Journal of clinical and experimental hepatology2026eng
Hepatic melioidosis is a clinically significant manifestation of Burkholderia pseudomallei infection, increasingly recognised across South and Southeast Asia. It primarily affects diabetic and immunocompromised individuals and is characterised by a subacute to chronic presentation that often mimics other causes of liver abscess. The pathogenesis of hepatic melioidosis reflects the interplay between bacterial virulence mechanisms and impaired host immunity, leading to intracellular persistence and necrotising hepatic lesions. Characteristic imaging features, including the honeycomb and necklace

Melioidosis is caused by Burkholderia pseudomallei and is found in soil and water in tropical and subtropical regions of the world. The bacterium is capable of evading the host's immune system, leading to the development of acute, subacute, or chronic invasive infections or potentially entering a latent state that may persist for an extended period. The true burden of melioidosis is vastly underestimated. Hot and humid climates with extreme weather conditions and rainy seasons are linked to increased melioidosis cases. Rice fields, building construction workers working on the muddy soil, and b

Melioidosis is an infectious, tropical and emerging disease, due to a bacterium of the hydrotelluric environment, Burkholderia pseudomallei, which is considered as a potential biological weapon because of its exceptional resistance and virulence capacities. Its worldwide spread, outside the original endemic foci of Southeast Asia and Australia, is favoured by global warming and the diabetes mellitus pandemic, which is the main predisposing factor. In humans, melioidosis is an opportunistic infection, following professional (rice farmers, soldiers) or accidental contamination, by inhalation or i

BACKGROUND: Melioidosis pneumonia, caused by the bacterium Burkholderia pseudomallei, is a serious infectious disease prevalent in tropical regions. Chest computed tomography (CT) has emerged as a valuable tool for assessing the severity and progression of lung involvement in melioidosis pneumonia. However, there persists a need for the quantitative assessment of CT characteristics and staging methodologies to precisely anticipate disease progression. This study aimed to quantitatively extract CT features and evaluate a CT score-based staging system in predicting the progression of melioidosis

Melioidosis is caused by percutaneous inoculation or inhalation of Burkholderia pseudomallei, predominantly among individuals with risk factors (diabetes mellitus, immunosuppression, etc.) from endemic areas of South Asia, Southeast Asia and Northern Australia. While some patients present acutely with sepsis and multi-organ failure, others present with a subacute to chronic course characterised by abscess formation. We present nine representative cases, each with an abscess at a separate site (lung, skin, bone/ joint, prostate, parotid gland, liver, spleen, brain and orbits). Using these cases

INTRODUCTION: Burkholderia pseudomallei is a gram-negative environmental bacterium and aetiological agent of melioidosis, a tropical infectious disease with diverse clinical presentations. We aimed to describe the epidemiological and clinical characteristics of melioidosis in northern Hainan and to determine the meteorological factors affecting its morbidity.
METHODS: We conducted a retrospective, multicentre, observational cohort study of 90 patients with melioidosis admitted to four general hospitals in northern Hainan from 2010 to 2020. Epidemiological, clinical presentation, laboratory an

AIMS: The goal of this study was to examine, for the first time, the virulence and pathogenicity of aerosolized Burkholderia pseudomallei, strain NCTC 13392, in BALB/c mice in order to develop an animal model for testing novel medical countermeasures (MCMs) for the treatment of human acute and subacute (a disease state between acute and chronic) melioidosis.
METHODS AND RESULTS: BALB/c mice were exposed to varying doses of aerosolized bacteria. Acute disease was seen in animals exposed to a very-high dose (≥103 CFU per animal) and death occurred 3-4 days postchallenge (pc). Bacteria were det

BACKGROUND: Melioidosis is endemic in tropical Australia and Southeast Asian countries and its causative organism Burkholderia pseudomallei is a recognized cause of pneumonia in these regions. Recent isolation of the organism in the soil of Kapasia, Gazipur, Bangladesh has proven its exposure among the population residing in endemic areas of our country. Pneumonia is the most common presentation of melioidosis. Acute, subacute and chronic pneumonia due to B. pseudomallei can present as primary or secondary pneumonia. Treatment of such cases are challenging as well. Till date, few cases of acut

Infection with Burkholderia pseudomallei can result in asymptomatic seroconversion, a single skin lesion that may or may not heal spontaneously, a pneumonia which can be subacute or chronic and mimic tuberculosis or rapidly progressive resulting in fatal overwhelming sepsis. Latency with subsequent activation of disease is well recognized, but very uncommon. Melioidosis also has a myriad of other clinical presentations and diagnosis is often delayed because of this and because of difficulties with laboratory diagnosis and lack of recognition outside melioidosis-endemic regions. The perception

Infection with Burkholderia pseudomallei causes the disease melioidosis, which often presents as a serious suppurative infection that is typically fatal without intensive treatment and is a significant emerging infectious disease in Southeast Asia. Despite intensive research there is still much that remains unknown about melioidosis pathogenesis. New animal models of melioidosis are needed to examine novel aspects of pathogenesis as well as for the evaluation of novel therapeutics. The objective of the work presented here was to develop a subacute to chronic caprine model of melioidosis and to

Melioidosis (Burkholderia pseudomallei) is a gram-negative bacterial infection that is highly endemic in Southeast Asia and Oceania. Pulmonary disease is the most common form of involvement. The clinical-radiologic thoracic manifestations of melioidosis can be classified as acute, subacute, subclinical, and chronic forms. Radiographic findings include nodular, alveolar, or mixed infiltration/consolidation with or without cavities. Pleural effusion, pneumothorax, and pericardial involvement can be seen. Melioidosis can easily be confused with other infections, especially tuberculosis. Suspicion

BACKGROUND: Melioidosis is an important cause of community-acquired sepsis in Southeast Asia and northern Australia, and pneumonia is the most common presentation. Clinical manifestations range from acute fulminant sepsis to chronic infection mimicking tuberculosis. Pneumonia may be the primary presenting feature, or it can develop secondary to initial disease at a distant focus.
METHODS: A prospective database of all melioidosis patients at Royal Darwin Hospital (Australia) between 1989 and 2010 was reviewed.
RESULTS: Of 624 patients with culture-confirmed melioidosis, 319 (51%) presented w

Between 1996 and 2002, 162 cases of pulmonary melioidosis were reported from Srinagarind Hospital, Khon Kaen, northeast Thailand, 90 acute vs 72 subacute/chronic. Patients averaged 50 years of age and half worked as farmers. The male to female ratio was between 2 and 3 to 1 depending on the subgrouping. Burkholderia pseudomallei was confirmed by a culture or a four-fold rise in titer in the majority of cases, while the others were presumptive diagnoses based on response to treatment. Pulmonary melioidosis presented as either acute fulminant pneumonia or as an indolent disease. The common concu

BACKGROUND: Burkholderia pseudomallei is an important cause of acute fulminant pneumonia and septicaemia in tropical regions of northern Australia and south east Asia. Subacute and chronic forms of the disease also occur. There have been three recent reports of adults with cystic fibrosis (CF) who presumably acquired B pseudomallei infection during extended vacations or residence in either Thailand or northern Australia.
METHODS: The clinical course, molecular characteristics, serology and response to treatment are described in four adult CF patients infected with B pseudomallei. Polymerase c

Burkholderia pseudomallei is the aetiological agent of melioidosis, a life-threatening bacterial disease occurring in many species of animals, including man. Infection in humans commonly manifests as one of three clinical presentations: acute, subacute or chronic disease. Investigations were undertaken to assess the suitability of BALB/c and C57Bl/6 mice as animal models for the different forms of human melioidosis. The course of infection in BALB/c mice was similar to that which occurs in acute human infection. By contrast, infection of C57Bl/6 mice appeared to mimic chronic human melioidosis

A preliminary survey was conducted for the prevalence of HIV infections in pulmonary tuberculosis and melioidosis patients in Ubon Ratchathani province, in Thailand, the second largest province in population which supplies labors to Bangkok metropolis. In this province, tuberculosis is prevalent in a higher rate than in most other provinces and melioidosis is endemic. Four HIV-seropositives were found in a total of 551 suspected and culture-positive cases of pulmonary tuberculosis, while no HIV-seropositive was found in 121 melioidosis patients. In view of the rapidly expanding HIV-infections

The chest radiographs of 183 patients with pulmonary melioidosis seen over a 4-year period were evaluated retrospectively. Disseminated nodular lesions occurred in 46 of 55 patients (84%) with the acute septicemic form of the disease and in 17 of 31 (55%) with the subacute or chronic form and positive blood cultures. Alveolar lesions occurred in 26 of 50 patients (52%) with the acute pneumonic form. There was no typical pattern of pulmonary infiltrates in 16 patients with the subacute pneumonic form, but in the chronic form, 17 of 31 patients (55%) had mixed infiltrates with cavities. The lesi

Melioidosis is being diagnosed with increasing frequency in the northern part of the Northern Territory, but the mortality rate remains high in the acute septicaemic form of the disease largely because of associated chronic debilitating illnesses. This paper reviews epidemiological and clinical features of human melioidosis in 37 cases seen between 1960 and 1979. Infection with Pseudomonas pseudomallei is most often contracted during the wet season by persons who have regular contact with soil or ground water, probably through pre-existing skin lesions or penetrating wounds and, occasionally,

During the 6-year period from 1965 through 1970, 39 patients with pulmonary melioidosis were treated at Fitzsimons Army Medical Center. Although this is a disease with a well-defined endemic area that does not include the United States, cases will no doubt continue to be seen in this country. These cases may result from acute infection in the endemic area, with subsequent travel to this country, or from delayed reactivation of a latent infection acquired months or years earlier. Given the potential for occurrence of this disease in the United States, continued awareness by the medical professi
