Increased risk of cancer in patients with mixed connective tissue disease: a population-based cohort study in Korea.

INTRODUCTION: We conducted a nationwide, population-based cohort study to evaluate the risk of cancer in Korean patients with mixed connective tissue disease (MCTD) compared with the corresponding risk in the general population. METHODS: Using the Korean National Health Insurance Service data, we identified 2,094 patients with newly diagnosed MCTD between 2004 and 2020 (16,409 person-years). Standardized incidence ratios (SIRs) were calculated to compare the overall and site-specific cancer risks between patients with MCTD and the age- and sex-matched general population. RESULTS: The overall

Global, regional, and national burden of colorectal and stomach cancers attributable to smoking and alcohol use, 1990-2023, and projections to 2050.

OBJECTIVE: To quantify temporal and geographic trends in mortality and morbidity attributable to these risk factors from 1990 to 2023 and to project disease burden through 2050. METHODS: Annual estimates of deaths, disability-adjusted life years (DALYs), years of life lost (YLLs), and years lived with disability (YLDs) were extracted by age, sex, and geographic region. Temporal trends in age-standardized rates were assessed using estimated annual percentage change (EAPC) derived from log-linear regression and joinpoint regression to identify inflection points. For projections through 2050, we

Comparison of robotic and laparoscopic gastrectomy using textbook oncologic outcome: a matched cohort study.

By evaluating whether a patient simultaneously satisfies multiple desirable goals, textbook oncologic outcome (TOO) offers a composite assessment of cancer-surgery quality. This study investigated whether TOO attainment after curative gastric cancer surgery differed according to the operative approach. We retrospectively reviewed 945 patients with gastric adenocarcinoma who received curative-intent robotic gastrectomy (RG; n = 161) or laparoscopic gastrectomy (LG; n = 784) between 2017 and 2025. Propensity score matching yielded 147 patients in each group. When each of the following was record

Nationwide Claims-Based Incidence and Associated Factors of Adverse Events after Gastric Endoscopic Submucosal Dissection.

BACKGROUND/AIMS: Most data on adverse events (AEs) following gastric endoscopic submucosal dissection (ESD) originate from high-volume expert centers, limiting their generalizability to routine clinical practice. Population-level claims-based evidence on AEs after gastric ESD remains limited, although such data cannot fully account for lesion- and procedure-specific risk factors. METHODS: We conducted a nationwide, retrospective cohort study using claims data from the Korean Health Insurance Review and Assessment Service. All patients who underwent ESD for gastric neoplastic lesions between 2

Giant Cystic Lymphangioma of the Transverse Mesocolon in a 7-Year-Old Child: A Case Report.

BACKGROUND: Cystic lymphangiomas are rare benign lymphatic malformations that are usually diagnosed in early childhood. Intra-abdominal lesions are uncommon, and involvement of the transverse mesocolon is particularly rare. CASE PRESENTATION: A 7-year-old girl presented with a 3-4 week history of vomiting, abdominal pain, progressive abdominal distension, poor appetite, abdominal fullness, generalized weakness, and irregular bowel movements. Examination revealed a large, well-defined, irregular, non-tender abdominal mass measuring approximately 12 × 10 x 12 cm and extending from the right fla

Nonlinear Associations of Body Muscle-to-Fat Ratio With Long-Term Prognosis in MASLD.

BACKGROUND: As weight-loss therapies are increasingly incorporated into clinical practice for MASLD, assessing body composition has become increasingly important. Body muscle-to-fat ratio (BMFR) may provide an intuitive and integrated measure of body composition balance, but its association with long-term prognosis in MASLD remains unclear. METHODS: We studied 83 006 UK Biobank participants with MASLD. BMFR was calculated as whole-body muscle mass divided by whole-body fat mass using bioelectrical impedance analysis. Restricted cubic spline, threshold-effect and two-piecewise Cox models asses

Short-term surgical outcomes of robotic versus laparoscopic radical gastrectomy for gastric cancer: a propensity score-matched retrospective cohort study.

Robotic gastrectomy (RG) is increasingly used for gastric cancer, but real-world comparative data from Chinese institutions during the early phase of robotic program implementation remain limited. We retrospectively identified patients who underwent radical gastrectomy between January 2015 and October 2025. The primary comparison included 54 patients who underwent RG and 162 who underwent Laparoscopic gastrectomy (LG). After exclusion of patients with missing propensity-score covariates, 53 RG and 143 LG patients were eligible for matching. One-to-two nearest-neighbor matching was performed us

Dynamic perturbation analysis: a modular rewiring framework for decoding cancer stage progression and identifying therapeutic vulnerabilities.

Disease progression is driven not only by transcriptional alterations but also by dynamic reorganization of molecular interaction networks. However, most existing approaches focus on differential gene expression and fail to capture modular-level reorganization. Here, we present dynamic perturbation analysis (DPA), a modular rewiring framework that quantifies dynamic perturbations across disease stages using a novel metric termed the degree of perturbation. Using gastric cancer (GC) progression as a model system, DPA revealed that extensive modular rewiring persists despite limited stage-specif

Short-Term Outcomes of Outpatient Endoscopic Resection for Selected Superficial Gastric Neoplasms: A Predominantly EMR-Based Real-World Cohort Study.

Background/Objectives: Although same-day discharge after gastric endoscopic resection has been explored, contemporary data on outpatient treatment using predominantly endoscopic mucosal resection (EMR)-based techniques remain limited. This study aimed to describe the technical and short-term clinical outcomes of outpatient endoscopic resection for selected superficial gastric neoplasms. Methods: We retrospectively reviewed 259 superficial gastric neoplasms from 254 consecutive patients who underwent outpatient endoscopic resection. The primary outcomes were short-term adverse events and unplan

A high-resolution spatial model of the primate gut microbiome defines niche-specific functions and host-adapted microbial metabolism.

The spatial organization of the gastrointestinal microbiome is shaped by strong physicochemical gradients along the gut, yet many region-associated microbial niches remain poorly resolved in humans. Here, we used the rhesus macaque (Macaca mulatta) as a spatially resolved primate model to profile 148 luminal and mucosal samples across the gastrointestinal tract using shotgun metagenomics, 2bRAD-M sequencing, and targeted bile acid metabolomics, complemented by comparative human datasets. Microbial communities exhibited strong spatial stratification along the gastrointestinal tract, while host

Danshen Injection for 5-Fluorouracil-Resistant Gastric Cancer: Mitochondrial Apoptosis and Enhanced Drug Activity.

Gastric cancer ranks among the leading causes of cancer-related death worldwide, and the progressive emergence of 5-fluorouracil (5-FU) resistance represents a critical barrier to the efficacy of conventional chemotherapy, particularly 5-FU-based regimens. Danshen injection (DI), a clinically approved intravenous preparation derived from Salvia miltiorrhiza, has demonstrated preliminary antineoplastic activity; however, the mechanistic basis underlying its reversal of 5-FU resistance in gastric cancer has not been systematically defined. This study aimed to evaluate the antitumor efficacy of D

An Atypical Presentation of Richter's Transformation in Chronic Lymphocytic Leukemia: Stroke-Like Onset With Multiorgan Involvement Including the Heart and Seminal Vesicles.

Chronic lymphocytic leukemia (CLL) is usually an indolent lymphoproliferative disorder. However, a small proportion of patients may develop Richter's transformation (RT), in which CLL transforms into an aggressive lymphoma. This transformation is often associated with rapid clinical progression and extranodal disease. Here, we describe a 72-year-old man diagnosed with CLL during evaluation for persistent cough and leukocytosis. Peripheral blood smear demonstrated smudge cells, and the diagnosis was confirmed by peripheral blood flow cytometry. As the patient was asymptomatic, he was managed wi

Size-dependent Effects of Polystyrene Micro- and Nanoplastics on Gastrointestinal Microbiota in Mice: Overlooked Axial Site-Specific Heterogeneity and Radial Mucosal-Luminal Compartment Divergence.

Accumulating evidence links micro- and nanoplastics (MNPs) exposure to gastrointestinal (GI) health impairment, especially via gut microbiota disruption. However, most studies focus on fecal or cecal luminal microbiota, neglecting the inherent spatial heterogeneity of the GI tract and distinct mucosal versus luminal microbial niche responses. In this study, multi-omics was applied to investigate the effects of polystyrene nanoplastics (PS-NPs, 100 nm) and microplastics (PS-MPs, 5 μm) on mouse GI microbiota following 4-week drinking water exposure. 16S rDNA sequencing revealed that MNPs altered

A lifetime perspective on resource use: spatial and ontogenetic trophic shifts in the blackspotted smooth-hound (Mustelus punctulatus).

Elasmobranchs are facing severe threats worldwide from overfishing and related high bycatch, and particularly so in the Mediterranean Sea, where 65% of sharks and rays are now at risk of extinction. Understanding their biology and ecological roles is crucial for the development of effective conservation strategies. This study analysed the trophic ecology of the vulnerable blackspotted smooth-hound (Mustelus punctulatus) in three sub-areas of the northwestern Adriatic Sea, combining stomach content and stable isotope analyses. We hypothesized that feeding strategies vary in response to differen

Case Report: Metastatic duodenal hepatoid adenocarcinoma: a rare pathology with exceptional response to dual immune checkpoint blockade.

Hepatoid adenocarcinoma (HAC) is a rare, malignant, extrahepatic tumor with hepatocellular differentiation. HAC is often associated with elevated serum alpha-fetoprotein (AFP) concentrations, which makes it difficult to distinguish from primary hepatocellular carcinoma (HCC). It is a difficult diagnosis based on histopathology, as the immunohistochemistry (IHC) profile often overlaps with hepatocellular carcinoma. HAC can arise from multiple organs, including the stomach, esophagus, colon, pancreas, duodenum, and peritoneum, with the stomach being the most common site. The treatment of HAC is

Genome-Wide DNA methylation profiling of tissue-plasma pairs identifies Novel cfDNA biomarkers for the diagnosis of gastric cancer.

Screening for gastric cancer (GC) is critical for improving patient survival, yet current clinical methods suffer from invasiveness and limited sensitivity. The aim of this study is to develop a machine learning model for the non-invasive diagnosis of GC using the whole-genome methylation profile of circulating free DNA (cfDNA) in plasma. Plasma cfDNA from 61 patients with GC and 29 healthy controls. First, whole‑genome bisulfite sequencing (WGBS) was performed on 29 paired GC and adjacent normal tissues to identify differentially methylated regions (DMRs). These tissue DMRs were then mapped t

Successful Endoscopic Management of Bouveret Syndrome by Transpyloric Stone Relocation and Intragastric Mechanical Lithotripsy.

Background: Bouveret syndrome is a rare form of gallstone ileus in which a gallstone migrates through a bilioenteric fistula and impacts at the gastric outlet or proximal duodenum. It affects predominantly frail elderly women, in whom an endoscopy-first strategy is desirable; however, complete endoscopic clearance is achieved in fewer than half of cases and becomes markedly less likely as stone size increases, meaning that large stones often prompt surgery. Case Presentation: A 95-year-old woman presented with four days of vomiting and obstipation. Computed tomography demonstrated gastric outl

Third lumbar transverse process syndrome with acute abdominal pain as the main clinical manifestation: A case report.

RATIONALE: Third lumbar transverse process syndrome is an uncommon musculoskeletal cause of referred abdominal pain. Because acute abdominal pain is appropriately evaluated first for visceral, vascular, urinary, and surgical emergencies, a spinal or paraspinal pain generator may be overlooked. PATIENT CONCERNS: A 66-year-old man presented with persistent left lower-quadrant abdominal pain radiating to the left lower back. Baseline pain intensity was 7/10 on a VAS, and the pain was aggravated by postural change, standing, and walking. DIAGNOSES: Serial laboratory tests, endoscopy, ultrasonogr
