Age-Specific Ultrasound Reference Data for the Laterolateral Rectal Diameter in Healthy Children.

Background: Transabdominal ultrasonographic measurement of rectal diameter is increasingly used as a non-invasive adjunct for assessing rectal distension and fecal loading in children. However, interpretation is limited by the lack of age-stratified reference data in pediatric populations without bowel dysfunction. This study aimed to characterize the distribution of the laterolateral (LL) rectal diameter across childhood and adolescence and provide preliminary age-stratified empirical reference percentiles. Methods: This single-center cross-sectional study included pediatric participants aged

Oncologic Outcomes and Adjuvant Chemotherapy Use in Early- Versus Late-Onset Colon Cancer: A Propensity Score-Matched Analysis.

BACKGROUND: The incidence of early-onset colon cancer is increasing, and younger patients are often treated more intensively on the assumption of worse prognosis. OBJECTIVE: To determine whether early-onset presentation is independently associated with prognosis, and to compare adjuvant chemotherapy use and effect between age groups. DESIGN: Retrospective cohort study with 1:3 propensity score matching and exact matching on stage. SETTINGS: A single tertiary referral center. PATIENTS: Patients who underwent curative resection for stage I to III colon cancer between 2012 and 2020. Early ons

Modifying quality and equity of care: emerging roles of genetic disease modifiers in β-hemoglobinopathies.

Genetic disease modifiers (GDMs) strongly affect disease and therapy outcomes, rendering their characterization essential for prognosis, treatment decisions, and therapy development. A paradigm for the importance of GDMs are β-hemoglobinopathies, for which a growing pipeline of emerging curative and disease-modifying therapies are based on the manipulation of GDMs. However, beyond their role as therapeutic targets, GDMs are also gaining clinical importance in the choice and planning of disease-modifying therapies, the selection and customization of curative therapies, and the equitable provisi

Gastrointestinal Involvement in Pediatric Behçet's Disease: A Focused Analysis from a Comprehensive Systematic Review.

Background: Gastrointestinal (GI) involvement is a clinically relevant but still incompletely characterized manifestation of pediatric Behcet's disease (PedBD). This systematic review provides a focused analysis of GI involvement in PedBD (addressing its frequency, clinical symptomatology, anatomical/endoscopic features, and therapeutic management) as a dedicated sub-analysis of a broader systematic review. Methods: A systematic search of MEDLINE, EMBASE, CINAHL, CENTRAL, and ClinicalTrials.gov was conducted, following PRISMA 2020 guidelines. Observational studies reporting GI involvement in p

Laser Fissurectomy Combined with Limited Lateral Internal Sphincterotomy for Chronic Anal Fissure: A Minimally Invasive Technique.

Chronic anal fissure (CAF) is a common benign anorectal condition characterised by severe pain and internal anal sphincter (IAS) hypertonia. Although lateral internal sphincterotomy (LIS) remains the surgical gold standard, the risk of postoperative faecal incontinence remains a clinically significant concern. We describe a minimally invasive hybrid technique combining 1470-nm diode laser fissurectomy with a limited closed LIS. The laser ablates the chronic ulcer triad whilst a tailored sphincterotomy confined to the distal one-third of the IAS addresses the underlying hypertonia. Based on pub

A Novel Quantitative Endoscopic Tumor Regression Grading System for Predicting Pathological Response Following Neoadjuvant Therapy in Rectal Cancer.

BACKGROUND: While pathological and magnetic resonance imaging-based tumor regression grading systems are widely utilized following neoadjuvant therapy for rectal cancer, existing endoscopic approaches rely on qualitative assessment without quantitative weighting of individual features. Although individual endoscopic features associated with response have been described, no weighted scoring system exists to integrate these features into an objective assessment tool. This lack of a statistically weighted grading framework leads to subjective clinical impressions and significant interobserver var

Evaluation of online dose-guided adaptive fractionation for intact prostate radiotherapy.

BACKGROUND: Dose-guided adaptive fractionation (DGAF) was proposed as a simpler alternative of adaptive radiation therapy, using organ variation simulation for proof of concept. However, its dosimetric benefit using actual daily anatomy changes in prostate cancer patients remains unclear. PURPOSE: The purpose of this study was to retrospectively evaluate the dosimetric benefit and potential risk of DGAF for prostate cancer patients treated with moderately hypofractionated radiotherapy and stereotactic body radiotherapy using daily cone-beam CT. METHODS: Patients assessed in this study were t

Leveraging spatial structure to design spatially-targeted malaria interventions at the community-scale.

Progress in malaria control has stagnated since the early 21st century in many countries, requiring new approaches such as the use of spatially-targeted interventions. However, evidence on the effectiveness of spatially-targeted interventions is mixed, depending on both the setting and intervention. In addition, the potential role of community health programs in implementing spatial targeting is underexplored, with models focusing on coarser spatial scales than those needed to guide programmatic activities. We developed a two-age-class, spatially-explicit model of malaria at the community-scal

Dosimetric impact of intrarectal gas removal by Foley catheter on image-guided adaptive brachytherapy in cervical cancer patients.

PURPOSE: Intraluminal gas in rectum can be observed in some cervical cancer patients who underwent brachytherapy, leading to increased rectal volume. This might increase radiation dose to the rectum, and result in rectal complications. The aim of the study was to investigate the effect of intrarectal gas drainage by rectal catheterization using Foley catheter on rectal dimensions, target dose, and adjacent organs at risk (OARs) doses in cervical cancer patients treated with image-guided adaptive brachytherapy. MATERIAL AND METHODS: Computed tomography (CT) images before and after rectal gas d

Dosimetric impact of computed tomography slice thickness-induced applicator reconstruction uncertainties in three-dimensional brachytherapy for cervical cancer.

PURPOSE: To evaluate the impact of computed tomography (CT) slice thickness on reconstruction accuracy of high-density tandem-ovoid applicators and resulting dose deviations in cervical cancer brachytherapy. MATERIAL AND METHODS: Seventeen cervical cancer patients treated with CT-based brachytherapy using stainless steel tandem-ovoid applicators (2.50 mm slice thickness) were retrospectively analyzed. Homologous 0.625 mm, 1.25 mm, and 5.00 mm image sets were reconstructed from original CT raw data. Applicators were reconstructed on each thickness, and treatment plans from 2.50 mm images were

Jujube pit ingestion leading to ileal perforation and secondary acute appendicitis in a child: a case report.

BACKGROUND: Acute appendicitis is a common pediatric surgical emergency. While most cases are primary, secondary appendicitis resulting from intra-abdominal pathologies like intestinal perforation is rare and diagnostically challenging. Sharp foreign body ingestion, such as jujube pits, can lead to intestinal perforation, peritonitis, and subsequent inflammation of adjacent organs, including the appendix. CASE DESCRIPTION: We present the case of an 8-year-old boy who presented with a one-day history of migratory right lower quadrant abdominal pain, nausea, and high fever (39.2 ℃). Laboratory

Development and independent regional validation of a dynamic ultrasound appendicitis score for suspected acute appendicitis: a prospective multicenter study.

BACKGROUND: Acute appendicitis is the most common surgical emergency. Although CT is highly accurate, radiation exposure is a major concern in children, young adults, and pregnant women. Ultrasound is radiation-free but operator-dependent, and current scores rely on static weights without accounting for important patient-finding interactions. OBJECTIVE: To develop and independently validate a Dynamic Ultrasound Appendicitis Score (Dynamic UAS) with interaction effects to improve risk stratification and potentially avoid unnecessary CT examinations. METHODS: A prospective multicenter study co

Endoscopic submucosal dissection for colitis cystica profunda in a 49-year-old woman: a case report with 52-month follow-up and sustained remission.

BACKGROUND: Colitis cystica profunda (CCP) is a rare benign colorectal lesion characterized by mucus-filled cysts within the submucosa. However, the durability of endoscopic submucosal dissection (ESD) for CCP beyond 48 months has not been documented. This case aims to fill that gap. ESD has emerged as a minimally invasive approach enabling both definitive diagnosis and treatment, but data on long-term efficacy remain limited, with no reported follow-up exceeding 48 months. CASE DESCRIPTION: A 49-year-old woman presented with a 6-month history of unformed stools. Colonoscopy revealed a submuc

Aberrant absorptive enterocyte differentiation is associated with altered ion transport and chronic inflammatory diahrea in IBD patients.

Diarrhea is a hallmark of Crohn's disease (CD) and ulcerative colitis (UC), often linked to a leaky epithelial barrier and an impaired fluid and electrolyte absorption. This study investigates whether a defect in enterocyte differentiation contributed to reduced absorptive, but increased leak protein expression in patients with Inflammatory bowel disease (IBD). Segmental biopsies from the terminal ileum to the rectum were collected from IBD patients and healthy controls (HC). mRNA and protein levels of key absorptive ion transporters and markers of cell proliferation and differentiation were a

Endoscopic Examination of Mitrofanoff Channel Findings and Subsequent Surgical Change of Management-Observations From a 2-Year Patient Cohort.

INTRODUCTION: A Mitrofanoff channel is a continent catheterisable channel, most commonly constructed from the appendix or small bowel (Monti channel) to provide an alternative to the urethra for intermittent catheterization. It is imperative for patients to be counselled on the risks of channel complications, including stomal stenosis, recurrent urinary tract infections (rUTI), and, less commonly, incontinence. Endoscopy of the Mitrofanoff (Mitrofanoscopy), is a diagnostic tool employed in these scenarios to visualize the channel and bladder. Herein, we aim to characterize the findings of Mitr
