Pediatric-inspired USC-ALL regimen vs hyper-CVAD in adults with Ph-negative and Ph-like acute lymphoblastic leukemia.

Adult acute lymphoblastic leukemia (ALL) outcomes remain inferior to pediatric results, motivating adoption of pediatric-inspired regimens (PIR) in adults. The optimal frontline regimen for high-risk subgroups, such as Philadelphia chromosome-like (Ph-like) ALL, has not been established. In this retrospective analysis, 160 adults with Philadelphia chromosome-negative B-cell or T-cell ALL treated between 2013 and 2024 received either the USC-ALL regimen, a pediatric-inspired protocol incorporating synchronized pegaspargase and high-dose methotrexate during two induction and six consolidation cy

Guardians of the germline, drivers of malignancy: piRNA-associated proteins as shared epigenetic regulators of infertility and cancer.

PIWI-interacting RNAs (piRNAs) and their associated proteins constitute an important regulatory system involved in genome stability, transposable-element repression, germ-cell development, and epigenetic regulation. Increasing evidence indicates that disruption of piRNA-associated proteins contributes to reproductive disorders and tumorigenesis through alterations in DNA methylation, histone modifications, RNA stability, and transposable-element activity. This review summarizes current evidence on the epigenetic functions of piRNA-associated proteins in infertility and cancer, with particular

Purpureocillium lilacinum Fungemia Masquerading as an Unidentified Yeast in a Heavily Immunosuppressed Patient With Relapsed Multiple Myeloma and Therapy-Related Mixed-Phenotype Acute Leukemia: A Case Report.

Purpureocillium lilacinum (formerly Paecilomyces lilacinus) is an environmental saprophytic mold and an increasingly recognized opportunistic pathogen in immunocompromised hosts, most often causing cutaneous, ocular, or sinopulmonary disease; bloodstream infection (fungemia) is rare. The initial microscopic morphology on Gram stain of a positive blood culture can simulate yeast, delaying correct identification and appropriate antifungal therapy. We describe a 67-year-old man with relapsed multiple myeloma and therapy-related mixed-phenotype acute leukemia, further complicated by a prior Mycoba

Subpicogram-Level Quantification of Trace Biomarkers Using an Automated High-Sensitivity Chemiluminescence Immunoassay Technology Based on a Novel Substrate Enhancement Strategy.

Trace biomarkers showed great potential for improving the diagnosis and monitoring of various diseases. However, the sensitivity of the current high-throughput detection platforms is limited, posing a significant challenge for the reliable measurement and clinical application of trace biomarkers. We developed an automated high-sensitivity chemiluminescence immunoassay (CLIA) technology with subpicogram-level sensitivity based on the modification of the substrate molecular structure and the synergy of surfactant and fluorescein. Using this technology, we constructed assay kits for five trace bi

Proteomic profiling of high purity extracellular vesicle subtypes from megakaryoblastic leukemia cells provides insights into myeloproliferative neoplasm biology.

Aim: Extracellular vesicles (EVs) are emerging as important mediators in myeloproliferative neoplasms (MPNs), but the characteristics of distinct EV subtypes remain unclear. This study aimed to characterize the molecular heterogeneity of MEG-01-derived large EVs (lEVs) and small EVs (sEVs). Methods: lEVs and sEVs were isolated from MEG-01 cell culture medium by differential centrifugation followed by density gradient ultracentrifugation. EVs were characterized by transmission electron microscopy, nanoparticle tracking analysis, immunoblotting, and on-bead flow cytometry. Quantitative proteomic

Maternal BLV Infection Markers and BoLA-DRB3 Polymorphism Are Associated with BLV Infection Outcomes in Dairy Youngstock.

Bovine leukemia virus (BLV) is a deltaretrovirus that establishes lifelong infection in cattle and remains highly prevalent in dairy herds. This study evaluated associations between maternal BLV infection markers, daughter BoLA-DRB3 variation, and BLV outcomes in dairy youngstock. We studied 201 dam-daughter pairs from five dairy farms. BLV status was assessed using ELISA and quantitative PCR (qPCR); maternal qPCR results were available for 126 dams, and daughters were sampled longitudinally during the youngstock period. BoLA-DRB3 genotypes were determined in 199 daughters, and genotype-level

An Atypical Presentation of Richter's Transformation in Chronic Lymphocytic Leukemia: Stroke-Like Onset With Multiorgan Involvement Including the Heart and Seminal Vesicles.

Chronic lymphocytic leukemia (CLL) is usually an indolent lymphoproliferative disorder. However, a small proportion of patients may develop Richter's transformation (RT), in which CLL transforms into an aggressive lymphoma. This transformation is often associated with rapid clinical progression and extranodal disease. Here, we describe a 72-year-old man diagnosed with CLL during evaluation for persistent cough and leukocytosis. Peripheral blood smear demonstrated smudge cells, and the diagnosis was confirmed by peripheral blood flow cytometry. As the patient was asymptomatic, he was managed wi

Targeting the eIF4F translation initiation complex disrupts BCR::ABL1 translation and overcomes drug resistance in CML.

Chronic myeloid leukemia (CML) is driven by the constitutively active BCR::ABL1 kinase, which promotes uncontrolled cell proliferation. Although protein-tyrosine kinase inhibitors (TKIs) have revolutionized CML treatment, resistance remains a significant barrier to curative therapy, particularly due to BCR::ABL1 mutations and the persistence of leukemia stem cells (LSCs). New treatment strategies that directly inhibit BCR::ABL1 protein expression are needed to achieve more effective therapy. Using a high-content antibody microarray and bioinformatics analyses, we investigated differences in th

Cell Death Induction by MO2455, a Novel Potential Anti-Cancer Agent in Monocytic and T-Cell Leukemia Cells.

Despite advances in targeted therapies, treatment resistance and disease recurrence remain major obstacles in leukemia management. Structural modification of clinically successful kinase inhibitors represents a promising approach for discovering novel anti-leukemic agents with improved biological activities. In the present study, we investigated the anti-leukemic activity and mechanism of action of MO2455, a newly synthesized imatinib-derived compound, in monocytic leukemia U937, THP-1, and T-cell leukemia CCRF-CEM cells. MO2455 showed substantial cytotoxic activity against all three cell line

Clinical Stage-Specific Alteration of hsa-miR-424-5p in Milk Small Extracellular Vesicles from Bovine Leukemia Virus Infection to Progression of Enzootic Bovine Leukosis.

Bovine leukemia virus (BLV) is an oncogenic retrovirus responsible for enzootic bovine leukosis (EBL) in cattle. Previous studies indicated that increased levels of the miRNA hsa-miR-424-5p in milk small extracellular vesicles (sEVs) may serve as an EBL biomarker in cattle; however, its alteration across the clinical stages of BLV infection to the onset of EBL remains unclear. This study evaluated the prognostic potential of hsa-miR-424-5p in milk sEVs across distinct clinical stages of BLV infection to EBL onset. qPCR analysis showed that the abundance of hsa-miR-424-5p was lower in milk sEVs

Programmable and selective silencing of oncogenic fusion transcripts by CRISPR-Cas13b via breakpoint sequence recognition.

Oncogenic gene fusions are key drivers of cancer, yet most remain undruggable by current therapies. Here, we establish CRISPR-PspCas13b as a personalizable platform for systematic silencing of various fusion transcripts. We reveal that recognition and cleavage of the breakpoint sequence by PspCas13b disrupt the fusion transcript, resulting in efficient protein depletion. PspCas13b cleavage produces aberrant, translation-incompetent target RNA species, a subset of which harbors internal deletions proximal to the cleavage site. We demonstrate that targeting the BCR::ABL1 breakpoint sequence effi

A Pilot Study of Condensed Busulfan, Melphalan, and Fludarabine Myeloablative Conditioning in Ex Vivo CD34-Selected Allogeneic Hematopoietic Cell Transplantation.

BACKGROUND: Myeloablative conditioning (MAC) with busulfan, melphalan, and fludarabine (Bu/Mel/Flu) given over 8 days in combination with rabbit antithymocyte globulin (ATG) and ex vivo CD34+-selected allogeneic hematopoietic cell transplantation (allo-HCT) leads to favorable disease control and consistently low rates of graft-versus-host disease (GVHD) in fit patients with myeloid malignancies. However, early regimen-related toxicity continues to be a major determinant of treatment tolerability, necessitating further optimization of the MAC regimen without compromising the efficacy of this pl

Neoantigenic receptor ROR1 promotes cell growth through diverse and complementary mechanisms.

Aberrant expression of neoantigenic receptor ROR1 has been identified in diverse lymphomas, including mantle cell lymphoma (MCL) and in a broad spectrum of nonlymphoid malignancies. While ROR1 has emerged as an attractive therapeutic target, its oncogenic role and the underlying mechanisms remain rather poorly characterized. Focusing on MCL, we found that ROR1 expression is heterogeneous and it can be induced under environmental stress. Inhibition of ROR1 expression resulted in cell-cycle suppression and apoptotic cell death. The ROR1 loss affected expression of numerous genes and altered phos

Multicenter validation of an explainable machine-learning model for APL, AMoL, and ALL.

BACKGROUND: Acute leukemias require rapid subtype identification to guide timely treatment. Differentiation of acute lymphoblastic leukemia (ALL), acute monocytic leukemia (AMoL/M5), and acute promyelocytic leukemia (APL/M3) often relies on specialized tests that may not be immediately available. This study evaluated differentiation only among patients already confirmed to have one of these three diagnoses, rather than screening an unselected population with suspected acute leukemia. METHODS: This multicenter retrospective study included 9,398 newly diagnosed, previously untreated patients wi

Pharmacological inhibition of MCL-1 disrupts mitochondrial cristae and depletes the human neural progenitor cell pool.

Myeloid cell leukemia 1 (MCL-1) is a canonical anti-apoptotic protein crucial for early neurodevelopment. Loss of MCL-1 results in embryonic-lethal neurodevelopmental defects that cannot be rescued by other anti-apoptotic proteins of the B cell lymphoma 2 (BCL-2) family. Here, we pharmacologically inhibit MCL-1 in human neural progenitor cells and find non-apoptotic roles for MCL-1 in sustaining mitochondrial cristae integrity, fatty acid oxidation and neural progenitor identity. MCL-1 inhibition disrupts mitochondrial ultrastructure, leading to a swollen network with disorganized cristae and

Extracellular vesicles in the immune regulation of hematological malignancies: mechanisms, prognostic significance, and therapeutic opportunities.

The progressive cross-talk between tumor cells and the immune microenvironment, which is required for tumor growth, progression, and dissemination, has been well documented in solid cancers and is increasingly recognized in hematological malignancies. Extracellular vesicles (EVs) are now emerging as essential facilitators of immune-tumor crosstalk. In hematological cancers, EVs from leukemia, lymphoma, and myeloma cells carry oncogenic proteins, lipids, cytokines, and non-coding RNAs that reprogram immune cells and establish tumor-promoting immune states, in which tumor cells are protected fro

Altered histone modifications H3K4me3 and H3K27ac are associated with abnormal clonality of peripheral B-cells in cattle with bovine leukemia virus.

B-cell clonality in bovine peripheral blood changes dynamically, with progression from polyclonal to oligoclonal in persistent lymphocytosis (PL), or to monoclonal expansion in enzootic bovine leukosis (EBL). The underlying mechanisms, however, remain poorly understood. In this study, ChIP-seq analysis of histone modifications H3K4me3 and H3K27ac in peripheral blood mononuclear cells (PBMCs) was performed across polyclonal, oligoclonal, and monoclonal B-cell states. PBMCs containing a polyclonal B-cell population exhibited active histone modification features associated with immune activation

LncRNA FGD5-AS1 promotes M2 polarization of tumor-associated macrophages to facilitate the malignant phenotype of non-small cell lung cancer cells.

BACKGROUND: Tumor-associated macrophages (TAMs) take up a high proportion among immune cells in the tumor microenvironment (TME), and their M2- polarized phenotype has been implicated in promoting non-small cell lung cancer (NSCLC) development. Long non-coding RNA FGD5-AS1 (FGD5-AS1) is up-regulated within multiple tumor types, which has been shown to modulate both TAMs polarization and metabolic reprogramming. However, its exact mechanisms in NSCLC are largely unclear. This study aims to investigate whether FGD5-AS1 regulates M2 polarization of TAMs and thereby influences the malignant progre
