Spontaneous Bowel Degloving and Evisceration Through Uterine Perforation Due to Unsafe Abortion: A Case Report.

Induced abortion is a common surgical procedure worldwide, regardless of whether it is performed for therapeutic or elective purposes. It contributes significantly to maternal mortality and morbidity. Uterine perforation is one of the major consequences that can occur while performing the procedure under unsafe conditions by unqualified personnel. It becomes a surgical emergency when bowel prolapses through the vaginal introitus. We present a case of a 35-year-old lady with bowel without its mesentery prolapsing through her vagina following a procedure of suction and evacuation for incomplete

Meat Consumption in Relation to Colorectal Cancer Incidence in Anatomical Subsites in the National Institutes of Health-AARP Diet and Health Study.

BACKGROUND: In the literature, the consumption of red and processed meat is associated with colorectal cancer (CRC) incidence. It still remains unclear whether the associations of different types of meat intake with CRC vary by subsite of the colorectum. OBJECTIVES: We investigated the association of red, white, and processed meat intakes with CRC incidence across anatomical subsites of the colon and rectum. METHODS: We analyzed data from 454,505 cancer-free adults in the National Institutes of Health-AARP Diet and Health Study cohort. During 6,896,619 person-years of follow-up, 9,461 CRC ca

Appendiceal endometriosis: an uncommon manifestation of deep infiltrating endometriosis.

Appendiceal endometriosis is a rare cause of acute appendicitis, accounting for approximately 1% of cases. This report describes a case of a female patient presenting with right iliac fossa pain and clinical signs of acute abdomen, who underwent emergency appendectomy. Histopathological analysis confirmed acute appendicitis secondary to endometriosis, establishing a direct pathological link. Subsequent imaging with transvaginal ultrasonography (TVUS) and MRI revealed multifocal deep infiltrating endometriosis (DIE) involving the rectosigmoid junction, retrocervical region and rectovaginal septu

Marine predator strikes rectally inserted eel fish causing colonic perforation: a rare case report and review of literature.

INTRODUCTION AND IMPORTANCE: Gastrointestinal perforation is a very common surgical emergency with significant mortality and morbidity, which needs urgent intervention. Foreign body-related perforations are very rare, accounting for less than 1% of cases. However, Colonic perforation by Eel fish is an extremely rare phenomenon; only three cases have been reported in the existing literature. Here, we report a rare case of traumatic colonic perforation with septic shock by a rectally inserted Eel fish. CASE PRESENTATION: A 25-year-old male presented to the emergency department with a 4-day hist

Perforated sigmoid colon diverticulitis initially presenting with pneumoperitoneum, pneumoretroperitoneum, and pneumomediastinum: A case report.

BACKGROUND: Complications occur in approximately 12% of cases of diverticulitis, with perforation occurring in up to 10% of complications. Typically, patient with perforated diverticulitis present intraperitoneally with abdominal pain and peritoneal signs. By contrast, pneumoretroperitoneum and pneumomediastinum are rare complications and lack typical symptoms, making their diagnosis difficult and often delayed, leading to increased morbidity and mortality. CASE SUMMARY: A 66-year-old man presented with lower abdominal pain for 3 days. On examination, his vital signs were stable, and the abdo

Double Trouble: A Rare Clinical Presentation of Bladder and Umbilical Endometriosis.

Endometriosis is a common condition among women where tissue resembling the uterine lining grows outside the uterus. This misplaced tissue can develop in several areas, including the peritoneum, pouch of Douglas, vagina, uterosacral ligaments, rectovaginal septum, rectosigmoid junction, bladder, and umbilicus. Although endometriosis most often affects the pelvic organs, cases involving the bladder are relatively rare. The exact cause of bladder endometriosis remains uncertain, and when it occurs alongside umbilical endometriosis (Villar's nodule), it is considered an infrequent clinical presen

Set-up and use of the "Asclepius tube": a slim drainage tube to facilitate saline-immersion endoscopic submucosal dissection.

BACKGROUND AND AIMS: Endoscopic submucosal dissection (ESD) is recognized as the reference standard for en bloc resection of large colorectal and gastric lesions, but conventional carbon dioxide-insufflation ESD presents challenges. The saline-immersion/irrigation technique (SITE) offers multiple advantages, especially when combined with a drainage tube. The video showcases the set-up and practical use of the "Asclepius tube" to optimize SITE-ESD, maintaining its advantages, allowing for a more efficient dissection, keeping an adequate balance of fluid and improving patient comfort. METHODS:

Appendiceal malakoplakia: a review of the literature and case presentation.

Background∕Objectives: Malakoplakia is an uncommon clinicopathological entity defined by the accumulation of dysfunctional macrophages containing undigested bacterial remnants and pathognomonic Michaelis-Gutmann bodies. Although it most frequently involves the gastrointestinal tract, particularly the colon, rectum, and rectosigmoid junction, appendiceal involvement remains rare. This case represents only the fourth reported occurrence of appendiceal malakoplakia in a young, immunocompetent adult without notable comorbidities, underscoring its exceptional clinical presentation. The junction between the midgut and hindgut co-localizes with the rectosigmoid junction.

Textbooks locate the junction between the midgut and hindgut where the vascular beds of the superior (SMA) and inferior (IMA) mesenteric arteries meet. In a previous study, we observed that the formation of the midgut corresponded with a pronounced thinning of its dorsal mesentery. We re-investigated, therefore, the location of the distal boundary of the midgut, making use of 3D reconstructions of serial sections of 36 human embryos between 4 and 13 weeks of development. Using the boundaries of the thin mesentery of the midgut as a criterion, the midgut-hindgut junction corresponds in 10-week

Technical Principles in Elective Surgical Treatment of Left Colon Diverticular Disease: A Scoping Review.

Left colon diverticular disease (LCDD) is prevalent in the aging populations of industrialized countries, with many patients requiring surgery. Elective surgery decisions should consider individual health conditions and quality of life. Typically, surgery is recommended six weeks after an acute episode. This scoping review, following PRISMA-ScR guidelines, analyzed the literature on elective LCDD surgery, focusing on inferior mesenteric artery (IMA) ligation, splenic flexure mobilization (SFM), surgical approach, and extent of resection. The databases searched included PubMed, Embase, and Coch

Incidence, demographics, and survival of patients with brain metastases from stage IV colorectal cancer: a population-based study from 2013 to 2023.

INTRODUCTION: Colorectal cancer (CRC) is the third most common cancer globally with rising incidence in the young population. While rare, brain metastasis (BM) in CRC patients leads to significant morbidity and mortality. The incidence of BM is expected to rise due to improved screening and treatments extending patient survival. However, limited screening guidelines exist to detect BM in patients with CRC. METHODS: Adult patients with colon and rectum tumors, with and without brain metastases, diagnosed between January 1, 2013, and December 31, 2023, were identified using ICD-10 codes from th

Exacerbation of previously undetected ulcerative colitis in a patient undergoing nivolumab for metastatic melanoma: A case report.

RATIONALE: In patients receiving immune checkpoint inhibitors, subclinical inflammatory bowel disease can be unmasked, leading to severe colitis. Ulcerative colitis, in particular, may remain undetected when initial symptoms are mild. This report describes a patient with metastatic melanoma whose previously unrecognized ulcerative colitis flared severely after initiating nivolumab. PATIENT CONCERNS: A 35-year-old man presented with chronic mild diarrhea, persisting for several months, which was initially overlooked. Three months after starting nivolumab therapy, he developed frequent diarrhea

Impar ganglion block with botulinum toxin in the management of perineal oncological pain: a case report.

Chronic perineal pain is a common complication among patients with cancer. Due to the complex innervation of this anatomical region, pain management is often challenging and significantly impacts patients' quality of life. We present the case of an elderly patient with advanced-stage rectosigmoid junction adenocarcinoma who experienced severe coccygeal-perineal oncologic pain. Despite a multimodal analgesic treatment approach, the patient exhibited refractoriness to conventional pain management. Consequently, an interventional analgesic approach was implemented using an impar ganglion block wi
