Pulmonary Mass as the First Clinical Manifestation of Histiocytic Sarcoma with Multi-Organ Involvement: A Case Report and Review of the Literature.

Histiocytic sarcoma is a malignant neoplasm with cells showing morphologic and immunohistochemical features of histiocytes. The lung and stomach are rare sites of involvement, and few have been reported in the existing literature. We report a 72-year-old woman with primary histiocytic sarcoma involving the right lung, stomach, and bilateral adrenal glands. Computed tomography revealed a large heterogeneous mass in the upper lobe of the right lung, regional lymphadenopathy, and masses within the bilateral adrenal glands. Gastric involvement was discovered via polypectomy during a routine gastri

Rosai-Dorfman Disease With Novel BRAF Fusion Involving the Central Nervous System.

Rosai-Dorfman disease is an uncommon histiocytic neoplasm that is often self-limiting; however, rarely, it exhibits aggressive behavior and may undergo biological transformation to histiocytic sarcoma. Pathogenic genetic drivers of Rosai-Dorfman disease include recurrent genetic abnormalities in KRAS, NRAS, ARAF, and MAP2K1. Mutation of BRAF is a well-known oncogenic driver in histiocytic neoplasms, including up to 50% of Langerhans cell histiocytosis lesions, Erdheim-Chester disease, and adult and juvenile xanthogranuloma. However, BRAF mutations have been documented in only 4 reported Rosai-

Splenic histiocytic sarcoma: Disease progression from the perspective of pathophysiology.

Splenic histiocytic sarcoma (SHS) is a rare, aggressive hematological malignancy with unclear progression and management. Our case illustrates the progression and pathophysiological processes of SHS and provides key data for the diagnosis, treatment and management of SHS. A 60-year-old female with incidentally detected splenic mass (6.0 cm × 5.7 cm) underwent splenectomy, confirmed as SHS in 2020. Post-op imatinib therapy was given. In 2022, hepatic metastases (2.4 cm × 2.9 cm) with pancytopenia led to supportive care. Lesions enlarged to 4.3 cm × 2.7 cm, leading to multi-organ failure and dea

Histiocytic Sarcoma: A Review and Update.

Histiocytic sarcoma (HS) is an ultra-rare hematopoietic neoplasm that frequently occurs in extranodal sites of adults. Clinically, HS demonstrates aggressive behavior and can arise de novo or in association with other hematological neoplasms. The median overall survival from the time of diagnosis is approximately six months. Histologically, HS is composed of sheets of large, round to oval cells with abundant eosinophilic cytoplasm and can be confused with a variety of benign and malignant conditions. Immunohistochemistry plays a crucial role in the diagnosis of HS, showing expression of CD163,

PD-L1 expression and response to immune checkpoint inhibitor therapy in malignant histiocytic neoplasms.

Malignant histiocytic neoplasms (MHN) are aggressive cancers without an established effective treatment. Recent studies have suggested possible role of immune checkpoint inhibitors (ICI), but predictors of response are unknown. We analyzed 26 MHNs to determine the frequency of PD-L1 expression and clinicopathologic correlates. PD-L1-assessment on tumor cells was manually scored negative or positive (≥1%). Median age at diagnosis was 59 years and PD-L1-positivity was seen in 73%. Those with ≥50% PD-L1 expression had better overall survival compared to PD-L1 < 50% (100% vs 42%; P-value:0.0324).

Misdiagnosed Histiocytic Sarcoma: A Case Report.

We report a 31-year-old male patient repeatedly hospitalized for mimicking infectious symptoms. Following standardized anti-infective and anti-inflammatory treatment, clinical evolution was discordant, characterized by treatment non-response and symptom exacerbation. Ultimately, the multidisciplinary team and repeated histopathological examinations diagnosed histiocytic sarcoma (HS). Unfortunately, the treatment opportunity was lost due to HS's high aggressiveness and rapid progression.

Extranodal Histiocytic Sarcoma: A Clinicopathologic Study of Two Cases and Literature Review.

Histiocytic sarcoma (HS) is an extremely rare hematopoietic neoplasm derived from the monocytic/histiocytic/dendritic cell lineage. While sporadic cases of primary soft-tissue HS have been reported, its clinicopathological and molecular features remain incompletely characterized. This study investigates the clinicopathological features of two cases of soft-tissue HS and reviews the relevant literature. Microscopically, the tumor cells exhibited a diffuse growth pattern, with morphologic features varying from spindle-shaped sarcomatoid to epithelioid subtypes. Occasional bizarre cells were obse

Establishment of human histiocytic sarcoma organoids dependent on the SHH/YAP pathway.

Histiocytic sarcoma is an extremely rare and aggressive malignant neoplasm characterized by immunophenotypic features of mature histiocytes. The mechanisms underlying its malignant transformation remain poorly understood; consequently, the development of effective therapies remains limited. Resected histiocytic sarcoma specimens were cultured using a modified air-liquid interface organoid method, serially passaged, and xenografted into NOD-scid IL2Rgnull mice. Tumors formed by xenografted organoids retained histological and genetic similarities with the original tumor. Genomic analysis reveale

Nodal histiocytic sarcoma mimicking metastatic carcinoma, a challenging diagnosis - Case report of an extremely rare hematolymphoid neoplasm with recent updates.

Histiocytic sarcoma (HS), a rare malignant neoplasm of hematolymphoid origin, belongs to the World Health Organization (WHO) classification of histiocytic and dendritic cell neoplasms. This rare aggressive malignancy occurs predominantly in adults and accounts for less than 1% of all hematolymphoid neoplasms. It arises from the monocytic/macrophage lineage and shares the histomorphologic and immunohistochemical features of mature tissue histiocytes. Its predominant presentation is extranodal, involving the intestinal tract, skin, and soft tissue, and it rarely presents with lymphadenopathy. Th

Primary pulmonary histiocytic sarcoma with nodal and esophageal metastasis in a dog-imaging and anatomopathological aspects.

BACKGROUND: This report presents a case of esophageal and nodal metastasis in a primary pulmonary histiocytic sarcoma in a dog. Histiocytic sarcoma is a malignant neoplasm originating from histiocytic cells, characterized by its aggressive nature, high metastatic potential and high mortality rate. Pulmonary involvement occasionally results in pleural effusion and regional lymph node involvement, such as the tracheobronchial, sternal, and mediastinal lymph nodes. Thoracic radiography is the preferred complementary diagnostic test when respiratory disorders are suspected, as it can identify pulm

Diffuse Large B-Cell Lymphoma Transdifferentiating Into Histiocytic Sarcoma: Case Report + Systematic Review.

A 68-year-old man with a history of diffuse large B-cell lymphoma (DLBCL) with cutaneous involvement presented with a new, rapidly growing, exophytic, bleeding tumor. Histopathologic and immunophenotypic characterization was consistent with a diagnosis of histiocytic sarcoma (HS). A subsequent lymph node biopsy also yielded a diagnosis of HS. Genetic analysis of this patient's original DLBCL bone marrow specimen and subsequent lymph node HS specimen identified identical p.G13D KRAS mutations. This case highlights, to our knowledge, the first case of systemic DLBCL with cutaneous involvement li

An In Vitro Approach to Evaluate the Impact of Autolysis and Formalin Fixation on the Detection of Canine Distemper Virus and Innate Immune Response Antigens.

Viral infections in humans and animals are increasing, and retrospective studies using formalin-fixed, paraffin-embedded (FFPE) samples reveal recurring outbreaks over past decades. However, the impact of pre-analytical factors like fixation and autolysis on immunohistochemistry (IHC) remains insufficiently understood. To examine how autolysis, fixation duration (6-72 h) and formalin concentration (2.5-25%) influence histology and IHC of canine distemper virus (CDV, Morbillivirus canis), interferon-β (IFN-β), and selected IFN-stimulated genes (ISGs), the study was conducted using an in vitro m

Durable Response in Histiocytic Sarcoma After Allogeneic Stem Cell Transplantation: A Case Report.

Background and Clinical Significance: Histiocytic sarcoma (HS) is a rare and aggressive form of malignant histiocytosis, often associated with poor prognosis. The diagnosis and management of HS are challenging due to the complexity of its pathogenesis, molecular profile, and the unclear cellular origin of histiocytic neoplasms, compounded by the limited literature on treatment strategies. Case Presentation: We report the case of a young patient with HS localized to the lymph nodes, spleen, and liver, who also presented with hemophagocytic lymphohistiocytosis (HLH) documented on bone marrow bio

Unifocal Langerhans Cell Histiocytosis.

Langerhans cell histiocytosis is a rare disease which is more common in the childhood. Clinical presentation may vary ranging from single to multiple organ involvement. Skeleton is often involved and in the maxillofacial region, it affects the mandible more than the maxilla. Radiographically, it may present as a well-defined radiolucency or an ill-defined radiolucency. The presence of Langerhans cells in an inflammatory background with prominent eosinophils gives a clue to the diagnosis. Confirmation is by positive immunohistochemical staining with S100, CD1a and CD207. Here, we report a case

EBV-positive inflammatory follicular dendritic cell sarcoma of fibroblastic reticular cell phenotype arising in intrapancreatic accessory spleen.

Epstein-Barr Virus (EBV)-positive inflammatory follicular dendritic cell sarcoma (EBV-IFDCS) is a rare tumor of low malignant potential. Abundant lymphocytes and plasma cells often infiltrate into this tumor tissue with lymphoid follicles and sarcoid-like granulomas, so that the differential diagnosis of this tumor includes inflammatory lesions such as IgG4-related disease, morphologically. This tumor typically arises in the spleen or liver, and shows follicular dendritic cell (FDC)-like immunophenotype. Here we presented a case of EBV-IFDCS arising in intrapancreatic accessory spleen, which w
