Axillary Accessory Breast Cancer Performed Reconstruction Using a Local Flap: A Case Report.

INTRODUCTION: Accessory breast cancer is rare, with approximately two-thirds of cases arising in the axillary region. Wide local excision is commonly performed; however, large axillary defects can lead to postoperative scar contractures and limited range of motion in the ipsilateral upper limb. CASE PRESENTATION: A 67-year-old woman presented with a palpable mass and erythema in the left axilla. Physical examination revealed a 2-cm indurated lesion with erythema in the left axilla. Ultrasonography revealed an irregular 14-mm hypoechoic mass with suspected dermal invasion. Skin punch biopsy su

Radiologically detectable microcalcifications in histopathologically confirmed ductal carcinoma in situ: The impact of cohort composition on reported prevalence.

PURPOSE: This study aimed to determine the prevalence of radiologically detectable microcalcifications in histopathologically confirmed ductal carcinoma in situ (DCIS) using specimen radiography and to evaluate clinicopathologic factors associated with their presence. METHODS: A total of 1201 consecutive patients who underwent wire-guided localization for non-palpable breast lesions were retrospectively identified. Patients without histopathologically confirmed DCIS, those without specimen radiographs, and those who had received neoadjuvant systemic therapy were excluded. The final cohort inc

Immunohistochemical and Molecular Features Distinguishing Metaplastic Breast Carcinoma With Squamous Differentiation From Cutaneous Squamous Cell Carcinoma.

Metaplastic breast carcinoma (MBC) with squamous differentiation can often be diagnosed by routine histopathologic evaluation, but cutaneous squamous cell carcinoma (cSCC) may enter the differential diagnosis in selected pure squamous carcinomas involving the breast/chest, particularly in limited samples or cases lacking definitive epidermal connection, ductal carcinoma in situ, or complete clinical information. Determining the site of origin is clinically important because management strategies for breast and cutaneous carcinomas differ substantially. We sought to identify immunohistochemical

Management of Radial Scars and Complex Sclerosing Lesions of the Breast: To Excise or Not to Excise?

BACKGROUND: Radial scars (RS) and complex sclerosing lesions (CSL) are benign breast lesions often detected incidentally on imaging. Although historically managed with surgical excision, emerging evidence suggests that conservative management may be appropriate in selected patients. This study evaluated rates of upgrade and progression of RS and CSL and examined factors associated with surgical management and these outcomes. METHODS: A retrospective cohort study was conducted at two academic centers affiliated with McGill University. Adult patients diagnosed with RS or CSL on image-guided cor

Case Report: Spindle cell metaplastic breast carcinoma.

BACKGROUND: Spindle cell metaplastic breast carcinoma (SpCC) is an uncommon subtype of metaplastic breast carcinoma. It often shows a triple-negative phenotype and can closely mimic malignant phyllodes tumor, primary breast sarcoma, and other spindle-cell lesions of the breast. CASE PRESENTATION: A 56-year-old woman presented with a 5.4-cm right breast mass. Core needle biopsy showed high-grade spindle cell carcinoma. The tumor expressed CKAE1/AE3, CK8/18, EMA, vimentin, TRPS-1, and partial GATA3, and was negative for ER, PR, HER2, p40, p63, CK5/6, S-100, CD34, and ERG; the Ki-67 index was 85

CBCRisk-Mastectomy: A Risk Prediction Tool to Aid Contralateral Prophylactic Mastectomy Decision Making.

PURPOSE: Unilateral breast cancer (BC) patients scheduled for mastectomy often choose to undergo contralateral prophylactic mastectomy (CPM), despite substantial declines in contralateral breast cancer (CBC) risk in recent decades. Models predicting absolute risk of future CBC can aid informed decision-making about CPM. CBCRisk is an existing CBC absolute risk prediction model trained on unilateral BC patients regardless of whether they had mastectomy. Here we developed CBCRisk-Mastectomy, tailored specifically to BC patients scheduled for mastectomy and considering CPM. PATIENTS AND METHODS:

Age-related differences in psychological distress and persistent postsurgical pain after surgery for management of non-invasive breast lesions.

PURPOSE: Younger women who have undergone surgery for non-invasive breast lesions may experience heightened psychological distress and persistent postsurgical pain (PPSP). We investigated whether younger age was associated with worse PPSP outcomes, and whether depression or anxiety contributed to these associations. METHODS: This was a secondary analysis of a cross-sectional survey of women (N = 821; Mean age = 60, range:26-88) who underwent surgical management of ductal carcinoma in situ (DCIS) or high-risk lesions (atypical ductal hyperplasia, atypical lobular hyperplasia, lobular carcinoma

Malignant adenomyoepithelioma of the breast with contralateral homologous tumor and ductal carcinoma in situ: two case reports.

INTRODUCTION AND IMPORTANCE: Malignant adenomyoepithelioma (MAME) is a rare biphasic breast tumor with no standardized management guidelines. Co-occurrence with other breast malignancies is extremely rare. CASE INTRODUCTION: We report two cases with long-term follow-up: a 59-year-old female with left invasive MAME and contralateral intermediate-grade ductal carcinoma in situ, and a 62-year-old female with bilateral MAME. Both underwent bilateral mastectomy with sentinel lymph node biopsy (SLNB). CLINICAL DISCUSSION: MAME may co-occur with homologous or breast malignancies of ductal origin, s

Florid Lobular Carcinoma In Situ (F-LCIS) Showing Predominantly Hypoechoic Masses With Obscured Margins and No Attenuated Posterior Echoes on Ultrasound: A Case Report.

Florid lobular carcinoma in situ (F-LCIS) is a rare disorder and has no established image findings to date. A 63-year-old woman was referred to our hospital for the detailed examination of the abnormality on screening mammography. Mammography showed a focal asymmetric density in the left breast. Ultrasound showed multiple round/oval masses with obscured margins, predominant low internal echoes, and no attenuated posterior echoes. MRI of the masses showed low signals on T1-weighted images, high signals on fat-suppressed T2-weighted images, fast enhancement followed by a washout pattern, and pre

Management of High-Risk Breast Lesions: A Comprehensive Update.

High-risk breast lesions (HRLs) encompass a heterogeneous group of pathologies that coexist with an adjacent or synchronous malignancy, confer an increased risk of future breast cancer, or warrant surgical consideration because of upgrade potential or local behavior. Despite their clinical importance, management of HRLs remains variable. The American Society of Breast Surgeons (ASBrS) and the Society of Breast Imaging are collaborating to establish unified, evidence-based guidelines for managing these lesions. While these recommendations are currently in development, this review summarizes the

Advancing Evidence of the Associations between specific Benign Breast Diagnoses and Future Breast Cancer Risk.

BACKGROUND: Benign breast disease (BBD) increases breast cancer risk; however, associations between specific BBD diagnoses and breast cancer risk are insufficiently studied. METHODS: We analyzed 131,075 benign breast biopsy episodes from 1994-2022 identified from eight Breast Cancer Surveillance Consortium registries. Pathological diagnoses were classified into 38 BBDs within 4 categories. For comparison, women with no prior breast biopsy and a negative screening mammogram (N=1,599,694) were identified. Associations of BBDs with invasive breast carcinoma and ductal carcinoma in situ (DCIS) we

Germline cancer predisposition gene mutations and upgrade rate to breast cancer among patients with pleomorphic and florid lobular carcinoma in situ.

BACKGROUND: Pleomorphic lobular carcinoma in situ (PLCIS) and florid lobular carcinoma in situ (FLCIS) are uncommon entities with variable rates of upgrades. Germline pathogenic variant cancer predisposition genes (PVs) are associated with increased breast cancer risk and influences management recommendations. DESIGN: Patients diagnosed with PLCIS and FLCIS over a 24-year period were identified. Clinical, radiologic, pathologic, treatment, and genetic testing was extracted from the electronic health record. The immediate rate of upgrade to invasive breast cancer and subsequent recurrence rate

Evaluation of exogenous estrogen use in patients with lobular neoplasia and its role in subsequent breast cancer development.

BACKGROUND: There is a paucity of data regarding the impact of exogenous hormones on future breast cancer risk in the setting of lobular neoplasia. We sought to determine if patients with a history of exogenous hormone use who develop lobular neoplasia are more likely to develop future breast cancer. METHODS: Following institutional review board approval, a single-institution retrospective review was conducted of patients diagnosed with lobular carcinoma in situ or atypical lobular hyperplasia between January 2009 and December 2017. The use of exogenous hormones and future breast cancer were

Spatial Integration of Protein and Chromosomal States Reveals Early Copy-Number Changes and Genotype-Associated Immune Neighborhoods in Serous Ovarian Cancer Evolution.

UNLABELLED: Detecting chromosomal copy-number alterations together with protein-defined cell states in intact tissue is critical for understanding early clonal evolution and microenvironmental interactions in cancer. We developed ORION-FISH, which integrates high-plex tissue imaging with a morphology-preserving DNA fluorescence in situ hybridization (DNA-FISH) workflow and single-cell registration, yielding measurements concordant with clinical FISH. In high-grade serous ovarian carcinoma (HGSOC), ORION-FISH recapitulated known chromosomal changes while revealing subclonal heterogeneity missed

Flat epithelial atypia of the breast: a pragmatic, context-integrated diagnostic framework within a biological continuum.

Flat epithelial atypia (FEA) is a diagnostically challenging lesion within the spectrum of columnar cell alterations of the breast, occupying a borderline position between benign columnar cell change and flat type intermediate nuclear grade ductal carcinoma in situ (DCIS). Despite extensive study and incorporation into classification systems, its definition remains largely dependent on subjective morphological thresholds rather than outcome-driven or genomic criteria, contributing to persistent diagnostic variability. This review examines the diagnostic framework of FEA, emphasising the primac

Comparative Analysis of the International Academy of Cytology Yokohama System and the Modified Masood Scoring Index in Fine Needle Aspiration Cytology of Palpable Breast Lesions.

INTRODUCTION: Breast cancer has surpassed cervical cancer in India, underscoring the importance of screening and early detection. Triple testing, including clinical examination, mammography, and fine needle aspiration cytology (FNAC), is crucial for assessing suspicious breast lesions. FNAC, a simple and minimally invasive method, effectively diagnoses benign and malignant lesions, aiding treatment, though overlapping morphological features can sometimes cause diagnostic errors. MATERIALS AND METHODS: This retrospective study involves patients referred for FNAC to the Department of Pathology

Mammographic surveillance in breast cancer patients aged 50 years or older: a synopsis of the Mammo-50 RCT.

BACKGROUND: Annual surveillance mammograms for an unspecified period, after treatment for early breast cancer, are widely practised in the United States of America and Europe. Current UK guidelines recommend annual mammograms for 5 years, then reverts to 3-yearly screening. The aim of this trial was to evaluate whether less than annual mammography was non-inferior in terms of breast cancer-specific survival and cost-effectiveness in women aged 50 years or older at diagnosis and 3 years post curative surgery. METHODS: We conducted a multicentre, randomised phase III trial of annual mammography
