Pregnancy and risk of melanoma: a matched retrospective cohort study.

Pregnancy-associated melanoma has long been debated, with hypotheses that gestational immune tolerance and elevated sex hormones may promote melanocytic transformation; however, large-scale, real-world evidence remains limited and conflicting. Using the TriNetX Global Collaborative Network, we conducted a retrospective cohort study of pregnant patients with documented first and third trimester encounters versus reproductive-age nonpregnant females with an encounter for contraceptive management and equivalent follow-up. Cohorts were 1:1 propensity-score matched on demographics, comorbidities re

Twenty-one years of cutaneous melanoma: stage shift, anatomical patterns and diagnostic trends from a Southern European tertiary center.

Cutaneous melanoma incidence continues to rise worldwide, while advances in dermoscopic surveillance increasingly influence the stage at diagnosis. Long-term single-center analyses offer a unique perspective on how demographic shifts, tumor biology, and diagnostic intensity interact over time. We retrospectively analyzed 4062 consecutive patients with histologically confirmed cutaneous melanoma diagnosed between 2003 and 2024 at a tertiary dermatology center in Southern Italy. Demographic characteristics, anatomical site, and Breslow thickness were collected. Temporal trends were assessed usin

Earlier detection of melanoma using in vivo reflectance confocal microscopy-a retrospective analysis.

INTRODUCTION: The incidence of melanoma has increased in recent decades. Non-invasive imaging devices, such as in vivo reflectance confocal laser microscopy (RCM), aid in the earlier detection of melanoma and decrease the number of patients that need treatment. RCM can visualise the skin to a depth of 200 μm at the cellular level. METHODS: We conducted a retrospective analysis of melanocytic lesions, including invasive melanoma, in situ melanoma, and nevi, treated at the University Hospital Dresden. The control group (CG) included lesions examined solely through dermoscopy from 01 January 201

Cutaneous melanoma in situ: a review.

Cutaneous melanoma in situ (MIS), also known as 'stage 0 melanoma', is a collection of malignant melanocytes in the epidermis and epithelial adnexa, without evidence of microinvasion to the papillary dermis. Distinct histological subtypes include lentigo maligna (LM), superficial spreading (SS) MIS and acral lentiginous (AL) MIS. LM is the most common subtype, usually diagnosed later in life (median age at diagnosis of 66-72 years) and associated with cumulative ultraviolet radiation exposure. SS MIS is associated with intense episodes of sun exposure and is more common on the trunk and extrem

Melanoma Arising in Tattoos: A Case Series and Scoping Review of the Literature.

BACKGROUND/OBJECTIVES: The prevalence of tattoos has risen globally in recent decades, ranging from 10% to 29%. Although rare, melanomas arising within tattoos are challenging for dermatologists due to the complexities in clinical and dermoscopic evaluation. In this article, we report two cases of melanoma on tattoos, review the reported cases in the literature, and examine the role of reflectance confocal microscopy (RCM) in improving the diagnosis of melanoma on tattooed skin. METHODS: We conducted a systematic literature search on Medline/Pubmed for the period from the inception of the dat

Local recurrence of non-lentigo maligna melanoma in situ and safety excision margins: A systematic review.

The strength of evidence supporting a 2-step wide local excision (WLE) with 5 mm safety margins for melanoma in situ non-lentigo maligna (non-LM) type, compared with narrower margins, is unclear. This review aims to compare the frequency of local recurrence (LR) with different safety margins, after the complete surgical excision (R0) of non-LM MIS. We performed a systematic literature search in PubMed, Scopus, and the Cochrane Library up to March 17, 2024. The PRISMA checklist was used. Of 3,047 articles retrieved, seven retrospective studies were included, enrolling a total of 1,526 non-LM MI

A Retrospective Study of Nevus-Associated Melanoma in situ.

INTRODUCTION: The frequency and type of nevus association in histology in melanoma in situ (MIS) is unclear. METHODS: We performed a retrospective study to investigate the characteristics, frequency, and type of adjacent nevus in histology (common or dysplastic) in MIS. Lentigo maligna (LM) and in situ acral lentiginous melanomas (ALMs) were excluded. RESULTS: From 1991 to 2023, there were 448 available non-LM/non-ALM MIS. MIS was nevus associated (NAM-MIS) in 353 cases (79%). Of these cases, 326 NAM-MIS cases (94%) had dysplastic nevus remnants. Among the dysplastic nevus MIS, 176 (54%) wer

Multiple Primary Melanomas: Clinical and Genetic Insights for Risk-Stratified Surveillance in a Tertiary Center.

Background: Patients diagnosed with melanoma are at increased risk of developing multiple primary melanomas (MPMs). Identifying clinical and genetic factors associated with MPM is critical for implementing personalized surveillance strategies. This study aims to describe the clinical, histopathological, and genetic characteristics of patients with MPM managed in a tertiary hospital and to contextualize findings within the current literature. Methods: We conducted a retrospective review of patients diagnosed with two or more primary melanomas between 2010 and 2023 at a tertiary dermatology unit

Local Recurrence and Survival in Patients With Melanoma In Situ.

IMPORTANCE: The overdiagnosis of melanoma in situ (MIS) is well documented. There is limited evidence on the rate of local recurrence of the non-lentigo maligna (non-LM)/non-acral lentiginous melanoma (non-ALM) subtypes. OBJECTIVE: To investigate local recurrence and prognosis in non-LM/non-ALM MIS, the histopathological clearance of the excisional biopsy margins, and the association with the size of wide excision margins. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study included patients with non-LM/non-ALM MIS diagnosed from 1991 to 2023 who were followed up for at least

A Case Report of a Comparative Approach in a Patient with Multiple Seborrheic Keratosis: Does It Work for SK-Like Melanoma Screening?

INTRODUCTION: The evaluation of pigmented lesions remains a daily diagnostic challenge. Dermoscopy enhances accuracy, but seborrheic keratoses (SKs), though benign, may mimic melanoma due to melanocytic-like features. In patients with multiple SK, detecting a malignant lesion can be difficult, especially when it shares clinical or dermoscopic characteristics with SK. CASE PRESENTATION: A 47-year-old man presented with multiple pigmented lesions on the face, scalp, and trunk. Most lesions were typical SK. However, one scalp lesion appeared clinically distinct with heterogeneous pigmentation. D

Anatomical Locations of Keratinocyte Carcinomas Among United States Veterans Affairs Healthcare System Patients in Minnesota: A Cross-Sectional Study.

Previous studies have demonstrated a higher prevalence of keratinocyte carcinomas (KC) on sun-exposed areas, particularly for squamous cell carcinoma (SCC) compared to basal cell carcinoma (BCC). Few studies in the United States (US) have compared the prevalence of BCC to SCC by anatomical location. The aim of this study was to determine the KC prevalence in a Midwestern US population standardised by relative tumour density (RTD) per anatomical area, which has not been previously reported in the US. Data was collected from Veterans Affairs' patients with biopsy-proven KCs from October 1999 to

Abatacept improves posttransplant survival and reduces endothelial injury syndromes in β-thalassemia major.

Iron overload in transfusion-dependent β-thalassemia (TDT) generates reactive oxygen species, predisposing to post-hematopoietic stem cell transplant (HSCT) endothelial activation. Abatacept prevents acute graft-versus-host disease (GVHD) by inhibiting CD80/CD86 on T cells, but CD80 is also expressed on neutrophils. Elevated neutrophil extracellular traps (NETs) at day +14 are associated with thrombotic microangiopathy (TMA) after HSCT, mechanistically linking endothelial activation to complement activation. We wanted to compare post-HSCT survival and incidence of endothelial injury syndromes

[A hard subcutaneous swelling: not always as malignant as it seems].

BACKGROUND: Nodular fasciitis is a rare, benign soft tissue tumor in young adults on the arms or trunk. CASE DESCRIPTION: A 24-year-old woman with previous pretibial melanoma in situ presented with a painless, rapidly growing subcutaneous hard swelling on the right forearm since two weeks. Ultrasound showed a subcutaneous, irregularly shaped, vascularized mass, suspicious for malignancy. Three days later, the swelling had subsided spontaneously. A new ultrasound showed a smaller, oval, sharply demarcated lesion with an inflammatory aspect. Histopathological-immunohistochemical examination of

Mohs micrographic surgery in the surgical treatment paradigm of melanoma in situ and invasive melanoma: A clinical review of treatment efficacy and ongoing controversies.

Mohs Micrographic Surgery (MMS) for treatment of melanoma offers several advantages over wide local excision (WLE), including complete histologic margin evaluation, same-day resection and closure, and sparing of healthy tissue in critical anatomic sites. Recently, a large volume of clinical data demonstrating efficacy in MMS treatment of melanoma was published, leading to emerging patient safety considerations of incurred treatment costs, risk of tumor upstaging, and failure of care coordination for sentinel lymph node biopsy (SLNB). MMS offers a safe, effective, and value-based treatment for

Synchronizing the Nomenclature Surrounding Synchronous Primary Cutaneous Melanomas: A Systematic Review.

OBJECTIVE: There is little consensus regarding the nomenclature and prognostic implications of synchronous melanomas. Here, we present a case of synchronous cutaneous melanoma and perform a systematic review of similar cases in the literature. METHODS: Pubmed and EMBASE databases were queried for relevant English-language articles published from inception until 2023. Cases of "multiple primary cutaneous melanomas" that occurred within a time frame of three months or less were included. Exclusion criteria included non-cutaneous melanomas and cases without specific time intervals or those occur

Recurrence Rate of Small Melanoma In Situ on Low-Risk Sites Excised With 5-mm Excisional Margin.

IMPORTANCE: The incidence of melanoma in situ (MIS) has increased significantly over the past decades, and traditional guidelines for treatment of MIS have been excision with a 5-mm clinical margin; however, current Australian and other guidelines now recommend 5- to 10-mm margins. This changed recommendation was largely driven by the outcomes of studies using Mohs micrographic surgery, and recent studies using Mohs micrographic surgery are advocating for even wider excisions up to 18 mm for clearance. OBJECTIVE: To assess the rate of recurrence of MIS excised with a 5-mm margin. DESIGN, SET

Trends in Cutaneous Melanoma in Nova Scotia With a Focus on 2007 to 2019.

BACKGROUND: Melanoma represents a significant public health challenge in Canada, contributing to the deaths of over 1000 individuals each year. Prince Edward Island and Nova Scotia were previously noted to have the highest incidence rates of melanoma in Canada. METHODS: Data from patients diagnosed with or dying from melanoma was extracted from the Nova Scotia Cancer Registry. TNM stage was available for cases diagnosed 2007 to 2017. Incidence (1992-2019) and mortality (1992-2021) rates were examined using Join Point Trend Analysis Software. RESULTS: Between 2007 and 2019, 2450 cases of in s

Outcomes for Malignant Pleural Effusions Because of Melanoma Treated With Indwelling Pleural Catheters.

BACKGROUND: Indwelling pleural catheters (IPCs) reduce dyspnea and improve quality of life in patients with malignant pleural effusions (MPEs). Data on outcomes of MPEs secondary to metastatic melanoma managed with IPCs are scarce. We aimed to evaluate outcomes of patients receiving IPCs for MPEs secondary to melanoma compared with other malignancies. METHODS: We identified patients from our prospectively collected database of all patients who had an IPC insertion for MPEs at our tertiary care center for melanoma between May 2006 and November 2018 and for nonmelanoma between May 2006 and June
