Mitochondrial Neurogastrointestinal Encephalopathy: A Case Report from Northeast of Iran.

Mitochondrial neurogastrointestinal encephalopathy (MNGIE) is a rare autosomal recessive disorder often misdiagnosed due to its heterogeneous presentation. We describe a 21-year-old man with a long history of episodic nausea and vomiting, weight loss, and intermittent diarrhea since childhood, initially misdiagnosed as Crohn's disease following a colonoscopy-induced perforation. Despite treatment with adalimumab, his gastrointestinal symptoms persisted. Subsequent investigations revealed dilated stomach and duodenum, and new neurological findings (ptosis, nasal speech) prompted further evaluat

Helicobacter pylori Infection - Advances, Challenges, and the Road Ahead.

The discovery of Helicobacter pylori has revolutionized the prevention and treatment of gastritis, peptic ulcers, and, most important, gastric cancer. In the past 40 years, enormous progress has been achieved in the clinical and basic research on H. pylori infection. However, challenges in preventing and treating H. pylori infection exist owing to the lack of an H. pylori vaccine and the ever-increasing resistance to antibiotics worldwide. This review summarizes the history of and key milestones in H. pylori research, provides an update on the understanding of the pathogenesis and global burde

The ethical labour of overdiagnosis research: Negotiating care in early detection.

Particularly in relatively well-funded healthcare systems, medicine increasingly seeks to detect disease before it causes symptoms. Early detection can prevent suffering and premature death, but it can also diagnose conditions that will never become harmful. Although the existence of overdiagnosis is supported by clinical and epidemiological literature, its implications for practice remain contested. This paper examines how clinicians and researchers decide what evidence of overdiagnosis should mean for good care and how they act on it (or not). Drawing on three years of ethnographic fieldwork

Dramatic regression of mass-forming gastric IgG4-related disease with inebilizumab: a case report.

BACKGROUND: Gastric immunoglobulin G4-related disease (IgG4-RD) is exceedingly rare, with fewer than 50 cases reported worldwide. Its mass-forming presentation frequently mimics malignancy, leading to high rates of unnecessary surgical resection. Inebilizumab, a CD19-directed monoclonal antibody and the first FDA-approved therapy for IgG4-RD, has not previously been described in gastric IgG4-RD. We report the first case of mass-forming gastric IgG4-RD treated with inebilizumab, achieving clinical, endoscopic, and histologic remission without surgical intervention. CASE PRESENTATION: A 52-year

Residual tumor deposits after neoadjuvant therapy and 3-year survival outcomes in gastric cancer: a single-center retrospective cohort study.

BACKGROUND: The prognostic relevance of residual tumor deposits after neoadjuvant therapy (ypTD) in gastric cancer remains insufficiently defined. This study evaluated associations between ypTD positivity and 3-year disease-free survival (DFS) and overall survival (OS) after curative-intent gastrectomy. METHODS: This single-center retrospective cohort study included 336 patients with locally advanced, clinically lymph node-positive gastric adenocarcinoma who underwent neoadjuvant therapy followed by R0 gastrectomy. Patients were classified by postoperative ypTD status. Survival analyses were

Extracellular vesicle biomarkers in gastric cancer: Opportunities and challenges.

Extracellular vesicles (EVs) are promising liquid biopsy candidates for gastric cancer because they can capture tumor-, immune-, microbial-, and microenvironment-related signals. We argue that the main challenge is no longer simply to find more candidates but to advance EV biomarkers with demonstrated diagnostic, therapeutic-monitoring, or prognostic value toward clinical translation. In this opinion article, we highlight EVs from nonblood biofluids, non-tumor-derived EVs, and underexplored EV cargo types, particularly small noncoding RNA cargoes, as sources of biomarker potential. We further

O-GlcNAcylation Regulation of SNAP29-Dependent Autophagy Activation Dictates Chemoresistance in Gastric Cancer.

Chemoresistance remains a major obstacle in gastric cancer (GC) treatment, particularly to standard fluoropyrimidine and platinum regimens. O-GlcNAc transferase (OGT) and O-linked β-N-acetylglucosamine modification (O-GlcNAcylation) are frequently elevated in cancers, yet their specific role in GC chemotherapy remains poorly defined. Contrary to its typical oncogenic function, this study demonstrates that low OGT/O-GlcNAcylation is associated with resistance to 5-fluorouracil (5-FU) or oxaliplatin (OXA) in GC. Clinically, high OGT expression correlates with better prognosis in patients receivi

Robotic Versus Laparoscopic Gastrectomy for Advanced Gastric Cancer: Prognostic Implications of Surgical Recovery and Oncologic Control in a Propensity Score-Matched Study.

BACKGROUND: Robotic gastrectomy may improve precision during radical lymphadenectomy, but its prognostic relevance in advanced gastric cancer remains uncertain. The authors hypothesized that robotic surgery may influence outcomes through improved recovery and oncologic control. METHODS: A prospectively maintained registry included 426 curative-intent minimally invasive gastrectomies from 2017 to 2025 (laparoscopic, n = 176; robotic, n = 250). Propensity score-matching yielded 154 pairs. Endpoints included severe morbidity (Clavien-Dindo grade ≥ III), postoperative inflammation, adjuvant chemo

Assessing Graded Histological Regression as Basis for Treatment Modification after Neoadjuvant Therapy in LAGC: An Exploratory Phase II Study.

BACKGROUND: Whether to change adjuvant regimen after ineffective neoadjuvant chemotherapy in locally advanced gastric cancer (LAGC) remains unclear. We conducted an exploratory study on whether graded histologic regression (GHR) < 50% was a potential indicator for regimen modification. PATIENTS AND METHODS: The study was a prospective, single-center, open-label, randomized, phase II trial. Patients were randomly assigned to group A (paclitaxel-modified regimen) and group B (original regimen) for adjuvant chemotherapy. The primary endpoint was disease-free survival (DFS), with secondary endpoi

Impact of recurrence on survival after gastric cancer curative surgery: a retrospective, single-center, and multistate model analysis.

BACKGROUND: Recurrence is the strongest determinant of mortality after curative gastrectomy for gastric cancer. Conventional Cox models do not explicitly account for recurrence as an intermediate event. Multistate models (MSMs) allow for the dynamic estimation of transition-specific risks across clinically relevant disease states. METHODS: Patients who underwent curative-intent gastrectomy between 2013 and 2023 were retrospectively analyzed. A 3-state MSM (alive and recurrence-free, alive after recurrence, and death) was constructed. State occupation probabilities and transition probabilities

MCOF: A Multi-Channel Attention-Based Omics Fusion Framework for Cancer Subtyping and Candidate Biomarker Prioritization.

Multi-omics molecular subtyping is complicated by small cohorts, heterogeneous data layers, and sensitivity to data splitting and feature selection. We developed multi-channel attention-based omics fusion (MCOF), an end-to-end framework that combines omics-specific projection, one-dimensional convolution, and integrated gradients for candidate biomarker prioritization. MCOF was evaluated against six baselines using five repeated stratified holdouts across four multi-omics datasets. In the restricted four-class breast invasive carcinoma task, MCOF achieved the highest observed mean for all five

Identification of heterogeneous subtypes of human dendritic cells from the lymph nodes of patients with gastric cancer.

OBJECTIVE: The subtypes of heterogeneous conventional dendritic cells (cDCs) and plasmacytoid dendritic cells (pDCs) in lymph nodes (LN) cannot be distinguished because they share similar surface markers. This study aimed to identify cDCs, pDCs, and other cell types in DC-enriched LNs from patients with gastric cancer (GC) by analyzing surface markers and gene expression. METHODS: Expression of surface markers, including CD141, CD1c, and CD123, in DC-enriched cells was analyzed by conventional flow cytometry and AbSeq using antibody-derived tags. Transcriptome and epitope analyses were used t

Vascular plasticity in the gastric cancer environment: mechanisms, prognostic significance, and concepts of therapeutic interventions.

Gastric cancer (GC) remains a major global health problem because of its aggressive behavior and frequent development of therapeutic resistance. In GC, as in other malignancies, blood vessels are essential for Tumor-cell survival and dissemination, thereby facilitating metastasis to distant organs. As tumors grow, they remodel the surrounding microenvironment, generating hypoxic, acidic, nutrient-poor conditions with elevated interstitial fluid pressure. Reactive oxygen species further support tumor growth, glycolytic metabolism, and neovascularization. Highly aggressive cancer cells can also

Progress in the Mechanism of Heat Shock Proteins in Gastric Cancer: From Biomarkers to Therapeutic Targets.

BACKGROUND: Gastric cancer remains a major challenge in public health, with low early diagnosis rates and poor prognosis for advanced patients. Heat shock proteins (HSPs) are highly conserved molecular chaperones that maintain proteostasis and promote cell survival under stress. Aberrantly elevated HSPs in gastric cancer are closely linked to tumor initiation, invasion, metastasis, and poor prognosis. This review aims to comprehensively synthesize the latest advances concerning HSPs in gastric cancer across three core dimensions: their utility as diagnostic/prognostic biomarkers, their regulat

Neoadjuvant Therapy for Gastric Adenocarcinoma with Signet Ring Cells: A Systematic Review and Meta-Analysis.

BACKGROUND: Signet ring cell-containing gastric cancer constitutes an increasingly recognized subtype. Although perioperative chemotherapy is the standard of treatment for locally advanced gastric cancer, its efficacy in signet ring cell-containing gastric adenocarcinomas remains controversial. This systematic review and meta-analysis aims to assess the impact of neoadjuvant chemotherapy compared to upfront surgery on overall survival. METHODS: A systematic search of MEDLINE, Scopus, and Cochrane was conducted up to January 2026. Eligible studies included adult patients with signet ring cell-
