Bisphosphonates in breast cancer radiotherapy: associations with skeletal complications and treatment outcomes - a retrospective cohort study.

INTRODUCTION: Breast cancer is the most diagnosed cancer in women worldwide. Radiotherapy is a cornerstone of treatment for both early-stage ductal carcinoma in situ (DCIS) and metastatic disease, but concerns about side effects remain. Bisphosphonates (BPs), particularly zoledronic acid (ZOL), have shown anti-tumor effects in preclinical models and reduce skeletal complications in patients with bone metastases. This study aimed to evaluate the radiosensitizing potential of BPs and their relationship with skeletal complications in breast cancer patients undergoing radiotherapy. METHODS: We co

Are breast screening recall rates associated with body mass index? Analysis of data from a population-based screening program.

BACKGROUND: Increased body mass index (BMI) is associated with increased risk of post-menopausal breast cancer, but little is known about its association with other key screening outcomes. This study investigates the association between BMI and the likelihood of being recalled after routine screening mammography. METHODS: To describe the associations between recall rates (crude and age-standardized) and BMI, descriptive statistics were estimated from screening events from clients attending the population-based BreastScreen Western Australia program. For those recalled, multivariable logistic

The landscape of the intestinal microbiome among patients with newly diagnosed invasive breast cancer and ductal carcinoma in situ (DCIS).

The intestinal microbiome shapes immune responses and is associated with patient outcomes in cancer following immunotherapy. We evaluated differences between the intestinal microbiome profiles of patients with early-stage invasive breast cancer (BC) and ductal carcinoma in situ (DCIS) by subtype using whole genome metagenomic sequencing. There were no significant differences in microbiome composition between DCIS and invasive BC as measured by alpha diversity (p = 0.20, ANOVA) or beta diversity (p = 0.52, PERMANOVA). Within invasive BC, patients with hormone receptor-positive (HR + )/HER2 + BC

Recurrence Rate Following Breast Conservation Surgery: A Retrospective Cohort Study.

BACKGROUND: Breast-conserving surgery (BCS) has become the standard treatment for early-stage breast cancer, offering oncological safety, better breast aesthetics, and enhanced health-related quality of life. The optimal margins for BCS have been debated. However, evidence on the impact of margin widths greater than 0 mm for invasive breast cancer remains limited. Cambridge Breast Unit (CBU) has adopted a policy where a single margin of < 1 mm (not at ink) for invasive disease would be acceptable, provided the other three radial margins are ≥ 1 mm. METHOD: Retrospective cohort study analyzed

Diagnostic and therapeutic challenges in mammary and extra-mammary Paget disease: a retrospective case series.

Mammary and extramammary Paget disease (PD; MPD, EMPD) is a rare neoplastic disorder characterised by intraepithelial infiltration of malignant cells with glandular differentiation. Diagnosis of PD remains challenging, as its clinical presentation frequently mimics inflammatory diseases or other cutaneous neoplasms. It is estimated that approximately 10-15 % of EMPD cases and up to 50 % of MPD cases are invasive at the time of definitive diagnosis, largely due to diagnostic delays that may last several years. In this retrospective case series, we analysed non-pigmented EMPD (n = 8) and MPD (n

Invasive Ductal Carcinoma of the Breast With an Uncommon Widespread Thick Hyperechoic Pattern: A Case Report.

A 77-year-old woman visited our hospital due to a palpable breast mass. Mammography only showed a focal asymmetric density. In addition to one normal-sized lymph node with a punctate hyperechoic pattern, ultrasound showed widespread, thick hyperechoic areas with indistinct margins and adjacent hypoechoic areas. The hypoechoic areas had both an irregular shape and focal internal punctate high echoes on grayscale ultrasound, as well as blood inflow on Doppler ultrasound. Magnetic resonance imaging (MRI) of the mass showed weak high signals on fat-suppressed T2-weighted images and slow initial en

Evaluation of a Simplified Technique for the Excision of Non-palpable Breast Cancer.

Breast cancer is the most common malignancy and the leading cause of cancer-related mortality among women, with early detection through screening programs significantly improving survival rates and reducing mortality. Accurate localization and excision of early-stage non-palpable lesions minimize the need for re-excision, prevent unnecessary removal of breast tissue, and improve cosmetic outcomes. This study aimed to optimize the wire localization technique for the excision of non-palpable breast cancer. A total of 46 female patients referred to the breast unit at Kasr Al-Ainy, Cairo Universit

Categorization of the Histopathological Diagnosis of Breast Core Needle Biopsies and the Correlation of the Risk of Malignancy With Diagnostic Accuracy.

INTRODUCTION: Core needle biopsy (CNB) is used to detect carcinoma of the breast, but it does not always provide a definitive diagnosis. Cases can be categorized into diagnostic categories to facilitate further management decisions. The United Kingdom's National Health Service Breast Screening Program uses five "B categories" for reporting CNBs of the breast. These categories can help bring uniformity to the reporting of biopsies. MATERIALS AND METHODS: Biopsy cores were assigned to one of the five B categories (B1-B5). Biopsy diagnoses were then correlated with the diagnoses from the resecti

Ductoscopy Facilitates Surgical Decision-Making in Management of Patients with Pathological Nipple Discharge.

Background/Objectives: We investigated the feasibility of ductoscopy in diagnosis and management of patients presenting with clinically pathological nipple discharge (PND). Methods: Mammary ductoscopy was performed on 57 breasts with PND in 54 patients for diagnostic and therapeutic purposes. Ductoscopic abnormalities included ductal irregularities, presence of erythematous patches, or presence of intraductal papillomas, whereas duct ectasia or presence of dense fluid was considered a benign finding. Results: Age older than 40 and any pathology in ultrasound or ductoscopy were significantly as

Artificial intelligence-assisted three-dimensional imaging of breast microinvasive carcinoma reveals larger invasive focus size in a substantial proportion of cases.

Microinvasive carcinoma of breast is a unique type of malignancy characterized by the presence of small invasive foci (less than 1 mm in diameter) in a background of carcinoma in situ. The disease is the earliest stage of breast invasive carcinoma development, and patients diagnosed with this disease are often treated conservatively. However, diagnosing microinvasive carcinoma based on a single tissue section may underestimate the invasive focus size. We developed a three-dimensional (3D) imaging method to re-evaluate the invasive focus size in microinvasive carcinoma cases in which the origin

Risk factors for ductal carcinoma in situ: comparisons with invasive breast cancer.

BACKGROUND: Ductal carcinoma in situ (DCIS) is a precursor of invasive breast cancer; however, the majority of DCIS tumors do not progress to invasive breast cancer. Elucidating shared and distinct risk factors for DCIS and invasive breast cancer may provide novel insights into the pathogenesis of breast cancer. METHODS: The Multiethnic Cohort Study was used to examine whether known risk factors for invasive breast cancer are associated with DCIS among a racially and ethnically diverse U.S. population of women. RESULTS: African American (HR 1.49, 95% CI 1.24-1.78), Native Hawaiian (HR 1.50,

Tumor-Infiltrating Mast Cells Enhance Neoadjuvant Therapy Efficacy in ESCC via Modulation of the Desmoplastic Microenvironment.

OBJECTIVE: Neoadjuvant therapy is essential for locally advanced esophageal squamous cell carcinoma (ESCC), but its efficacy requires improvement. This study investigated the role of tumor-infiltrating mast cells (MCs) in treatment response and prognosis. METHODS: An integrated approach was employed, combining single-cell RNA sequencing of paired specimens from one ESCC patient pre- and post-neoadjuvant therapy, immunofluorescence analysis of a retrospective cohort of 68 treatment-naïve ESCC surgical specimens, bioinformatics analysis of public datasets, and in vitro mast cell activation assa

Correlates of Ki-67 proliferation index in a cohort of women with suspected breast cancer in Lusaka, Zambia.

Ki-67 is a key biomarker of tumor proliferation in breast cancer, yet its clinical correlates in this population, where disease is often detected at earlier stages, remain underexplored. This study aimed to identify factors independently associated with high Ki-67 expression among women investigated for suspected breast cancer at Unilabs Laboratory, Lusaka, Zambia. A retrospective cross-sectional analysis was conducted on 208 women suspected of having breast cancer through a laboratory-based screening program in Lusaka, Zambia (2019-2024). Demographic, clinical, and pathology data were extract

Multiparametric Breast MRI for the Differentiation of Atypical Ductal Hyperplasia and Ductal Carcinoma In Situ in Postmenopausal Women.

Introduction Atypical ductal hyperplasia (ADH) is a proliferative epithelial breast lesion associated with an increased risk of progression to ductal carcinoma in situ (DCIS) or invasive carcinoma. Although breast MRI is highly sensitive for lesion detection, its ability to differentiate biologically related non-mass enhancement (NME) lesions such as ADH and DCIS remains incompletely defined. Aim The aim of this exploratory study was to investigate whether predefined morphologic, dynamic, and functional breast MRI parameters, such as lesion size, postcontrast signal intensity increase (wash-in

Vacuum-assisted breast biopsy vs core needle biopsy: a systematic review and meta-analysis.

OBJECTIVES: Vacuum-assisted breast biopsy (VABB) and core needle biopsy (CNB) are percutaneous biopsy methods used for the assessment of suspicious breast lesions. This systematic review and meta-analysis focused on comparative diagnostic performance outcomes of lesions biopsied with VABB or CNB. MATERIALS AND METHODS: Studies comparing VABB to CNB were searched in PubMed and Cochrane Library. Pooled risk ratios (RR) with 95% CI using random-effects models were calculated for atypical ductal hyperplasia (ADH) and ductal carcinoma in situ (DCIS) underestimation rates, repeat biopsy rate, conco

Binary and Ternary Classification Prediction for Breast Cancer and Breast Sclerosing Adenosis With Interpretable Artificial Intelligence From Clinical and Imaging Features: A Retrospective, Diagnostic Accuracy Cohort Study.

BACKGROUND: Sclerosing adenosis (SA) and breast cancer (BC) often exhibit overlapping clinical, imaging, and pathological characteristics, making them difficult to differentiate. SA may also coexist with BC (SA + BC), including ductal carcinoma in situ (SA-DCIS) and invasive breast cancer (SA-IBC), which complicates diagnosis even when core-needle biopsy (CNB) suggests SA. This study aimed to develop interpretable AI-based binary and ternary classification models that leverage clinical and imaging features to distinguish SA-only from SA + BC and to further differentiate among SA-only, SA-DCIS,

Multiple Cystic Components With Bleeding Are Important Diagnostic Findings for Encapsulated Papillary Carcinomas of the Breast: A Case Report.

Encapsulated papillary carcinoma (EPC) is a rare papillary breast lesion with known favorable biology, but is not fully understood of its image findings. A 78-year-old woman with a screening mammography-detected breast mass was referred to our hospital. Mammography showed an oval mass, 4.3 cm in size, with circumscribed margins in her left breast. Ultrasound showed that the mass consisted of high-echo areas and many cystic areas. Magnetic resonance imaging (MRI) of the mass showed weak high signals in some areas on T1-weighted images and mixed high and low signals with fluid-fluid level format

Computer-Aided Quantitative Image Analysis of Multiple Human Epidermal Growth Factor Receptor 2 Immunohistochemistry Assays in Breast Carcinoma.

CONTEXT.—: Quantitative image analysis (QIA) is increasingly applied for immunohistochemistry (IHC)-based biomarker assessment in pathology. With the introduction of drugs targeting breast carcinomas (BCs) with low human epidermal growth factor receptor 2 (HER2) expression levels, a need for more accurate and reproducible HER2 scoring is warranted. QIA has the potential to increase HER2 IHC scoring reproducibility. However, QIA reproducibility should be agnostic and uninfluenced by different IHC assays. OBJECTIVE.—: To compare HER2 scoring accuracy and reproducibility by expert pathologists a

Development and Validation of an Algorithm to Achieve Complete Registration of Danish Patients with Carcinoma In Situ of the Breast, 2008-2023.

PURPOSE: Registration of carcinoma in situ (CIS) lesions in the national Danish Breast Cancer Group (DBCG) database is incomplete. We aimed to develop and validate a computer-based algorithm designed to identify patients with pure CIS of the breast, defined as CIS without concurrent breast cancer (BC), and with no history of BC or ipsilateral CIS. A secondary aim was to assess the completeness of CIS registration in the DBCG database. PATIENTS AND METHODS: We developed an algorithm to identify patients diagnosed with pure CIS between 2008 and 2017 using Systematized NOmenclature of MEDicine c
