Preoperative MRI for pleomorphic lobular carcinoma in situ: Implications for surgical management.

BACKGROUND: Pleomorphic lobular carcinoma in situ (PLCIS), a more aggressive variant of classic LCIS (CLCIS), demonstrates higher rates of upgrade to invasive carcinoma at surgical excision. Mammography may underestimate disease extent, whereas MRI may delineate lesions due to sensitivity. METHODS: Our institutional pathology database was queried for patients diagnosed with PLCIS on core needle biopsy (CNB) from 2002 to 2025 who underwent surgical excision. Preoperative MRI and MRI-guided biopsies were reviewed. Radiologic, pathologic, surgical, and outcome data were collected. Proportions we

Incidence and Treatment Patterns of Pleomorphic Lobular Carcinoma in Situ: A Population-based Analysis After the Introduction of ICD-O-3 Code 8519/2.

INTRODUCTION: A dedicated morphology code (ICD-O-3 8519/2), introduced in 2018, made population-level description of PLCIS possible for the first time. PATIENTS AND METHODS: Women with lobular carcinoma in situ diagnosed during 2018 to 2023 were identified in the SEER 17-registry file; incidence came from the 21-registry file. Treatment was compared by logistic regression adjusted for age, race and ethnicity, and diagnosis year. RESULTS: Of 11,170 women, 288 (2.6%) had PLCIS. Age-adjusted incidence was 0.08 per 100,000 woman-years (classic, 4.68), rising 8.7% annually and peaking at ages 65

Clinical Outcomes and Chemoprevention in High-Risk Breast Lesions: A Comparison of Atypical Ductal Hyperplasia and Lobular Carcinoma In Situ.

BACKGROUND: Although atypical ductal hyperplasia (ADH) and lobular carcinoma in situ (LCIS) are both high-risk breast lesions, their management differs in clinical practice. We compared subsequent breast cancer outcomes between the 2 groups. METHODS: We retrospectively reviewed patients with surgically excised ADH or LCIS treated between January 2008 and December 2025. Patients with concurrent breast cancer were excluded. ADH cases upgraded to ductal carcinoma in situ (DCIS) or invasive carcinoma at excision were analyzed separately and excluded from the primary analysis. The primary endpoint

Pleomorphic and florid lobular carcinoma in situ of the Breast: A systematic review of current evidence and knowledge gaps.

INTRODUCTION: Pleomorphic (PLCIS) and florid (FLCIS) lobular carcinoma in situ are uncommon entities, characterized by significant architectural distortion and cellular atypia. Their rarity poses three key clinical challenges: diagnostic variability, histologic upgrade and risk of local recurrence (LR). Currently, no standardized management guidelines exist. This systematic review provides the most comprehensive synthesis to date of the available evidence on clinical, radiologic, pathologic, and molecular characteristics of P/FLCIS, and evaluates outcomes associated with different treatment st

Breast cancer genetics and risk assessment 101: What women's health care providers need to know.

Breast cancer affects the lives of many women and their loved ones. Women's health practitioners have an important role in identifying women at increased risk for breast cancer and guiding them toward appropriate counseling, imaging surveillance, and preventive care. Familial cancer syndromes account for a substantial proportion of breast cancer diagnoses. When family history or other clinical features suggest a hereditary predisposition, practitioners should consider recommending genetic testing and genetic counseling to help mitigate risk. The U.S. National Comprehensive Cancer Network offer

Adherence to Endocrine Prevention in Patients with Atypical Hyperplasia and Lobular Carcinoma In Situ: Promising Trends from Real-World Use of Low-Dose Tamoxifen.

BACKGROUND: Endocrine prevention is a well-established preventative strategy for women with high-risk lesions (HRL). The objective of the study was to evaluate factors associated with endocrine prevention adherence in this patient population. PATIENTS AND METHODS: We performed a retrospective cohort study of all women referred for atypical ductal and lobular hyperplasia (ADH/ALH) and lobular carcinoma in situ (LCIS) between 2019 and 2025. Data pertaining to eligibility, initiation, and adherence to endocrine prevention were extracted with univariate analyses performed to evaluate factors asso

Comparative analysis of clinicopathological characteristics and prognosis between E-cadherin-negative breast lymphoepithelioma-like carcinoma and E-cadherin-negative invasive lobular carcinoma.

BACKGROUND: E-cadherin-negative breast lymphoepithelial-like carcinoma (E-Cad-neg BLEC) is a rare malignant tumor. To date, no study has compared the clinicopathological characteristics and prognosis between E-Cad-neg BLEC and E-Cad-neg invasive lobular carcinoma (ILC). METHODS: To clarify the relationship between E-Cad-neg BLEC and E-Cad-neg ILC, we analyzed 12 patients with E-Cad-neg BLEC, comparing their clinicopathological characteristics and tumor prognosis with those of 578 patients with E-Cad-neg classical invasive lobular carcinoma (c-ILC) or 43 patients with E-Cad-neg pleomorphic lob

Adherence to and tolerance of low-dose tamoxifen versus standard-dose tamoxifen in women at increased risk of breast cancer.

OBJECTIVE: Tamoxifen 20 mg daily for 5 years decreases breast cancer (BC) risk by 49% in women with a family history of the disease and by up to 85% in women with a personal history of high-risk lesions. The TAM-01 study demonstrated that patients with ductal carcinoma in situ (DCIS), lobular carcinoma in situ (LCIS), atypical ductal hyperplasia (ADH) or atypical lobular hyperplasia (ALH) had a 50% reduction in cancer events with 5 mg daily for 3 years. We aimed to determine whether the use of low-dose tamoxifen (LDT) leads to better adherence and tolerance than standard-dose tamoxifen (SDT).

A pilot study of a behavior-change intervention for preventive therapy among women at high risk for invasive breast cancer.

BACKGROUND: The use of preventive therapy in women at high risk for invasive breast cancer is associated with a significant reduction in breast cancer risk, however, uptake remains suboptimal outside of clinical trials. We conducted a pilot study of a behavior-change intervention to encourage high-risk women to take preventive therapy. METHODS: Women aged 35–69 with a history of lobular carcinoma in situ (LCIS) or atypical hyperplasia (AH) receiving care at MD Anderson Cancer Center, Houston, Texas, were eligible. Following cognitive testing, the tool was field-tested using a pre-post design,

Axillary Accessory Breast Cancer Performed Reconstruction Using a Local Flap: A Case Report.

INTRODUCTION: Accessory breast cancer is rare, with approximately two-thirds of cases arising in the axillary region. Wide local excision is commonly performed; however, large axillary defects can lead to postoperative scar contractures and limited range of motion in the ipsilateral upper limb. CASE PRESENTATION: A 67-year-old woman presented with a palpable mass and erythema in the left axilla. Physical examination revealed a 2-cm indurated lesion with erythema in the left axilla. Ultrasonography revealed an irregular 14-mm hypoechoic mass with suspected dermal invasion. Skin punch biopsy su

Management of Radial Scars and Complex Sclerosing Lesions of the Breast: To Excise or Not to Excise?

BACKGROUND: Radial scars (RS) and complex sclerosing lesions (CSL) are benign breast lesions often detected incidentally on imaging. Although historically managed with surgical excision, emerging evidence suggests that conservative management may be appropriate in selected patients. This study evaluated rates of upgrade and progression of RS and CSL and examined factors associated with surgical management and these outcomes. METHODS: A retrospective cohort study was conducted at two academic centers affiliated with McGill University. Adult patients diagnosed with RS or CSL on image-guided cor

CBCRisk-Mastectomy: A Risk Prediction Tool to Aid Contralateral Prophylactic Mastectomy Decision Making.

PURPOSE: Unilateral breast cancer (BC) patients scheduled for mastectomy often choose to undergo contralateral prophylactic mastectomy (CPM), despite substantial declines in contralateral breast cancer (CBC) risk in recent decades. Models predicting absolute risk of future CBC can aid informed decision-making about CPM. CBCRisk is an existing CBC absolute risk prediction model trained on unilateral BC patients regardless of whether they had mastectomy. Here we developed CBCRisk-Mastectomy, tailored specifically to BC patients scheduled for mastectomy and considering CPM. PATIENTS AND METHODS:

Age-related differences in psychological distress and persistent postsurgical pain after surgery for management of non-invasive breast lesions.

PURPOSE: Younger women who have undergone surgery for non-invasive breast lesions may experience heightened psychological distress and persistent postsurgical pain (PPSP). We investigated whether younger age was associated with worse PPSP outcomes, and whether depression or anxiety contributed to these associations. METHODS: This was a secondary analysis of a cross-sectional survey of women (N = 821; Mean age = 60, range:26-88) who underwent surgical management of ductal carcinoma in situ (DCIS) or high-risk lesions (atypical ductal hyperplasia, atypical lobular hyperplasia, lobular carcinoma

Florid Lobular Carcinoma In Situ (F-LCIS) Showing Predominantly Hypoechoic Masses With Obscured Margins and No Attenuated Posterior Echoes on Ultrasound: A Case Report.

Florid lobular carcinoma in situ (F-LCIS) is a rare disorder and has no established image findings to date. A 63-year-old woman was referred to our hospital for the detailed examination of the abnormality on screening mammography. Mammography showed a focal asymmetric density in the left breast. Ultrasound showed multiple round/oval masses with obscured margins, predominant low internal echoes, and no attenuated posterior echoes. MRI of the masses showed low signals on T1-weighted images, high signals on fat-suppressed T2-weighted images, fast enhancement followed by a washout pattern, and pre

Management of High-Risk Breast Lesions: A Comprehensive Update.

High-risk breast lesions (HRLs) encompass a heterogeneous group of pathologies that coexist with an adjacent or synchronous malignancy, confer an increased risk of future breast cancer, or warrant surgical consideration because of upgrade potential or local behavior. Despite their clinical importance, management of HRLs remains variable. The American Society of Breast Surgeons (ASBrS) and the Society of Breast Imaging are collaborating to establish unified, evidence-based guidelines for managing these lesions. While these recommendations are currently in development, this review summarizes the

Advancing Evidence of the Associations between specific Benign Breast Diagnoses and Future Breast Cancer Risk.

BACKGROUND: Benign breast disease (BBD) increases breast cancer risk; however, associations between specific BBD diagnoses and breast cancer risk are insufficiently studied. METHODS: We analyzed 131,075 benign breast biopsy episodes from 1994-2022 identified from eight Breast Cancer Surveillance Consortium registries. Pathological diagnoses were classified into 38 BBDs within 4 categories. For comparison, women with no prior breast biopsy and a negative screening mammogram (N=1,599,694) were identified. Associations of BBDs with invasive breast carcinoma and ductal carcinoma in situ (DCIS) we
