The effectiveness of unimodality organ preservation surgery vs radiotherapy for early hypopharyngeal cancer: a systematic review protocol.

OBJECTIVE: The objective of this review is to determine the comparative effectiveness of unimodality organ preservation surgery versus radiotherapy on oncological and functional outcomes in patients with early hypopharyngeal cancer. INTRODUCTION: Early hypopharyngeal cancer is difficult to detect and, therefore, rarely diagnosed, as patients are often asymptomatic. Radiotherapy is considered the main treatment, although this modality has been compared to the previously used open surgical approach, which may not reflect current surgical options. This is reflected in the available literature, w

Diagnostic Testing for Laryngopharyngeal Reflux Disease: The Role of 24-hour Hypopharyngeal-Esophageal Multichannel Intraluminal Impedance-pH Monitoring.

Laryngopharyngeal reflux disease (LPRD) presents as a constellation of nonspecific upper aerodigestive tract symptoms and signs. Among many objective tools available on the market, hypopharyngeal-esophageal multichannel intraluminal impedance and pH monitoring (HEMII-pH) is considered to date the gold standard technique for diagnosing LPRD, as well as tailoring treatment according to the different LPR profiles. More studies are needed to further validate and standardize diagnostic criteria and evaluate long-term outcomes of patients diagnosed and treated for LPRD using HEMII-pH.

A rapid and highly sensitive fluorescence immunochromatographic test strip for pepsin detection in human hypopharyngeal saliva.

Pepsin serves as a potential biomarker for laryngopharyngeal reflux disease (LPRD). The reported methods for detecting pepsin still have limitations, such as long reaction times and low sensitivity. Therefore, it is crucial to develop a rapid, simple and sensitive detection method. In this study, a fluorescence immunochromatographic test strip was designed for point-of-care diagnostics of LPRD. To obtain optimal fluorescence immunochromatographic conditions, key experimental factors, including microsphere particle size, binding pH, and resuspension dilution ratio, were systematically evaluated

Weak acid and pepsin reflux induce laryngopharyngeal mucosal barrier injury: A rabbit-model-based study.

OBJECTIVE: Using rabbit models, this study simulated the laryngopharynx's response to the synergistic effects of various acidic reflux environments and pepsin to investigate the response mechanism underlying weak acid reflux and pepsin in the mucosal barrier injury of laryngopharyngeal reflux. METHODS: The rabbits were divided into six groups, and the original larynx was recorded for each group. During the study period, rabbits were sprayed with different doses of acid and pepsin solutions and monitored for hypopharyngeal mucosal transient impedance before and after modeling. After the experi

Disparities in stage at diagnosis of head and neck tumours in Brazil: a comprehensive analysis of hospital-based cancer registries.

BACKGROUND: The advanced stage of cancer is a determining factor in poor prognosis. Head and neck squamous cell carcinomas (HNSCC) are highly incident in Brazil, but similarly to many Low and Middle-Income Countries, data is limited regarding the proportion of tumours diagnosed at advanced clinical stages and the main factors associated with it. Therefore, this study aimed to identify the factors associated with advanced stage of HNSCC in Brazil. METHODS: Cross-sectional study based on secondary data collected from Hospital-based Cancer Registries (HBCR) between 2000 and 2017. Descriptive dat

Clinicopathological Significance of Extranodal Extension in Hypopharyngeal and Laryngeal Squamous Cell Carcinoma.

BACKGROUND: The AJCC cutoff value of 2 mm for the extranodal extension (ENE) distance was determined from an analysis of patients with or without adjuvant therapy. The purpose of this study was to find out the ENE distance that reflects prognosis only in patients with head and neck squamous cell carcinoma (SCC) who received adjuvant therapy. METHODS: The ENE distance was defined for 109 patients who underwent surgery for SCC of larynx or hypopharynx as a primary tumor. RESULTS: To standardize patient conditions, only 26 patients who received additional postoperative treatment were analyzed.

Mucosal integrity of the larynx and hypopharynx and the response to proton pump inhibitors in patients with laryngopharyngeal reflux.

PURPOSE: To evaluate whether impairment of integrity of laryngeal and hypopharyngeal mucosa in laryngopharyngeal reflux (LPR) could be related to response to proton pump inhibitors (PPIs). METHODS: Chronic hoarseness with Reflux Finding Score (RFS) > = 7 was evaluated using the Reflux Disease Questionnaire (RDQ) and Reflux Symptoms Index (RSI). Upper endoscopy was performed and samples of the laryngeal posterior commissure were collected to evaluate ex vivo laryngeal mucosal integrity by transepithelial resistance (TER) using Ussing chamber. In vivo, hypopharyngeal integrity was measured usin

Hypopharyngeal Amyloidosis: A Rare Cause of Dysphagia.

Amyloidosis localized to the head and neck most commonly affects the laryngo-tracheobronchial tree or tongue. It rarely affects the hypopharynx, with only 3 cases reported in literature. All of these cases describe females presenting with hypopharyngeal amyloid masses and associated multiple myeloma or monoclonal gammopathy of undetermined significance. They were treated with chemotherapy and CO2 laser, respectively. In comparison, our case identifies a 69-year-old female, with a history of MALT lymphoma but no concurrent malignancy. She initially presented with dysphagia and was diagnosed wit

Intraoperative leakage testing in free jejunal transfer for hypopharyngeal reconstruction: Impact on pharyngocutaneous fistula and anastomotic stenosis rates.

Intraoperative leakage testing (IOLT) is widely used in gastrointestinal surgery. Its application in free jejunal transfer (FJT) for hypopharyngeal reconstruction remains unexplored. We reviewed 49 consecutive patients who underwent FJT with IOLT between June 2021 and February 2024. IOLT was performed using a bulb syringe to inject saline through one nasal cavity while occluding the contralateral nostril and mouth. Despite the jejunum-oesophageal anastomosis remaining unoccluded, rapid saline injection generated sufficient pressure for effective leak detection. Pharyngocutaneous fistula rate w

Frailty adversely affects outcome following free-flap reconstruction in patients with head and neck cancer.

INTRODUCTION: Frailty is a multisystem aging-associated syndrome characterised by physiological decline and increased vulnerability to stressors. There will be a need for elderly, and possibly frail patients, to undergo complex and lengthy free-flap reconstruction for large head-and-neck defects. The purpose of this study was to evaluate the impact that frailty has on post-operative complications, functional outcomes, and overall survival in this cohort. METHODS: A retrospective case-control study of patients undergoing free-flap reconstruction of head-and-neck defects was carried out between

In the battle of survival: transcriptome analysis of hypopharyngeal gland of the Apis mellifera under temperature-stress.

BACKGROUND: Temperature is one of the essential abiotic factors required for honey bee survival and pollination. Apart from its role as a major contributor to colony collapse disorder (CCD), it also affects honey bee physiology and behavior. Temperature-stress induces differential expression of genes related to protein synthesis and metabolic regulation, correlating with impaired gland function. This phenomenon has been confirmed in mandibular glands (MGs), but not in Hypopharyngeal glands (HGs), potentially affecting larval nutrition. RNA-seq analysis was performed using HGs tissue at low (23

[Thinking out of the tube: endoscopic findings in oral cavity, pharynx and larynx during flexible esophagogastroduodenoscopy].

During a flexible esophagogastroduodenoscopy (EGD), the oral cavity and significant regions of the pharynx (oropharynx, hypopharynx), as well as the larynx, can usually be examined with a high resolution. Numerous benign and malignant findings of considerable clinical relevance can occur in this region of the digestive tract, which can be inspected during a gastroscopy and treated subsequently. Remarkably, head and neck tumors represent a heterogeneous group of malignant diseases and account for ca. 4 % of cancers diagnosed in men in Germany. This review presents benign and malignant findings a

A population-based study of incidence trends of head and neck epithelial cancers in northeastern Spain, 1994-2018.

BACKGROUND: Head and neck cancer (HNC) is the seventh most common cancer worldwide. Incidence rates of these tumors vary between countries and change over time depending on the prevalence of risk factors such as tobacco and alcohol consumption, betel nut chewing habits or human papillomavirus infection. This makes it necessary to periodically study HNC incidence trends in each geographic area. METHODS: To determine trends in the incidence of HNC, all cancer cases diagnosed in Girona and Tarragona (northeastern Spain) between 1994 and 2018, except mesenchymal and hematological neoplasms, were

Nobiletin and 5‑demethylnobiletin ameliorate hypopharyngeal squamous cell carcinoma by suppressing TGF‑β‑mediated epithelial‑mesenchymal transition.

Hypopharynx squamous cell carcinoma accounts for 5% of all diseases diagnosed in Mexico. It is associated with poor oral hygiene, alcohol consumption and tobacco use and is usually diagnosed at an advanced stage, with metastasis to the lymph nodes. Metastasis from primary tumors occurs via a complex process called epithelial-mesenchymal transition (EMT), in which epithelial cells gradually acquire characteristics of mesenchymal cells, enabling their spread. Flavonoids have anticancer effects. In the present study, the effects of the polymethoxyflavones nobiletin (Nob) and 5-demethylnobiletin (
