[Simultaneous VMAT for Multiple Hepatic Metastases Resulting in Disease Control in a Super-Elderly Patient with Transverse Colon Cancer-A Case Report].

A man in his 90s with a history of gastrectomy for gastric cancer was diagnosed in December of year X with a circumferential transverse colon carcinoma causing endoscopic obstruction. He underwent laparoscopic partial resection the following month. Pathology revealed a 65×35 mm tumor(tub1-pap>tub2), corresponding to pStage Ⅱa. His carcinoembryonic antigen(CEA)dropped from 11.0 ng/mL preoperatively to 2.9 ng/mL postoperatively. By August of the same year, CEA levels began to rise again. Magnetic resonance imaging detected three hepatic metastases. A 10 mm lesion in segments 5 lay adjacent to th

Polysaccharides of Atractylodes Macrocephala Koidz effectively improve the EpCAM+/- genotype-related inflammatory bowel diseases via modulating gut microbiota.

The diminished EpCAM increases the susceptibility to inflammatory bowel disease (IBD). PAMK (polysaccharides of Atractylodes macrocephala Koidz) demonstrates the promotion for the intestinal health. To study the efficacy and mechanism of PAMK on mitigating IBD related to the downregulated EpCAM, it was administrated to the dextran sulfate sodium (DSS)-treated both WT and EpCAM+/- mice. DSS caused more pronounced damage to the mucosal muscular layer in the EpCAM+/- mice, increased the activity of MAPKs and upregulated the MMPs and inflammatory cytokines, but downregulated the tight junction-rel

[A Case of Transverse Colon Cancer with Pancreatic Head and Gastric Invasion Treated with Radical Surgery after Preoperative Chemotherapy].

The patient was a 67-year-old woman who presented to our hospital with abdominal pain. She was diagnosed with transverse colon cancer with obstructive colitis, pancreatic head, and gastric invasion(cT4bN1bM0, cStage ⅢC). After the placement of a colonic stent to relieve the obstruction, 5 courses of preoperative chemotherapy(FOLFOXIRI+bevacizumab) were administered for local control. Tumor shrinking was observed, and the response was judged to be a partial response. Following a rest period, radical surgery(extended right hemicolectomy and pancreaticoduodenectomy)was performed. Pathological fin

Pelvic Sepsis Secondary to Necrotic Coloanal Anastomosis After Perineal Proctosigmoidectomy Leading to Emergent Abdominoperineal Resection: A Rare Case and Complication.

Abdominoperineal resection (APR) is a surgical procedure that involves the complete removal of the anal canal, anal sphincter complex, rectum, and a portion of the sigmoid colon with creation of an end colostomy. This procedure is primarily indicated for patients with low rectal cancers where the anal sphincters cannot be spared and anal cancers refractory to chemoradiation or recurrent anal cancers. It can also be performed in patients with severe perianal Crohn's disease, complex anorectal fistulae, or patients with severe trauma to the anus/rectum where the sphincter complex is significantl

The saline-immersion/irrigation technique for endoscopic submucosal dissection of a challenging colonic lesion using double-balloon endoscopy and a novel type of advanced imaging.

BACKGROUND AND AIMS: Endoscopic submucosal dissection (ESD) offers effective en bloc resection of large colorectal lesions but can be technically challenging, especially for lesions with poor access. This video showcases successful use of the saline-immersion/irrigation technique (SITE), double-balloon endoscopy (DBE), and a novel type of advanced imaging for resection of such a lesion in a patient with a long sigmoid colon that compromised stability. METHODS: A 64-year-old woman underwent ESD for an 80-mm Paris 0-Is lesion within the transverse colon. DBE was used to enhance access and stabi

In Vitro Investigation of Equine Gut Microbiota Alterations During Hypoglycin A Exposure.

Hypoglycin A is a plant-derived protoxin that causes atypical myopathy in equids. In atypical myopathy-affected horses, metabolomic and microbiome studies have reported alterations in metabolic markers and faecal microbiota composition, pointing to a potential disruption of microbial homeostasis. However, in vivo observations are strongly confounded by host-related factors, underscoring the need for controlled in vitro approaches. To address this, we used an in vitro static batch fermentation model simulating the equine colon to investigate the direct effects of hypoglycin A on microbiota comp

Predicting Alzheimer's Disease Diagnosis, a Decade or more Years before Onset using the Electronic Health Record and Random Forest Machine Learning Models.

INTRODUCTION: There is need to detect and intervene in pre-clinical phases of Alzheimer's disease (AD). Electronic health records (EHRs) may help predict AD using machine learning methods. METHODS: We identified EHRs for 19,473 cases with AD and 111,922 controls. Records spanned 10 or more years prior to AD diagnosis. We trained a random forest model (employing 5-fold cross-validation with 2,499 features) to predict AD 10 years prior to its onset using a 75/25% train/test split and then computed permuted feature importance. RESULTS: We achieved an area under the ROC curve of 0.80. Feature im

Minimally Invasive Repair Options for Symptomatic Larrey Hernia in an Adult: A Case Report and Literature Review.

Morgagni-Larrey hernias (MLH) are rare congenital anterior diaphragmatic defects, and Larrey hernias are a less common variant that typically occurs on the left side. Their rarity and often asymptomatic nature in adults make diagnosis and treatment challenging. A 44-year-old woman experienced intermittent left upper quadrant abdominal pain for one year, with associated nausea and vomiting. Her past medical history included a laparoscopic cholecystectomy and a right inguinal hernia repair. Imaging confirmed a left-sided anterior diaphragmatic defect consistent with a Larrey hernia. Laparoscopic

Tocilizumab in immune checkpoint inhibitor-induced myositis, myocarditis, and myasthenic syndrome: a rare case report and review of the literature.

Immune-related adverse events (irAEs) associated with immune checkpoint inhibitors (ICI) are becoming more frequent with the growing use of these agents in routine clinical oncology. This mechanism enhances the anti-tumor immune response, increasing the risk of activating autoreactive T cells, and leading to immune-mediated manifestations. The spectrum of these manifestations is widely variable. ICI-associated myositis is among the most severe irAEs, being a rare but serious complication with significant morbidity and mortality, particularly when accompanied by myocarditis and/or myasthenic cr

The Gut Microbiome as a Modulator of Traumatic Brain Injury Pathology and Symptoms.

Traumatic brain injuries (TBIs) affect millions annually, leading to devastating neurobehavioral consequences and increasing risk for neurodegenerative and psychiatric diseases. However, therapies are lacking. Starting in 2018, dysbiosis of the gut microbiome was identified as an acute, and potentially chronic, pathology originating from TBI. Recent studies established that the microbiome contributes to the evolution of TBI pathology and functional impairments. The gut microbiome is the collection of microorganisms that inhabit the stomach through colon and is indicated as a contributor to myr

Safe endoscopic resection of a giant pedunculated colonic polyp after epinephrine volume reduction with clip placement.

BACKGROUND AND AIMS: Resection of giant pedunculated colorectal polyps can be challenging because of adverse events such as bleeding and technical difficulties in snaring. Epinephrine injection may reduce tumor size, but its efficacy for giant lesions is limited. METHODS: A 59-year-old woman presented with a 50-mm pedunculated polyp in the transverse colon. A total of 20 mL of diluted epinephrine (1:10,000) was injected into the apex and stalk of the polyp, and 6 hemostatic clips were placed near the stalk base. Three weeks later, a follow-up colonoscopy was performed. RESULTS: After a 3-wee

Metal Bezoar-Induced Gastrointestinal Obstruction: A Rare Clinical Presentation.

Bezoars are accumulations of indigestible or non-food material within the gastrointestinal tract, capable of causing obstruction, bleeding, or perforation. We report the case of a 25-year-old male patient with a history of chronic polysubstance abuse who presented with a one-month history of persistent vomiting, severe epigastric pain, and minimal tolerance to liquids. Abdominal radiography and computed tomography revealed metallic foreign bodies in the stomach and colon. Endoscopic removal was deemed unfeasible due to the size and characteristics of the objects; therefore, open surgery was pe

Expansion of the phenotype in ZFX neurodevelopmental disorder in a family.

With new and increasingly sensitive techniques for genetic testing, genes that are newly related to a phenotype or disease are still identified, warranting for adequate phenotyping. Recently, 11 variants in the ZFX gene were reported to cause a distinct X-linked neurodevelopmental disorder in 18 male patients, and a female was reported with hyperparathyroidism in concurrence with a ZFX variant. In this report, we present a male patient and his mother with a new likely pathogenic variant in the ZFX gene (NM_003410.4(ZFX): c.2363C > G, p. Pro788Arg). The phenotype of the patient includes a globa

Oral nutritional management in short bowel syndrome.

Patients with short bowel syndrome (SBS) have less than 200 cm of functional small intestine resulting in significantly impaired digestion and absorption of nutrients and fluids. When malabsorption becomes severe and cannot be compensated by oral intake, patients develop intestinal failure (IF) and require parenteral support (PS) to survive. Over time, however, intestinal adaptation may improve absorption and reduce the need for PS. Oral nutrition plays a central role in the management of SBS and can positively influence the clinical outcomes. This review synthesizes current knowledge on the r

Long-Standing, Giant, Right-Sided Inguinal Hernia Containing the Right Colon and Greater Omentum: A Case Report.

Giant inguinal hernias are rare, long-standing conditions characterized by massive herniation of intra-abdominal organs into the scrotum. Their chronicity, unusual contents, and associated comorbidities make diagnosis and surgical management particularly challenging. We report the case of a 63-year-old man with a right-sided inguinal hernia present for 39 years. His medical history included hypertension, ischemic heart disease, hypercholesterolemia, and diabetes mellitus. Preoperative assessment included recommendations for respiratory rehabilitation to optimize pulmonary function. During surg
