A 26-year-old Arab had epigastric pain and bloody mucous stools. At coloscopy distal proctitis and a polyp at the rectosigmoid junction were discovered. The polyp had granulomatous inflammatory changes and eggs of Schistosoma mansoni with a characteristic egg shell, central miracidium and lateral spine. Bilharziasis should be considered in the differential diagnosis of chronic inflammatory disease of the intestines.

There has been a considerable amount of experience with removal of rectal foreign bodies inserted transanally. Until recently removal of objects above the rectosigmoid junction has required laparotomy. In a patient who presented with pelvic pain and changed bowel habits, barium enema and sigmoidoscopic examinations demonstrated a nonspecific asymmetric narrowing of the colon at the rectosigmoid junction with normal mucosal appearance. Computed tomography revealed the etiology to be retroperitoneal fibrosis with pelvic extension and entrapment of the colon at this level. The importance of CT in diagnosis of this unusual manifestation of retroperitoneal fibrosis is discussed.

The motor activity of colonic segments proximal to the rectosigmoid junction are poorly understood. We investigated colonic peristalsis (high-amplitude propagated contractions; HAPCs) in 20 healthy volunteers by means of a colonoscopically positioned manometric probe and low-compliance infusion system. A new method for urinary diversion to the rectum was elaborated in animal experiments and currently has been used in 19 patients. Reflux of the rectal content to the colon and to the upper urinary tract is prevented by the fashioning of an intussusception valve at the rectosigmoid junction. The rectum is augmented by anastomosing an ileal patch to the anterior rectal wall. A transverse colostomy protects the construction for 6 to 8 weeks. From 1976 to 1985, 66 elective side to end colorectal anastomosis according to Baker's technique were performed. 36 of those 66 anastomoses (54.5%) were performed lower than 10 cm from the anal verge. Our series includes 17 cases of diverticular disease, 1 case of post radiotherapy stenosis of the rectosigmoïd junction and 49 carcinomas. 50% of all carcinomas were Dukes' C or D lesions and more than 34.8% of all lesions were subobstructive. Results are reported for 335 patients hospitalized for diverticular disease of the symptomatic colon: bowel movement disorders, abdominal pains and/or rectorrhagia. Of 81 patients that received operative surgery, 62 presented inflammatory stenosis or perforation. Recurrence is prevented by surgical treatment combining sigmoidectomy and exeresis of the rectosigmoid junction.
