Case Report: Digital fabrication of a music splint oral appliance for lip irritation in a brass player.

Playing a wind instrument requires coordinated orofacial function, and oral conditions may influence both comfort and musical performance. Among brass musicians, dental crowding or irregular incisal morphology may cause mechanical irritation of the lip while playing. This case report describes the fabrication and evaluation of a music splint for a professional trombonist with recurrent lower-lip pain while playing. A conventionally fabricated provisional splint was first provided, followed by a definitive splint designed using a digital workflow based on intraoral scanning and computer-aided d

Predictors of Delayed Cleft Lip and Palate Surgery: A Case-Control Study in Addis Ababa, Ethiopia.

BACKGROUND: Orofacial clefts are common anomalies, and timely treatment is essential for optimal outcomes. In low- and middle-income countries, surgical delays are frequent, yet contributing factors remain understudied. This study aims to identify and quantify these factors at a tertiary medical center in Ethiopia. METHODS: An institution-based unmatched case-control study was conducted among 98 patients between January 2023 and December 2024. Cases included patients undergoing surgery after the recommended age (cleft lip >6 mo; cleft palate >12 mo), whereas controls received timely surgery.

Early presurgical nasoalveolar molding for bilateral cleft lip and palate: a case report.

BACKGROUND: Bilateral cleft lip and palate is a severe orofacial anomaly associated with wide alveolar defects and complex nasal deformities. Surgical repair of such a defect is challenging and increases the risk of compromised esthetic and functional outcomes. Early presurgical nasoalveolar molding (PNAM) is a non-surgical orthopedic technique used to reduce cleft severity and improve nasal form prior to surgical repair. This case demonstrates the tangible clinical benefits of early intervention in optimizing surgical conditions and outcomes in a complex bilateral cleft presentation. CASE PR

Voice Therapy in Patients with Cleft Lip and Palate: A Narrative Review.

OBJECTIVES: Cleft lip and palate (CLP), as one of the most common congenital anomalies in children, can result in voice disorders. Incidence of CLP and its consequential impact may lead to laryngeal pathologies of the vocal cords, and symptoms may include hoarseness, harshness, breathiness, decreased loudness, and abnormal pitch. Considering the limited number of findings on the efficacy of voice therapy in patients with CLP, the purpose of this narrative review is to aggregate the results of previous studies, explore their findings and restrictions, and identify gaps to present more appropria

Orofacial Strength in a Typically Developing Pediatric Population: A Comparison of the Iowa Oral Performance Instrument and Tongueometer.

PURPOSE: Pressure-based assessments of tongue strength are becoming increasingly common in dysphagia management. However, whether different manometric devices produce comparable measurements for the same tasks, particularly in pediatric populations, remains insufficiently clarified. The purposes of this study were to establish age- and sex-specific reference values for tongue, buccal, and lip strength in a pediatric population using the Iowa Oral Performance Instrument (IOPI) and the Tongueometer (TO) and to systematically examine the effects of device, age, and sex on these measurements, as w

An Anatomy-driven Algorithm for Aesthetic Lip Enhancement: Integrating Structural and Volumetric Approaches.

BACKGROUND: Aesthetic lip enhancement has traditionally been dominated by volumetric approaches, particularly hyaluronic acid fillers. However, lip aging involves both volumetric loss and structural changes, including elongation of the cutaneous upper lip (ergotrid), which cannot be adequately addressed with fillers alone. Despite the wide range of available techniques, no standardized algorithm currently guides treatment selection based on specific anatomical targets. OBJECTIVES: To propose and evaluate a novel, anatomy-driven therapeutic algorithm for aesthetic lip enhancement, integrating

Unilateral macrostomia repair using modified muscle reconstruction: Long-term anthropometric evaluation of static and smile symmetry.

BACKGROUND: Macrostomia, or transverse facial cleft, substantially impairs facial esthetics and oral function. Although various repair techniques have been reported, achieving satisfactory bilateral symmetry, particularly during dynamic expressions such as smiling, continues to be a challenge. Here, we introduce our modified muscle reconstruction technique for unilateral macrostomia repair and evaluate long-term outcomes in static and smile symmetry using anthropometric measurements. METHODS: A retrospective analysis was conducted on all patients who underwent unilateral macrostomia repair at

Scavenger Receptor Class B Type 1 Mediated Transport Enables Delivery Across Capillarized Liver Sinusoids in Metabolic Dysfunction-Associated Steatohepatitis.

Liver fibrosis in metabolic dysfunction associated steatohepatitis (MASH) lacks effective therapies, in part because therapeutic agents poorly penetrate the capillarized sinusoidal endothelium. As fibrosis progresses, liver sinusoidal endothelial cells (LSECs) become defenestrated and restrict trans-sinusoidal exchange, thereby limiting drug access to hepatic stellate cells (HSCs) within the perisinusoidal space. Here, we developed a lipoprotein mimetic liposomal nanocarrier, SV@SRB-Lip, to exploit scavenger receptor class B type 1 (SR-B1)-associated transendothelial transport across capillari

Cheilitis Glandularis: A Report of Two Cases.

Cheilitis glandularis is a rare benign disorder affecting the vermilion border of the lower lip, the occurrence of which is unknown and still remains a myth. some attributed its formation because of the increased expression of surface aquaproteins, hyperplasia from chronic irritation by lip biting, and ultraviolet rays from the sun resulting in squamous epithelial metaplasia but unrelated to inflammation. This article describes the classical clinical picture of cheilitis glandularis in two cases, one related to amelogenesis imperfecta, which is significant for understanding the diagnosis and i

Concomitant reconstruction of the nose and upper lip using an extensive paramedian frontal flap: A technical note.

BACKGROUND: The paramedian forehead flap (PFF), based on the supratrochlear artery, is a ``gold standard'' for nasal reconstruction due to its robust vascularity and excellent color and texture match. However, its use for simultaneous reconstruction of both nose and upper lip following trauma or oncological resection has not been previously reported. OBJECTIVE: This technique aims to describe an extensive PFF incorporating both forehead and scalp tissues, enabling concomitant reconstruction of the nose and upper lip. MATERIAL AND METHOD: The procedure involves at least three stages. The firs

ROS-Responsive Hydrogel for Spatiotemporal Cascade Regulation of Ferroptosis and Fibrosis for Myocardial Protection.

Myocardial infarction (MI)-induced cardiac injury is mainly divided into two phases: the acute necrotic phase and the repair phase. Given the distinct therapeutic requirements of each phase-salvaging dying cardiomyocytes and reducing infarct size in the acute phase, while achieving controlled fibrosis to promote effective ventricular remodeling in the repair phase-we designed an ROS-responsive hydrogel co-delivery system (Met/Anti-34a@Gel) loaded with metformin (Met) and Anti-34a liposomes (Anti-34a Lip) to achieve precise regulation of both the acute and repair phases of MI. In the highly oxi

Decoding Atrial Fibrillation Heterogeneity: A Nationwide Phenome-Wide Association Study in England.

BACKGROUND: Age and sex influence atrial fibrillation (AF) prevalence and outcomes, yet comprehensive data on sex- and age-specific health care utilization in patients newly diagnosed with AF remain limited. OBJECTIVES: We aimed to address this gap using a phenome-wide association study (Phe-WAS) approach. METHODS: We conducted a retrospective cohort study using nationwide Clinical Practice Research Datalink data linked to Hospital Episodes Statistics. Adults diagnosed with AF between 1998 and 2016 (n = 199,433) were matched 1:1 by age and sex to non-AF control subjects. Primary and secondar

Mechanistic insights into TLR2-mediated acute pancreatitis exacerbation via macrophage-ferroptotic acinar cell crosstalk.

Ferroptosis is closely linked to increased inflammatory responses and plays a vital role in the development of acute pancreatitis. We investigated how this process contributes to pancreatic injury in AP and examined the interactions between ferroptotic acinar cells and macrophages. Our study profiles the crosstalk mechanisms between ferroptotic acinar cells and macrophages. These findings, derived from human peripheral blood cohorts, were validated in vivo and in vitro. Multi-omics analyses revealed a reduction in pancreatic acinar cells owing to abnormal ferroptosis activation, as confirmed i

In Situ-Forming Bioresponsive Liposome-Anchored Hydrogel Restores Redox-Immune Homeostasis in Mucosal Macrophages for Radiation-Induced Rectal Injury Repair.

Radiation-induced rectal injury (RRI) is a prevalent complication of pelvic radiotherapy that severely affects the life quality of patients, but its effective treatment remains a major challenge in the clinic. Herein, we identify that the redox-immune dyshomeostasis of mucosal macrophages as a major pathological driver of RRI-associated acute inflammation and chronic fibrosis and developed an in situ forming reactive oxygen species (ROS)-responsive liposome-anchored hydrogel (Lip@H151@TK-PF127 Gel) for effective RRI repair. For this purpose, liposomes were further grafted with mannose moieties

Reduced Risk of MASLD and Cardiometabolic Disease Following Renal Denervation Therapy in Patients With Resistant Hypertension.

INTRODUCTION: Autonomic dysfunction increases the risk of cardio-kidney-metabolic (CKM) syndrome including metabolic dysfunction-associated steatotic liver disease (MASLD), type 2 diabetes (T2D), cardiovascular disease (CVD) and chronic kidney disease (CKD). Renal denervation reduces renal sympathetic nerve activity and lowers blood pressure independent of long-term pharmacotherapy, with potential downstream effects on metabolic and hepatic pathways. We aimed to evaluate the impact of renal denervation, compared with intensive antihypertensive therapy, on cardiometabolic disease risk. METHODS

Exercise-based cardiac rehabilitation after stroke and the risk of incident dementia: an observational study from a global federated network.

INTRODUCTION: Dementia is a common and disabling complication after stroke. Exercise-based cardiac rehabilitation (EXCR) may influence vascular and inflammatory mechanisms relevant to cognitive decline. We investigated the association between EXCR initiation after stroke and incident dementia. PATIENTS AND METHODS: Using TriNetX, we identified adults with ischaemic stroke or TIA. Patients with recorded ICD-10-CM codes indicating an NIHSS score ≥ 16 were excluded from the primary analysis. Patients initiating EXCR within 6 months were compared with those without documented post-index EXCR. Pro
