Minimal residual disease in multiple myeloma: the movement towards response-adapted therapy.

In recent years, minimal residual disease (MRD) testing has emerged as a powerful prognostic tool in multiple myeloma. Multiple meta-analyses have demonstrated that achieving MRD negativity is associated with significantly improved progression-free survival (PFS), leading to its acceptance as a surrogate endpoint in clinical trials to support accelerated drug approval. Looking forward, MRD status is poised to be incorporated into treatment decision-making; to guide escalation and de-escalation of therapy with the aims of improved progression free survival with limited treatment toxicity. Complicated hepatic cyst with an incidental pelvic hydatid cyst: a case report.

INTRODUCTION: Hydatid disease, a zoonosis caused by Echinococcus granulosus, most commonly affects the liver and lungs. Primary pelvic localization is exceptionally rare (< 2% of cases) and poses significant diagnostic challenges, particularly in patients without classic epidemiological risk factors. This case report highlights the complexities of managing such an atypical presentation. CASE PRESENTATION: A 56-year-old Arab man from an urban area, with a history of mechanical aortic valve replacement and treatment with warfarin, presented with acute abdominal pain and fever. Imaging revealed

Whole-Genome Sequencing and Single-Cell Transcriptomics Identify Human Leukocyte Antigen Loci and a COG5 Variant in Aortic Dissection.

BACKGROUND: Aortic dissection (AD) is a lethal cardiovascular emergency with a complex and underexplored genetic basis. METHODS: We performed whole-genome sequencing on 151 patients with sporadic Stanford type B AD and 965 healthy controls in a Chinese population as a discovery phase analysis to assess both common and rare variants. Cross-ancestry meta-analyses incorporating 4 external genome-wide association data sets were used to identify population-specific risk loci. To pinpoint functional candidates, we integrated epigenomic profiles and single-cell transcriptomes from human aortic tissu

Advancing Heart Failure Classification Beyond Left Ventricular Ejection Fraction: Unsupervised Machine Learning Insights From the Japanese Nationwide Acute Decompensated Heart Failure Cohort: JROADHF.

BACKGROUND: Heart failure (HF) classification based on left ventricular ejection fraction (LVEF) inadequately captures HF heterogeneity, limiting phenotype-specific therapies. While machine learning (ML) has been applied to HF with preserved ejection fraction, comprehensive approaches across the full LVEF spectrum remain scarce. This study aimed to identify HF phenogroups across the entire LVEF spectrum using unsupervised ML, moving beyond the LVEF-centric and fragmented understanding of HF. METHODS: We analyzed 13 238 patients with acute HF from a nationwide, multicenter Japanese cohort (JRO

Cardiac Myosin-Binding Protein C for Diagnosis of Myocardial Infarction in Patients With Prior Coronary Artery Bypass Grafting.

BACKGROUND: Diagnosing myocardial infarction in patients with prior coronary artery bypass grafting (CABG) remains challenging, primarily because of lower diagnostic accuracy of high-sensitivity cardiac troponin (hs-cTn). cMyC (cardiac myosin-binding protein C), because of its faster release kinetics and greater abundance, may offer improved diagnostic performance in this population. METHODS: The diagnostic accuracy of cMyC was directly compared with that of hs-cTnT and hs-cTnI in a prospective, international, multicenter, diagnostic study enrolling adult patients presenting to the emergency

A Visual Protocol for Selecting, Initiating, and Monitoring Hemodialysis or Peritoneal Dialysis in Adults with Uremic-Stage Chronic Kidney Disease.

Uremic-stage chronic kidney disease requires timely kidney replacement therapy, but the transition from advanced chronic kidney disease to hemodialysis (HD) or peritoneal dialysis (PD) is often fragmented. Late referral, incomplete modality education, delayed access planning, and insufficient early follow-up contribute to avoidable urgent starts, catheter exposure, hospitalization, and poor patient experience. This article describes a reproducible, visually demonstrable transition-care protocol for a hospital-based adult nephrology service. The workflow moves from eligibility screening and cli

Proton Therapy and Immune Preservation: Dosimetric Rationale, Clinical Evidence, and Implications for the Immunotherapy Era.

Radiation-induced lymphopenia (RIL) is a clinically significant and potentially modifiable consequence of radiotherapy, independently associated with worse survival and attenuated immunotherapy efficacy across multiple disease sites. Its mechanistic basis lies in the exquisite radiosensitivity of circulating lymphocytes and the irradiation of hematopoietically active bone marrow and lymphatic structures within and adjacent to standard treatment fields. Proton therapy, through its fixed beam angles and lack of exit dose by virtue of the Bragg peak phenomenon, delivers substantially less integra

Extracellular Vesicle-Mediated microRNA Signaling in Osteosarcopenia: Emerging Insights into Age-Related Bone-Muscle Crosstalk.

Osteosarcopenia combines low bone strength with sarcopenia and increases the risks of falls, fractures, disability, and death. Recent reviews have catalogued extracellular vesicle (EV)-associated microRNAs in muscle-bone crosstalk, but intracellular microRNA function, EV association, experimentally administered EVs, and physiological tissue-to-tissue transfer are often discussed together. This focused narrative review applies an author-defined, non-validated four-tier framework to separate these inference levels using prespecified criteria for tissue-of-origin tracing, EV characterization, car

Deciphering Polyomavirus-Mediated Carcinogenesis in the Genitourinary Tract.

Human polyomaviruses, particularly BK polyomavirus (BKPyV) and JC polyomavirus (JCPyV), establish persistent infections within the genitourinary tract and undergo reactivation under conditions of immunosuppression. Although their clinical relevance is well established in transplant-associated nephropathy and haemorrhagic cystitis, accumulating molecular, pathological, and epidemiological evidence suggests that these viruses may also contribute to genitourinary carcinogenesis. This review examines the emerging role of BKPyV and JCPyV in urothelial, upper urinary tract, renal, and prostate malig

Does COVID-19 Vaccination Protect Against Post-COVID Cardiovascular Conditions?

Cardiovascular diseases, such as acute myocardial infarction, heart failure, and arrhythmias, have been associated with SARS-CoV-2 infection, representing an important cause of long-term morbidity and mortality. In addition to preventing primary infection, COVID-19 vaccination has emerged as a protective factor against post-COVID cardiovascular outcomes. This study aims to analyse the evidence supporting a causal relationship between COVID-19 vaccination and the prevention of post-COVID cardiovascular conditions, based on classical and contemporary epidemiological criteria, including the Bradf

Regulatory mechanisms of E3 ubiquitin ligases in cholesterol homeostasis.

Dysregulated cholesterol homeostasis fundamentally drives atherosclerotic cardiovascular disease (ASCVD) and related metabolic disorders. Some E3 ubiquitin ligases have emerged as central post-translational nodes that orchestrate cholesterol balance via a sophisticated dual-mode mechanism. Chronologically, they execute rapid, direct ubiquitination and degradation of rate-limiting enzymes (e.g., HMGCR, SQLE), LDL receptors, and cholesterol efflux transporters for acute lipid adaptation, while simultaneously orchestrating slower, transcriptional reprogramming by targeting upstream sensors (e.g.,

Fatal oral ingestion of a sectioned transdermal fentanyl patch in an opioid-naïve youth with gender incongruence: a forensic case report from Japan.

BACKGROUND: Fentanyl is a central driver of the global opioid crisis due to its extreme potency and abuse liability. Japan has historically maintained a low prevalence of opioid use disorders; however, the diversion of prescription pharmaceuticals has emerged as a discrete threat, particularly among adolescents and young adults. Herein, we report a fatal case of acute intoxication caused by the deliberate oral misuse of a transdermal fentanyl patch. CASE PRESENTATION: An adolescent (late teens) with a history of depression and gender incongruence died at home. The deceased had no history of c

Surface chemistry and size-related particle attributes shape lipidomic responses to polystyrene micro- and nanoplastics in zebrafish liver cells.

MICRO: and nano-plastics (MNPs) are emerging environmental pollutants, yet the influence of surface functionalization on their hepatotoxicity remains poorly understood. In this study, zebrafish liver (ZFL) cells were exposed plain and surface-functionalized polystyrene microplastics (1 μm) and nanoplastics (50 nm) to assess their effects on cell viability, generation of reactive oxygen species (ROS), lipidomic responses, and gene expression. As a complementary analysis, UV-treated particles were evaluated on cell viability and ROS production. While most treatments did not impair cellular viabi

Curcumin attenuates beauvericin-induced global DNA methylation alterations in liver and colon cells.

Beauvericin (BEA) is an emerging mycotoxin with various reported toxic effects. Since BEA is considered non-genotoxic, epigenetic mechanisms may contribute to its toxicity. This study investigated the effects of BEA on global DNA methylation (5-mC) and hydroxymethylation (5-hmC), the expression of genes involved in these processes, and the modulatory potential of curcumin (CUR). After 24 h of BEA exposure, the IC50 values were 15.93 μM in HepG2 cells and 16.57 μM in HCT116 cells, decreasing to 0.98 μM and 0.30 μM after 48 h. Co-treatment with 5 μM CUR significantly restored cell viability. BEA

Selective vulnerability of the spinal cord in a rat model of asphyxia-induced cardiac arrest.

Spinal cord injury after cardiac arrest (CA) can cause devastating neurological deficits, including paraplegia. Although lumbar spinal cord injury is clinically important, regional comparisons remain limited. In this study, we examined region-associated vulnerability of the thoracic and lumbar spinal cord using a rat model of asphyxial CA (ACA). Adult male Sprague-Dawley rats underwent sham surgery or ACA, and thoracic and lumbar spinal cord tissues were collected 24 h after return of spontaneous circulation (ROSC) from ACA survivors with hindlimb paralysis. Histopathological analysis demonstr

Lacticaseibacillus paracasei CM6 postbiotics alleviate SO/SP-induced hepatic lipotoxicity in HepG2 cells: Insights into lipid-redox homeostasis, glutamate metabolism and PLA2G4A-dependent lysophospholipid remodeling.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is characterized by metabolic imbalances and lipid-induced cellular toxicity. Although postbiotics are emerging as reliable functional food ingredients for metabolic disorder management, molecular mechanisms underlying their hepatoprotective effects remain incompletely elucidated. In this study, we integrated microbial whole-genome sequencing with paired intracellular and extracellular metabolomic profiling to explore the regulatory effects of the Lacticaseibacillus paracasei CM6 postbiotic against sodium oleate/sodium palmitate
