Postoperative management after oesophagectomy: An evidence-based approach.

Oesophagectomy is a complex surgical procedure that requires meticulous multidisciplinary care. The procedure, which typically requires multiple incisions, involves tumour resection and creation of a gastric conduit followed by its anastomosis to the remaining oesophagus in the thorax or the neck. Although respiratory failure is the leading cause of postoperative morbidity, non-invasive ventilation is frequently unfeasible. Special care is required to secure the multiple catheters and drains inserted during the procedure; accidental dislodgement of any of which may increase postoperative morbi

Histological Evidence for the Presence of a Glandular Stomach in the Mudskipper Scartelaos histophorus From the Mekong Delta, Vietnam.

This study describes the gross anatomy and histological organization of the digestive tract of the mudskipper Scartelaos histophorus from the Mekong Delta, Vietnam. Thirty adult specimens were collected from intertidal mudflats, and the digestive tract was examined using gross anatomical observation and routine paraffin histology with haematoxylin and eosin staining. Gross anatomical observation differentiated the digestive tract into the oesophagus, stomach, intestine and rectum, whereas histological examination focused on the stomach, intestine and rectum. The stomach was characterized by pr

Mapping Molecular Transitions in Barrett's-Associated Oesophageal Adenocarcinoma via Multi-Omics Integration and Pathway Activity Modelling.

Background/Objectives: Interpretation of high-dimensional molecular profiling data requires computational strategies that go beyond single-layer analyses to resolve coordinated biological programs. Although multi-omics integration has advanced latent structure discovery, translating these cross-omics signals into interpretable, functionally meaningful molecular states remains a central challenge. Methods: Here, we present an interpretable, pathway-centric multi-omics integration framework that combines expression, DNA methylation, and copy-number alteration data to capture nonlinear cross-omic

A systematic review on the clinical implementation of 4D particle therapy.

Intrafractional motion in the thoracic and abdominal regions remains a major challenge in particle therapy representing one of the key anatomical changes during radiotherapy. Interfractional changes further contribute to cumulative uncertainties and may require adaptive strategies over the course of treatment. The high dose conformality and steep dose fall-off of particle beams, combined with motion-induced range variations, make dose delivery highly sensitive to temporal and anatomical changes. This systematic review provides an overview of clinically implemented 4D treatment approaches addre

Aorto-Oesophageal Fistula Presenting With Massive Upper Gastrointestinal Haemorrhage in Early Childhood: A Case Report.

Aorto-oesophageal fistula is an exceptionally rare but life-threatening cause of upper gastrointestinal bleeding in children. Early diagnosis is challenging because of its nonspecific presentation and rapid clinical deterioration. We report the case of a previously healthy young child who presented with massive haematemesis following a short viral prodrome and rapidly developed haemorrhagic shock. Initial laboratory investigations demonstrated severe acute anaemia requiring activation of the massive haemorrhage protocol and blood product resuscitation. Computed tomography imaging revealed abno

First Documented Fatal Gastric Obstruction Associated with Ingestion of Plastics and Vegetation in a Juvenile Kogia breviceps.

The stranding network of the Basque Country (SAREUS) attended a stranded juvenile female Kogia breviceps on Zarautz (Gipuzkoa, Spain) on 2 October 2025. The necropsy revealed severe pathological findings: (1) plastic and plant debris occupying most of the volume of the main stomach, (2) pulmonary congestion, pink froth in respiratory airways and presence of sand in the oesophagus, and (3) haemorrhagic liver and meninges together with serosanguineous liquid in the pericardium. Results indicate that the individual stranded alive and drowned because of a debilitated condition most likely caused b

The combined porcine-avian model: An innovative training tool for paediatric retroperitoneoscopic pyeloplasty.

INTRODUCTION: Simulation-based training is increasingly recognised as a cornerstone of modern minimally invasive paediatric surgery, providing a safe environment for deliberate practice despite limited operative exposure. However, training models reproducing the specific ergonomic constraints of paediatric retroperitoneoscopic pyeloplasty remain scarce. Building on our established neonatal avian simulator, we adapted a complementary hybrid model designed to reproduce the anatomical orientation, key procedural steps, and working angles encountered during retroperitoneoscopic dismembered pyelopl

Delayed thoracoscopic repair helps preserve native oesophagus in long-gap oesophageal atresia.

BACKGROUND: Preservation of the native oesophagus is generally considered the preferred strategy for long-gap oesophageal atresia (LGOA). This study aimed to assess whether delayed thoracoscopic repair of LGOA was associated to increase rate of oesophageal preservation. METHODS: Retrospective single-centre cohort study including patients with Gross type A or B managed between January 2006 and December 2025. Patients undergoing delayed anastomotic attempt were grouped according to operative approach (open or thoracoscopic). The primary outcome was successful native oesophageal preservation. Se

Type IV laryngo-tracheo-esophageal cleft with CPAM in a preterm twin- a case report.

BACKGROUND: Laryngo-tracheo-esophageal cleft (LTEC) is a rare congenital anomaly caused by incomplete separation of the foregut. Type IV defects are exceptionally rare and typically carry high mortality and morbidity despite surgical interventions. CASE PRESENTATION: We report a preterm neonate born at 34 + 1 weeks' gestation from a monochorionic diamniotic twin pregnancy. Antenatal ultrasound detected a large microcystic congenital pulmonary airway malformation (CPAM) with mediastinal shift, absent gastric bubble, and suspected esophageal pouch. Immediately upon birth, bedside ultrasound ide

Peri-oesophageal metastases of pulmonary adenocarcinoma presenting as dysphagia in a dog.

Primary pulmonary neoplasia represents approximately one percent of canine cancers. Although it is recognised that many of these cases may not display classic respiratory signs, dysphagia is not an expected presentation. This report describes a twelve-year-old Border Collie that presented with a history of progressive dysphagia and weight loss over several weeks. Diagnostics in a general practice clinic identified a pulmonary mass on thoracic radiographs. Referral was recommended for advanced diagnostics due to clinical deterioration in the absence of a definitive diagnosis, with humane euthan

Deposition and clearance of airborne microplastics in the human respiratory tract.

Αirborne microplastics (MPs) comprise an important exposure pathway to humans and an emerging health hazard. Published MP concentrations and size distributions were applied to a dosimetry model to determine their deposition and clearance in the human respiratory tract (HRT). Inhaled MPs deposited primarily in the extra-thoracic region with smaller MPs accumulated in the tracheobronchial and alveolar regions. The retained mass after exposure of one day was estimated to range from 5.2 × 10-4 to 9.48 μg based on different exposure concentrations. Moreover, after one day, approximately 60 % of the

Mechanisms underpinning the progression of nondysplastic Barrett's oesophagus to cancer.

Barrett's oesophagus is the metaplastic precursor to oesophageal adenocarcinoma; it is initiated by exposure of the squamous oesophageal epithelium to acid reflux. Wound repair mechanisms lead to the migration of columnar cells into the denuded lower oesophageal niche. These columnar cells remain exposed to inflammatory acid and bile reflux, which serves as a mutagenic pressure and induces the appearance of intestinal phenotypic features. The patchwork accumulation of genomic instability after TP53 loss provides a cancerised field where dysplastic clones can be selected for. In our review, we

Longitudinal computed tomography body composition changes in patients receiving curative radiotherapy for non-small cell lung cancer.

BACKGROUND AND PURPOSE: To determine whether longitudinal changes on routine thoracic computed tomography (CT) predict overall survival (OS) in non-small cell lung cancer (NSCLC) after curative radiotherapy and identify dose predictors of adverse tissue changes. MATERIALS AND METHODS: We performed a retrospective, single-centre study of 231 stage I-IV NSCLC patients who had at least two follow-up scans. In total, 2708 CT scans were analysed (median follow-up, 22 months; range, 1-97). Automated segmentation quantified left ventricular (LV) myocardium, L1 skeletal muscle (SKM) and fat volumes.

Outcome measures used to assess gastrointestinal tract involvement in adults and children with systemic sclerosis: a scoping review.

OBJECTIVES: The gastrointestinal (GI) tract is commonly affected in children and adults with systemic sclerosis (SSc) leading to substantial morbidity including malnutrition, growth failure, and poorer quality of life. We aimed to identify the GI measures used in SSc, describe their performance characteristics, and assess their applicability for use in juvenile-onset SSc (jSSc) trials. METHODS: We performed a scoping review of GI outcome measures in SSc studies from 1994-2024. Studies with at least one GI outcome measure were included, and test performance details were extracted. RESULTS: Fr

Evaluation of the gastrointestinal tract in juvenile systemic sclerosis: the paediatric gastroenterologist perspective.

The gastrointestinal (GI) tract is one of the most affected organs in systemic sclerosis (SSc), described in 42-78% of children with juvenile-onset SSc (jSSc). GI disease negatively impacts quality of life and increases disease-specific morbidity and mortality. Immune-mediated vascular, mucosal and neuromuscular compromise of the GI tract results in heterogeneous disease and variable clinical symptoms, severity, and disease progression. The burden of GI disease has a significant impact on clinical outcomes, and yet diagnosis and monitoring of GI disease remain challenging. The problem is multif
