Efficacy of Intratonsillar Steroid Injection for Reducing Palatine Tonsil Size in Tonsillar Hypertrophy: A Randomized Controlled Trial.

INTRODUCTION: Tonsillectomy is a common treatment for obstructive sleep apnea (OSA) and remains one of the most frequently performed surgical procedures worldwide. However, postoperative morbidity is a persistent clinical concern. Intratonsillar steroid injection may offer a non-surgical alternative by reducing the size of hypertrophic palatine tonsils. OBJECTIVE: this study aims to evaluate the efficacy and safety of intratonsillar steroid injections compared to intralesional injections of normal saline solution (NSS) in tonsillar hypertrophy. STUDY DESIGN: Prospective study (Randomized con

Elucidating the role of pre-existing immunity in mucosal lymphoid architectural dynamics following live attenuated influenza vaccination.

Mucosal immune responses in the upper respiratory tract are central to preventing influenza infection. The live attenuated influenza vaccine (LAIV) is delivered intranasally, making it a valuable model for investigating mucosal immunity in humans. In this study, we examined structural and cellular changes in palatine tonsils following LAIV vaccination in cohorts of children and adults undergoing tonsillectomy. Using quantitative immunofluorescence microscopy, we assessed germinal centre and mantle zone architecture over time and quantified the distribution of IgG- and IgA-positive cells within

Extreme conformal prediction: Reliable intervals for high-impact events.

BACKGROUND: Prehospital (ambulance) care can reduce morbidity and mortality from trauma. Yet, there is a dearth of effective evidence-based interventions and implementation strategies. Emergency Medical Services Traumatic Shock Care (EMS-TruShoC) is a novel bundle of five core evidence-based trauma care interventions. High-Efficiency EMS Training (HEET) is an innovative training and sensitization program conducted during clinical shifts in ambulances. We assess the feasibility of implementing EMS-TruShoC using the HEET strategy, and feasibility of assessing implementation and clinical outcomes

CXCR5 identifies stem-like resident memory CD8⁺ T cells enriched for latent EBV specificity in tonsils.

CXCR5+ CD8+ T cells emerged as key mediators of antiviral immunity in the context of chronic infection. However, their functional attributes and tissue distribution remain incompletely defined, especially in relation to antigen specificity. Here, we investigated the anatomical localization and antiviral properties of CXCR5+ CD8+ T cells across multiple sites throughout the human body, with an emphasis on oropharyngeal lymphoid tissues. Tonsils harbored the highest frequencies of CXCR5+ CD8+ T cells compared to other tissues, many of which expressed Granzyme K and concurrently displayed tissue

Single-cell analysis reveals multi-faceted features of B cell development, together with age-associated B cell subpopulations.

The development and maturation of B lymphocytes are critical for adaptive immunity, relying on tightly regulated gene programs within specialized microenvironments shaped by extracellular matrix and neighboring cells. However, high-dimensional, integrated analyses of B cell heterogeneity, gene regulation, and external factors during development remain limited. Here, we analyze single-cell transcriptomic and B cell receptor (BCR) sequencing data from B cells and surrounding cells in bone marrow, tonsil, and peripheral blood. We reveal the dynamics of gene regulation, the heterogeneity of conven

Disordered differentiation and cellular senescence in pediatric Hodgkin Reed-Sternberg cells.

Hodgkin lymphoma (HL) is characterized by inflammatory lesions with relatively few (typically <1%) pathogenic Hodgkin Reed-Sternberg (HRS) cells. The scarcity of malignant HRS cells in HL has historically limited characterization of these disease drivers, and pathogenic mechanisms underlying HL are incompletely defined. Using multiparameter flow cytometry to purify HRS cells and immune cell populations from pediatric HL lesions and control tonsils, we sought to define transcriptional programs in pediatric HRS cells. HRS cells are thought to arise from transformed germinal center B cells. Inter

SegJointGene: joint cell segmentation and spatial gene prioritization by information entropy guided convolutional neural networks.

Spatial sequencing technologies enable the single-cell-level study of molecular organization in tissues. Revealing such spatial patterns relies on accurate cell segmentation. In complex tissues with dense cell packing, segmentation based solely on nuclear staining is insufficient for accurate cell boundary detection. This limitation arises because accurate segmentation necessitates the delineation of cell morphology, which is driven by molecular activities such as cytoskeletal dynamics, cell-cell adhesion, and intercellular signaling. Thus, integrating molecular information, including gene or

Clinical Parameters Predicting Obstructive Sleep Apnoea Syndrome and Its Severity in Children Referred to a Sleep Laboratory in Myanmar.

BACKGROUND: Paediatrics obstructive sleep apnoea syndrome (OSAS) is often underdiagnosed by medical practitioners due to its subtle and nonspecific symptoms in children. This study aimed to determine the prevalence of OSAS and identify clinical predictors of its occurrence and severity. METHODS: Medical records of children (2-18 years) who underwent polysomnography (PSG) at a sleep laboratory in Myanmar during 2012-2023 were reviewed retrospectively. Children with incomplete PSG data, a prior PSG record in the same sleep laboratory, or a history of adenotonsillectomy were excluded from the st

Tonsillar Microbial Signature of Fusobacterium nucleatum Associated With Epstein-Barr Virus Latency.

Epstein-Barr virus (EBV) acquired in childhood is asymptomatic, with the tonsils as the entry site to infect B lymphocytes and establish latency. Little is known about whether the microbiota influences EBV persistent infection. We aimed to characterize the tonsil microbiome with different EBV latency types. A cohort of 60 children with tonsillar hyperplasia were included. EBV latency types (0/I/II/III) and lytic phase were determined by immunohistochemistry. 16S NGS of lymphoid tonsillar tissue was carried out with the Illumina platform. Bioinformatic analysis was carried out with QIIME2 2023.

The spectrum of tonsillar involvement in chronic lymphocytic leukemia (CLL).

Tonsillar enlargement in chronic lymphocytic leukemia (CLL) either with or without symptoms has been rarely reported. We describe 22 cases of CLL with tonsillar involvement and demonstrate this is a CLL disease site that may be under-recognized in routine practice. Tonsillar involvement can be divided into three categories: (1) symptomatic CLL tonsillar involvement with upper airway obstruction (UAO) requiring therapy managed with: (a) tonsillectomy without systemic therapy when tonsillar enlargement is disproportionate to overall disease state, (b) tonsillectomy and systemic CLL therapy, and

Modulation of gut immunity and microbiota by Bacteroides thetaiotaomicron confers dose-dependent protection against necrotic enteritis in broiler chickens.

Necrotic enteritis (NE), caused by Clostridium perfringens, remains a major challenge in broiler chicken production. Probiotics represent a promising alternative strategy to mitigate NE by modulating intestinal immunity and microbiota composition. This study evaluated the effects of oral administration of the commensal bacterium Bacteroides thetaiotaomicron on immune responses, microbial profiles, and lesion development in broiler chickens challenged with C. perfringens. One-day-old male broilers (n = 120) were assigned to four groups and received either a low (1 × 10⁷ CFU) or high (5 × 10⁷ CF

Chiari I Malformation: Review and Update of Current Treatment Options.

The pathophysiology of Chiari malformation type I (CM-I) is complex, involving structural abnormalities at the craniovertebral junction that result in herniation of the cerebellar tonsils through the foramen magnum. In this study, we aim to present and evaluate current treatment options for CM-I, with a focus on evidence-based clinical outcomes and emerging surgical techniques. A comprehensive literature review was conducted using a PubMed search with MeSH terms such as "Arnold-Chiari Malformation, Type 1," "syringomyelia," and "Arnold-Chiari malformation*/surgery."
