Menthol-based self microemulsifying drug delivery system for augmented intestinal absorption of edoxaban.

BACKGROUND: Clinical effectiveness of edoxaban is hindered by poor solubility and limited permeability due to P-glycoprotein (P-gp) efflux. AIM: The aim was to hasten dissolution and intestinal permeability of edoxaban by loading into microemulsion system consisting of menthol as oil and inhibitor of P-gp. METHODS: Pseudo-ternary phase diagram was constructed at 37°C and microemulsion comprising menthol, tween 80, ethanol and water at ratio of 20:30:30:20 was selected and evaluated. RESULTS: Microemulsion's droplet size was 163.47 nm, its polydispersity index was 0.28, and its zeta potentia

Distribution of Somatostatin and Its Receptors in the Intestinal Tract in Healthy Patients and Patients with Type 2 Diabetes.

BACKGROUND: Somatostatin is a peptide hormone exerting actions through 5 different somatostatin receptors (SSTR1-5). Somatostatin-positive cells have previously been found in intestinal mucosa and may be implicated in the pathophysiology of type 2 diabetes. Here, we characterized the distribution of somatostatin-positive cells and the mRNA expression profiles of somatostatin and SSTR1-5 along the intestinal tract of healthy individuals and patients with type 2 diabetes. METHODS: Mucosal biopsies were collected using double-balloon enteroscopy along the entire small intestine (with 30 cm inter

Organ-specific equivalent uniform dose constraints and radiobiological parameters for radiation treatment planning of abdominal tumors.

BACKGROUND: Radiation therapy planning (RTP) for abdominal tumors often requires multiple dose-volume constraints for each organ at risk (OAR), which vary with different fractionation schemes. This variability can complicate biologically optimized treatment planning. PURPOSE: This study aims to extract fractionation-independent dose constraints based on the concept of equivalent uniform dose (EUD), utilizing published clinical data for a range of fractionation regimens. The goal is to derive organ-specific radiobiological parameters as well as the EUD for each OAR that can be used in biologic

Is It Time to Replace the Duodenal Self-Expandable Metal Stent with Endoscopic Ultrasonography-Guided Gastroenterostomy for Malignant Gastric Outlet Obstruction in Patients with Pancreatic Cancer?

Malignant gastric outlet obstruction (MGOO) occurs in 2% to 25% of patients with pancreatic ductal adenocarcinoma (PDAC) who do not undergo surgical intervention. Over the past decade, duodenal self-expandable metal stent (D-SEMS) has been widely used for MGOO and has demonstrated high technical (89.1% to 100%) and clinical (85.7% to 94.3%) success rates. Endoscopic ultrasonography-guided gastroenterostomy (EUS-GE) has emerged as a promising alternative with comparable technical (89.3% to 98.9%) and clinical (89.0% to 100%) success rates. Notably, EUS-GE reduces the 6-month reintervention rate

Robotic SADI-S in modern bariatric surgery: A systematic review and limited meta-analysis of comparative and single-arm evidence.

Obesity affects over one billion people worldwide, with bariatric and metabolic surgery remaining the most durable treatment for severe disease. Single-anastomosis duodeno-ileal bypass with sleeve gastrectomy (SADI-S) is an increasingly utilised procedure in primary, revisional, and super-obese patients due to its metabolic efficacy and streamlined surgical design. Robotic assistance has been adopted to facilitate technically demanding components of SADI-S; however, the available evidence is limited to small observational studies with few laparoscopic comparators. This systematic review and li

Partial Duodeno-Ileal Diversion with Magnetic Compression Anastomosis: A Novel Approach for the Treatment of Type 1 Diabetes-Associated Metabolic Derangements in a Porcine Model.

BACKGROUND: Metabolic surgery has shown effectiveness in the management of some patients with type 2 diabetes (T2D). However, the evidence for metabolic surgery in type 1 diabetes (T1D) remains scarce. This pilot study aimed to evaluate the feasibility, safety, and metabolic impact of partial duodeno-ileal diversion (PDID) using a novel magnetic compression small bowel anastomosis (MCA) system in a T1D porcine model. METHODS: Ten adult Yorkshire pigs were subjected to intravenous infusion of streptozotocin (STZ) to induce T1D. Diabetic pigs were randomized to either PDID via MCA or control wi

Review of Familial Adenomatous Polyposis: Current Understanding and Clinical Management.

Familial adenomatous polyposis (FAP) is an autosomal dominant hereditary colorectal cancer (CRC) syndrome caused by germline pathogenic variants in the adenomatous polyposis coli (APC) gene. FAP is typically characterized by the development of hundreds to thousands of adenomatous polyps throughout the colon and rectum, with a nearly 100% chance of developing CRC if left untreated. Duodenal cancer is the second leading cause of cancer for patients with FAP; however, gastric cancer has become more prevalent in recent years with improved surveillance of the colon, rectum, and duodenum. Patients f

The role of the y(+)L-type amino acid transporter 2 in promoting zinc absorption as zinc proteinate with moderate chelation strength in broiler primary duodenal epithelial cells.

No study is currently available regarding the roles of the amino acid transporters in promoting mineral absorption in animals. To study whether the y(+)L-type amino acid transporter 2 (y+LAT2) participates in enhancing Zn absorption as Zn proteinate with moderate chelation strength (Zn-Prot M) in broilers, three experiments were carried out to investigate the effects of y+LAT2 silencing and overexpression on zinc (Zn) absorption as Zn-Prot M and related gene expressions in broiler primary duodenal epithelial cells (BPDECs) contrasted with Zn sulfate (ZnSO4). In experiment 1, three pairs of siR

Isolation of Streptococcus mutans in the gastrointestinal tract of corpses.

OBJECTIVE: The oral-gut axis, the pathway by which oral bacteria reach the intestine, has recently attracted attention. However, no recent studies have isolated live Streptococcus mutans, a major pathogen of dental caries, in the gastrointestinal tract. In the present study, we isolated S. mutans from the gastrointestinal tract of corpses. METHODS: Fifty corpses from forensic autopsies (ages 0-94 years, median age 49) were used. Samples were taken from the oral cavity and gastrointestinal tract (esophagus, stomach, duodenum, small intestine, and large intestine) using sterile swabs. S. mutans

Prior high fiber intake impinges on the cellular responses of mesenteric adipose and intestinal tissues to subsequent high fat feeding.

While high-fiber diets (HfiDs) promote weight loss, their long-term efficacy is limited by rapid weight regain upon returning to high-fat diets (HFDs). Using C57BL/6J mice in diet-switching paradigms, we characterized tissue-specific responses to HfiD-to-HFD transitions through single-nucleus and spatial transcriptomics. HfiD pre-feeding enhanced mesenteric white adipose tissue progenitor/adipocyte sensitivity to subsequent HFD exposure. In the intestine, HfiD prevented HFD-induced immune-enterocyte expansion in the duodenum and reversed the enterocyte-to-goblet cell shift in the colon while m

Defining the Intestinal eCBome and Oxylipin Signaling Systems in a TDP-43 Mouse Model of Frontotemporal Dementia.

Frontotemporal dementia (FTD) is a group of early onset and progressive disorders, characterized by degeneration in the frontal and temporal lobes, and subsequent deterioration in cognition, personality, social behavior, and language, with aggregates of the RNA-binding protein TDP-43 being present in ~45% of the cases. We reported alterations of the endocannabinoidome (eCBome) in the brain of a TDP-43 mouse model of FTD. Here we investigated the small intestinal eCBome, oxylipins, and, preliminarily, the gut microbiome. The duodenum, jejunum, and ileum of TDP-43 overexpressing versus wildtype

Genetic and metabolic regulation of eggshell quality in aging hens: Integrated multi-omics insights into CYP7A1, CALM1, and cholic/stearic acid metabolism.

While breeding improvements have prolonged the laying cycle for greater productivity, a concurrent deterioration in eggshell quality is commonly observed in aged hens. To investigate the genetic basis of this deterioration, we conducted the multi-omics analyses on the hypothalamus, liver, duodenum, and uterus from 100-week-old Rhode Island Red hens. We identified significant genomic regions associated with total eggshell weight during 99-100 weeks (TESW99-100) and eggshell brownness at 100 week (ESB100). Transcriptomic profiling revealed distinct gene expression patterns across tissues, indica
