Minimal Oral Water Immersion Method May Shorten Insertion Time During Unsedated Colonoscopy: A Randomized Controlled Trial.

BACKGROUND AND AIMS: Unsedated colonoscopy often increases procedural pain and prolongs insertion time, especially in the sigmoid colon with acute luminal angulation. This study aimed to evaluate the efficacy and safety of preprocedural enhanced oral water intake (PWI) for unsedated colonoscopy. METHODS: This randomized controlled trial (RCT) enrolled 372 eligible patients. They were randomized to the conventional air colonoscopy (CAC), water-assisted colonoscopy (WAC), and PWI groups to receive the assigned interventions. Primary outcomes included insertion time, visual analog scale (VAS), c

Multiorgan Segmentation in Magnetic Resonance Images for Gynecological Brachytherapy Using Residuals in Residual Dense Block.

BACKGROUND: Deep learning has significantly improved medical image segmentation, particularly in complex applications such as gynecological brachytherapy (BT). This study proposes a novel U-Net variant, the residual in residual dense block U-Net (RU-Net), to address the segmentation of organs-at-risk (OARs), which are often distorted due to BT probe insertion. Accurate segmentation is essential for effective treatment planning; however, task-specific datasets are scarce. To tackle this, a dedicated magnetic resonance imaging (MRI) dataset was developed, focusing on the precise delineation of O

Synchronous Leiomyomas of the Sigmoid Colon and Ileocecal Valve Presenting as Colonic Polyps in a Young Male.

Gastrointestinal (GI) leiomyomas are rare benign smooth muscle tumors, most commonly arising in the esophagus and stomach. Their occurrence in the colon is uncommon, and synchronous involvement of multiple colonic sites is exceedingly rare. We report a case of a young male who underwent colonoscopic evaluation for GI symptoms, which revealed polypoidal lesions in the sigmoid colon and at the ileocecal valve. Both lesions were endoscopically resected. Histopathology with immunohistochemistry is essential for accurate diagnosis and to differentiate these lesions from GI stromal tumors. Recogniti

Spontaneous Mesenteric Hematoma Following Severe Coughing in a Patient on Dual Antiplatelet Therapy: A Case Report.

Spontaneous mesenteric hematoma (SMH) is a rare cause of acute abdominal pain and may mimic more common intra-abdominal conditions, creating diagnostic challenges. A 67-year-old man with hypertension, hyperlipidemia, coronary artery disease status post four-vessel coronary artery bypass grafting, and chronic dual antiplatelet therapy with aspirin and clopidogrel presented with acute worsening lower abdominal pain. He reported a severe coughing episode during a recent bronchitis-like illness approximately two weeks before presentation, followed by lower abdominal pain in the same general region

Case Report: Delayed complete transection of the sigmoid colon after severe pelvic fracture leading to septic shock.

Severe pelvic fractures caused by high-energy trauma may be complicated by bowel injuries. These injuries may be evident at initial presentation or remain occult, resulting in delayed diagnosis. Delayed complete transection of the sigmoid colon associated with pelvic fractures is extremely rare. We describe a 57-year-old woman who was admitted to the Intensive Care Unit (ICU) following a traffic accident that resulted in a Young-Burgess lateral compression type III pelvic ring injury, hemorrhagic shock, impaired consciousness, and multiple associated injuries. Abdominal CT on post-injury days

Disease- and Region-Specific Molecular Patterns Involving miRNAs and Associated Signalling Pathways and Epithelial Cell-Fate Responses in PSC-Associated Ulcerative Colitis.

Patients with primary sclerosing cholangitis-associated ulcerative colitis (PSC-UC) have an elevated risk of colorectal cancer compared with ulcerative colitis (UC), but the molecular mechanisms linking chronic inflammation to epithelial cell fate remain unclear. The present study investigated disease- and region-specific molecular patterns involving miRNAs and associated PI3K/AKT, STAT3, and NF-κB signalling pathways. miR-21, miR-181a, miR-181b, and key pathway components were analysed in paired ascending and sigmoid colon biopsies from patients with PSC, PSC-UC, UC, and controls. Possible association of pyogenic liver abscess with an inflamed sigmoid colonic lipoma: a case report and literature review.

BACKGROUND: Pyogenic liver abscess (PLA) is a severe infectious condition that is often associated with an identifiable biliary or gastrointestinal source. Benign colonic lesions are rarely considered during the etiological evaluation of PLA. CASE PRESENTATION: We report the case of a 47-year-old man admitted with febrile diarrhea. Contrast-enhanced computed tomography (CT) demonstrated confluent hepatic abscesses involving segments II, III, and IV, measuring 10 cm × 9 cm × 7 cm, without a CT-detectable biliary source. The etiological investigation identified an approximately 2 cm inflammator

Case report: Fatal hematogenous gastrosplenic mucormycosis in an immunocompetent adult following sigmoid perforation, septic shock, and short-term stress glucocorticoids.

BACKGROUND: Mucormycosis is an aggressive angioinvasive fungal infection that predominantly affects chronically immunosuppressed individuals. Hematogenous dissemination of Mucorales causing synchronous gastric full-thickness perforation and total splenic infarction is exceptionally rare, particularly in immunocompetent hosts without chronic comorbidities. To our knowledge, this presentation has not been reported in a patient with only three days of stress-dose glucocorticoid therapy following sigmoid perforation and septic shock. CASE PRESENTATION: We report a fatal case of synchronous gastri

Elective transabdominal preperitoneal repair of an incidentally detected spigelian hernia with massive sigmoid colon protrusion through a small fascial defect: a case report.

INTRODUCTION AND IMPORTANCE: Spigelian hernia is a rare type of ventral hernia. The hernia orifice is generally narrow, and incarceration of the contents - usually omentum or small bowel - is a common problem. CASE PRESENTATION: A 79-year-old man underwent laparoscopic cholecystectomy for acute cholecystitis. Preoperative computed tomography (CT) incidentally revealed a left Spigelian hernia with protrusion of the sigmoid colon through a small fascial defect. Considering the potential risk of future incarceration, elective hernia repair was planned, and laparoscopic transabdominal preperitone

A case of parasitic fibroid in unscarred abdomen mimicking pedunculated uterine fibroid: a case report.

INTRODUCTION AND IMPORTANCE: Parasitic leiomyomas are a type of extra-uterine fibroid that develop outside the uterus and thus receive their blood supply from surrounding structures. While most cases are iatrogenic, spontaneous occurrences are rare and often pose diagnostic challenges due to atypical locations. CASE PRESENTATION: We report the case of a 28-year-old woman, para 2, with no prior abdominal or pelvic surgery, who presented with a 5-year history of intermittent lower abdominal pain. Ultrasound suggested a pedunculated mass arising from the uterine fundus, mimicking an ovarian endo

Giant primary hydatid cyst of the sigmoid mesocolon: a surgical case report.

Giant primary hydatid cysts of the sigmoid mesocolon are exceptionally rare, and their exact site of origin may be difficult to establish before surgery. We report a 51-year-old woman with an 8-year history of progressive abdominal distension and vague abdominal discomfort. CT showed a multilocular cystic lesion of size 30 × 22 × 28 cm extending from the pelvis to the upper abdomen, with internal septations and multiple daughter cysts. Echinococcus serology was positive, with no radiological evidence of hepatic or pulmonary hydatid disease. The patient had received several previous courses of

Pelvic splenosis and endometriosis in adolescent with hereditary spherocytosis.

The patient is a 16-year-old with a history of hereditary spherocytosis. At age 12, she underwent a laparoscopic partial converted to total splenectomy for intraoperative bleeding. The spleen was placed in a retrieval bag and removed via morcellation. She first reported abdominal pain at age 8, with dysmenorrhea after menarche at age 13. Her dysmenorrhea was managed with a progesterone IUD. Due to worsening pain, she underwent MRI which showed multiple lobulated structures throughout the pelvis and ventral abdomen, suggestive of endometriomas versus splenules. Laparoscopy revealed dozens of bl

An unusual colo-ovarian fistula of advanced colorectal carcinoma: A case report and literature review.

In colorectal cancer, the invasion of the colon is not unusual, but the perforation of all layers of the colon or rectum and the presentation of the patient with generalized peritonitis is rare. Colo-ovarian fistulas have as their starting point either in the sigmoid colon, rectum, appendix or, more frequently, as a benign or malignant ovarian tumor. The present study reports an extremely rare case of colorectal cancer with perforation at the rectosigmoid junction into a metastatic ovarian tumor. Intraoperatively, a colo-ovarian fistula with diffuse peritonitis was diagnosed. The treatment con

Laparoscopic Anterior Gastropexy with a Technical Modification Using the Round Ligament for Internal Hernia of the Stomach after Sigmoid Colostomy: A Case Report.

INTRODUCTION: Internal hernia involving the stomach associated with a sigmoid colostomy is extremely rare. The hernia defect is typically located between the colostomy limb and the left abdominal wall. Because only a few cases have been reported, no standardized management strategy has been established, particularly in patients in whom immediate colostomy closure is not appropriate. CASE PRESENTATION: A woman in her 70s underwent a laparoscopic Hartmann's procedure with sigmoid colostomy for obstructive sigmoid colon cancer with para-aortic lymph node metastasis. On POD 9, the patient develop

Percutaneous endoscopic sigmoidopexy for recurrent sigmoid volvulus in high-risk adults: a systematic review.

BACKGROUND: Recurrent sigmoid volvulus in frail, comorbid adults recurs frequently after detorsion and carries significant risks with resectional surgery. Percutaneous endoscopic sigmoidopexy (PES), defined as fixation of the sigmoid colon to the abdominal wall without an indwelling tube, has been proposed as a tube-free, minimally invasive alternative. OBJECTIVE: To synthesise outcomes of strict PES (fixation-only; no tube) for recurrent sigmoid volvulus in adults at high surgical risk. METHODS: PRISMA-guided review (2000-31 July 2025) registered prospectively with PROSPERO CRD420251111664.

Dual single-port laparoscopic surgery for metastatic low-grade serous ovarian carcinoma: a case report of an innovative minimally invasive approach adhering to no-touch isolation technique.

The management of peritoneal metastases in young patients with ovarian cancer demands a delicate balance between oncological radicality and quality of life. We present the case of a 34-year-old woman with a final pathological diagnosis of primary advanced-stage (FIGO IIIC) low-grade serous ovarian carcinoma (LGSC) of the left ovary, with metastases to the sigmoid colon, pelvic peritoneum, vesical peritoneum, diaphragm, and omental surface. The diagnosis was confirmed by comprehensive immunohistochemistry (PAX8+, WT1+, ER+, PR+, CA125+, p53 wild-type) and complete histopathological examination.

Successful treatment of recurrent epithelioid trophoblastic tumor (ETT) with pembrolizumab after pembrolizumab: A case report and review of literature in treatment of ETT.

BACKGROUND: Epithelioid Trophoblastic Tumors (ETT) are rare, which makes an optimal treatment regimen difficult to determine. Since PD-L1 is expressed in placental tissue and in gestational trophoblastic neoplasia (GTN), immunotherapies such as pembrolizumab have been proposed as possible treatments. CASE PRESENTATION: The case presented is a 50-year-old postmenopausal female who presented with postmenopausal bleeding and a large, fungating cervical mass. Cervical biopsies confirmed ETT. The patient underwent primary debulking surgery with radical abdominal hysterectomy, bilateral salpingo-oo

Pathologic complete response after first-line tislelizumab, cetuximab, and dabrafenib in microsatellite-stable BRAF V600E-mutant metastatic colorectal cancer: a case report.

BRAF V600E-mutant metastatic colorectal cancer (mCRC) is characterized by aggressive disease behavior and limited responsiveness to conventional systemic therapy. Although BRAF/EGFR-targeted therapies have improved clinical outcomes, durable complete remission remains uncommon, particularly in microsatellite-stable (MSS) disease. We report the case of a 26-year-old woman with MSS, BRAF V600E-mutant sigmoid colon adenocarcinoma and synchronous liver and omental metastases (cT4aN2M1c, stage IVC). Pretreatment biopsy demonstrated high programmed death-ligand 1 (PD-L1) expression (combined positiv
