BACKGROUND: Contemporary Australian guidelines for the management of melioidosis recommend a longer duration of intravenous antibiotics during the intensive phase of treatment; recurrent disease is now far less common. A high rate of adverse drug reactions (ADRs) to the currently recommended minimum of 3 months of oral trimethoprim/sulfamethoxazole (TMP/SMX) eradication therapy questions the incremental value of this phase of treatment.
METHODS: In this pilot, single-center, open-label randomized controlled trial, adults with culture-confirmed melioidosis received either prolonged intravenous

Pulmonary melioidosis is frequently mistaken for pulmonary tuberculosis because of overlapping clinical and radiological features, often delaying appropriate treatment. We conducted a retrospective comparative cohort study at a tertiary care center in southern India between January 2018 and August 2025 to compare chest computed tomography (CT) features of microbiologically confirmed melioidosis and tuberculosis in diabetic adults who underwent CT within 30 days of symptom onset. A total of 151 patients were included: 46 with melioidosis and 105 with tuberculosis. Cavitation (n = 81/105 [77.1%]

Introduction Septic arthritis (SA) is a potentially destructive joint infection requiring timely diagnosis and treatment. Published adult data from Cambodia are limited, and local microbiological patterns may differ from those reported in other settings. Objective The objective of this study is to describe the demographic, clinical, laboratory, and available microbiological characteristics of adults treated for SA in a tertiary hospital in Cambodia. Materials and methods We conducted a small, single-center retrospective descriptive case series of 25 adults admitted to Medicine Ward B-A5, Calme

Burkholderia pseudomallei, the causative agent of melioidosis, is a gram-negative bacterium historically endemic to Southeast Asia and Northern Australia. However, recent environmental detection along the Gulf Coast and an increasing number of reported cases in non-travelers have raised questions regarding potential endemicity and environmental establishment in the Southern United States. We report a case of putatively locally acquired pulmonary melioidosis in a Florida resident with no recent international travel history, illustrating the diagnostic and therapeutic challenges posed by this ra

BACKGROUND: Pulmonary melioidosis may be clinically indistinguishable from other causes of community-acquired pneumonia (CAP), delaying appropriate microbiological investigation and antimicrobial therapy. A four-variable clinical prediction score incorporating monsoon-season presentation, diabetes mellitus, shock at presentation, and extrapulmonary focal abscesses was previously developed to identify melioidosis among patients with CAP. We aimed to externally validate this score in a prospective multicentre cohort and explore whether its performance could be improved by adding readily availabl

The Damage-Response Framework (DRF) provides a conceptual model for understanding the pathogenesis of parasitic infections by distinguishing biological infection from clinical infectious disease. Rather than viewing parasite virulence as an intrinsic property, the DRF proposes that disease results from dynamic host-parasite interactions, with host damage determined by the magnitude and quality of the immune response. Both insufficient immune responses, permitting parasite proliferation, and excessive inflammation, causing immunopathology, can lead to disease. Using representative examples, inc

BACKGROUND: Melioidosis is not a nationally notifiable disease in India, and the absence of systematic surveillance has contributed to substantial under-recognition and fragmented epidemiological data. We aimed to determine the hospital-based prevalence of culture-confirmed melioidosis among hospitalised adults at high risk of melioidosis in India.
METHODS: We conducted a prospective, multicentre, hospital-based prevalence study between Jan 1, 2023, and Dec 31, 2025, across 15 tertiary care centres in India. Adults (aged ≥18 years) hospitalised with predefined community-acquired infectious sy

Emerging respiratory infectious diseases increasingly arise at the human-animal-environment interface and challenge clinicians with rapidly evolving diagnostic, therapeutic, and infection-control decisions. This narrative review summarizes clinically relevant viral and bacterial pathogens that may present as severe respiratory disease, but are easily mistaken for common community-acquired infections during early illness. We focus on zoonotic influenza A viruses, including contemporary H5N1, Middle East respiratory syndrome coronavirus (MERS-CoV), hantaviruses, Nipah virus, and inhalational bac

PURPOSE: This review aims to summarize the diagnostic and therapeutic challenges associated with rare bacterial infections, with particular focus on Whipple's disease, melioidosis, Lemierre's syndrome, and nocardiosis. It also evaluates the clinical utility of emerging molecular diagnostic techniques, particularly polymerase chain reaction (PCR) and metagenomic next-generation sequencing (mNGS), in improving pathogen identification. The primary research question concerns how advances in molecular microbiology may enhance the early diagnosis and targeted treatment of these infections.
METHODS:

Burkholderia pseudomallei (B. pseudomallei) is the causative agent of melioidosis, a potentially serious and life-threatening infectious disease in humans. This pathogen invades the bloodstream and infects multiple organs, including the lungs, skin, liver, spleen, bones, and brain. The enzymes polyphosphate kinase 1 (Ppk1), type III secretion system cluster 3 (TTSS3)-effector protein (BopA), and trehalose-phosphate phosphatase (TPP) are vital for the survival and pathogenicity of B. pseudomallei, playing central roles in oxidative phosphorylation, stress responses, metabolism, biofilm formatio

Melioidosis, caused by Burkholderia pseudomallei, produces a wide range of clinical manifestations and may be difficult to diagnose in the pediatric population. We describe the case of a 12-year-old girl with poorly controlled type 1 diabetes mellitus that posed diagnostic challenges. Over 4 months, she experienced relapsing fever, multiple splenic abscesses with a 'target sign' on ultrasonography, right-sided preseptal cellulitis, and a left thigh ulcer. Repeated cultures of superficial swabs and blood yielded no growth of bacteria. Radiological mimicry and culture-negative results raised con

Melioidosis, caused by Burkholderia pseudomallei, remains challenging to diagnose promptly, partly due to limited use of virulence factors as diagnostic targets. Most existing biomarkers fail to capture the dynamic expression of bacterial virulence factors during active infection. In vivo‑upregulated virulence determinants, such as Hcp1, POMCR/PPEP, and GroEL, are intrinsically superior diagnostic targets. Hcp1 is a paradigm: it is the most advanced virulence factor translated into a point‑of‑care test, with prospective validation in endemic settings. Mechanistic understanding of virulence reg

INTRODUCTION: The shortage of physicians in rural areas of the USA has led medical schools to focus on recruiting and selecting students who will choose to work in non-urban settings. The purpose of this study was to examine the effect of student characteristics previously correlated to choosing rural practice (ie being older, being male, being raised in a rural community, having a spouse or significant other who was raised in a rural community, having a spouse or significant other whose parents live in a rural community, having parents with high school education or less, and graduating from a

BACKGROUND: Attention deficit hyperactivity disorder (ADHD) is a common and growing problem and a leading cause of child referrals to Child and Adult Mental Health Services (CAMHS). It is a drain on resources across nationally funded support agencies and associated with negative outcomes such as early criminality, school disruption and antisocial behaviour. Mainstream interventions (pharmacological and behavioural) demonstrate effectiveness whilst implemented, but are costly, often have unwanted side effects and do not appear to be affecting long-term outcomes.Development of a robust evidence

Individuals with type 2 diabetes are at increased risk of acquiring melioidosis, a disease caused by Burkholderia pseudomallei infection. Although up to half of melioidosis patients have underlying diabetes, the mechanisms involved in this increased susceptibility are unknown. We found that B. pseudomallei-infected PBMCs from diabetic patients were impaired in IL-12p70 production, which resulted in decreased IFN-γ induction and poor bacterial killing. The defect was specific to the IL-12-IFN-γ axis. Defective IL-12 production was also observed during Mycobacterium tuberculosis infection, in wh

Mucuna pruriens (Mp) is an annual and perennial legume which belongs to the family Fabaceae having different types of therapeutic activity. Anti-oxidative, anti-inflammatory, anti-epileptic, anti-microbial, etc. are the example of some most common activities of Mp. It is widely utilized as a potent aphrodisiac. The anti-Parkinsonian activity of Mp was explored since the nineteenth century. The neuroprotective activity of Mp was shown by several researchers. Levodopa (L-DOPA) is the important constituents responsible for the anti-Parkinsonian activity of Mp. Apart from L-DOPA, several other imp

BACKGROUND: Understanding synapse-specific effects of neuroinflammation can provide mechanistic and therapeutically relevant insights across the spectrum of neurological diseases.
METHODS: We applied neuron-specific proteomic biotinylation in vivo, differential centrifugation of brain for crude synaptosome enrichment (P2 fraction) and mass spectrometry (MS) analysis of biotinylated proteins to derive native-state proteomes of Camk2a-positive neurons and their corresponding P2 synaptic compartments. Next, in an in vivo model of systemic lipopolysaccharide (LPS) dosing, we examined the effects
