An unusual colo-ovarian fistula of advanced colorectal carcinoma: A case report and literature review.

In colorectal cancer, the invasion of the colon is not unusual, but the perforation of all layers of the colon or rectum and the presentation of the patient with generalized peritonitis is rare. Colo-ovarian fistulas have as their starting point either in the sigmoid colon, rectum, appendix or, more frequently, as a benign or malignant ovarian tumor. The present study reports an extremely rare case of colorectal cancer with perforation at the rectosigmoid junction into a metastatic ovarian tumor. Intraoperatively, a colo-ovarian fistula with diffuse peritonitis was diagnosed. The treatment con

Increasing Neoadjuvant Radiation for Rectosigmoid Cancer: An Unintended Consequence of Modern Rectal Cancer Care?

BACKGROUND: Rectal cancer outcomes have improved with advances, including neoadjuvant radiation (NART), yet its utility for rectosigmoid cancer (RSJC) remains uncertain. We hypothesized that multimodality rectal cancer care has led to increased use of NART in rectosigmoid cancer (RSJC). Our objectives were to 1) assess the use of NART over time in RSJC and 2) identify factors associated with its use. METHODS: The National Cancer Database was reviewed (2004-2022) for adults with clinical Stage II-III rectosigmoid cancer who underwent surgery. The use of NART in these patients was examined over

Spontaneous Transanal Small Bowel Evisceration with Distinct CT Findings: A Case Report.

INTRODUCTION: Transanal small bowel evisceration is an extremely rare and life-threatening surgical emergency that primarily occurs in debilitated elderly patients. Preoperative computed tomography (CT) can be useful for identifying the viability of eviscerated small bowel and other intra-abdominal pathologies. CASE PRESENTATION: In this study, we report the case of an 81-year-old woman who presented with sudden anal protrusion of small bowel loops. Computed tomography (CT) demonstrated a rectal wall defect, pneumoperitoneum, and herniation of the small bowel with features suggestive of stran

Rectosigmoid obstruction after a uterosacral ligament suspension.

A woman in her 40s presented on post-operative day 2 following vaginal hysterectomy and uterosacral ligament suspension (USLS) with severe abdominal pain, distention and emesis. A distended colon was seen on CT, and rectal contrast confirmed complete obstruction at the rectosigmoid junction. She was taken urgently to the OR by the general surgery, urogynecology and reconstructive pelvic surgery teams for flexible sigmoidoscopy and diagnostic laparoscopy, where the right-sided uterosacral stitches were found to obstruct the colon. The obstruction resolved after releasing the right uterosacral s

McKittrick-Wheelock Syndrome Caused by Giant Tubulovillous Adenomas With Malignant Transformation: A Case Report.

Giant villous adenomas of the colon are a rare cause of chronic secretory diarrhea. These lesions can produce excessive mucus, fluid, and electrolyte loss, leading to life-threatening hypokalemia, hyponatremia, and acute kidney injury. Chronic secretory diarrhea with electrolyte imbalance and possible acute kidney injury caused by secretory giant tubulovillous adenomas constitutes McKittrick-Wheelock syndrome, a rare condition described in 257 patients worldwide. We report two cases of McKittrick-Wheelock syndrome caused by giant tubulovillous adenomas of the rectum and rectosigmoid junction w

Barotrauma-induced Rectosigmoid Perforation Following Accidental High-pressure Compressed Air Insufflation: A Rare Case Report.

Colorectal barotrauma due to accidental high-pressure compressed air insufflation is rare. Prompt recognition and intervention are critical. In this report, we present a case of accidental anorectal insufflation of compressed air leading to hollow viscus perforation. Radiographs and computed tomography imaging demonstrated massive pneumoperitoneum, pneumomediastinum, dilated small bowel loops, and diffuse sigmoid wall edema. Emergency laparotomy revealed a 3 cm × 4 cm perforation at the rectosigmoid junction. A Hartmann's procedure was performed with thorough peritoneal lavage and pelvic drain

Primary Rectal Burkitt Lymphoma in a Child: A Case Report and a Review of the Literature.

INTRODUCTION: Primary colorectal lymphomas are exceptionally rare in the pediatric population, with Burkitt lymphoma representing an even rarer subset. The nonspecific symptoms can often mimic benign conditions, leading to delays in diagnosis. We report a case of primary rectal Burkitt lymphoma in a child who presented with isolated lower gastrointestinal bleeding. CASE PRESENTATION: A 10-year-old previously healthy boy presented with a 3-week history of painless, bright red blood per rectum. He had no systemic B symptoms, and his physical examination was unremarkable except for fresh blood o

Contribution of the sphincter of O'Beirne in the pathophysiology of slow transit constipation as shown by low resolution manometry.

BACKGROUND: Key features of the pathophysiology of slow transit constipation are known. Colonic motility is markedly reduced, related to increased sympathetic activity, and retrograde propulsive activity is present, originating from the rectosigmoid junction (the sphincter of O'Beirne) and rectum. Furthermore, the sphincter of O'Beirne does not persistently relax when propulsive motor activity descends upon it, creating a barrier for stool transit into the rectum. METHODS: Our aim was to assess whether low-resolution manometry can provide information on the role of the sphincter of O'Beirne i

When Hygiene Turns Hazardous: An Unusual Case of Rectosigmoid Impaction by a Health Faucet Requiring Laparotomy-Assisted Extraction.

Rectal foreign bodies (RFBs) represent an uncommon but challenging surgical emergency. While most cases result from intentional insertion for sexual gratification, accidental insertion is exceedingly rare. Management depends on the size, shape, location, and associated complications of the foreign body. We present a rare case of patient-reported rectal insertion of a health faucet nozzle resulting in rectosigmoid impaction that required laparotomy-assisted transanal extraction. A 40-year-old male presented with anorectal pain, scant bleeding per rectum, mucoid discharge, and lower abdominal pa

When Size Matters: Surgical Challenges of Giant Rectal Foreign Bodies.

Rectal foreign bodies are an uncommon but increasingly recognized presentation in emergency colorectal surgery. Object size - particularly transverse diameter - and rural, low-resource settings add complexity to both diagnosis and treatment. A 57-year-old man from a rural area of southern Colombia was referred to a regional hospital for a retained lithic rectal foreign body (a stone). Plain abdominal radiography showed a cone-shaped radiopaque object. After multiple failed transanal extraction attempts, exploratory laparotomy was performed. A conical stone approximately 15 cm in diameter at it

Anatomical Location Is Associated with Clinicopathological Features and Long-Term Oncological Outcomes in Mucinous Colorectal Adenocarcinoma: A Population-Based Analysis of 40,698 Patients from the SEER Database.

Background: Mucinous adenocarcinoma (MAC) of the colorectum is a biologically distinct histological subtype whose prognostic significance may vary substantially according to primary tumor location. The impact of anatomical site on clinicopathological characteristics and long-term survival in MAC remains incompletely characterized. This study aimed to evaluate the influence of tumor location on oncological outcomes in patients with mucinous colorectal cancer using a large population-based dataset. Methods: Patients diagnosed with mucinous colorectal adenocarcinoma between 2000 and 2023 were ide

Effects of time-restricted feeding and high-intensity interval training on thermogenesis-related protein abundance in white adipose tissue of diabetic rats.

Many new mothers who need antidepressant or mood-stabilising drug treatment may wish to breastfeed their infants, but are hesitant to do so because of possible harmful effects of the medication on the infant. This article reviews current data on drug excretion into breast milk and the effects on the breast-fed child, and provides recommendations for the use of the different psychotropic drugs in lactating women. Relevant literature was identified through systematic searches of MEDLINE, EMBASE and the Science Citation Index Expanded (ISI) from 1966 to February 2005. The present knowledge is bas

The Association of KRAS and P53 Gene Mutations and MDM2 Expression with the Occurrence of Colorectal Cancer.

BACKGROUND: Colorectal cancer (CRC) encompasses various cancers located in the rectosigmoid junction, rectum, and anus, as well as parts of the colon. Globally, CRC is the second leading cause of cancer-related mortality and the third most prevalent malignancy. The KRAS oncogene was found to be mutated in 30 to 50% of CRC cases, leading to dysregulated cellular functions. Furthermore, the connection between KRAS and P53 gene mutations and the incidence of both colorectal and breast cancer remains an area of interest. METHODS: This comprehensive narrative review was carried out by mining data

[MRI-based prognostic factors predicting surgical complexity in rectal cancer].

BACKGROUND: Colorectal cancer is one of the most prevalent oncological diseases, with rectal cancer accounting for a substantial proportion of cases. Surgical intervention remains the cornerstone of treatment, with its success significantly influenced by individual pelvic anatomy. Nevertheless, there is a notable scarcity of studies specifically focused on evaluating surgical complexity based on pelvic MRI pelvimetry. OBJECTIVE: To identify the prognostic factors of pelvic MRI pelviometry that affect the complexity of surgical treatment of rectal tumors. MATERIAL AND METHODS: A retrospective

Technique and results of novel intracorporeal "overlap" colorectal anastomosis for laparoscopic and robotic surgery.

BACKGROUND: Laparoscopic colorectal procedures are widely adopted for colorectal cancer surgery. The double-stapling technique for circular colorectal anastomosis carries leakage rates of 11.2-13.4% and elevates stricture risk. In 2024, we introduced an innovative intracorporeal linear isoperistaltic "overlap" colorectal anastomosis; preliminary data from ten patients confirmed its safety and feasibility. This study aims to assess the feasibility and safety of the colorectal "overlap" anastomosis technique for laparoscopic and robotic surgery on a larger group of patients. METHODS: An observa
