BACKGROUND: The presence of bacteria in bile is an important factor in the formation of pigment gallstones. The bile of healthy people is sterile and bacteria in the biliary system come from endogenous infection from the gut. Yet, the route of bacterial translocation into the bile duct is still unclear. Theoretically, two routes exist: one is through the intestinal barrier and the other is by direct reflux from the sphincter of Oddi. This study was undertaken to explore the relationship between the effectiveness of intestinal barrier and the formation of pigment gallstones in hamsters. METHOD

Impact of sodium-glucose cotransporter 2 inhibitors on bleeding, hospitalizations, and other adverse outcomes in atrial fibrillation patients with and without diabetes.

INTRODUCTION: Emerging evidence suggests that sodium‑glucose cotransporter 2 inhibitors (SGLT2is) may be associated with a reduced risk of arrhythmias and related complications. OBJECTIVES: We aimed to evaluate the impact of SGLT2is on atrial fibrillation (AF)-associated adverse outcomes. PATIENTS AND METHODS: All anticoagulated patients with AF diagnosed between January 2014 and December 2020 were identified from a federated electronic medical record database (TriNetX), and followed‑up for 3 years. A 1:1 propensity score matching (PSM) analysis was performed to balance SGLT2i and non‑SGLT2i

Extreme conformal prediction: Reliable intervals for high-impact events.

BACKGROUND: Prehospital (ambulance) care can reduce morbidity and mortality from trauma. Yet, there is a dearth of effective evidence-based interventions and implementation strategies. Emergency Medical Services Traumatic Shock Care (EMS-TruShoC) is a novel bundle of five core evidence-based trauma care interventions. High-Efficiency EMS Training (HEET) is an innovative training and sensitization program conducted during clinical shifts in ambulances. We assess the feasibility of implementing EMS-TruShoC using the HEET strategy, and feasibility of assessing implementation and clinical outcomes

Impact of temperature, pH, and L-tryptophan supplementation on indole-3-acetic acid biosynthesis in Bacillus subtilis strain ESU181.

The bovine respiratory syncytial virus (BRSV) known as Bovine orthopneumovirus according to the international classification is one of the most important etiological agents of respiratory diseases in calves. At present, rapid and reliable methods to detect and measure the concentrations of this pathogen are needed. The objectives of the survey are developing the real-time polymerase chain reaction (PCR) to identify and quantify the BRSV RNA and, based on it, determining the number of the virus genomes in the respiratory tract of sick animals during the disease outbreaks. The nucleocapsid (N) p

Interplay between Tetanus, Diphtheria, Acellular Pertussis Vaccination in Pregnancy and Infant Pneumococcal Vaccination in Shaping Infant Pneumococcal Vaccine Serotype Carriage.

Источник пометил публикацию как отозванную

BACKGROUND: Tetanus, diphtheria and acellular pertussis (Tdap) vaccination during pregnancy blunts the infant humoral immune response following primary immunization with pneumococcal conjugate vaccines (PCVs). While this effect typically resolves after the booster dose for most vaccine serotypes, its impact on nasopharyngeal carriage of pneumococcal vaccine serotypes remains unclear. METHODS: A total of 3298 nasopharyngeal swabs were collected from infants aged 6-30 months attending daycare centers in Belgium between 2018 and 2022, along with data on maternal Tdap vaccination status (clinical

Full-Spectrum Analysis With Machine Learning for Quantitative Assessment of Lateral Flow Immunoassays: A Platform Approach.

Lateral flow immunoassays (LFIAs) provide rapid point-of-care results but lack quantitative capabilities. This study presents a platform technology integrating full-spectrum analysis (400-700 nm) with machine learning to enhance qualitative LFIAs with semi-quantitative assessment capabilities. We analyzed 241 clinical nasopharyngeal specimens using portable spectrometry to capture gold nanoparticle optical signatures from SARS-CoV-2 rapid tests as a validation model. Systematic evaluation of normalization strategies revealed T-C differential outperformed T/C ratio normalization. Signal process

Pharyngeal airway changes after functional orthodontic treatment: A retrospective case-control study on a pediatric population.

BACKGROUND: Initiating orthodontic treatment before the pubertal peak results in more pronounced long-term craniofacial changes in the maxilla and adjacent structures. Dental malocclusion correction through maxillary expansion has been shown to significantly increase the patency and decrease the airflow resistance in several airway compartments, ranging from the nares to the epiglottis plane. OBJECTIVES: We aimed to assess the impact of treatment with a removable functional orthodontic appliance on the dimensions of selected sections of the upper respiratory tract in pediatric patients, with

Respiratory Syncytial Virus Genotype ON1 Within-host Populations in Hospitalized Children: Deletion of the 72-Nucleotide Duplication in the G Gene.

BACKGROUND: Respiratory syncytial virus (RSV) is the major pathogen of lower respiratory tract infection in infants and young children. Molecular epidemiology studies allow the surveillance of circulating RSV genotypes worldwide. However, comprehensive data about the genetic variability of RSV strains within an individual host is lacking. The objective of this study was to investigate the genetic within-host variability of RSV strains in pediatric patients in Heidelberg, Germany. METHODS: Nasopharyngeal swabs (NPS) were prospectively screened for RSV from children admitted with acute respirat

Comparison of two novel options for resuspension of inverted U-shaped nasopharyngeal flap following endonasal access to cranio-vertebral junction: Pilot cadaveric analysis and illustrative case report.

Suturing in the narrow endonasal corridor can be technically challenging. For some endonasal procedures, including inverted U-shaped nasopharyngeal flap (IUNF) harvest for cranio-cervical junction exposure, the ability to support meticulous flap resuspension offers clinical utility. Two novel suturing techniques offer promise: (1) use of barbed suture and (2) Durastat dural repair device, obviating the need for knot tying and endonasal needle driver/supination, respectively. Perform subjective and objective evaluation of IUNF resuspension with: (1) running mucosal suturing using endonasal need

Long-term efficacy of endoscopic endonasal transsphenoidal marsupialization using a nasoseptal mucosal flap for petrous apex cholesterol granuloma: systematic review and two case reports.

PURPOSE: Endoscopic endonasal treatment for petrous apex cholesterol granuloma (PACG) allows for natural drainage to the nasopharynx. The high postoperative recurrence rate of PACG is a problem, and a method of permanent drainage is desirable. A systematic review of the literature was conducted to investigate which techniques can achieve permanent drainage. METHODS: We report new symptomatic PACG cases treated by endonasal transsphenoidal marsupialization using a nasoseptal mucosal flap (NSMF). To verify the safety, efficacy, and long-term preventive effect on recurrence using an NSMF during

The Effect of a 5-Day Course of Azithromycin on Streptococcus Pneumoniae Carriage and Antimicrobial Resistance Among Kenyan Children Discharged From Hospital.

BACKGROUND: Mass azithromycin distribution reduces child mortality in some settings, potentially through reductions in nasopharyngeal carriage of Streptococcus pneumoniae, but has been associated with increased antimicrobial resistance. Individual-level data are lacking on the impact of azithromycin on antimicrobial resistance over time. METHODS: We analyzed data from a double-blind, randomized placebo-controlled trial (ClinicalTrials.gov; NCT02414399) which followed 1398 hospitalized Kenyan children to evaluate the impact of a 5-day course of oral azithromycin at discharge from hospital on p

A Potentially Lethal Course of a Nasogastric Tube: Case Report.

Nasogastric tube (NGT) insertion is a common procedure, often performed blindly. We report a rare, near-fatal complication in a 44-year-old woman where an NGT traversed the internal jugular vein, superior vena cava, and perforated the right atrium into the pericardial cavity. Prompt imaging and surgical retrieval via sternotomy prevented tamponade or exsanguination. No esophageal injury was found; the likely entry point was the nasopharynx. A sealed duodenal perforation was managed conservatively. This case emphasizes the importance of verifying NGT placement before use and the role of confirm

Eighteen-year laboratory-based surveillance of human coronavirus OC43 in a single tertiary hospital in the Republic of Korea: Temporal inflection, seasonal stability, and age-dependent risk.

Human coronavirus OC43 (HCoV-OC43), a member of the betacoronavirus genus, is a seasonally circulating respiratory virus that persists globally; however, long-term laboratory-based surveillance data remain limited. We analyzed multiplex real-time PCR test results collected at Dankook University Hospital between 2007 and 2024 to comprehensively evaluate long-term annual trends, seasonal structure, and age-specific detection patterns of HCoV-OC43. Based on nasopharyngeal swab specimens, the annual positivity rate of HCoV-OC43 ranged from 0.16% to 3.05%. A marked suppression in detection was obse

Molecular detection and phylogenetic analysis of KI polyomavirus and WU polyomavirus in respiratory tract samples from children under five.

BACKGROUND AND AIM: We aimed to investigate the frequency of KI and WU polyomavirus in respiratory tract samples, the affected groups by these viruses and their clinical characteristics. Although the viruses had been discovered in 2007, their pathogenicity and virulence have not been fully elucidated yet. METHODS: We examined 182 nasopharyngeal aspirate/swab samples obtained pediatric patients younger than five years and sent to our laboratory between 2016 and 2019. The viruses were investigated by nested-polymerase chain reaction using primers targeting partial VP1 gene regions. The positive

Primary NK/T cell lymphoma in the uterus: report of an unusual case.

BACKGROUND: Natural killer/T-cell lymphoma (NKTCL) is a highly invasive non-Hodgkin lymphoma (NHL) characterized by extranodal onset, commonly observed in the nose and nasopharynx. NHL in the female reproductive system is rare, mostly secondary, occurring in the ovaries, cervix, uterine body, and endometrium. The prognosis of this disease is extremely poor, with most patients dying within a few months of diagnosis. Owing to its rarity, the prognostic factors are difficult to ascertain. Here, we report a case of extranodal NK/T-cell lymphoma in the uterus related to pregnancy. CASE REPORT: A 3

Establishment of a Point-of-Care Testing Method for Rapid Detection of Multiple Respiratory Virus Antigens Based on a Dual-Drive Microfluidic Chip.

The global impact of pandemics has intensified the demand for rapid, cost-effective, sensitive, and reliable POCT methods. Herein, we developed a multiplex pathogen detection system based on a dual-drive microfluidic chip, enabling simultaneous detection of RSV, FluA, FluB, and SARS-CoV-2 antigens from a single sample via parallel-arranged detection units within microchannels. Monoclonal antibodies targeting conserved viral proteins were cross-paired to identify optimal pairs. The chip's labeling and reference regions were designed, and key parameters were optimized to enhance performance. Cut
