Targeted Preconditioning and Cell Transplantation in the Murine Lower Respiratory Tract.

Transplantation of airway basal stem cells could achieve a durable cure for genetic diseases of the airway, such as cystic fibrosis and primary ciliary dyskinesia. Recent work demonstrated the potential of primary and pluripotent stem cell-derived basal cells to efficiently engraft into the mouse trachea after injury. However, there are many hurdles to overcome in translating these approaches to humans, including developing safe and efficient methods for delivery in larger animal models. We propose a model that targets preconditioning and cell delivery to intrapulmonary airways using a microbr

Endogenous nitric oxide synthase inhibitor asymmetric dimethylarginine ameliorates lipopolysaccharide-induced airway inflammation and bronchial hyperreactivity in mice.

OBJECTIVES: Asymmetric dimethylarginine (ADMA) is an endogenous nitric oxide synthase inhibitor. ADMA concentrations are augmented in lung, plasma samples of asthmatics. It remains unclear whether the increased concentration of ADMA in airway inflammation contributes to the pathophysiology or acts as a protective mechanism. The objective of this study was to evaluate the effects of exogenous ADMA on airway inflammation in mice. METHODS: Ex vivo/in vivo airway inflammation models were used. Tracheal tissues, isolated from female Balb/c mice were incubated with lipopolysaccharide (LPS), ADMA, o

Knowledge-Driven Multiple Instance Learning With Hierarchical Cluster-Incorporated Aware Filtering for Larynx Pathological Grading.

Pathological grading of laryngeal squamous cell carcinoma (LSCC) based on whole-slide image (WSI) is crucial for the diagnosis, treatment and prognosis. According to pathologists' knowledge, tumor regions are highly associated with grading. However, existing multiple instance learning (MIL) methods tend to overrepresent weakly relevant non-tumor regions and irrelevant background, leading to poor grading performance and interpretability. Motivated by the above problems, we propose an end-to-end knowledge-driven MIL network with hierarchical cluster-incorporated aware filtering, i.e. HCF-MIL. Fi

Unravelling the Complexity of Congenital Tracheal Stenosis with 'O' Rings.

OBJECTIVES: Congenital tracheal stenosis (CTS) in paediatric patients is a rare condition, accounting for only 0.3% to 1% of all laryngotracheal stenoses. It is characterised by complete cartilaginous 'O'-shaped tracheal rings replacing the normal posterior membranous portion. This monocentric case series analyses management strategies and assesses surgical and functional outcomes. METHODS: Twenty-one patients (12 males, 9 females) underwent surgery for CTS. Nineteen patients received slide tracheoplasty, while two underwent tracheal resection with primary anastomosis. Pre- and postoperative

Contemporary outcomes after 100 consecutive HeartMate 3 LVAD implantations: a single-centre cohort study from a Swiss tertiary centre.

The extra ocular (eye) muscles are one of the principal tissues involved in the autoimmune-mediated inflammation of Graves' ophthalmopathy (GO). Several eye muscle proteins are targeted by autoantibodies or sensitized T lymphocytes, or both, and include: G2s, which is now identified as the terminal 141 amino acids of the winged-helix transcription factor FOXP1, the flavoprotein (Fp) subunit of the mitochondrial enzyme succinate dehydrogenase, the so-called "64kDa protein", a non-tissue specific membrane protein called 1D and the calcium binding protein calsequestrin. Of these, antibodies again

Tracheal resection anastomosis in neurologically compromised patients: no longer a contra-indication, with promising outcomes.

OBJECTIVES: To highlight our experience in the management of patients with severe subglottic and/or upper tracheal stenosis associated with neurological co-morbidities. Effective tips and tricks in the surgical technique were emphasised. METHODS: This study included 33 patients with neurological co-morbidities who underwent tracheal and/or cricotracheal resection anastomosis. To decrease the anastomotic tension, suprahyoid release was performed and trachea-hyoid detensioning stitches were placed. Surgiflo® was applied over the anastomosis to enhance healing. Surgical and functional outcomes w

Direct Inoculation of Pathogenic Yeasts into Mouse Lung by Trans-tracheal Instillation.

Pathogenic dimorphic fungi are endemic in certain regions and can cause from subclinical infections to systemic mycoses. These pathogens are associated with high mortality and morbidity rates and represent emerging infectious threats to human populations worldwide. Because most fungal pathogens cause lung infection or use the lung as a route to disseminate to other organs, mammalian pulmonary infection models are crucial tools in the advancement of medical mycology. Many different types of animal models of fungal infection have been developed, with murine models being hailed as the gold standa

Endoscopic tracheoesophageal fistula closure-Electrocautery combined with esophageal clip application in pediatric patients.

OBJECTIVE: A tracheoesophageal fistula (TEF) is an abnormal communication between the esophagus and trachea, most often associated with esophageal atresia (EA), a rare congenital malformation affecting 1 in 2400-4500 live births. While surgical repair of EA/TEF is typically completed in infancy, recurrent or missed TEF can be diagnosed later. Open repair can be technically challenging, often requiring re-do thoracotomy or neck dissection. We describe our center's experience, using endoscopic electrocautery in combination with esophageal clips for closure of TEF. METHODS: We conducted a retros

Effectiveness and feasibility of orogastric tube for surfactant delivery in moderate or very preterm neonates with respiratory distress syndrome: an open-label randomized controlled trial.

To assess the feasibility and effectiveness of administering surfactant via an orogastric feeding tube used as a thin tracheal catheter, compared with the Intubation-Surfactant-Extubation (InSurE) method, in preterm neonates with respiratory distress syndrome (RDS). This was hospital-based, randomized controlled trial. The intervention group (Group A) received surfactant via an orogastric tube with direct laryngoscopy while maintaining continuous positive airway pressure (CPAP) therapy. The control group (Group B) received surfactant using the standard InSurE technique. Data collected included

Temporal Progression of Tracheal Wound Healing After Partial-thickness Rat Injury Analyzed by Histology and qRT-PCR.

BACKGROUND/AIM: Wound healing is difficult to study due to interspecies variation in healing mechanisms, rates, and genetic factors. This study aimed to characterize tracheal wound healing in Wistar rats (Rattus norvegicus) following a thin, partial-thickness injury using a modified Derby-Perry Excavator. MATERIALS AND METHODS: Four male Wistar rats underwent endotracheal wounding under ketamine anesthesia. Each rat was euthanized at one, three, five, or six days post-injury. Excised tracheas were bisected for histological examination (Hematoxylin and Eosin staining) and quantitative RT-PCR a

Fabrication and Functional Integration of a Bioengineered Tracheal Graft With Angiogenic and Epithelial Regeneration Potential for Airway Restoration.

Tracheal defects caused by trauma, malignancy, congenital anomalies, or prolonged intubation remain a significant clinical challenge, particularly for long-segment replacements where conventional solutions such as autologous tissue grafts, allografts, and synthetic implants have shown limited long-term success. Tissue-engineered tracheas (TETs) have emerged as a promising alternative by integrating advances in scaffold design, fabrication techniques, and biological augmentation. Recent progress in scaffold materials, including decellularized natural matrices, synthetic polymers, and hybrid com

Identification of differentially expressed genes associated with tracheal injury recovery in a rabbit model of septic shock.

BACKGROUND: Sepsis is a syndrome caused by the host's inflammatory response to an infection with an unknown mechanism. This study aimed to identify differentially expressed genes (DEGs) potentially involved in the development and recovery of tracheal injury from septic shock. METHODS: Nine New Zealand white rabbits were randomized to control (CON), septic shock model (SS), and septic shock norepinephrine treatment (SSNE) groups (each group n = 3). The SS and SSNE groups were injected with lipopolysaccharide to induce septic shock. The SSNE group was administered Ringer lactate with norepineph

Endotracheal sialolipoma: A rare case report.

RATIONALE: Sialolipomas arising in the tracheobronchial tree are exceedingly rare. To date, only 6 cases of tracheobronchial sialolipoma have been documented in the English medical literature. A comprehensive understanding of the characteristics of this tumor is vital for accurate diagnosis and the development of effective treatment strategies. This study seeks to elucidate its features through a detailed case report and literature synthesis, aims to prevent diagnostic pitfalls and guide optimal treatment for this exceedingly rare tumor. PATIENT CONCERNS: A 28-year-old male was referred to Yu

Primary tracheal Ewing sarcoma.

Primary Ewing sarcoma of the trachea is an exceptionally uncommon malignancy that often presents with non-specific airway symptoms, leading to diagnostic delays. We report a woman in her early 30s with progressive cough, dyspnoea, hoarseness and stridor who was initially mismanaged as having common airway disease. Imaging revealed a near-obstructing tracheal mass, which was successfully excised bronchoscopically. Histopathology examination, immunohistochemistry and fluorescent in situ hybridisation confirmed the diagnosis of Ewing sarcoma with EWSR1 gene rearrangement. The patient was subseque

Single-stage functional lateral hemilaryngocricoidectomy for a locally advanced chondrosarcoma invading the hemicricoid.

Complete resection of cricoid lesions is often extensive, causing defects that require major reconstruction. We describe the use of a novel reconstruction technique in a patient with open organ preservation surgery to treat a grade II chondrosarcoma centred at the left hemicricoid. The pre-, intra- and postoperative course is described. Circumferential release of the trachea was made, ensuring it was below the right remaining hemicricoid. A stepladder tracheal defect was created and the trachea was advanced superiorly until the left tracheal hemirings were intussuscepted and rotated to recreat

Hospital Floor Hallway Boarding: A Systematic Review of Outcomes.

OBJECTIVES: The prolonged boarding of admitted patients in the emergency department (ED) increases crowding, a significant challenge for hospitals. Some hospitals have implemented hallway boarding by placing patients in inpatient hallways when standard rooms are unavailable. However, the impact of hallway boarding on patient preferences, clinical outcomes, and patient safety remains uncertain. STUDY DESIGN: Systematic review and meta-analysis. METHODS: We conducted a systematic review and meta-analysis of studies comparing boarding patients in an inpatient hallway with those in the ED or a s

Tracheal resection and mediastinal tracheostomy for cervical parastomal recurrence after laryngectomy and radiochemotherapy followed by several complications: a rare case report.

BACKGROUND: Patients with parastomal recurrence have poor prognosis and pose a great challenge to the surgeons. Surgical resection is the only curable choice. Treating this patient group with a patent tracheostoma and a stable airway is the main objective.  CASE PRESENTATION: We present an extremely rare case of a parastomal relapse after laryngectomy and radiochemotherapy for laryngeal carcinoma. Partial resection of the manubrium, medial part of the right clavicle, medial parts of the 1st and 2nd ribs, proximal part of the trachea below the cervical tracheostomy and subtotal thyroidectomy we

Enhanced proliferation tracer reveals Dorsal-Ventral asymmetry in tracheal epithelial Renewal.

To elucidate spatiotemporal dynamics of tissue renewal, we developed evProTracer, an enhanced dual-recombinase lineage tracing system for cumulative in vivo labeling of proliferating cells. Robust longitudinal tracing using evProTracer in murine tracheal epithelium revealed near-complete homeostatic turnover (91.6 ± 1.29% epithelial replacement over 25 weeks), while basal cell-specific Trp63-evProTracer uncovered a dorsally biased proliferation pattern, contributing 33.88 ± 1.44% of total epithelial renewal over 6 months, with early differentiation bias toward club cells. These data demonst

Penetrating Cervical Spine Injury without Spinal Cord Damage: A Case Report.

CASE: We report a case of a 65-year-old man attempting suicide through anterior neck stabbing with an ice pick. Computed tomography showed spinal canal penetration at C4-5 near the midline, sparing the trachea, esophagus, and major vessels. The foreign body was removed under general anesthesia; cerebrospinal fluid leakage was noted, and the wound was tightly closed with a fibrin-based sealant. No motor or sensory deficits were present preoperatively or postoperatively. CONCLUSION: This case shows that penetrating cervical spinal canal injuries may occur without neurological impairment.
