Knowledge gaps in tubular gut tumours: a critical appraisal of the 6th edition of the World Health Organization classification of tumours.

Gastrointestinal cancer is a global health problem. In the new 6th edition of the World Health Organization Classification of Tumours (WCT) of the Digestive System, updated evidence and guidance is provided for the aetiology, pathogenesis, diagnosis, classification, grading, staging and prognosis of these tumours. However, significant knowledge gaps remain, some of which are addressed in this review. To provide for a research framework to fill these knowledge gaps, we focused on precursor lesions and rare cancers, where the need for new evidence is most urgent. Areas with a need for further re

Boerhaave Syndrome: Challenges Faced to Achieve Positive Outcomes.

Boerhaave syndrome, the spontaneous barogenic rupture of the oesophagus, is lethal when unrecognised and its cardiorespiratory mimicry frequently delays diagnosis. Optimal management remains debated. We retrospectively reviewed four consecutive patients with spontaneous oesophageal perforation managed over two years at a tertiary surgical-gastroenterology unit. Diagnosis was confirmed by water-soluble oral-contrast computed tomography; presentation, operative findings, complications and outcomes were analysed and compared with the contemporary literature. The four patients were initially asses

A Systematic Review of Post-surgical Adverse Events Related to Hem-O-Lok Clips: A Commonly Used Device With Potential Complications.

Since their introduction, Hem-o-lok clips (HOLCs) have become widely accepted and increasingly reliable devices among surgeons performing minimally invasive surgery due to their effectiveness in achieving vascular pedicle control and haemostasis. Although HOLCs are generally regarded as safe devices, various complications have been reported. This review aimed to identify and summarise complications occurring at any time following HOLC use, from their initial introduction in January 1999 through December 2025. This literature review was conducted in accordance with the PRISMA (Preferred Reporti

Prior endoscopic surveillance and diagnosis of Barrett-related dysplasia and cancer: evidence from a nationwide Dutch cohort.

BACKGROUND: In patients with Barrett oesophagus (BE), surveillance aims to detect dysplasia or oesophageal adenocarcinoma (EAC) early. Whether most asymptomatic patients undergoing endoscopic treatment are identified through surveillance remains uncertain. OBJECTIVE: To assess the contribution of surveillance to the diagnosis of dysplasia and cancer. DESIGN: Patients in the Netherlands with curative treatment for BE-related dysplasia or EAC between 2008 and 2018 were identified from the Barrett Expert Center Registry for endoscopic therapy and the Cancer Registry for surgery or chemoradiothe

Eosinophilic Oesophagitis Beyond the Oesophagus: High Prevalence of Chronic Rhinosinusitis-Type Symptoms and Their Impact on Symptom Severity in Swiss EoE Cohort Study Patients.

BACKGROUND: Both eosinophilic oesophagitis (EoE) and chronic rhinosinusitis (CRS) are type 2 inflammatory conditions sharing pathogenic mechanisms, therapeutic responses and atopic associations. Data on the prevalence of CRS in EoE patients is missing. AIMS: Our aim was to assess the prevalence of CRS-type symptoms in EoE patients, the quality of life and shared characteristics of patients with both diseases. METHODS: We assessed the point prevalence of CRS in Swiss Eosinophilic Esophagitis Cohort Study patients using a validated screening questionnaire comprising four symptom-based diagnost

Routine Preoperative Oesophagogastroduodenoscopy Before Roux-En-Y Gastric Bypass: A Retrospective Cohort Analysis From Queensland, Australia.

INTRODUCTION: The role of routine preoperative oesophagogastroduodenoscopy (POOGD) with routine biopsies prior to Roux-en-Y gastric bypass (RYGB) remains controversial. Whether POOGD should be routinely offered or reserved for symptomatic and/or at-risk patients is heavily debated. This study aimed to determine the proportion of patients undergoing RYGB in whom POOGD altered surgical management. METHODS: A retrospective review of medical records from 2016 to 2024 was performed using data from Surgery Brisbane, Queensland, Australia. Demographic data, clinical characteristics, POOGD findings,

Evolution of endoluminal treatment of Zenker's diverticulum and complete pharyngo-oesophageal obstruction.

PURPOSE OF REVIEW: Endoscopic approaches to surgery of the foregut, in particular, the upper oesophageal sphincter (UES), continue to develop, providing options for management that were previously unavailable. This review examines endoluminal surgical tunnelling approaches (third space endoscopy) to treat Zenker diverticulum and the more rare condition of complete oesophageal stenosis. RECENT FINDINGS: Endoluminal surgical approaches to the UES such as Z-POEM (per-oral endoscopic myotomy) that utilize gastroscopes and upper gastrointestinal endoscopic instrumentation, are increasingly being u

Apnoeic oxygenation with high-flow nasal oxygen in adult patients undergoing endoscopic pharyngolaryngeal surgery: a clinical review.

Endoscopic pharyngolaryngeal surgery requires access to the base of tongue, pharynx, larynx, trachea and/or upper oesophagus using a rigid or flexible endoscope for diagnostic or therapeutic surgical management of benign or malignant disease. The anaesthetist must deliver a safe and effective shared airway management strategy. This strategy is aimed at optimising the surgical view and access to pathology, whilst avoiding airway obstruction, maintaining oxygenation, facilitating carbon dioxide clearance, and minimising the risk of airway fire. Various strategies exist to achieve this depending

The natural history of paraquat-associated stomatitis: A four-stage model.

BACKGROUND: Paraquat-associated stomatitis (PAS) is a poorly characterized feature of paraquat poisoning and is presented as a binary finding. Despite its diagnostic potential, its evolution and prognostic value remain undefined. This pooled individual-patient analysis aimed to delineate the natural history of PAS and develop a structured, clinically applicable staging system. METHODS: Data from 170 published paraquat poisoning cases were aggregated at the individual-patient level. Variables included exposure-to-symptom interval, lesion morphology, topography, and survival outcome. Temporal a

Distinct oesophageal microbiota is associated with paediatric eosinophilic oesophagitis.

BACKGROUND: Eosinophilic oesophagitis (EoE) is a chronic immune-mediated disease in which oesophageal microbiota may vary according to disease activity and treatment. We aimed to characterise oesophageal microbiota in treatment-naïve children with EoE before and after proton pump inhibitor (PPI) therapy, and to assess spatial variability along the oesophagus. METHODS: In this multicentre longitudinal study, oesophageal biopsies were obtained from proximal-mid and distal oesophagus of treatment-naïve children with newly diagnosed EoE at baseline and after standardised PPI induction therapy, an

The Role of Indocyanine Green Fluorescence Angiography in Reducing Anastomotic Leaks after Esophagectomy - A Comprehensive Meta-Analysis of Comparative Studies.

Anastomotic leaks (AL) after esophagectomy contribute to significant morbidity (up to 30%) and mortality (5-10%). Indocyanine green (ICG) fluorescence angiography offers real-time perfusion assessment, potentially reducing AL compared to conventional methods. This meta-analysis evaluates ICG's efficacy in reducing AL after esophagectomy with gastric pull-up, focusing on cervical versus intrathoracic anastomoses. This systematic review and meta-analysis followed PRISMA guidelines. A comprehensive search of PubMed and Google Scholar from 2005 to the present was performed using MeSH terms to iden

HLA genotype, clinical and immunofluorescence findings of 30 patients with type VII collagen antibody positive subepidermal bullous disease.

Epidermolysis bullosa acquisita (EBA) and mucous membrane pemphigoid are mucocutaneous blistering disorders that are associated with antibodies that target type VII collagen (anti-col7). There are relatively few studies that closely investigate the clinical phenotype and HLA genotype of patients who are anti-col7 positive. We therefore characterized all patients with anti-col7-positive subepidermal bullous disease that were treated at our centre. In total, 30 patients were identified, 29 had mechanobullous skin lesions and all had oral ulceration. The most common sites for oral lesions were th

Pyloric stenosis following hydrochloric acid-based bathroom cleaner ingestion in a toddler.

Accidental ingestion of corrosive household substances can lead to serious injuries in young children. While damage to the oesophagus is most common, injuries affecting the stomach, especially the pylorus, are rare and can be easily missed. We describe a toddler who developed persistent vomiting after swallowing a hydrochloric acid-based bathroom cleaner. Investigations revealed complete blockage at the gastric outlet due to scarring. She was successfully treated with surgery. This case highlights the importance of early recognition and timely intervention, especially when the child continues

Ectopic ACTH production from oesophageal neuroendocrine carcinoma causing Cushing syndrome.

Ectopic Cushing syndrome (ECS) occurs when a non-pituitary tumour secretes excess adrenocorticotropic hormone, leading to hypercortisolism. ECS is most commonly associated with small cell lung cancer and pancreatic neuroendocrine tumours, but rarely with oesophageal neuroendocrine carcinoma (NEC). We report the case of a man in the 60s initially treated for locally advanced large cell NEC of the oesophagus who later developed ECS. Oesophageal NECs account for less than 0.04% of all neuroendocrine tumours and carry a poor prognosis, with large cell subtype being particularly uncommon. Despite e

Complete Communicating Tubular Oesophageal Duplication in an Adolescent Presenting With Haematemesis and Long-Standing Dysphagia.

Complete communicating tubular oesophageal duplication is a rare congenital foregut anomaly that may remain undiagnosed until adolescence because symptoms can be intermittent. A 17-year-old boy with long-standing intermittent solid-food dysphagia presented with moderate haematemesis. Upper gastrointestinal endoscopy identified an ulcerated inflammatory stricture 26 cm from the incisors, an accessory orifice, and a diverticulum-like opening. Biopsies showed ulcerative and regenerative changes with glandular and intestinal metaplasia without dysplasia. Barium oesophagography and cervicothoracoab

First record of Macrobilharzia (Digenea: Schistosomatidae) from Phalacrocorax lucidus in South Africa, with molecular evidence suggesting a distinct species.

Schistosomatid flukes are mainly found in the vascular system of their definitive hosts. Avian schistosomatids, represented by 13 genera, are blood parasites affecting snails and aquatic birds in their larval and adult stages, respectively, with the genus Macrobilharzia Travassos, 1922 being a poorly documented clade, with limited records globally. During a parasitological investigation, adult schistosomatids (prevalence = 57%) were recovered from the intestinal mesenteric veins of white-breasted cormorants (Phalacrocorax lucidus; n = 14), sampled during July 2024 and March 2025 at Loskop Dam,

Hepatic Manifestations and Response to Treatment in Deficiency of Adenosine Deaminase 2.

BACKGROUND AND AIMS: Deficiency of adenosine deaminase 2 (DADA2) is caused by mutations in the ADA2 gene and results in an auto-inflammatory state. Hepatosplenomegaly, with or without abnormalities in liver enzymes, has been reported in DADA2. Anti-tumour necrosis factor (anti-TNF) therapy is the standard of care for the prevention of recurrent ischemic strokes and reduction of inflammatory burden. However, little is known about the extent of hepatic involvement and the utility of non-invasive tools for identifying liver disease and monitoring treatment response. METHODS: Retrospective analys

Carinal tracheo-oesophageal fistula repair using adjacent gastric conduit wall after Ivor-Lewis oesophagectomy: a case report.

BACKGROUND: Neo-oesophagus–airway fistula after transthoracic oesophagectomy is rare but life-threatening. Standard therapeutic options, including stenting, primary repair with muscle or omental interposition, and diversion with delayed reconstruction, can be contraindicated in the context of sepsis and significant mediastinal contamination. We describe an emergency, staged approach employing the adjacent viable gastric conduit wall as a local flap to achieve closure of a carinal fistula in a hemodynamically unstable patient.
