Non-ampullary duodenal mixed neuroendocrine-non-neuroendocrine neoplasm initially diagnosed as adenocarcinoma: a case report.

INTRODUCTION: Mixed neuroendocrine-non-neuroendocrine neoplasms (MiNENs) are rare tumors composed of neuroendocrine and non-neuroendocrine components. Their occurrence in the duodenum is extremely uncommon. Preoperative diagnosis is difficult because the neuroendocrine component may not be detected in biopsy specimens. We report a rare case of non-ampullary duodenal MiNEN initially diagnosed as adenocarcinoma. CASE PRESENTATION: A 56-year-old man was found to have a tumor in the duodenal bulb during routine endoscopic examination. Biopsy revealed moderately to poorly differentiated adenocarci

Bouveret Syndrome Mimicking Gallbladder Malignancy: A Diagnostic Challenge in an 85-Year-Old Woman.

Bouveret syndrome is a rare complication of cholelithiasis in which a gallstone enters the duodenum via a cholecystoenteric fistula, causing gastric outlet obstruction. Because of chronic inflammation and nonspecific symptoms, the condition can easily be misdiagnosed as a gastrointestinal malignancy. An 85-year-old woman presented with severe weight loss (10 kg), night sweats, and non-bloody vomiting. CT imaging revealed massive aerobilia, a thickened gallbladder wall, and an ill-defined mass at the gastroduodenal junction. These findings raised strong suspicion for a malignant process in the

Duodenal atresia associated with apple-peel configuration of distal small bowel in a late preterm neonate: a rare case report and literature review.

INTRODUCTION: The association of duodenal atresia with apple-peel (Christmas tree) configuration of the distal small bowel is exceptionally rare, with only a few cases reported in the literature. This unusual combination is of particular interest because it may provide evidence supporting a vascular etiology for selected cases of duodenal atresia. We report a neonate with type III duodenal atresia associated with apple-peel bowel and absent proximal superior mesenteric artery anatomy. CASE PRESENTATION: A late-preterm (35 + 5 weeks) female neonate with antenatal suspicion of jejunal atresia p

A Retrospective Study of Lemmel's Syndrome: Single Centre 15-year Experience.

Lemmels's syndrome is obstructive jaundice due to periampullary duodenal diverticula in the absence of a neoplasm or choledocholithiasis. The aim of this study is to present our 15-year experience on this rare pathology. Endoscopy, imaging, and histopathology records of all the patients with obstructive jaundice who were managed in surgical unit 3 in our institute between January 2010 and December 2024 were reviewed retrospectively. Patients who were found to have periampullary duodenal diverticula were included in this study. Descriptive statistics were used to summarize the data. Of 10 cases

Resection of a Large Intra-Abdominal Desmoid Tumor with Preoperative Superior Mesenteric Artery Balloon Catheter Placement: A Case Report.

INTRODUCTION: Large tumors occupying the abdominal cavity can hinder surgical exposure, particularly of the posterior aspect, increasing the risk of arterial bleeding. Because arterial hemostasis is difficult, hemodynamic instability may occur. This report describes the safe use of preoperative balloon catheter placement in the superior mesenteric artery (SMA), the tumor's primary feeding vessel, to control arterial bleeding through on-demand intraoperative occlusion. CASE PRESENTATION: A 40-year-old woman with a history of 2 cesarean sections presented with abdominal distension. An intra-abd

Duodenal perforation masquerading as an intra-abdominal abscess: successful management with pedicled falciformopexy.

Duodenal perforation typically presents as an acute abdomen. In contrast, presentation as an intra-abdominal abscess is rare and may delay diagnosis. We report a 25-year-old male who presented with 8 days of upper abdominal pain and 4 days of progressive abdominal distension. Despite marked abdominal distension, he had no signs of peritonitis. Laboratory investigations showed marked leukocytosis. Chest radiography showed no classical subdiaphragmatic free air, whereas computed tomography confirmed pneumoperitoneum. Emergency exploratory laparotomy revealed an intra-abdominal abscess with dense

Outcomes of laparoendoscopic rendezvous for cholecysto-choledocolithiasis: A single series of 389 cases from a teaching hospital in Santiago, Chile.

BACKGROUND: The ideal management for cholecysto-choledocholithiasis remains controversial. A one-stage approach technique offers advantages over the two-stage approach, mainly by reducing postoperative complications, length of stay, and costs. This study aimed to describe the clinical and perioperative outcomes of laparoendoscopic rendezvous (LERV) for treating cholecysto-choledocholithiasis in elective and emergency settings. METHODS: All patients with cholecysto-choledocholithiasis who underwent simultaneous cholecystectomy and endoscopic retrograde cholangiopancreatography (ERCP) at a teac

Elective combined surgical management of concurrent gastrocolic and duodenocolic fistulae in longstanding Crohn's disease.

Internal fistulization is a recognized complication of Crohn's disease, reflecting its transmural inflammatory nature and cumulative bowel damage over time. Internal fistulae occur in approximately 20-35% of patients with Crohn's disease, characteristically involving enteroenteric, ileosigmoid or enterovesical communications. In contrast, fistulous involvement of the stomach or duodenum is exceedingly rare, with only isolated cases and small case series reported in the literature. We report the case of a man in his 60s with longstanding Crohn's disease who was incidentally found to have concur

Duodenum inversum presenting with intestinal obstruction in a 40-day-old infant: a case report.

BACKGROUND: Duodenum inversum is a rare congenital anomaly characterized by an abnormal course of the duodenum with preservation of a normally positioned duodenojejunal junction. Clinical presentation is highly variable, ranging from incidental findings to symptoms of upper gastrointestinal obstruction. Due to its rarity, diagnosis and management remain challenging, particularly in pediatric patients. CASE PRESENTATION: We report the case of a 40-day-old female infant presenting with recurrent postprandial vomiting and poor weight gain since the first days of life. Initial clinical, metabolic

Laparoscopic Right Hemicolectomy Using A Cranial-First Approach: Technical Description and Early Outcomes.

There are various surgical approaches for laparoscopic radical right hemicolectomy. However, the Henle's trunk and middle colic vessels often reveal anatomical variations, and improper handling may lead to vascular injury. This protocol presents a "cranial-first" approach combined with complete mesocolic excision (CME) to attempt to overcome the aforementioned technical difficulties. 86 cases were collected from September 2023 to April 2026. First, the gastrocolic ligament was opened to expose the right gastroepiploic vessels and the pancreatic head. The colic branches of Henle's trunk and the

Introduce 4π dynamic spot-scanning proton arc therapy (SPArc-4π) for pancreatic cancer: a comparative planning study with IMPT.

OBJECTIVE: Stereotactic body radiotherapy with a simultaneous integrated boost (SBRT-SIB) for locally advanced pancreatic cancer (LAPC) is constrained by the proximity of serial gastrointestinal organs at risk (OARs). Conventional intensity-modulated proton therapy (IMPT) relies on a limited number of fixed beam angles, which may restrict dose shaping around these critical structures. This study evaluated whether non-coplanar spot-scanning proton arc therapy (SPArc-4π) could improve OAR sparing while preserving robust target coverage. APPROACH: 10 patients with LAPC were retrospectively selec

Pancreaticoduodenectomy for Mass-Forming Localized Duodenal Amyloidosis: A Case Report.

INTRODUCTION: Localized duodenal amyloidosis is extremely rare, with only a few cases having been documented in the literature. To the best of our knowledge, we report the first case of localized amyloid light-chain (AL) λ-type duodenal amyloidosis diagnosed after pancreaticoduodenectomy (PD). CASE PRESENTATION: A 71-year-old asymptomatic man was referred to our hospital after plain radiography revealed a calcified mass in the upper abdomen. Contrast-enhanced CT revealed a 58-mm exophytic mass with prominent peripheral calcification on the dorsal aspect of the descending duodenum, showing con

Gastrointestinal Manifestation of Light Chain Deposition Disease: Beware of the Snakeskin Duodenum.

Gastrointestinal manifestations of light chain deposition disease are exceedingly rare and are characterized by gastrointestinal bleeding, nutritional deficiencies and protein-losing enteropathy. Diagnosis is challenging due to nonspecific symptomatology and indolent disease progression. We describe the case of a 69-year-old woman presenting with melena, chronic diarrhea, and recurrent sepsis. Gastroscopy revealed a highly unusual snakeskin appearance of the duodenum, with histopathology and immunofluorescence confirming light chain deposition disease. This case highlights the importance of pr

A Continuously-Resolved 3D Organisation of the Rat Stomach.

The anatomy of the stomach informs about the specific inter-regional functions of the digestive process. Eight whole stomachs from Sprague-Dawley rats were stained with phosphotungstic acid (PTA) and imaged using a SkyScan 1172 μ CT scanner. Following segmentation of the mucosal-submucosal and muscle-serosal layers, thickness, fractional anisotropy, and muscle fibre orientation were calculated and reconstructed over three-dimensional anatomies. In the mucosal-submucosal layers, thickness increased from the forestomach through the glandular corpus to the antrum, with characteristic transitio

Fatal rebleeding of a residual duodenal varix caused by portal hemodynamic redistribution after selective embolization.

Duodenal varices are a rare form of ectopic varices that develop as a consequence of portal hypertension and may cause life-threatening gastrointestinal bleeding. Due to complex portosystemic collateral pathways, optimal treatment strategies have yet to be established. We herein report a fatal case of ruptured duodenal varices in a man in his 50s with decompensated liver cirrhosis. The patient presented with hematochezia and hemorrhagic shock. Endoscopic hemostasis was unsuccessful, and emergency transileocolic venous obliteration (TIO) was performed. Contrast-enhanced computed tomography reve

One threshold does not fit all: assay-specific validation of anti-tTG IgA tests for biopsy-free diagnosis of pediatric celiac disease.

BACKGROUND: The ESPGHAN biopsy-free diagnostic algorithm for pediatric celiac disease (CeD) requires serum IgA anti-tissue transglutaminase antibody (anti-tTG IgA) concentrations ≥10 times the upper limit of normal (ULN) in the first blood sample, together with positive IgA endomysial antibodies (EMA) in a second blood sample. However, the transferability of the 10×ULN anti-tTG IgA threshold across different analytical platforms remains uncertain owing to the lack of assay harmonization. METHODS: Eighty-four children (6-10 years) who underwent duodenal biopsy during the diagnostic work-up for

Adult Ascaris suum roundworms and their antigens modulate the porcine intestinal microbiome and metabolome.

The porcine roundworm, Ascaris suum, affects host intestinal physiology, microbiota composition and metabolic profiles. To better understand the mediators of these alterations, six pigs each were experimentally exposed to (i) 30 adult roundworms via gastric tube, or (ii) received injections into the duodenum of A. suum total excretory-secretory (ES) antigens, ES antigens secreted exclusively from the parasites' body surface (trans-cuticular ES), or cuticular somatic antigens, respectively. Six further pigs served as untreated controls. Microbiome composition in faeces, ingesta and mucosa sampl

Muir-Torre syndrome in a patient with advanced duodenal carcinoma.

Muir-Torre syndrome (MTS) is a rare autosomal dominant disorder characterized by sebaceous neoplasms and visceral malignancies and is regarded as a phenotypic variant of Lynch syndrome. Although colorectal and genitourinary cancers are the most common visceral malignancies in MTS, duodenal cancer is extremely rare. We report a case of advanced duodenal cancer associated with MTS treated with multidisciplinary therapy. A 62-year-old man with a history of multiple colorectal cancers and sebaceous carcinomas presented with vomiting and epigastric pain. Upper gastrointestinal endoscopy revealed a
