (MPNs), including polycythemia vera, essential thrombocythemia, and primary myelofibrosis, are driven by canonical JAK2, CALR, or MPL mutations that constitutively activate JAK-STAT signaling. Yet, the clinical reality reveals a sobering paradox: JAK inhibitors modify symptoms but fail to eradicate the malignant clone, reduce mutant allele burden, or reverse fibrosis. This therapeutic ceiling suggests that oncogenic signaling alone cannot explain disease persistence, progression, or transformation to secondary acute myeloid leukemia

An Atypical Presentation of Richter's Transformation in Chronic Lymphocytic Leukemia: Stroke-Like Onset With Multiorgan Involvement Including the Heart and Seminal Vesicles.

Chronic lymphocytic leukemia (CLL) is usually an indolent lymphoproliferative disorder. However, a small proportion of patients may develop Richter's transformation (RT), in which CLL transforms into an aggressive lymphoma. This transformation is often associated with rapid clinical progression and extranodal disease. Here, we describe a 72-year-old man diagnosed with CLL during evaluation for persistent cough and leukocytosis. Peripheral blood smear demonstrated smudge cells, and the diagnosis was confirmed by peripheral blood flow cytometry. As the patient was asymptomatic, he was managed wi

Thrombotic and hemorrhagic complications in NPM1-mutated AML: Association with APL-like immunophenotype and impact on early mortality.

NPM1-mutated acute myeloid leukemia (AML) is generally classified as favorable risk; however, early mortality remains a concern. A subset of NPM1-mutated AML exhibits a CD34-low/negative and HLA-DR-low/negative immunophenotype resembling acute promyelocytic leukemia (APL), raising the possibility that these patients may also share an increased susceptibility to coagulation abnormalities. While thrombotic and hemorrhagic complications are well recognized in APL, their frequency and prognostic significance in NPM1-mutated AML remain incompletely defined. In this single-center retrospective study

Prognosis and treatment-related toxicities in Down syndrome acute lymphoblastic leukemia in JCCG ALL-B12.

Children with Down syndrome (DS) have an increased risk of acute lymphoblastic leukemia (ALL). Although recent reports describe some improvement, survival in DS-ALL remains inferior to that of non-DS-ALL. This study evaluated the prognosis, relapse patterns, and treatment-related toxicities of DS-ALL in the Japan Children's Cancer Group ALL-B12 trial, a nationwide prospective study using risk-adapted therapy. A total of 1,804 patients, including 44 DS-ALL, were analyzed. Risk classification used standardized criteria. The 5-year event-free survival and overall survival in DS-ALL were 70.5% and

SOHO State of the Art Updates and Next Questions | The Evolving AML Treatment Paradigm.

The treatment paradigm for fit, younger adults with acute myeloid leukemia (AML) is evolving toward the frontline use of lower-intensity, better-tolerated therapies that more effectively target disease biology. For over 45 years, intensive chemotherapy (IC) has endured as the cornerstone of induction therapy for fit AML patients. Clinical outcomes have slowly improved during this time; however, long-term survival remains suboptimal, and IC-related toxicity is significant. For older AML patients, clinical outcomes have been transformed by the introduction of hypomethylating agents in combinatio

The Utility of Computed Tomography of the Chest at the Time of Induction Chemotherapy in Patients with Acute Leukemia.

Background: Compared to chest X-ray, computed tomography (CT) imaging of the chest is more sensitive and specific for identifying pulmonary abnormalities. However, patients with acute leukemia routinely undergo chest X-ray before beginning chemotherapy, and the benefit of obtaining baseline chest CT remains unclear. Methods: In this retrospective study, 50 patients with newly diagnosed acute leukemia who underwent chest X-ray and chest CT without contrast before beginning induction chemotherapy were included. CT imaging reports were independently evaluated by two hematologist-oncologists for i

Adjunctive Herbal Medicine Use and Chemotherapy-Induced Thrombocytopenia in Acute Myeloid Leukemia: A Nested Case-Control Study.

BACKGROUND: Systemic chemotherapy increases the risk of thrombocytopenia in patients with acute myeloid leukemia (AML), which may lead to risk of bleeding and treatment interruption. Accumulating evidence indicates that traditional Chinese medicinal herbs (TCMH) may accelerate symptom relief. We hypothesized that integrating TCMH alongside standard treatment may decrease the risk of chemotherapy-induced thrombocytopenia (CIT) in patients with AML. AIM: To compare the likelihood of CIT among patients with AML who received adjunctive TCMH plus standard treatment and those who received standard
