MRGPRX2-expressing mast cells are increased in the GI tract of individuals with active inflammatory bowel disease and hereditary α-tryptasemia.

INTRODUCTION: Hereditary α-tryptasemia (HαT), defined by increased TPSAB1 copy number and elevated basal serum tryptase, is associated with mast cell (MC)-mediated symptoms. However, its role in gastrointestinal disease remains unclear. Because intestinal MCs express the non-IgE-dependent activation receptor MRGPRX2, we investigated whether MRGPRX2 expression and MC phenotypes are altered in individuals with HαT in the context of inflammatory bowel disease (IBD). METHODS: We genotyped 854 biobanked IBD samples to identify individuals with HαT. Spatial transcriptomic analysis was performed on

Adult Pan-Colonic Intussusception Secondary to Poorly Differentiated Cecal Adenocarcinoma: A Rare Presentation of Stage IIA Colon Cancer.

Adult intussusception is rare, typically associated with an underlying pathology, and often presents with nonspecific symptoms. We report the case of a 68-year-old man presenting with diarrhea, abdominal pain, and weight loss. CT suggested volvulus versus intussusception, and emergent laparotomy revealed massive pan-colonic intussusception extending from the small bowel into the right colon and continuing through the transverse and descending colon to the sigmoid. A subtotal colectomy with end ileostomy was performed. The postoperative course was complicated by high serous drain output and tra

Ischemic Colitis Associated With Tirzepatide Therapy in a Young Female Patient: A Case Report.

Tirzepatide has been approved for the management of obesity, obstructive sleep apnea, and type 2 diabetes mellitus; while gastrointestinal side effects are commonly seen, ischemic colitis has been only rarely described. We present the case of a 32-year-old woman with no prior medical history who developed hematochezia and abdominal pain after six weeks of tirzepatide therapy for weight loss. Laboratory evaluation showed neutrophilic leukocytosis without anemia or electrolyte abnormalities, and stool studies were negative for infection. A CT of the abdomen and pelvis excluded obstruction or wal

Case Report: Idiopathic Wunderlich syndrome complicated by a perinephric abscess with fistula formation to the descending colon.

Wunderlich syndrome (WS) is a rare clinical condition characterized by spontaneous, non-traumatic renal hemorrhage. Although secondary infection of a renal hematoma has occasionally been reported, progression to a perinephric abscess with fistula formation to the descending colon has not been previously documented. We present a unique case of idiopathic WS in a 61-year-old diabetic woman with poorly controlled blood sugar who developed a spontaneous left subcapsular hematoma without an identifiable etiology. During hospitalization, follow-up contrast-enhanced computed tomography (CECT) reveale

An overview of commercially available polymers for colon-targeted drug delivery systems.

The colon consists of the cecum, transverse colon, ascending colon, descending colon, and sigmoid colon. Colonic diseases such as inflammatory bowel disease (IBD), irritable bowel syndrome (IBS), colon cancer, and various other conditions may occur due to alterations in pathophysiology or factors such as environmental influences, dietary habits, and immunological variations. Conventional oral dosage forms require higher doses and pose the risk of exposing non-colonic tissues to the drug, potentially leading to adverse reactions. Successful colon targeting can be accomplished achieved by levera

Evaluation of Remnant Esophageal Perfusion Using Indocyanine Green Fluorescence Imaging during Ivor Lewis Esophagectomy after Total Pharyngolaryngectomy: A Case Report.

INTRODUCTION: Head and neck cancer frequently coexists with synchronous or metachronous esophageal cancer, owing to shared carcinogenic exposures such as alcohol and tobacco use. In patients undergoing esophagectomy after total pharyngolaryngectomy (TPL), surgical reconstruction poses significant challenges due to dense adhesions and altered cervical anatomy. The McKeown procedure requires cervical manipulation, demanding careful preservation of the tracheal stoma and free jejunal graft blood supply. In contrast, the Ivor Lewis procedure eliminates the need for cervical maneuver but necessitat

The regional landscape of the human colon culturome in health and cystic fibrosis.

UNLABELLED: Cystic fibrosis (CF) alters gut physiology, yet its impact on microbial communities across colonic regions (ascending, transverse, descending colon) and microhabitats (lumen, mucosa) remains incompletely understood. Here, we applied culturomics to characterize gut microbiota in a small cohort of 32 individuals (22 non-CF, 10 CF). Persons with CF (pwCF) exhibited significantly higher viable bacterial loads than non-CF individuals, particularly in mucosal samples. Anaerobes predominated overall, with relative enrichment of aerobes in the mucosa of pwCF. Alpha diversity was reduced in

A case of descending colon cancer achieving a complete response with preoperative capecitabine plus oxaliplatin therapy following colorectal stenting.

A 71-year-old woman with obstructive descending colon cancer and synchronous lymph node and liver metastases underwent colorectal stenting to relieve colonic obstruction. Preoperative capecitabine plus oxaliplatin (CapeOX) was administered for four cycles. Subsequent imaging showed marked shrinkage of the primary tumor, with nodal and hepatic lesions no longer detectable. Laparoscopic left hemicolectomy was then performed. Histopathological examination demonstrated a pathological complete response (CR), with no viable tumor cells in the primary lesion and no lymph node metastasis. Postoperativ

Descending Colon Epiploic Appendagitis Mimicking Diverticulitis: A Case Report.

Epiploic appendagitis is a rare, self-limiting cause of acute abdominal pain that often mimics more common conditions, such as diverticulitis, leading to frequent misdiagnosis and unnecessary interventions. We report a case of a 46-year-old male who presented with sudden-onset, sharp, localized left lower quadrant pain without systemic symptoms. Physical examination revealed localized tenderness without peritoneal signs, and laboratory tests showed only mild leukocytosis and slight elevation of inflammatory markers. Contrast-enhanced computed tomography demonstrated an oval, fat-density lesion

Evaluating the correlation of intestinal ultrasound and fecal calprotectin with endoscopic severity scores in assessing disease activity in ulcerative colitis patients.

BACKGROUND AND AIMS: Ulcerative colitis (UC) is a chronic inflammatory bowel disease. Noninvasive markers, including fecal calprotectin (FC) and intestinal ultrasound (IUS), have emerged as promising tools for assessing UC activity. To establish the correlation between noninvasive test IUS and fecal calprotectin with the gold standard invasive test UCEIS for assessing disease activity in patients with UC. MATERIALS AND METHODS: Sixty UC patients were enrolled, and comprehensive demographic, clinical, and biochemical data were collected. FC levels, IUS findings, UCEIS scores, and endoscopic Ma

Prediction of Massage Force and Intra-abdominal Wall Deformation During Massage by a Digital Twin Model Based on an Abdominal Finite Element Model.

PURPOSE: Constipation is a prevalent disorder of the digestive system, and abdominal massage is commonly employed to alleviate its symptoms due to its non-invasive nature and minimal side effects. This study aims to develop an accurate correlation model that links massage force (MF), massage depth (MD), and intra-abdominal wall deformation (IAWD) based on patient-specific data, thereby providing quantitative standards and personalized treatment options for the use of clinical massage devices. METHODS: The study utilized finite element method (FEM) and machine learning (ML) techniques to const

Successful Surgery for Type III Colonic Atresia Using the Side-to-Side Santulli Procedure: Rescue Treatment During the Practice of Global Pediatric Surgery.

Colonic atresia (CA) is a rare congenital intestinal condition that requires surgical intervention. The surgical approaches for its treatment may vary from primary anastomosis to stoma creation. We report a case of CA treated with side-to-side anastomosis and Santulli-type colostomy. A three-year-old girl with a history of CA who had undergone ascending colostomy in the neonatal period visited the surgical mission of the World Surgical Foundation for definitive surgical management. The details of the operative findings in the neonatal period are unknown. Preoperative contrast enema revealed an

Robot-assisted laparoscopic nephrectomy for fish bone-induced perforation from the descending colon into a renal cyst: A case report.

Fish bone ingestion is common in Japan, but gastrointestinal perforation is rare. We report a 67-year-old man with a fish bone perforating the descending colon and penetrating a renal cyst, causing severe retroperitoneal infection. CT revealed a linear hyperdense structure extending into the cyst with intralesional gas. Conservative therapy failed, and robot-assisted laparoscopic surgery was performed, including fish bone removal, colonic serosal repair, and nephrectomy. Recovery was uneventful, and the patient remained recurrence-free at 6 months. This is the first report of robot-assisted la

Intestinal mucosal Schwann cell hamartoma.

Mucosal Schwann cell hamartoma (MSCH) is a recently proposed disease condition. It is a benign neurofibroma that differs from neurofibromas present in type-1 neurofibromatosis. It is a form of polypoid lesion that develops in the mesenchyme, presenting with pure Schwann cell growth in the lamina propria and exhibiting diffuse S-100 protein staining. This study presents the case of a 51-year-old man, a patient who underwent routine endoscopy, which revealed a 0.5 cm polyp in the descending colon, followed by pathological and immunohistochemical examination, which verified the diagnosis of intes

Development and Validation of an Ileocolonoscopy-Based Nomogram for Predicting Perianal Penetrating Complications in Patients with Crohn's Disease.

BACKGROUND: Perianal penetrating complications (PPC) in Crohn's disease (CD) are inadequately predicted. PPC risk correlates with ileocolonoscopic scores, however, the association between specific ileocolonoscopic features and it remains unclear. This study aimed to identify predictive ileocolonoscopic features and develop a dedicated PPC nomogram, thereby enabling proactive management of high-risk patients. This tool is designed as a prediction model, not a clinical decision-making instrument. METHODS: CD patients from two centers between January 1, 2012 and July 31, 2025 are enrolled: the F

Urethral Diverticulum Mimicking Malignancy on FDG PET/CT.

Female urethral diverticulum is an uncommon pathologic entity. An 80-year-old woman who underwent left hemicolectomy for descending colon cancer 2 years ago was referred for FDG PET/CT for restaging. FDG PET/CT showed a hypodense FDG-avid periurethral lesion suspicious for malignancy. The FDG-avid periurethral lesion corresponded to a urethral diverticulum, which was initially detected on a pelvic MRI 1 year ago. This case indicates that urethral diverticulum should be included in the differential diagnosis of FDG-avid urethral or periurethral lesions. Careful correlation with the patient's hi
