55 patients with tumours of the gastrointestinal tract were examined by CT, using a paraffin emulsion as a negative oral contrast medium. These were compared with 119 normal patients. The appearance of normal and tumour bearing portions of the gut wall against the contrast medium was studied. Under hypotonic conditions the gut wall could regularly be distinguished from surrounding organs and gut content. At oesophagogastroscopy a web was seen in the upper oesophagus in a female of 73 years with dysphagia. Because she also had a smooth tongue, a low serum iron level and anaemia, the syndrome of Plummer-Vinson was diagnosed. After treatment with ferrous fumarate the dysphagia, the web and the anaemia disappeared and the serum iron rose. The symptomatology of this syndrome is discussed. Remarkably, the pathogenesis is not completely known. There are indications that this uncommon syndrome is a premalignant disorder.

From 1980 to 1989, 46 patients underwent surgery for pathological gastro-oesophageal reflux disease; 28 with Nissen technique, nine with Hill technique and nine with a modified Belsey technique. 11 patients (24%) experienced postoperative complications, 21% after Nissen, 33% after Hill and 22% after Belsey. Mortality rates in the USSR for the major cancer sites have been computed for the period 1986-88 from official numbers of certified deaths and population estimates provided by the World Health Organization databank, and compared with rates for 26 other European countries. The aim of our study was to assess the reproducibility of the findings of Hambraeus et al. In their group of patients with squamous cell carcinoma of the oesophagus who underwent preoperative radiotherapy and resection, the histopathological malignancy grading score system was found to predict survival. We studied nine patients with squamous cell cancer of the mid-thoracic oesophagus, who had received radiotherapy followed by surgery and survived over 12 months. We did not confirm the prognostic value of the histopathological malignancy grading score system.

Columnar lined oesophagus (Barrett's oesophagus) carries a risk for the development of adenocarcinoma. Epithelial dysplasia appears to be a precursor but the utility of this marker for predicting subsequent adenocarcinoma is unsettled. Ultrastructural changes associated with exsheathment of the infective juveniles of Haemonchus contortus are described. We report a 58-year-old man with recurrent ulceration in a Barrett's oesophagus whose iron-deficiency anaemia was in fact due to a caecal carcinoma. Recent reports suggest an association between these two disorders. A Barrett's oesophagus should not be blamed for iron deficiency unless neoplasms of the colon have been excluded.

The phenotype of 165 gastrointestinal stromal tumours was studied by immunohistochemistry. In each case the phenotype was compared to the histological diagnosis. The phenotype was muscle in 49 tumours (30%), neural in 18 (11%), histiocytic in 20 (12%) and mixed in five (3%); 68 tumours (41%) were positive for vimentin only, four tumours had no markers and one tumour was positive for keratin only. The expression of the haeme-binding protein, lactoferrin, was studied in human gastric tissues displaying normal, benign hyperplastic or malignant histology. Oesophageal problems are common in patients with scleroderma, but the association of primary biliary cirrhosis and scleroderma is uncommon. A Barrett's oesophagus identified in a patient with primary biliary cirrhosis and scleroderma is described. The Barrett's oesophagus was probably a complication of scleroderma and resulted from low lower-oesophageal sphincter pressure and severe gastro-oesophageal reflux.

Five patients with congenital broncho-oesophageal fistula confirmed by fistulectomy are presented. Clinical symptoms were chronic or recurrent pulmonary infection, and three patients experienced coughing after swallowing.
