Pulmonary Mass as the First Clinical Manifestation of Histiocytic Sarcoma with Multi-Organ Involvement: A Case Report and Review of the Literature.

Histiocytic sarcoma is a malignant neoplasm with cells showing morphologic and immunohistochemical features of histiocytes. The lung and stomach are rare sites of involvement, and few have been reported in the existing literature. We report a 72-year-old woman with primary histiocytic sarcoma involving the right lung, stomach, and bilateral adrenal glands. Computed tomography revealed a large heterogeneous mass in the upper lobe of the right lung, regional lymphadenopathy, and masses within the bilateral adrenal glands. Gastric involvement was discovered via polypectomy during a routine gastri

Beta-Klotho Protein Expression in Healthy Human Tissues and Liver Biopsies From Patients With MASLD or MASH.

BACKGROUND AND AIMS: Biologics based on the structure of fibroblast growth factor (FGF) 19 and 21 show strong beneficial effects in the treatment of metabolic dysfunction-associated steatotic liver disease (MASLD), including compensated cirrhosis. Studies in rodents indicated that the effectiveness of these drugs relies on the presence of transmembrane protein beta-klotho (KLB). However, the tissue expression profile of KLB and its regulation in liver disease remain poorly characterized. Here, we aim to investigate KLB protein expression in healthy human tissues and liver biopsies from patient

[A Case of Toxic Epidermal Necrolysis during Chemotherapy for Advanced Gastric Cancer].

A 52-year-old man was diagnosed with unresectable advanced gastric cancer, staged as cT4a(SE), N+, M1(H0, P0, CY1), Stage ⅣB. First-line chemotherapy with SOX plus nivolumab was initiated. Nivolumab was added from the second course. Six days after the first nivolumab dose, the patient developed a pruritic skin rash, diagnosed as Grade 2 drug eruption. Chemotherapy was continued with topical steroids and antihistamines. However, at the start of the 3 course, the rash worsened and was diagnosed as toxic epidermal necrolysis(TEN), prompting emergency hospitalization under dermatology care. At adm

Long-Term Follow-Up of a Complicated Rupture of Multiorgan Hydatid Cysts: A Case Report.

Hydatid disease (echinococcosis) remains a serious health problem in underdeveloped countries. Although the liver is the primary site of infection, other organs may also be involved. Multifocal involvement of multiple abdominal organs is rare but can occur. Spontaneous cyst rupture constitutes a critical surgical emergency with significant mortality. We discussed a case managed surgically for perforated hydatid disease. A two-year follow-up of a giant complicated cyst involving the liver, stomach, and spleen was presented.

Artificial Intelligence for Spleen-Stomach Disorders in Traditional Chinese Medicine: Integrating Knowledge Graphs with Intelligent Diagnosis and Treatment.

Spleen-Stomach disorders are prevalent clinical conditions in Traditional Chinese Medicine (TCM). The complex diagnostic and treatment model used in TCM is based on a "symptom-pattern-disease-formula" framework that heavily relies on practitioners' experience. However, this model faces several challenges, including ambiguous knowledge representation, unstructured data, and difficulties with knowledge sharing. Recent advancements in artificial intelligence, natural language processing, and medical knowledge engineering have significantly improved research on knowledge graphs (KGs) and intellige

Oral transmission of rock bream iridovirus (RBIV) drives delayed systemic infection and organ specific immune reprogramming in rock bream (Oplegnathus fasciatus).

Rock bream iridovirus (RBIV) is a major threat in marine fish aquaculture, but the role of oral transmission in disease progression remains poorly understood. Here, we compared intraperitoneal (i.p.) and oral infection routes in rock bream (Oplegnathus fasciatus) to evaluate viral replication, pathology, and immune responses. Fish infected via the i.p. route showed rapid systemic spread and 100 % mortality within 17-35 days post infection (dpi). In contrast, oral infection caused delayed mortality, occurring 15-20 days later, reflecting the additional mucosal barriers encountered during viral

Successful surgical removal of a retroperitoneal paraganglioma in the celiac artery trifurcation in a dog.

BACKGROUND: Paraganglioma (PGL) is a general term for tumors that originate in the paraganglia in dogs, most commonly reported in the carotid and aortic bodies. Reports on surgical treatment are rare because these tumors develop near large blood vessels, and their prognosis remains unclear. In dogs, the indications for and safety of surgical procedures involving the celiac artery (CA) root and the dissection of its major branches have not been established. To the best of our knowledge, this is the first reported case of CA root involvement in canine PGL. CASE PRESENTATION: Surgery was perform

Jejuno-jejunal intussusception in an adult due to inflammatory fibroid polyp: Tale of two unusual cases.

Intussusception in adults is rare, accounting for 1-2% of intestinal obstruction, often caused by neoplasia. Jejuno-jejunal intussusception is especially uncommon and usually linked to benign lesions. Symptoms are vague, including colicky abdominal pain, vomiting and gastrointestinal bleeding, often delaying diagnosis until exploratory laparotomy. We present two cases of adult jejuno-jejunal intussusception caused by inflammatory fibroid polyps (IFPs), rare benign submucosal tumours mostly found in the stomach and rarely (4.3%) in the jejunum. The first case involved a 17-year old male with ab

Ocular Insights: Exploring Uveitis as a Manifestation of Celiac Disease.

Celiac disease (CD) is an autoimmune condition that mainly affects the small intestine and often leads to issues with nutrient absorption. People with CD commonly experience symptoms like diarrhea, bloating, stomach pain, fatty stools, unintended weight loss, and low iron levels. In addition to its intestinal manifestations, CD is also associated with a range of extraintestinal features and comorbid conditions. Although uveitis is a rare manifestation of CD, we report, to our knowledge, the first documented case from Pakistan of posterior uveitis secondary to CD in an 11-year-old girl. The pat

Dachengqi decoction for the treatment of acute pancreatitis: a comprehensive analysis based on metabolites, pharmacokinetics, and metabolites efficacy mechanisms.

Dachengqi decoction (DCQD) is a traditional Chinese medicine formula consisting of three botanical drugs and one mineral drug. It has anti-inflammatory, gastrointestinal motility, and microcirculation improvement effects. A large number of studies have shown that DCQD has a significant impact on the treatment of acute pancreatitis (AP). However, owing to the multi-component and multi-target characteristics of traditional Chinese medicine, it is difficult for modern scientists to understand the role of DCQD in treating AP. Therefore, We sorted out the literature data related to DCQD published i

Spontaneous Isolated Symptomatic Celiac Artery Dissection: A Case Report.

The celiac artery, also known as the celiac trunk, originates from the abdominal aorta at the level of the 12th thoracic vertebra. Arising from the anterior aorta, this ultrashort artery plays an integral role in supplying blood to the stomach, liver, spleen, pancreas, and proximal duodenum. Arterial dissection is a life-threatening emergency most frequently reported in the aorta, while visceral arterial dissections remain uncommon or may be undetected. A dissection involves a tear in the artery wall, which can lead to serious complications, including compromised arterial supply, end-organ isc

Unexpected finding of cholecystogastric fistula in a patient undergoing laparoscopic cholecystectomy: A case report.

BACKGROUND: Gallbladder stones are a common occurrence, with a prevalence of approximately 10% in the Pakistani population. A rare but potentially fatal complication of gallstones is cholecystogastric fistulas. The underlying mechanism involves chronic inflammation due to cholelithiasis, causing gradual erosion and eventually leading to fistula formation. CASE SUMMARY: We present a rare case of a cholecystogastric fistula in a 40-year-old female patient, successfully managed with an open surgical approach. The patient initially presented with a 6-month history of intermittent epigastric pain,

Bouveret syndrome in a young patient: A case report and review of literature.

BACKGROUND: Bouveret syndrome is a rare cause of gastric outlet obstruction resulting from the passage of a gallstone into the duodenum or stomach through a biliary-enteric fistula. It is a complication of chronic cholelithiasis and accounts for only 1%-3% of all gallstone-related intestinal obstructions. CASE SUMMARY: A 49-year-old male presented with nausea, vomiting, and upper abdominal pain. Imaging revealed a large gastric stone and a cholecystoduodenal fistula, confirming Bouveret syndrome. An initial endoscopic attempt to remove the stone was unsuccessful, necessitating surgical interv

The persistent risk of secondary malignancies in gastric neuroendocrine tumor survivors: a population-based analysis.

Gastric neuroendocrine tumors (G-NETs) are rare neoplasms with a favorable survival rate, yet they present a significant risk for second primary malignancies (SPMs). This study aims to estimate the relative risks of SPMs in G-NET survivors, exploring variations across key patient characteristics. Patients diagnosed with G-NETs were identified from the Surveillance, Epidemiology, and End Results database (2000-2021). Standardized incidence ratios (SIRs) and excess absolute risks (EARs) were calculated to assess SPM risk stratified by age at diagnosis, gender, race, latency period, marital statu

Cytological Assessment of Metastatic Gastrointestinal Stromal Tumor in Ascitic Fluid.

Gastrointestinal stromal tumors (GISTs) are mesenchymal neoplasms typically arising in the stomach and small intestine and rarely involving extra-gastrointestinal organs. While fine-needle aspiration (FNA) offers reliable diagnosis, exfoliation of GIST cells into body fluids is exceedingly rare and diagnostically challenging. We report a case of a 69-year-old male with metastatic GIST in ascitic fluid. The cells revealed atypical epithelioid morphology with high nuclear-to-cytoplasmic ratio and nuclear irregularities. Immunohistochemistry (IHC) demonstrated positivity for CD117 and DOG1. Diagn
