First culture-confirmed melioidosis case in Mozambique: a wakeup call for better diagnostics and clinical awareness.

Melioidosis is a disease caused by Burkholderia pseudomallei and is an underrecognised cause of severe infection in sub-Saharan Africa. In Mozambique, where infectious diseases are prevalent, and laboratory capacity remains limited, it has not previously been reported. We report the first culture-confirmed case of melioidosis in a 37-year-old man living with advanced HIV. He presented with fever, diffuse ulcerative skin lesions, and pulmonary infiltrates, and was empirically treated for sepsis. He died on day 15 of hospitalisation before culture results were available. The isolate was identifi

Microbial diversity creates a global firewall against pathogens in soil.

Soil is a critical ecological contributor to plant and animal health. In this issue of Cell Host & Microbe, Xiong et al. use global metagenomic data to show that human pathogens linked to diseases like tuberculosis, melioidosis, and sepsis are widespread in humid and agricultural soils harboring reduced microbial diversity.

Tibial Osteomyelitis due to Burkholderia pseudomallei in a Non-diabetic Patient: A Case Report.

INTRODUCTION: Melioidosis, a potentially fatal infectious human disease labeled "silent killer disease," caused by Burkholderia pseudomallei, is an environmental Gram-negative bacillus endemic to tropical regions. To our knowledge, this is one of the few reported cases of the rare melioidosis infection from the Mizoram state bordering Myanmar and Bangladesh. CASE REPORT: We report a case of culture-confirmed B. pseudomallei osteomyelitis in a 29-year-old non-diabetic, immunocompetent adult from an endemic region of Myanmar. CONCLUSION: The diagnosis of melioidosis requires a high index of su

Targeting inflammasome, IL-1β, and coagulation in melioidosis.

Burkholderia pseudomallei is a Gram-negative bacterium that causes melioidosis, a disease endemic in Southeast Asia and Northern Australia and increasingly detected in other tropical regions. B. pseudomallei is resistant to many antibiotics and no vaccine for melioidosis is currently available. The disease, particularly its pneumonic form, has high mortality, often due to sepsis. Accumulating evidence from our and other groups has shown that inflammasome and toll-like receptors (TLR) activation during B. pseudomallei lung infection causes an excessive inflammatory response that becomes deleter

CRISPR-based environmental detection of Burkholderia pseudomallei identifies sanitation gaps and melioidosis risk in northeast Thailand.

Environmental exposure to Burkholderia pseudomallei, the causative agent of melioidosis, remains poorly characterised due to the low sensitivity of conventional detection methods. Here, we develop CRISPR-BEEPs, a sensitive and resource-efficient CRISPR-based assay, and evaluate its performance against conventional culture-based plate inspection using double-qPCR as the reference standard. CRISPR-BEEPs demonstrated higher sensitivity (93.5% vs 19.4%) and high specificity (100% vs 98.0%). We apply the assay to water samples from natural and piped sources across 15,118 km² in northeast Thailand,

Interaction effects of physicochemical factors on the growth of Burkholderia pseudomallei in soil microcosms.

BACKGROUND: Burkholderia pseudomallei (B. pseudomallei), the causative agent of melioidosis, inhabits diverse ecological niches such as soil, surface water, and rhizospheres. Soil serves as its primary reservoir. Understanding the ecological niche of B. pseudomallei in soil is crucial for developing strategies to reduce bacterial presence. Therefore, this study investigated the interaction effects of soil parameters, namely pH, salinity, temperature, moisture content, carbon to nitrogen (C/N) ratio, and iron content, on B. pseudomallei proliferation through factorial soil microcosm experiments

Comparative Performance and Species-Specific Recovery Biases of Culture-Based Methods for Campylobacter Detection in Food Products: A Systematic Review and Meta-Analysis.

Campylobacter is the primary bacterial cause of foodborne gastroenteritis globally. While international standards recommend a tiered approach for detection, emerging evidence suggests that selective protocols may introduce species-specific recovery biases. This systematic review and meta-analysis evaluated the diagnostic performance of established culture-based detection protocols across diverse food matrices. Following PRISMA 2020 guidelines, we searched multiple databases for studies reporting 2 × 2 diagnostic accuracy data through October 2024. Ten studies comprising 43 method comparisons a

Endostemonine I as a Multi-Target Inhibitor of Kaposi's Sarcoma-Associated Herpesvirus Oncogenic Pathways: An Integrative Computational Study.

Background/Objectives: Kaposi's sarcoma (KS) is an angioproliferative malignancy caused by Kaposi's sarcoma-associated herpesvirus (KSHV), characterized by aberrant angiogenesis, chronic inflammation, and endothelial cell transformation. Given the multi-factorial nature of KS pathogenesis, strategies that simultaneously modulate multiple mo-lecular targets are considered more promising than single-target approaches. However, effective multi-target therapeutic agents for KS remain limited, prompting this study to employ an integrative in silico pipeline. Methods: An integrative in silico pipeli

Melioidosis Presenting As a Chronic Splenic Abscess: A Diagnostic Challenge.

Melioidosis is a potentially fatal infection caused by Burkholderia pseudomallei, often presenting as various clinical manifestations and is commonly misdiagnosed in endemic regions, leading to delayed diagnosis. We report the case of a middle-aged female patient with uncontrolled diabetes mellitus and occupational exposure as a farmer, who presented with chronic fever and abdominal pain of one year duration. She had a prior history of a treated lumbar abscess where microbiological diagnosis was missed. Imaging revealed a multiloculated splenic abscess with features of impending rupture. Possi

Hospital-Based Cross-Sectional Study of Burkholderia pseudomallei Seroreactivity Among Febrile Patients in Northernmost Vietnam: Near-Neighbor Bloodstream Isolates, Environmental Correlates, and Spatial Clustering.

Melioidosis is a potentially fatal tropical disease caused by the environmentally mediated bacterium, Burkholderia pseudomallei. In Vietnam, melioidosis is not nationally reportable and little is known regarding its epidemiology and distribution in the northernmost mountainous provinces. To address this gap, a hospital-based cross-sectional study was conducted across six northernmost provinces of Vietnam: Son La, Dien Bien, Lai Chau, Lao Cai, Ha Giang, and Cao Bang. Objectives were to (a) elucidate the distribution and environmental correlates of B. pseudomallei seroreactivity, and (b) identif

The Great Mimicker in Bone: A Rare Case of Melioidosis With Multifocal Skeletal Manifestations and Diagnostic Challenges.

Melioidosis, caused by the Gram-negative bacillus Burkholderia pseudomallei (B. pseudomallei), is an emerging infectious disease endemic to Southeast Asia and northern Australia, well known for its protean manifestations and aptly termed "the great mimicker." Skeletal involvement, although an established entity, remains underreported, particularly when affecting multiple bones simultaneously in the setting of newly diagnosed diabetes mellitus. We report a 35-year-old male patient with newly diagnosed type II diabetes mellitus who presented with fever and right knee pain. Surgical debridement o

Neuromelioidosis in a nutshell-Pattern and Endemicity.

Timely and accurate imaging diagnosis is critical in cases of fever, altered sensorium, seizures, or headaches that do not respond to conventional clinical management. This pictorial essay aims to highlight, through case illustrations, the critical role of imaging in identifying Burkholderia pseudomallei-a rare but significant infectious agent responsible for a characteristic pattern of brain and spinal cord involvement. The condition, known as neuromelioidosis, is endemic in the tropical regions of Southeast Asia and Northern Australia including coastal regions of Tamil Nadu and Karnataka in

Mucosal-associated invariant T (MAIT) cells are reduced and dysfunctional in acute melioidosis.

Burkholderia pseudomallei (BP), the causative agent of melioidosis, is a major cause of sepsis in Southeast Asia, especially in people with diabetes mellitus (DM). The role of Mucosal-associated invariant T (MAIT) cells; innate-like T cells important for antibacterial immunity; in melioidosis is unknown. We measured MAIT cell activation by BP in vitro using co-culture assays with THP-1 cells, and evaluated MAIT cell frequency, activation, and function ex vivo in an observational cohort (n = 120) of melioidosis patients and endemic controls with and without DM in Thailand. We show that BP induc

Characterization of an isogenic bimA mutant in the ATS2021 strain of Burkholderia pseudomallei.

Encephalomyelitis is uncommon but is associated with Burkholderia pseudomallei strains whose bimA gene, encoding an actin-based motility protein, resembles bimA from Burkholderia mallei (designated bimABm). We previously characterized the virulence of B. pseudomallei wild-type ATS2021 (bimABm). In a mouse model of ATS2021 aerosol exposure, lesions and inflammatory cytokines in the brain were seen early post-exposure and reached the central nervous system (CNS) through the olfactory nerve. In the current study, the bimA allele in the wild-type ATS2021 was replaced with the bimABp, and virulence
