Application and Complications of the Tunnelized Facial Artery Musculomucosal Flap in Repairing Floor of Mouth Defects.

BACKGROUND: This study evaluates the efficacy and complications of the tunnelized facial artery musculomucosal (tFAMM) flap for repairing floor of mouth defects, comparing it with the nasolabial island (NLI) flap. METHODS: A retrospective analysis was conducted on 12 patients who underwent tFAMM flap reconstruction at the Fifth Affiliated Hospital of Sun Yat-sen University from 2018 to 2024. Data from 8 patients receiving NLI flap reconstruction served as a control group. Outcomes included surgical parameters and complications, and we analyzed UW-QOL scores at 1 month postoperatively. Treatment pathways for oral tongue or floor of mouth reconstruction: A comparative analysis of islanded facial artery myomucosal flap and free anterolateral thigh flap.

PURPOSE: The free anterolateral thigh (ALT) flap is the mainstay for reconstructing intraoral defects after oral tongue (OT) or floor of mouth (FOM) squamous cell carcinoma (SCC) resection, but the islanded facial artery myomucosal (iFAMM) flap has emerged as an alternative for selected patients. This study compared perioperative outcomes, functional recovery, and hospitalization costs between the two reconstructive pathways in cT1-2 N0 OT/FOM SCC patients. PATIENTS AND METHODS: This single-center retrospective cohort study included patients undergoing resection of cT1-2 N0 OT/FOM SCC with re

Three-Dimensional Tongue Reconstruction Using a Trishul-Pattern Sensate Suprafascial Anterolateral Thigh Flap: Technique and Functional Outcomes.

BACKGROUND: Tongue reconstruction after oncologic glossectomy requires restoration of 3-dimensional contour, palatal contact, sensation, and floor-of-mouth support. A flat fasciocutaneous inset may close the defect but may not recreate the functional anatomy of the tongue. We describe a Trishul-pattern sensate suprafascial anterolateral thigh flap for subtotal and near-total tongue reconstruction, with selective use of a chimeric motor-innervated vastus lateralis component for floor-of-mouth reconstruction. METHODS: This descriptive case series was conducted from March 2023 to March 2025. Six

Body Mass Index-Related Changes in Tibial Morphology: Considerations for Intramedullary Nailing.

Glypican-3 (GPC3) mRNA and protein are expressed in >80% of human hepatocellular carcinomas (HCC) but not in normal tissues except for placenta and fetal liver. The oncofetal antigen GPC3 is a glycosylphosphatidyl inositol-anchored membrane protein and may be secreted. It is a novel tumor marker for human HCC: GPC3 protein was present in sera from 40-50% of HCC patients, but was not detected in sera from patients with liver cirrhosis or chronic hepatitis, or in sera from healthy individuals. alpha-Fetoprotein (AFP) and PIVKA-II (protein induced by vitamin K absence or antagonist-II), are well

Evaluating differentiation grade as a guide for decision-making in elective neck dissection for early-stage oral squamous cell carcinoma: a population-based cohort study.

BACKGROUND: Early-stage oral squamous cell carcinoma (OSCC) frequently poses a risk of occult lymph node metastasis, complicating neck management decisions. Elective neck dissection (END) is a critical strategy for mitigating this risk, though its criteria for selection continue to be debated. This study evaluated whether tumor differentiation can guide END decisions in early-stage OSCC. METHODS: Patients with stage I/II OSCC were identified from the Surveillance, Epidemiology, and End Results (SEER) database. Kaplan-Meier and Cox regression analyses assessed cancer-specific survival (CSS) an

Solitary fibrous tumor of the sublingual gland: a case report.

OBJECTIVES: To emphasize the importance of considering solitary fibrous tumor (SFT) in the differential diagnosis of sublingual gland masses, particularly those presenting with unusual symptoms like tongue deviation. METHODS: A case of a 41-year-old female with a painless sublingual mass and tongue deviation was analyzed. Clinical presentation, imaging, surgical management, histopathological findings, immunohistochemical results, and follow-up were systematically reviewed. Management consisted of complete surgical excision of the sublingual gland and the tumor, with preservation of the lingua

Transfusion rates in oral squamous cell carcinoma patients undergoing free flap reconstruction in oral and maxillofacial surgery.

INTRODUCTION: Oral squamous cell carcinoma (OSCC) frequently requires extensive resections and microvascular free flap reconstruction, procedures associated with relevant perioperative blood loss. This study aimed to quantify transfusion rates in OSCC patients and identify procedure- and patient-specific predictors of transfusion to guide evidence-based patient blood management (PBM). MATERIAL AND METHODS: In this retrospective cohort study, 566 OSCC patients undergoing free flap reconstruction over five years were analyzed. Transfusion rates were determined, and predictors were assessed usin

Non-Smoking, Non-Drinking Oral Squamous Cell Carcinoma Is Associated with an Immune-Modulated Clinical Phenotype.

BACKGROUND: Non-smoking, non-drinking (NSND) oral squamous cell carcinoma (OSCC) is increasingly recognized, yet its clinicopathologic and immune-related correlates remain incompletely defined. METHODS: We retrospectively studied 243 surgically treated patients with previously untreated primary OSCC (2011-2020). Patients were classified as NSND or smoking and/or drinking (SD). Immune-modulating conditions and preoperative systemic immune-inflammatory indices (NLR, LMR, SIRI, AISI) were assessed, and overall (OS) and disease-specific survival (DSS) were analyzed. RESULTS: Eighty-five patients

Pyogenic Granuloma in the Floor of Mouth: A Case Report.

Pyogenic granuloma is a common benign, non-neoplastic inflammatory hyperplasia of keratinized tissue, typically arising in response to chronic irritation, trauma, or hormonal influences. Recently it has been classified under vascular tumor and termed as a lobular capillary hemangioma by International Society for the Study of Vascular Anomalies. It presents as a solitary, hemorrhagic, often pedunculated, nodules of variable size which typically appears most of the times in the gingiva. This report presents the case of a 43-year-old male with a pyogenic granuloma on the floor of the mouth. This

Technique for Difficult Wharton's Duct Cannulation.

The comparative ease of cannulating Stensen's duct is contrasted by the acknowledged difficulty in cannulating Wharton's duct. Although access to Wharton's duct can be facilitated with a floor-of-mouth incision, it is desirable to avoid this open approach. We present a technique emphasizing the use of a tapered 4-0 Prolene suture to cannulate Wharton's duct. This guide directs placement of a 26-gauge angiocatheter through a modified Seldinger technique. The suture guide is then removed to permit infusion of saline with expansion of the duct to allow for passage of a 0.38-mm guidewire and progr

Enhancing the functional outcome: a modified bow incision technique for release of tongue adhesions after floor of the mouth resections.

INTRODUCTION: Postoperative scarring in the floor of the mouth (FOM) after tumor resection and reconstruction can reduce tongue mobility. To restore mobility, the tongue often has to be surgically detached. However, functional limitations often recur because of new scar formation. METHODS: A modified incision technique resulting in a tongue-pedicled flap was evaluated in patients with reduced tongue mobility due to scar adhesions following tumor resection of the floor of mouth and tongue. All patients underwent surgical detachment of the tongue 12-24 months after tumor resection. Tongue mobil

Giant epidermoid cyst of the floor of mouth masquerading as a foregut duplication cyst: a case report and review of the literature.

Epidermoid cysts are benign lesions composed of an epithelial lining filled with keratinous debris. The absence of skin appendages distinguishes them from true dermoid cysts. While they may occur in various anatomical locations, they rarely exceed the 5-cm threshold used to define 'giant' lesions. We report a unique case of a giant epidermoid cyst arising from the floor of the mouth, reaching a maximum diameter of 10 cm and initially mimicking a foregut duplication cyst on imaging. We describe its clinical presentation, radiological evaluation, surgical management, and histopathological diagno

Pattern of Progression of Oral Potentially Malignant Disorders From Real-World Data: A 24-Year Study.

BACKGROUND: Oral potentially malignant disorder (OPMD) prognostication is limited by uncertainty of which lesions will progress and the aggressiveness of lesion progression. We defined patterns and kinetics of progression in a large cohort of OPMD subjected to serial clinical and histologic examinations. METHODS: This 24-year study evaluated the pattern of disease progression in 668 patients with 1313 OPMDs. RESULTS: The disease progression pattern was grouped as (a) non-progressive (82.5%), (b) progressive, where initial low-grade dysplasia advanced to high-grade dysplasia in subsequent les

AJCC staging evolution and its clinical impact on resectable oral cancers: A decade-based cohort analysis.

PURPOSE: To assess clinical impact of American Joint Committee on Cancer (AJCC) 7th, 8th, and 9th edition transitions and staging patterns on resectable oral squamous cell carcinoma (SCC). METHODOLOGY: A decade-long cohort (2014-2025) was analyzed across three AJCC editions. Depth of Invasion (DOI)'s prognostic value was assessed using chi-square tests, while vermillion lip reclassification in the 9th edition was evaluated using risk ratios. Stage migration and treatment-impacting transitions were quantified using transition matrices and heatmaps. Kaplan-Meier curves with risk tables were gen

Incidence and Site-Specific Risk Factors of Jaw Osteoradionecrosis for Head and Neck Patients in the IMRT/VMAT Era: Evidence From a Large, Hospital-Based Cohort.

PURPOSE: Osteoradionecrosis (ORN) of the jaw remains a serious late complication of head and neck cancer (HNC) radiotherapy. Although intensity-modulated radiotherapy (IMRT) and volumetric-modulated arc radiotherapy (VMAT) are expected to reduce ORN risk, large real-world data in the modern era are limited. This study evaluated the incidence, latency, and risk factors of ORN across radiotherapy modalities in a large hospital-based cohort. MATERIALS AND METHODS: We retrospectively analyzed 6199 patients with HNC treated with curative radiotherapy between 2006 and 2020. ORN was defined as expos

Adverse Airway Events Following Head and Neck Cancer Reconstruction Without Elective Tracheostomy.

OBJECTIVE: Prior literature has examined characteristics associated with the decision to perform elective tracheostomy (ET); however, the risk of adverse airway events (AAEs) in patients managed without ET remains unclear. We aimed to identify predictors of AAEs in patients managed without ET and to evaluate postoperative outcomes associated with ET. DATA SOURCES: Embase, PubMed, Web of Science, Scopus, and Ovid MEDLINE (January 1, 2009-January 6, 2026). REVIEW METHODS: A systematic review and meta-analysis were conducted following PRISMA guidelines. Risk of bias was assessed for included st

Global Epidemiological Insights: Temporal Trends and Disparities in Oral Cavity Cancer Mortality in the United States, 1999-2024.

BACKGROUND: Oral cancer constitutes a significant public health burden globally and has long been associated with substantial socioeconomic, racial, and regional disparities worldwide. Population-level mortality surveillance in high-income countries such as the United States can provide crucial epidemiological evidence and methodological references for global cancer control and prevention strategies. However, national-level updated evidence on long-term trends, subgroup heterogeneity, and pre- and post-COVID-19 pandemic trends in oral cancer mortality in the United States remains limited. This
