Oncological impact of universal endoscopic submucosal dissection for large Barrett's cancers.

BACKGROUND: Oncological principles favour en bloc R0 excision for curative endoscopic resection. In Barrett's neoplasia, endoscopically curable cancers include T1a and selected early T1b disease. Although endoscopic mucosal resection (EMR) and endoscopic submucosal dissection (ESD) are established treatments, optimal lesion selection remains debated. OBJECTIVE: To evaluate the oncological impact of two selective resection strategies: (1) prioritising ESD for suspected Barrett's cancers >15 mm and (2) a historical approach reserving ESD mainly for advanced cancers. DESIGN: Multicentre retrosp

Laparoscopic Nissen fundoplication in children with a pre-existing gastrostomy: challenges and tips.

PURPOSE: Patients with an in-situ gastrostomy tube(G-tube) often require laparoscopic fundoplication (LF) for refractory gastro-oesophageal reflux disease. We evaluated the feasibility and outcomes of LF, identifying the need for gastrostomy detachment. METHOD: All LF performed between 2014 and 2024 by a single surgeon in a tertiary paediatric centre were retrospectively analysed. Clinical data, operative technical aspects and outcomes were analysed. RESULTS: Of 170 LF,33 children had a G-tube. In 28 cases (85%, Group A), LF was completed without detaching the gastrostomy (median operating t

Enhancement of oral bioavailability of risedronate through xyloglucan rafts.

Bisphosphonates irritate the stomach and oesophagus and have a very limited absorption. The purpose of this study was to increase risedronate (RDN) oral bioavailability by causing a raft to form in the stomach. The creation of a raft prevents the irritation of the stomach and oesophagus caused by bisphosphonates. FTIR, TGA, and DSC were used to characterise the RDN, XLG, and the created formulation. In addition to a cell viability analysis utilising Caco-2 cells, the release of RDN was investigated in 0.1 N HCl, 0.5 N HCl, 1 N HCl, and simulated gastric fluid (SGF). For the pharmacokinetic inv

Cooperative spatial modelling of hospital compliance with minimum caseload requirements.

BACKGROUND: Minimum caseload requirements (MCRs) ensure medical treatment quality but may negatively affect spatial accessibility to health care. Previous studies optimised the caseload distribution via spatial models, with a focus on balancing spatial concentration and accessibility with centralised case redistribution models. This study seeks to capture hospitals as active participants in MCR policy decisions by incorporating their intentions and motivations regarding MCRs within their spatial context, considering current caseloads and neighbouring hospital distances. METHODS: The study mod

Frequent synaptophysin expression in SMARCA4-deficient undifferentiated carcinoma of the oesophagus: a diagnostic pitfall with therapeutic implications.

BACKGROUND: SMARCA4-deficient neoplasms are aggressive tumours typically arising in the thoracic region, often responding to immunotherapy despite poor prognosis. Although rare, these tumours can also occur in the gastrointestinal tract, including the oesophagus. Given the potential for misdiagnosis, particularly when tumours present with undifferentiated morphology, this study aimed to identify key diagnostic features of SMARCA4-deficient undifferentiated carcinoma of the oesophagus (SMARCA4-deficient UC) and highlight the clinical importance of accurate diagnosis. MATERIAL AND METHOD: A ret

Preclinical Studies on Plant Based-Antacid Formulations as New Therapies for Gastro-Oesophageal Reflux Disease.

Background/Objectives: Gastro-oesophageal reflux disease (GERD) refers to a disease in which stomach acid rises into the oesophagus. Currently, proton pump inhibitors (PPIs) are the most commonly used medications to treat GERD. However, long-term use of PPIs is not free from side effects, and new treatment strategies are needed. The present study was conducted to evaluate the gastroprotective potential of four different formulations containing both antiacids and medicinal plants considered useful for the treatment of GERD. Methods: The protective effects of the formulations on gastric ulcers i

Dieulafoy lesion of the gastrointestinal tract: what's new? A narrative review.

Dieulafoy's lesion is a rare but potentially life-threatening cause of gastrointestinal bleeding because of an abnormally dilated submucosal artery that erodes the overlying epithelium without a primary ulceration. Typically located in the proximal stomach near the esophagogastric junction, Dieulafoy's lesions can also occur throughout the whole gastrointestinal tract, including the oesophagus, small bowel, and colon. Rare cases have been reported in the respiratory tract. Dieulafoy's lesion accounts for approximately 3.5% of gastrointestinal bleeding cases, with mortality rates ranging from 2

Can bile salts affect the contractile oesophageal activity associated with gastroesophageal reflux disease?

Bile salts are essential molecules that have evolved beyond their digestive surfactant properties to act as relevant pathophysiological signalling modulators. In the gastroesophageal reflux disease (GERD), recurrent exposure of the oesophagus to harmful agents of gastric and duodenal origin, including bile salts, results in chronic inflammation and damage to the oesophageal mucosa, ultimately promoting the progression of the normal oesophageal epithelium to pre-malignant or malignant lesions, such as Barrett's oesophagus and adenocarcinoma. Although the acidity of the refluxed material has lon

Are radiology residents safe to report feeding nasogastric (NG) tubes on chest X-rays?

OBJECTIVES: The task of issuing reports on whether nasogastric (NG) tubes are safe for enteral nutrition on chest X-ray (CXR) often falls to radiology residents. The aims of this study are to evaluate whether radiology residents are formally trained and their performance in interpreting NG tube position on CXR. METHODS: Radiology residents were invited to participate in an online study evaluating NG tube position on CXR. The CXR images comprised 20 NG tubes, 14 of which were correctly sited, while 4 were in the distal oesophagus and 2 in the lung. RESULTS: Twenty-eight (of 185, 15%) radiolog

Basal crypt dysplasia in Barrett's oesophagus: ready for prime time?

Barrett's oesophagus (BE) continues to rise in prevalence alongside oesophageal adenocarcinoma; understanding and identifying early neoplastic changes is critical. Crypt dysplasia (CD) in BE is an emerging concept characterised by dysplasia confined to the crypt base without surface involvement. Recent studies suggest that CD shares molecular alterations with low-grade and high-grade dysplasia, such as TP53 mutations and chromosomal instability, and may represent an early phase of neoplasia. Grading of CD remains inconsistent, with limited correlation to clinical outcomes, although classifying

A case report of persistent phrenic nerve injury following pulsed field ablation using a pentaspline catheter.

BACKGROUND: Pulsed field ablation (PFA) is an emerging non-thermal ablation modality for atrial fibrillation (AF), characterized by its myocardial selectivity and reduced risk of collateral damage to extracardiac structures such as the oesophagus and phrenic nerve (PN). While transient phrenic nerve injury (PNI) has been reported, persistent diaphragmatic paralysis remains exceedingly rare. CASE PRESENTATION: We report the case of a 49-year-old man with paroxysmal AF, refractory to medical therapy, who underwent pulmonary vein isolation using the FARAPULSE PFA system. The procedure was uneven

Clinical significance of hiatus hernia on Barrett's oesophagus: a scoping review.

BACKGROUND: Hiatal hernia (HH) is commonly observed in patients with Barrett’s oesophagus (BO), a premalignant condition that may progress to oesophageal adenocarcinoma (OAC). While HH has been implicated in BO pathogenesis, it is not formally recognised as a major risk factor in leading international clinical guidelines. This systematic scoping review aimed to evaluate the role of HH in the development, progression, and treatment outcomes of BO. METHODS: A systematic scoping review was conducted, searching through databases (PubMed, Medline, Embase, Scopus). RESULTS: A total of sixty-six arti

Common carotid artery pseudoaneurysm and frontal brain abscess secondary to oesophageal foreign body in a paediatric patient.

Young children often present with sharp foreign body (FB) ingestion. While the risk of complications is low when the FB is urgently removed, perforation of the gastrointestinal tract may lead to further interventions and treatments. A boy between 12 and 24 months old presented to the emergency department with a safety pin lodged within his mid-oesophagus. Nine days following removal, he had developed a right common carotid pseudoaneurysm (PSA) and right frontal brain abscess due to unrecognised oesophageal perforation by the FB. He underwent a burr hole craniotomy for the brain abscess and rig

Esophageal erosion by a PDA occluder device following MAPC closure in tetralogy of Fallot.

Tetralogy of Fallot (TOF) is the commonest cyanotic congenital heart disease encountered in paediatrics. Major aortopulmonary collaterals (MAPCAs) often complicate TOF when surgical correction is delayed, and a transcatheter closure is commonly done prior to definitive surgery. Embolization of occluding devices is a known complication that typically occurs in the vessels or in cardiac chambers. We report a young boy who presented with 1-month dysphagia post-transcatheter closure of a large MAPCAs. Barium revealed a filling defect confirmed by endoscopy as an impacted occluding device in the oe

Oesophageal tissue engineering: optimisation of stereotactic robotic cell injection in decellularised oesophageal scaffolds.

PURPOSE: Oesophageal substitution following atresia repair, caustic damage or cancer of the oesophagus can be challenging. We and others are working on engineering oesophageal tissue using a combination of decellularised oesophagi and cell injection. So far this has been achieved using highly operator-dependent techniques. This study aimed to establish a reproducible method for cell delivery into scaffolds. METHODS: To improve consistency, a stereotaxic robotic platform was adapted to deliver a suspension of porcine gelatin and cells in a 1:1 ratio. The scaffold was mounted on a 3D-printed ro

Video-assisted thoracoscopic surgical management of benign esophago-pulmonary fistula.

Benign esophago-pulmonary fistula (EPF) is a rare clinical condition. This report highlights the case of a 58-year-old woman with a 10-year history of recurrent episodes of cough, throat irritation, dyspepsia and recurrent haemoptysis. Upper gastrointestinal (GI) endoscopy and chest CT detected a fistulous communication between the mid-oesophagus and the right lower lobe lung parenchyma, with bronchiectatic changes in the lower lobe. This patient underwent a video-assisted thoracoscopic surgery (VATS) division of EPF with right lower lobectomy. Intraoperatively, the fistula was identified on e

A novel dynamic in-vitro model of esophageal button battery ingestion demonstrating benefits of honey, but not acetic acid.

PURPOSE: Retained esophageal button batteries (BBs) cause significant morbidity in children. Current guidelines recommend oral honey administration in children over 1 year and esophageal irrigation with 0.25 % acetic acid (ACA) after BB removal, but supporting evidence is limited. We aimed to develop a dynamic in vitro esophageal model to evaluate these interventions. METHODS: Our novel model consisted of fresh porcine oesophagi suspended in an environment simulating body temperature, peristalsis, and saline perfusion. BBs or sham BBs were placed in the oesophagus. Honey was administered at s

A giant pericardial cyst as rare cause of reflux and chest discomfort: a case report.

BACKGROUND: Pericardial cysts are a rare form of congenital mediastinal cystic lesion. They are usually asymptomatic and are often discovered incidentally, although some patients may present with a variety of different symptoms. The presence of small, asymptomatic pericardial cysts does not need treatment; however, large and/or symptomatic cysts may require surgical intervention. CASE SUMMARY: We report a case of a 42-year-old man with a 7-year history of dysphagia and non-specific thoracic complaints. Multiple gastroenterological examinations and proton pump inhibitor therapies did not find

Females exhibit greater cardiac volume reductions with expiratory muscle loading during submaximal exercise compared to males.

Intrathoracic pressure (ITP) modulates venous return, ventricular filling and ejection throughout the respiratory cycle, and its fluctuations are accentuated during exercise. We sought to determine whether ITP manipulation during submaximal exercise alters cardiac volumes differently in males and females. Young, healthy participants completed two protocols (both n = 30, 15 females): (i) inspiratory loaded breathing (75 W and 30% Wmax) and (ii) expiratory loaded breathing (100 W and 60% Wmax). We aimed to increase the inspiratory and expiratory load by +20 cmH2O oesophageal pressure (absolute s
