Forty-seven patients with oesophageal carcinoma were managed in 6 years' time. Average duration of illness was 5.5 months. History of chronic smoking and/or tobacco chewing was present in 80.85% of patients. Carcinoma included squamous cell variety (80.85%) and adenocarcinoma (19.15%). Cardiopulmonary complications account for half the deaths associated with upper gastrointestinal endoscopy. The incidence of hypoxia at the time of upper gastrointestinal endoscopy can be greatly reduced by the administration of supplemental oxygen via nasal cannulae. Current evidence suggests that lower oesophageal sphincter (LOS) competence depends upon both pressure and length. Existing devices can measure length and pressure, but not simultaneously. Pre-operative investigations, although providing useful information, were unreliable in predicting resectability in patients with malignant oesophageal lesions. In a prospective study of the frequency and clinical importance of heterotopic gastric mucosa in the upper oesophagus, 634 consecutive veteran patients (98% male), undergoing endoscopy for various gastrointestinal complaints, were evaluated. Primary malignant melanoma is a very rare tumour of the oesophagus, and although this is the first case reported in a black patient in South Africa, the clinical, radiographic and histopathological features were characteristic of this malignancy. A 44-year old male with carcinoma of the upper third of the oesophagus and invasion of the trachea recently underwent surgical treatment at our institution. After oesophageal resection tracheal and main bronchial leaks were unsuccessfully closed with pleural patches. The stomach was subsequently used to seal the remaining leaks. The post-operative recovery was uneventful and the patient was discharged after a hospital stay of 41 days.

A total of 19,225 malignant neoplasms were recorded in Kingston & St. Andrew, Jamaica over the 30-year period 1958-1987. The most prevalent cancers in males were those of prostate (12.2%), stomach (10.9%) and lung (10.7%). In females 24.1% of cancers occurred in breast and 20% were recorded for invasive carcinoma of the cervix. During the seven-year period from 1983 to 1990, 210 patients with gastrointestinal disorders were treated by laser at Aker Hospital. A safe and easily employed method of removal of a blunt foreign body from the oesophagus is described. The present case was a newly born infant with Pierre Robin's syndrome and congenital heart disease. A nasal tube had become displaced and the clinical condition did not permit endoscopic removal. By means of an inflatable balloon catheter introduced through the tube to the stomach, the foreign body could be removed without difficulty.
