Advances in Indocyanine Green Near-Infrared Fluorescence for Extrahepatic Biliary Identification in Laparoscopic Cholecystectomy for Acute Cholecystitis: A Narrative Review.

Laparoscopic cholecystectomy, a minimally invasive procedure with rapid recovery, remains the standard surgical intervention for acute cholecystitis. Acute inflammation of the gallbladder can cause severe edema, dense adhesions, and anatomical distortion within Calot's triangle, frequently obscuring tissue planes and increasing the risk of iatrogenic bile duct injury, a significant surgical complication. Indocyanine green fluorescence serves as a valuable intraoperative navigation tool. Its hepatobiliary metabolic characteristics enable real-time near-infrared visualization of extrahepatic bil

Liver Transplant in Primary Sclerosing Cholangitis: Results in Our Center.

Primary sclerosing cholangitis (PSC) is a chronic cholestatic disorder characterized by inflammation and subsequent fibrosis of the intrahepatic and extrahepatic bile ducts. Its clinical course is variable and may progress to advanced cirrhosis, requiring liver transplantation as the treatment of choice. At our center, we conducted a retrospective observational study of patients who underwent liver transplantation due to PSC between 1998 and 2024. The objectives were to evaluate post-transplant survival, identify the most frequent cause of mortality, determine the incidence of incidental chola

Perihilar Extrahepatic Cholangiocarcinoma With Typical and Atypical Manifestations: Diagnostic Resolution During Autopsy.

Extrahepatic cholangiocarcinoma is a rare malignant neoplasm characterized by aggressive behavior and poor prognosis. It typically presents in the seventh to eighth decades of life, with a slight male predominance, and most commonly manifests with symptoms related to biliary obstruction; however, atypical clinical presentations may occur. We report the case of a 71-year-old man who presented with acute pancreatitis. Imaging studies failed to identify an underlying neoplasm, and the diagnosis was established only during the autopsy, which revealed a perihilar extrahepatic cholangiocarcinoma inv

Implementing Continuous Nursing Care After Kasai Procedure in Infants with Biliary Atresia.

Biliary atresia (BA) is a severe cholangiopathy of infancy characterized by progressive obstruction of the extrahepatic bile ducts. Kasai portoenterostomy remains the standard initial treatment, but postoperative complications such as cholangitis, malnutrition, and progressive jaundice remain common and can compromise long-term outcomes. Conventional discharge guidance often lacks continuity, leading to poor family adherence and delayed recognition of complications once the infant returns home. To address this gap, we developed and implemented a standardized protocol for continuity of nursing

[Analysis of challenges in the surgical management of gallbladder neoplasms].

Gallbladder cancer is a highly malignant tumor of the biliary system. Radical resection remains the only treatment that can significantly improve patient prognosis. However, surgical management of gallbladder cancer in clinical practice still faces numerous challenges: intraoperative frozen section pathology and postoperative pathological examination are critical diagnostic tools, yet their accuracy is limited by medical technology and the inherent complexity of pathological diagnosis. Surgical strategies involving the extent of hepatectomy, lymph node dissection, and management of extrahepatic bi

Comparative Evaluation of Standard Cholangiography, Intravenous, and Intracholecystic Indocyanine Green Fluorescence Cholangiography During Elective Laparoscopic Cholecystectomy: Results of a Three-Arm Randomized Trial.

Background and Objectives: Bile duct injury is a relatively rare, but critical complication of laparoscopic cholecystectomy and is most commonly attributed to misinterpretation of biliary anatomy. Intraoperative biliary imaging may enhance anatomical recognition and reduce operative uncertainty, yet the optimal imaging modality remains debated. This study aimed to compare conventional intraoperative X-ray cholangiography with two fluorescence-based techniques-intravenous and intracholecystic indocyanine green fluorescence cholangiography-with respect to biliary visualization, perioperative out

Autoantibodies in the diagnostics, prognostics and follow-up of primary biliary cholangitis.

Primary biliary cholangitis (PBC), is a chronic autoimmune liver disease, characterized by cholangiopathy, cholestasis and in the long-term fibrosis, biliary cirrhosis and ultimately end-stage liver disease unless liver transplantation is applied. The diagnostics of PBC is based on biochemical tests, autoantibody measurements and liver histopathology. Abdominal ultrasound, later magnetic resonance cholangiopancreatography (MRCP) or endoscopic ultrasound can be effective methods for imaging the intra- and extrahepatic bile ducts. Concerning autoantibodies, anti-mitochondrial (AMA) and PBC-speci

Prognostic Significance of Cystic Biliary Dilatation in Primary Sclerosing Cholangitis.

BACKGROUND/AIMS: Primary sclerosing cholangitis (PSC) is a chronic, progressive disease characterized by inflammatory and fibrotic strictures in the intrahepatic and/or extrahepatic bile ducts. Typical cholangiographic findings include multifocal, annular, short bile duct strictures with minimal upstream dilatation. However, in some patients, this dilatation progresses to cystic dimensions. The current study aimed to investigate whether the clinical features and prognosis of patients with cystic dilatation (CD) of the bile ducts differ from those of patients with classic cholangiographic PSC.

How to efficiently establish animal models of cholangiocarcinoma: challenges and inspiration.

Cholangiocarcinoma (CCA) consists of a class of malignant tumors of the biliary system, with a rising incidence and carries a poor prognosis. Developing reliable animal models of CCA is crucial to elucidate the pathogenesis, identify novel therapeutic targets, and assess treatment efficacy. Among these models, the in situ primary cholangiocarcinoma model which closely mimics clinical reality, serves as a robust tool for evaluating drug efficacy, simulating tumor progression characteristics, and facilitates immunotherapy strategies. Although in situ primary CCA models have been utilized in prec

[Excerpt from the 2025 EASL practice guidelines for the diagnosis and treatment of extrahepatic cholangiocarcinoma].

In recent years, significant progress has been made in the imaging, molecular characterization, and systemic treatment of extrahepatic cholangiocarcinoma (CCA). However, early-stage detection, accurate classification, and effective management of CCA possess challenges. Therefore, the EASL commissioned an expert panel to draft evidence-based recommendations on account of recent research advances and significant differences in CCA subtypes for the management of extrahepatic CCA, including distal and perihilar CCA, with particular attention to the integration of emerging molecular concepts and pr
