Immunophenotyping of Human B Lymphocytes from Peripheral Blood Mononuclear Cells.

Flow cytometry has made it possible to identify human B cell subsets based on surface and intracellular antigens, a process known as immunophenotyping. CD19 and CD20 are commonly expressed in all B cells; the presence or absence of CD27 separates memory B cells from naïve cells, while IgD and IgM are used to distinguish naïve from transitional B cells. Markers such as CD38, CD27, and CD24 are helpful to identify plasmablasts and immature B cells. CD21 points to mature B cells, CD10 is on immature B cells, CD138 marks plasma cells, and CD5 is high in regulatory B cells. Memory B cells have less

Clinical Flow Cytometric Testing in Chronic Lymphocytic Leukemia.

Flow cytometry is the cornerstone for establishing the diagnosis of chronic lymphocytic leukemia (CLL), owing to its characteristic and well-defined immunophenotype that enables accurate distinction from other leukemias and lymphomas. Beyond diagnosis, flow cytometry provides essential prognostic information and allows sensitive detection of minimal residual disease (MRD), a strong predictor of clinical outcome. CLL MRD assessment is increasingly used to guide risk stratification, therapeutic decision-making, and treatment duration in the era of targeted therapies and immunotherapies. This cha

Measurable Residual Disease.

Measurable residual disease (MRD) serves as a critical biomarker of prognosis, treatment efficacy, and clinical outcome. It captures the presence of residual tumor cells below the detection threshold of conventional microscopy. Multiparametric flow cytometry (MFC) offers a rapid, cost-efficient, and widely applicable platform for MRD detection through leukemia-associated immunophenotypes (LAIPs) and deviation-from-normal (DfN) antigen maturation patterns. This technique underpins a wide range of clinical applications, including risk stratification, therapeutic decision-making, and post-transpl

Current perspectives on high-risk genomic profiles in chronic myeloid leukemia: from bench to clinical practice.

Chronic myeloid leukemia (CML) is defined by BCR::ABL1 rearrangement, yet its genomic architecture extends well beyond this single driver. High-risk genomic profiles (HGP)-additional chromosomal abnormalities in Philadelphia-positive cells, Philadelphia chromosome-associated rearrangements, and high-risk somatic mutations detected by next-generation sequencing (NGS)-are present at diagnosis in roughly one-third of chronic-phase (CP) patients and shape response, progression, and survival. We propose a clinically actionable taxonomy of HGP based on biological origin, detection method, and TKI se

TP53 mutations and IGHV in naïve chronic lymphocytic leukemia: retrospective analysis of data from a Spanish collaborative network.

This retrospective study involving 225 centers assessed TP53 and IGHV molecular status and stereotyped BcR IG subsets in a large cohort of patients with CLL in Spain using data from the RED53 project. Three analytical reference centers determined the mutational status of treatment-naïve CLL patients requiring treatment following the ERIC guidelines between May 2016 and March 2021. Of 1097 patients included (58.4% men), 90.8% had unmutated TP53. Of the 100 patients (9.2%) with mutated TP53, most (97.0%) had single-hit TP53 mutations, of which 88.3% were pathogenic. IGHV-UM mutational status (57

Survival Outcomes in Chronic Lymphocytic Leukemia During the Novel Agent Era: A Population-Based Analysis.

Chemoimmunotherapy (CIT) had long been used to treat chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL) until 2014 when the first novel agent, ibrutinib, was approved in the United States. Numerous novel therapies have since become available. Nevertheless, as novel therapies have largely replaced CIT in CLL/SLL, its impact on survival has yet to be examined on the population level. In this study, we used the SEER (Surveillance, Epidemiology, and End Results) database to compare survival in patients with CLL/SLL before and after the approval of the first novel therapy. We include

Ca2+-dependent pathways controlled by Piezo1 activity in human erythroid progenitor cell line: focus on KCa3.1 channels, ANO6 lipid scramblases and pannexins-1.

BACKGROUND: Piezo1 is a mechanosensitive Ca2+-permeable ion channel that plays a crucial role in the Ca2+ signaling processes of human red blood cells (RBCs). Ca2+ influx through Piezo1 controls the activity of various molecules that are crucial for RBC physiology and pathophysiology, including Ca2+-activated K+ channels of intermediate conductance (KCa3.1, or Gardos channels), ANO6 lipid scramblases and pannexins-1 (Panx1). Erythropoiesis, the differentiation of erythropoietic stem cells in the bone marrow to myeloid progenitor cells and then to mature (RBCs, is traditionally studied using di

Targeting PRMT5 with indole-based arylsulfonylhydrazides: a novel epigenetic strategy for the treatment of acute and chronic leukemia.

Hematological malignancies, including acute myeloid leukemia (AML), acute lymphoblastic leukemia (ALL), chronic myeloid leukemia (CML), and chronic lymphocytic leukemia (CLL), are characterized by extensive epigenetic dysregulation. Protein arginine methyltransferase 5 (PRMT5) is an important epigenetic regulator that contributes to leukemic stem-cell maintenance, impaired differentiation, therapeutic resistance, and immune evasion. Although PRMT5 is a promising therapeutic target, no PRMT5 inhibitor has yet been approved by the FDA. In this study, we investigated a mini-library of indole-base

Prognostic implications of TET2 variant allele frequency in chronic myelomonocytic leukemia: a multicenter study.

Chronic myelomonocytic leukemia (CMML) is a heterogeneous myeloid neoplasm with highly variable prognosis. TET2 is mutated in ∼60% of patients, previously linked to favorable outcomes. However, no study has quantitatively characterized TET2 variant allele frequency (VAF) as a continuous, nonlinear prognostic variable, nor leveraged it to refine contemporary CMML risk stratification systems. We examined the prognostic role of TET2 VAF in 1206 patients across a UK discovery cohort (DC; TET2-mutated, 433/645 [67%]), an international validation cohort (validation cohort 1 [VC1]; TET2-mutated, 141/

A case of lysozyme-induced nephropathy with improved renal function following treatment for chronic myelomonocytic leukemia.

Chronic myelomonocytic leukemia (CMML) is a myelodysplastic/myeloproliferative neoplasm characterized by persistent peripheral blood monocytosis. Although renal involvement can occur in patients with CMML, it may be overlooked. In addition, renal impairment is not included among the current criteria for initiating treatment for CMML.We report the case of a 75-year-old man with lysozyme-induced nephropathy that was initially considered to be diabetic nephropathy. He had a 20-year history of diabetes mellitus and presented to our hospital with weight loss and progressive anemia. After further ev

IκBζ target genes in chronic lymphocytic leukemia.

IκBζ, an atypical IκB protein induced downstream of Toll-like receptor (TLR) signaling, is pivotal in orchestrating inflammatory responses and modulating inflammation-associated B cell malignancies. However, the transcriptional and epigenetic programs regulated by IκBζ in both normal and malignant B cells remain poorly described. Here, we analyzed chronic lymphocytic leukemia (CLL) cells by epigenomic profiling to dissect the molecular function of IκBζ in leukemia. In primary human B cells and CLL samples, chromatin immunoprecipitation sequencing (ChIP-seq) and cleavage under targets and tagme

Acute and long-term outcomes of CD20-targeting CAR T-cell therapy in relapsed or refractory B-NHL: MB-CART20.1 Lymphoma phase I trial.

INTRODUCTION: Anti-CD19 CAR T-cell therapy has improved outcomes in B-cell non-Hodgkin lymphoma (B-NHL), but only about half of patients achieve durable benefit. As CD20 is expressed in > 90% of B-NHL, this phase I trial evaluated the CD20-directed CAR T-cell product MB-CART20.1 in adults with relapsed/refractory CD20-positive B-NHL without curative treatment options. METHODS: In this phase I dose-escalation trial, patients received 1.0 × 10⁵ cells/kg body weight (BW) MB-CART20.1 at the safety dose level (SDL) or 1.0 × 10⁶ cells/kg BW at dose level 1 (DL1). RESULTS: Recruitment was terminate
