Incidence of first and subsequent fractures in multiple myeloma patients: a parallel cohort study using UK CPRD dataset.

UNLABELLED: Based on a UK primary care dataset, patients with Multiple Myeloma (MM) experience a significantly higher index fracture rate from 1 year prior to MM diagnosis onwards and a higher subsequent vertebral fracture rate, compared to non-MM controls. There is potential for earlier MM diagnosis and reducing subsequent fracture risk. PURPOSE: Whilst multiple myeloma (MM) is known to increase fracture risk, the incidence of subsequent fractures is poorly described. Here, we describe the incidence of index and subsequent fractures in a real-world cohort of MM patients, compared to non-MM c

MGMT, NUPR1, NDRG2, and GLI1 Gene Promoter Methylation in Glioblastoma Tissues and Association with Clinical Characteristics and Therapeutic Outcomes.

Glioblastoma (GBM) is the most prevalent and devastating form of primary brain tumors in adults, with dismal survival despite advancements in treatment modalities. The current study sought to develop clinically significant prognostic models for GBM patients by comprehensively profiling MGMT, NUPR1, NDRG2, and GLI1 gene promoter methylation in GBM tissues vs. non-neurooncological disease (NND) and their association with clinical characteristics and therapeutic outcome. This was further evaluated by in silico functional enrichment analysis. NUPR1, NDRG2, and GLI1 gene promoter methylation were s

Primary Cutaneous Peripheral T-Cell Lymphoma With TFH Phenotype With Divergent Clonal B- and Plasma-Cell Populations and DNMT3A Mutation.

Primary cutaneous peripheral T-cell lymphoma with T-follicular helper phenotype (pcTFH-PTCL) is rare, and concurrent clonal B-lineage proliferations are uncommon. This lymphoma subtype presents unique diagnostic challenges due to its rarity and potential for misdiagnosis. We present a case of pcTFH-PTCL in an 88-year-old man with pruritic nodules on the shoulders, later developing multifocal cutaneous lesions including a prominent eyebrow nodule. Biopsies showed dense dermal infiltrates of CD4 positive T cells expressing TFH markers (PD-1, ICOS, and BCL6). Three lesions demonstrated an identic

HER2 heterogeneity between primary cancers and brain metastases: a retrospective analysis of 185 matched pairs across multiple cancer types.

BACKGROUND: HER2 overexpression drives tumor aggressiveness and poor prognosis, serving as a crucial therapeutic target in various malignancies. While HER2’s role in breast cancer metastasis and prognosis is well-documented, its heterogeneity in brain metastases (BM) across different primary tumors remains understudied. This investigation examined HER2 status in matched primary cancers and BM across multiple cancer types to evaluate HER2 alterations during metastases. METHODS: We conducted a retrospective analysis of 185 matched primary tumor-BM pairs. HER2 expression was assessed using immuno

Cancer-specific survival patterns in patients with bone metastasis: a registry-based analysis of 13,742 patients.

With increasing opportunities for patients with bone metastasis (BM) to benefit from local surgical intervention, accurate survival analysis across different primary cancers remains challenging. Current analytical frameworks commonly rely on single-center, pan-cancer cohorts and provide insufficient integration of cancer-specific characteristics. In this retrospective, multicenter, registry-based cohort study, baseline demographic and clinical characteristics of 13,742 patients with AJCC stage IV or TNM stage M1 metastatic cancer were collected from 42 studies registered in the cBioPortal for

A differentiated thyroid carcinoma-specific graded prognostic assessment for brain metastases: derivation in a 10,306-patient cohort and external validation.

BACKGROUND: Brain metastases from differentiated thyroid carcinoma (DTC) are rare but associated with significantly worse prognosis compared to extracranial metastatic sites. Existing prognostic models for brain metastases are not tailored to DTC, limiting risk stratification and therapeutic guidance in this population. METHODS: We conducted a retrospective cohort study of 10,306 patients with DTC treated at a single tertiary cancer center from 2011 to 2025. Patients with radiologically or pathologically confirmed brain metastases were included. Multivariable Cox regression was used to identi

Clinical trial endpoints for metastases-directed therapy in oligometastatic cancer: a review and Delphi consensus on behalf of the EORTC-ESTRO OligoCare consortium.

Oligometastatic cancer is characterised by a low volume of metastases to a small number of anatomical sites. However, evaluating the impact of metastases-directed therapies (MDTs) on overall survival or quality of life is often challenging. Current clinical trials use a wide range of primary endpoints that might not be validated or suited to MDT. To address this issue, we did a systematic review of international trial registries, alongside a Delphi consensus process involving 30 experts and five patient representatives. The aim was to identify preferred primary endpoints for MDT trials in olig

Cross-tissue atlas of mucosa-associated lymphoid tissue lymphomas reveals intratumoral heterogeneity and microenvironmental subtypes.

Mucosa-associated lymphoid tissue (MALT) lymphoma, a common primary extranodal B cell lymphoma that arises in diverse mucosal tissues, has a pathogenesis closely linked to the supportive lymphoma microenvironment (LME). However, its microenvironmental heterogeneity and therapeutic implications remain unclear. Here, we report a cohort of 89 MALT lymphoma samples from multiple anatomical sites, profiled by bulk (n = 89), single-nucleus (n = 16), and spatial (n = 9) RNA sequencing. Integrative analysis of 120 bulk transcriptomes from our cohort and two independent cohorts, guided by cell-type-res

MYD88 gene and protein: molecular architecture, signalling mechanisms and clinical implications in lymphoid malignancies.

MYD88 is a signal-transducing adaptor protein that plays a central role in Toll-like receptor and interleukin 1 receptor signalling through activation of the NF-κB pathway. The somatic L265P mutation in MYD88 represents a recurrent gain-of-function alteration in mature B-cell neoplasms, most notably lymphoplasmacytic lymphoma (LPL)/Waldenström macroglobulinaemia (WM) (>90%), IgM monoclonal gammopathy of undetermined significance (27%-87%) and activated B-cell-type diffuse large B-cell lymphoma (DLBCL), particularly those arising in immune-privileged sites such as the central nervous system (CN

Integrative cfDNA profiling from low-pass whole-genome sequencing enables tissue-of-origin prediction in cancer.

Cancer type classification is challenging due to tumor heterogeneity and undefined tissue of origin (TOO), particularly in cancers of unknown primary (CUP) and multiple primary cancers (MPC). Accurate TOO identification is critical for guiding treatment and prognosis. We developed a stacked ensemble machine learning classifier that integrates 11 multidimensional cfDNA features spanning genomic, fragmentomic, methylation/repeat, and microbial signals. Base models were constructed using five algorithms, including Deep Learning, Distributed Random Forest, Gradient Boosting Machine, Generalized Li

Exploring Primary Care Interventions to Increase Lung, Breast, and Cervical Cancer Screening With Individuals Experiencing Marginalization: A Scoping Review.

IntroductionScreening for lung, breast, and cervical cancer can reduce morbidity and mortality by enabling earlier detection and treatment. However, participation remains below target levels, particularly among individuals experiencing marginalization. While primary care providers (PCPs) can play a key role in improving screening uptake, few reviews have focused on interventions targeting individuals experiencing marginalization that are aimed at PCPs, delivered by PCPs, or that occurred in a primary care setting.MethodsThe aim of this scoping review was to examine the extent and types of PCP

Predicting distant metastatic sites of cancer using perturbed correlations of miRNAs with competing endogenous RNAs.

Cancer metastasis is the dissemination of tumor cells from the primary tumor site to other parts of the body via the lymph system or bloodstream. Metastasis is the leading cause of cancer associated death. Despite the significant advances in cancer research and treatment over the past decades, metastasis is not fully understood and difficult to predict in advance. In particular, distant metastasis is more difficult to predict than lymph node metastasis, which is the spread of cancer cells to nearby lymph nodes. Distant metastatic sites is even more difficult to predict than the occurrence of d

BACKGROUND: Cancer of unknown primary (CUP) remains a diagnostic and therapeutic challenge with limited treatment guidance. Caudal type homeobox 2 (CDX2), a transcription factor indicative of intestinal differentiation, may define a distinct subset of CUP with potential sensitivity to 5-fluorouracil (5-FU)-based chemotherapy. This study examines outcomes in patients with CDX2-positive CUP and the predictive value of CDX2 expression for response to 5-FU-based regimens. PATIENTS AND METHODS: We analyzed data from CDX2-positive CUP patients in the Mayo Clinic Cancer of Unknown Primary registry.
