A patch-based bolus technique for total scalp irradiation: dosimetric impact of conformity and reproducibility.

Total scalp irradiation (TSI) for scalp angiosarcoma remains technically challenging because of the large, highly curved target geometry and the need for reliable bolus application. This study aimed to develop a practical method to improve bolus conformity using commercially available bolus and to evaluate the dosimetric impact of bolus reproducibility in volumetric-modulated arc therapy (VMAT)-based TSI. An anthropomorphic head phantom was used to simulate TSI conditions. A three-segment 'patch bolus' method was developed to enhance conformity and setup stability. Treatment plans were generat

Paediatric Scalp Squamous Cell Carcinoma With Suspected Intracranial Extension Mimicking Cerebral Abscesses in a Patient With Xeroderma Pigmentosum: A Case Report and Literature Review Abstract.

OBJECTIVES: Squamous Cell Carcinoma (SCC) is among the most frequently reported malignancies in XP; however, intracranial extension is exceedingly rare and may pose a significant diagnostic challenge when radiologic findings mimic those of cerebral abscess. METHOD: The case of a 7-year-old male child with XP who developed an ulcerative lesion on the scalp and underwent surgical excision is reported. Xeroderma Pigmentosum (XP) is a rare autosomal recessive genodermatosis characterized by extreme sensitivity to ultraviolet radiation and an increased predisposition to cutaneous malignancies.. R

Thin Thoracodorsal Artery Perforator Free Flap for Scalp Resurfacing After Malignant Skin Tumor Excision.

BACKGROUND: Scalp defects after malignant tumor excision often require free tissue transfer for durable coverage. The thoracodorsal artery perforator (TDAP) flap remains underused despite its suitability for thin elevation. METHODS: Fourteen patients (6 men, 8 women; mean age 62 years) treated with a thin TDAP free flap for scalp resurfacing after malignant tumor excision between 2016 and 2026 were retrospectively reviewed. All flaps were performed by a single surgeon at two institutions. RESULTS: TDAP elevation was completed in 14 of 16 attempts (87.5%); in 2 patients no reliable perforator

Progressive Pseudorheumatoid Dysplasia Presenting with Cauda Equina Compression: A Case Report.

PURPOSE: Fresh vegetables are essential human dietary components but are increasingly implicated in foodborne infections caused by Escherichia coli. Contamination may occur during cultivation, irrigation, handling, and retail display, and the presence of pathogenic or antimicrobial-resistant E. coli poses significant public health risks. Despite widespread vegetable consumption in Ogun State, Nigeria, limited data exist on the resistance and virulence characteristics of E. coli in these foods. This study therefore assessed antimicrobial resistance profiles and virulence genes in E. coli isolat

Platelet-Rich Plasma Injections and Scalp Cutaneous Malignancies: Clinical Implications and Knowledge Gaps.

Platelet-rich plasma (PRP) injections are an effective and increasingly utilized treatment for androgenetic alopecia; however, the delivery of concentrated growth factors raises theoretical concerns regarding cutaneous malignancy risk in chronically ultraviolet (UV)-exposed, hair-thinning scalps. In this article, the current hypotheses linking PRP to scalp skin cancer risk are reviewed, including potential oncogenic mechanisms, such as platelet-derived growth factor (PDGF)-mediated signaling, and protective effects, such as improved hair-mediated photoprotection and antiproliferative findings

Diagnosis, treatment, and prognosis of scalp angiosarcoma: A case report.

RATIONALE: Scalp angiosarcoma is a rare malignant vascular tumor with the characteristics of occult early symptoms, no specificity, a high misdiagnosis rate, strong invasion, and poor prognosis. The 5-year survival rate is <30%. The accumulation of rare cases is of great significance for optimizing diagnosis and treatment. PATIENT CONCERNS: A 78-year-old man presented with a dark-brown nodule on the top of the head 2 months ago, which progressively enlarged with ulceration, erosion, and easy bleeding. Topical antibiotic therapy was ineffective. Imaging showed a mass with an unclear inferior b

Electrochemotherapy in the treatment of basal cell carcinoma of the head and neck: a case report.

BACKGROUND: Skin areas chronically exposed to solar radiation, such as the face, neck, scalp, and upper limbs, are at significantly increased risk of developing basal cell carcinoma. The objective of this case report is to describe the treatment of a basal cell carcinoma located on the chin using electrochemotherapy in a patient with advanced heart disease, a condition that contraindicated invasive surgical intervention. CASE PRESENTATION: An 83-year-old Caucasian female patient presented with a persistent nodular lesion on the chin, present for approximately 6 months. The diagnosis of cutane

Metastatic Cutaneous Adnexal Carcinoma of the Head and Neck With Durable Response to an Aromatase Inhibitor Combined With a Cyclin-Dependent Kinase 4/6 Inhibitor.

BACKGROUND: Cutaneous adnexal carcinomas are a very rare group of cancers arising from appendageal structures of the skin. More than half occur in the head and neck region. No standard treatments exist for metastatic disease. METHODS: A 48-year-old postmenopausal female presented with an enlarging scalp lesion. Biopsy showed adenocarcinoma that was positive for estrogen and progesterone receptors. She underwent wide local excision with adjuvant radiation. Unfortunately, she presented with metastatic disease 1.5 years later. RESULTS: Given the tumor's hormone receptor profile, she was started

SMARCB-1-Positive Rhabdoid Squamous Cell Carcinoma Following Liver Transplant: Systematic Review and Unique Case Report.

We present a rare case of squamous cell carcinoma with rhabdoid features (SCCR), an aggressive histological variant of SCC, in a 63-year-old man with a history of liver transplantation, alongside a review of the literature on previously reported cases (n = 14). SCCR is characterized by large, ovoid, or polygonal carcinoma cells with eosinophilic cytoplasmic inclusions, eccentric nuclei, and prominent nucleoli. While typically associated with INI-1-inactivating mutations and linked to immunosuppression, particularly post-transplantation, the exact pathogenesis of SCCR remains unclear. The patien

A giant challenge: Hybrid management of a scalp and neck cirsoid aneurysm.

BACKGROUND: Cirsoid aneurysms are rare, high-flow arteriovenous malformations (AVMs) of the scalp and neck, formed by direct arteriovenous shunts without an intervening capillary bed. They may present with pulsatile swelling, bruit, cosmetic disfigurement, pain, or hemorrhage. Giant, diffuse lesions with feeders from multiple vascular territories are uncommon and pose significant therapeutic challenges. CASE DESCRIPTION: A 24-year-old male presented with a 10-12-year history of an enlarging occipital mass, which had rapidly increased in size over the preceding 2-3 years. Examination revealed

Lethal medical course of an untreated giant cutaneous squamous cell carcinoma.

Cutaneous squamous cell carcinoma (cSCC) is the second most common malignant skin tumor in Caucasian populations and occurs predominantly on sun-exposed areas of the body. In Germany, which has a nationwide skin cancer screening program, cSCC is usually detected in early tumor stages and can curatively be treated by surgical excision. We report the case of a 59-year-old man who had a giant untreated cSCC of the scalp and died from the local and systemic effects of the aggressively growing tumor. A subsequent medicolegal autopsy was performed. It is noteworthy that the patient had refused medic

Inverted Follicular Keratosis in Young Adult: A Rare Entity in the Differential Diagnosis of Skin Tumors.

INTRODUCTION: Inverted follicular keratosis (IFK) is a rare benign tumor arising from the follicular infundibulum, typically presenting as a solitary lesion on the head and neck of older adults. The actual incidence of inverted follicular keratosis is likely underreported due to frequent misdiagnosis as more common cutaneous lesions, including seborrheic keratosis, verruca vulgaris, or pyogenic granuloma. CASE PRESENTATION: We reported a rare case of IFK in a 26-year-old male with a solitary, asymptomatic, pedunculated lesion on the scalp. The patient presented with a 0.7 cm-sized lesion demo

A 17-Year-Old Male Adolescent With a Giant Isolated Scalp Neurofibroma Associated With a Calvarial Defect: A Case Report.

BACKGROUND Scalp neurofibromas are uncommon benign peripheral nerve sheath tumors and are typically associated with neurofibromatosis type 1 (NF1). Isolated scalp neurofibromas without NF1 findings are extremely rare, and large lesions with associated calvarial remodeling are even more exceptional. This study presents a giant occipital neurofibroma in a young patient without any diagnostic criteria of NF1 and discusses its clinical, radiological, surgical, and histopathological features in the context of current literature. Diagnosis and treatment of apocrine carcinoma of the head and neck: A systematic review.

BACKGROUND: Apocrine carcinoma is a rare malignancy of apocrine sweat glands that can arise in the head and neck. These lesions frequently resemble benign adnexal processes, which may contribute to diagnostic delay. Owing to the rarity of this tumor, treatment recommendations are based largely on isolated case reports. METHODS: A systematic review was performed according to the PRISMA guidelines. PubMed, Embase, and CINAHL databases were queried from inception through March 2025. Studies were included if they reported at least one case of head and neck apocrine carcinoma with demographic, tre

Aggressive multifocal recurrent porocarcinoma of the face: a case report and review of the literature.

Eccrine porocarcinoma (EPC) is a rare, aggressive skin cancer arising from eccrine sweat glands, comprising less than 0.01% of all skin cancers. Management is especially complex in immunosuppressed patients due to increased tumor aggressiveness, recurrence, and limited treatment options. We present a 69-year-old woman with recurrent facial EPC following liver and kidney transplantation for Wilson's disease and hepatitis C. Despite multiple surgeries and two courses of radiation, she experienced multifocal locoregional recurrence. Immunotherapy was contraindicated due to high graft rejection ri

Clinical, Histopathologic, and Surgical Aspects of Dermatofibrosarcoma Protuberans on the Head vs Other Locations: Retrospective Single-center Case Series of 235 Tumors.

BACKGROUND: Dermatofibrosarcoma protuberans (DFSP) is a rare tumor that affects the head in 10-15% of cases. Head DFSP is often associated with more complex surgery and a higher rate of local recurrence. OBJECTIVE: To compare the clinical, histopathologic, and surgical characteristics of head DFSP vs body DFSP in order to identify potential risk factors. METHODS: Retrospective study. RESULTS: Of 235 DFSP, 32 (13.6%) were located on the head. Histologically, 71.0% of head DFSP vs 7.4% of body DFSP were purely subcutaneous with no dermal involvement (P<.001). Uncommon non-traumatic scalp entities: a pictorial review.

Non-traumatic emergent scalp abnormalities are uncommon and may be easily overlooked on routine neuroimaging. However, by nature of its superficial anatomic location, the scalp is frequently the first site of physical abnormality noted by patients. Although scalp pathology is rarely the primary indication for obtaining cross-sectional imaging, the rich vascular supply to the scalp means that this region is commonly linked to disease processes in the skull, intracranial vasculature, and more distant anatomical structures. In this pictorial essay, we present a unique spectrum of scalp lesions en
