Three-dimensional dynamic optical coherence tomography for breast tumor margin assessment.

Intraoperative margin assessment techniques are needed to reduce the re-excision rate in breast-conserving surgery. Optical coherence tomography (OCT) is a non-invasive imaging technique capable of rapid three-dimensional (3-D) imaging of the internal microstructure of tissues. However, there is often low contrast between morphological features in breast tissue. Dynamic OCT (d-OCT), which provides additional contrast derived from the temporal variance of the OCT signal caused by intrinsic motion within the tissue, may provide a solution. However, few studies have applied it to breast tumor mar

Abnormal breast pathology after breast reduction surgery: A case-series and a 10-year retrospective chart review.

BACKGROUND: Incidental abnormal breast pathology identified by histological analysis following breast reduction surgery is a rare complication. This may vary from benign and premalignant pathologies to invasive breast carcinoma. Such unexpected diagnoses can pose a challenge for plastic surgeons. Few studies have examined the treatment course for this patient population. We aimed to determine whether patients with incidental breast pathology following breast reduction mammoplasty were more likely to be treated conservatively or more aggressively through prophylactic mammoplasty. METHODS: A re

Real-World Application of Alliance ACOSOG Z11102: How Many Patients Can be Spared Mastectomy?

BACKGROUND: ACOSOG Z11102 demonstrated that breast-conserving surgery (BCS) with radiation is safe for multiple ipsilateral breast cancers (MIBCs), with re-excision and mastectomy conversion rates of 32.4% and 7.1%, respectively. Our objective was to evaluate the applicability of ACOSOG Z11102 in real-world practice. METHODS: A retrospective review of MIBC patients was performed to collect clinical and demographic information. Pathology was re-reviewed by two breast pathologists assessing distance between foci and presence of significant pathology in intervening tissue (incidental invasive ca

Long-term follow-up confirms patients with high-risk breast lesions can be successfully managed at a multidisciplinary conferences.

BACKGROUND: The management of high-risk breast lesions is controversial. There is a lack of long-term follow-up studies to evaluate clinical management decisions. METHODS: We included 267 consecutive high-risk breast lesions with pathology-radiology concordance that were prospectively recommended for surgery or follow-up at a multidisciplinary conference. The 267 lesions included 149 papillomas and 118 other high-risk lesions. The 149 papillomas included 119 benign papillomas, 17 atypical papillomas, 6 papillomas with adjacent atypical ductal hyperplasia (ADH), 7 papillomas with adjacent atyp

Breast Cancer Polygenic Risk Score Associated with Outcomes after In Situ Breast Disease.

BACKGROUND: Ductal carcinoma in situ (DCIS) and lobular carcinoma in situ (LCIS) are preinvasive breast lesions. DCIS is treated more aggressively, as it is more likely to develop into invasive disease than LCIS. Both DCIS and LCIS face an elevated risk of contralateral breast cancer. It is thus important to identify those at high risk of further breast disease in order to personalize treatment. METHODS: This study evaluated whether the 313-SNP breast cancer polygenic risk score (PRS313) can predict the likelihood of developing ipsilateral or contralateral breast cancer after diagnosis of DCI

A systematic review and meta-analysis of imaging characteristics and upgrade rates in noninvasive lobular neoplasia of the breast.

BACKGROUND: Non-invasive lobular neoplasia (NLN) encompasses a range of lobular breast lesions that may precede invasive breast cancer. Microcalcifications detected through mammography play a crucial role in evaluating breast lesions and are often associated with NLN. This study focuses on the prevalence and significance of microcalcifications in NLN, noting that they can be the sole radiographic finding in many cases. While mammography is highly sensitive for detecting microcalcifications, it has limitations in diagnosing NLN, as some lesions may not show up on scans. Advanced imaging techniq

High-Risk Breast Lesions on Core Biopsy: Rates of Upgrade and Management Recommendations.

High-risk lesions of the breast confer an increased risk for breast cancer. Due to wide variations in the reported rate of upgrade to carcinoma at excision, optimal management of these lesions is controversial. Intraductal papillomas, mucocele-like lesions, radial scars, and complex sclerosing lesions without atypia found on core biopsy do not require excision. Flat epithelial atypia and classic lobular neoplasia can be managed conservatively while nonclassic forms of lobular carcinoma in situ should undergo excision. In most cases, atypical ductal hyperplasia should be surgically excised, tho

Impact of concurrent lobular carcinoma in situ on recurrence outcomes in patients with classic and pleomorphic invasive lobular carcinoma of the breast.

PURPOSE: We investigated outcomes of invasive lobular carcinoma (ILC) with or without concurrent lobular carcinoma in situ (LCIS) in patients with classic or pleomorphic ILC. METHODS: We retrospectively analyzed a single-institution database of patients with stage I-III ILC. We compared tumor features, treatment, and recurrence free survival (RFS) in patients with ILC-alone versus ILC + LCIS stratified by ILC tumor subtype. Multivariable Cox proportional hazards models were used for multivariate analysis. RESULTS: Of the 786 cases of ILC, 542 were classic and 92 were pleomorphic, with 70.6%

A Mendelian randomization study investigating the causal associations of 35 blood and urinary metabolite biomarkers with breast cancer development.

Breast cancer is the most common cancer among women worldwide. The present study utilized a bidirectional Mendelian randomization methodology to explore the causal associations between 35 blood and urinary metabolic markers and distinct subtypes of breast cancer. The MR-PRESSO method was employed to detect and correct for pleiotropic effects, with statistical significance adjusted using the false discovery rate (FDR). The findings revealed significant causal links between certain metabolic markers and specific breast cancer subtypes. Specifically, glucose (OR: -0.187; 95% CI: -0.344 to -0.030)

Electrical characteristics of the extracellular fluid in the body segments of Apis mellifera bees.

INTRODUCTION: The use of conventional imaging techniques, namely mammography (MMG) and ultrasound (US), for breast cancer (BC) detection in women at high risk for the disease does not bring optimal results in many cases. AIM: The present study evaluated the effectiveness of magnetic resonance (MR) mammography (MRM) in cases where US and MMG failed to detect suspected breast lesions. MATERIAL AND METHODS: The study group consisted of 379 women who had had no breast pathologies detected by US and MMG. This group was then divided into 4 groups according to the relative risk of breast cancer dev

A Novel Method of Management of High Output Chyle Leak Following Neck Dissection: Indocyanine Green Fluorescence-guided Robotic Transthoracic Thoracic Duct Ligation: A Case Series.

Diabetes mellitus is a chronic metabolic disorder characterized by persistent hyperglycaemia and is associated with multiple systemic complications. Chronic hyperglycaemia and related metabolic disturbances can impair bone remodelling and compromise bone quality and microarchitecture, increasing skeletal vulnerability. The effects of different antidiabetic treatment combinations on diabetes-associated skeletal alterations remain insufficiently characterized. We investigated the effects of canagliflozin alone and in combination with metformin and fenugreek seed extract on glycaemic control and

Altered cytokeratin 5 expression in breast lobular myoepithelial cells.

AIMS: Cytokeratin 5 (CK5) is a surrogate maker of progenitor cells and early glandular and myoepithelial cells (MECs) in the breast, and CK5 expression in breast MECs varies from ducts to lobules, and from normal to diseased tissue. However, the mechanisms underlying immunophenotypic alterations of CK5 expression in MECs remain unclear. METHODS: CK5 expression in MECs of 20 normal breast samples, 58 ductal carcinoma in situ (DCIS; including 21 DCIS with extensive lobular involvement), 11 atypical ductal hyperplasia (ADH), 18 non-invasive lobular neoplasia consisting of 11 atypical lobular hyp

Upgrade Rates and Breast Cancer Development Among Germline Pathogenic Variant Carriers with High-Risk Breast Lesions.

BACKGROUND: High-risk lesions (HRL) of the breast are risk factors for future breast cancer development and may be associated with a concurrent underlying malignancy when identified on needle biopsy; however, there are few data evaluating HRLs in carriers of germline pathogenic variants (PVs) in breast cancer predisposition genes. METHODS: We identified patients from two institutions with germline PVs in high- and moderate-penetrance breast cancer predisposition genes and an HRL in an intact breast, including atypical ductal hyperplasia (ADH), flat epithelial atypia (FEA), and lobular neoplas

Postlumpectomy imaging: is there a role for the study of the contralateral breast?-a retrospective cohort.

BACKGROUND: Some patients with breast cancer submitted to breast-conserving surgery might benefit from a postlumpectomy imaging examination previously to radiation therapy. This aims to document the complete removal of cancer and might be accomplished using mammogram with breast and axillary ultrasonography. These modalities study not only the affected side but also the contralateral side. In fact, it is well-documented that women with breast cancer have an increased risk for contralateral breast cancer. Thus, we intended to evaluate the value of postlumpectomy imaging undertaken before adjuva

Upgrade Rates of Variant Lobular Carcinoma In Situ Compared to Classic Lobular Carcinoma In Situ Diagnosed in Core Needle Biopsies: A 10-Year Single Institution Retrospective Study.

The primary aim of this study was to determine the upgrade rates of variant lobular carcinoma in situ (V-LCIS, ie, combined florid [F-LCIS] and pleomorphic [P-LCIS]) compared with classic LCIS (C-LCIS) when diagnosed on core needle biopsy (CNB). The secondary goal was to determine the rate of progression/development of invasive carcinoma on long-term follow-up after primary excision. After institutional review board approval, our institutional pathology database was searched for patients with "pure" LCIS diagnosed on CNB who underwent subsequent excision. Radiologic findings were reviewed, rad

A dedicated structured data set for reporting of invasive carcinoma of the breast in the setting of neoadjuvant therapy: recommendations from the International Collaboration on Cancer Reporting (ICCR).

AIMS: The International Collaboration on Cancer Reporting (ICCR), a global alliance of major (inter-)national pathology and cancer organisations, is an initiative aimed at providing a unified international approach to reporting cancer. ICCR recently published new data sets for the reporting of invasive breast carcinoma, surgically removed lymph nodes for breast tumours and ductal carcinoma in situ, variants of lobular carcinoma in situ and low-grade lesions. The data set in this paper addresses the neoadjuvant setting. The aim is to promote high-quality, standardised reporting of tumour respon

Immediate and delayed risk of breast cancer associated with classic lobular carcinoma in situ and its variants.

OBJECTIVE: To determine the risk of breast cancer due to lobular carcinoma in situ (LCIS). METHODS: This retrospective IRB-approved study identified cases of LCIS after percutaneous breast biopsy from 7/2005 to 7/2022. Excluded were cases with less than 2 years of imaging surveillance or a concurrent ipsilateral breast cancer diagnosis within 6 months of the LCIS diagnosis. Final outcomes of cancer versus no cancer were determined by pathology at surgical excision or the absence of cancer on imaging surveillance. RESULTS: A total of 116 LCIS lesions were identified. The primary imaging findi

Surgical outcomes of total duct excision in the diagnosis and management of nipple discharge.

INTRODUCTION: Total duct excision (TDE) is performed for the diagnosis and management of nipple discharge. The Association of Breast Surgery's recent guidelines recommend considering diagnostic surgery for single-duct, blood-stained or clear nipple discharge, and for symptomatic management. METHODS: We retrospectively reviewed the diagnostic and surgical outcomes of all cases of TDE between January 2013 and November 2019. RESULTS: In total, 259 TDEs were carried out: 219 for nipple discharge, 29 for recurrent mastitis, 3 for screening abnormalities and 8 for breast lumps. Of the nipple disch
