Cholangiocarcinoma (CCA) is an aggressive malignancy originating from the epithelial lining of the intrahepatic or extrahepatic bile ducts. Although rare globally, its mortality closely mirrors incidence due to late-stage presentation of the disease and limited curative options. While surgical resection and liver transplantation remain the cornerstone treatments for those with resectable disease, endoscopic techniques have emerged as versatile tools for diagnosis, therapy, and palliation. In recent years, there have been major advancements in endoscopic therapies, including radiofrequency abla

We report a rare case of biliary aplasia in an infant who presented with persistent jaundice and elevated direct bilirubin despite having cholic stools. Imaging confirmed the absence of extrahepatic bile ducts, consistent with biliary atresia. Intraoperatively, there was a complete absence of an extrahepatic biliary tree consistent with biliary hypoplasia. We identified a cryptogenic hepatic plate directly fused to the right portal vein on further dissection. Despite the atypical anatomy, a modified Kasai portoenterostomy was successfully performed. The patient showed progressive clinical impr

Background: Biliary complications (BC) affect 5-32% of liver transplant (LT) patients and include strictures, leaks, stones, and disease recurrence. Their risk increases with extended criteria donor (ECD) livers, contributing to early graft dysfunction. Normothermic liver machine perfusion (NLMP) helps reduce bile duct (BD) damage overall, but anastomotic region issues persist. This study assessed hyperspectral imaging (HSI) as a non-invasive method to evaluate BD viability during NLMP. Methods: Eleven donor livers underwent NLMP with HSI at the start and end. Seven were transplanted; four wer

Cholangiocellular carcinoma (cholangiocarcinoma, CCA) is a heterogeneous group of malignant epithelial tumors of the bile ducts, with varying classifications and molecular characteristics. CCA can arise in intrahepatic, perihilar, or extrahepatic bile ducts, with its incidence rising globally, particularly in Southeast Asia due to liver fluke infections. Other risk factors include primary sclerosing cholangitis, viral hepatitis, gallstones, congenital bile duct malformations, and cirrhosis. CCA is often diagnosed at advanced stages and has a poor prognosis. This article summarizes the complex

OBJECTIVE: Biliary atresia (BA) is a progressive fibro-obliterative disease of the extrahepatic bile ducts. Laparoscopic Kasai portoenterostomy (LKPE) has emerged as a minimally invasive alternative to the open approach. We aimed to evaluate the feasibility, perioperative outcomes, and early prognostic factors of LKPE in a single center.
METHODS: We conducted a retrospective single-center study. Feasibility, operative metrics (including the learning curve), incidence of cholangitis, native liver survival, and clinical outcomes were assessed using standard statistical methods.
RESULTS: Among

BACKGROUND AND OBJECTIVE: Primary sclerosing cholangitis (PSC) is an autoimmune biliary fibrosing disease characterized by inflammation and injury of the intra- and/or extrahepatic bile ducts. The pathogenesis of PSC is poorly understood but is believed to be multifactorial, involving genetic predisposition, immunological dysregulation, and environmental influences. These may include disturbances in the gut-liver axis such as immune dysfunction in the colon and liver, alterations in the fecal and biliary microbiome, conjugation of bile acids into toxic species, and compromised intestinal epith

OBJECTIVES: To evaluate the adjunctive diagnostic value of three-dimensional MR cholangiopancreatography (3D MRCP) for identifying biliary atresia (BA) in infants with cholestasis.
MATERIALS AND METHODS: This retrospective two-center study evaluated the adjunctive diagnostic performance of 3D MRCP beyond ultrasound (US) using receiver operating characteristic (ROC) analysis. The cohort from center 1 was divided into training (n = 770) and validation (n = 330) sets, with center 2 as the test set (n = 252). The optimal cut-off for the MR triangular cord thickness (MR-TCT) was derived from the a

BACKGROUND: Cholangiocarcinomas (CCAs), including perihilar CCA (pCCA) and extrahepatic CCA (eCCA), are aggressive cancers with poor prognosis. Most patients are diagnosed at advanced stages. Jaundice and infection, commonly present at diagnosis, can delay treatment initiation. This study aimed to evaluate the impact of these conditions on time to treatment and patient prognosis in a real-world setting.
METHODS: This study included 104 patients with advanced pCCA and eCCA who were evaluated in multidisciplinary team discussions at the University Hospital of Bordeaux between July 2015 and July

Disorders of bile formation and bile flow along the intra- and extrahepatic bile ducts are summarised under the term cholestasis. Clinically, conditions resulting in retention of biliary constituents such as bile acids within hepatocytes (termed primary cholestasis) need to be distinguished from diseases characterised by bile duct injury (termed cholangiopathies). Some cholangiopathies may also cause retention of biliary constituents within hepatocytes, resulting in secondary cholestasis. Genetic variants in a multitude of genes can contribute to the development of both primary cholestasis and

Feline fibrocystic hepatobiliary lesions (FHLs) and extrahepatic biliary malformations (EBMs) are supposedly common, but prevalence studies and detailed gross and histologic descriptions within a large cohort are lacking. Our aims were to identify the prevalence of FHLs and EBMs within a large feline population, characterize gross and histologic features, and report comorbidities. Of 140 cats, 20 (14.3%) had either FHLs and/or EBMs. FHLs were most common (15/20, 75%) and included cystic biliary lesions (CBLs, 14/20, 70%) and congenital hepatic fibrosis (CHF, 1/20, 5%). CBLs had gross variabili

The study was prepared to explain the morphological features of the biliary duct system, gall bladder, duodenal papilla, extrahepatic ducts, and pancreatic ducts of the white Pekin Duck (Anas platyrhynchos) using gross, casting, ultrastructural, and histochemical techniques. The gall bladder in the liver's cystic fossa is not visible from the right lobe's parietal surface. Bile is drained from the liver via the intrahepatic bile duct, with one main duct in the left lobe branched into two secondary ducts and three hepatic ducts in the right lobe. The hepatocystic duct transports bile to the gal

Portal cavernoma cholangiopathy (PCC) is a rare condition characterized by abnormalities of the intrahepatic and extrahepatic bile ducts and the gallbladder, resulting from long-standing chronic portal vein thrombosis (cPVT). These vascular changes often occur in the setting of non-cirrhotic, non-neoplastic portal hypertension. Its prevalence among cPVT patients ranges from 4% to 92%, with an unweighted average of 45.2%, increasing to 67.0% in studies using magnetic resonance cholangiopancreatography (MRCP) or endoscopic retrograde cholangiopancreatography (ERCP). Although most patients remain

BACKGROUND: Strongyloides stercoralis (S. stercoralis), is a prevalent parasitic worm that infects humans. It is found all over the world, particularly in tropical and subtropical areas. Strongyloidiasis is caused mostly by the parasitic nematode S. stercoralis. Filariform larvae typically infest humans by coming into contact with dirt, such as by walking barefoot or through exposure to human waste or sewage.
CASE SUMMARY: A 35-year-old male presented to our department with a 10-year history of abdominal pain and diarrhea, which had recently recurred for the past 3 months. A computed tomograp

INTRODUCTION AND SIGNIFICANCE: Gallstone ileus is a mechanical intestinal obstruction caused by gallstones or stones from the common bile duct entering the intestinal canal through a biliary-intestinal fistula and becoming impacted. Multidetector computed tomography (MDCT) scanning is the most important auxiliary diagnostic method for this disease.
CASE PRESENTATION: We report a rare case of gallstone ileus involving an elderly female patient and analyze the characteristics of the MDCT images: atrophic cholecystitis, gallstones, the formation of a local duodenal-gallbladder fistula, pneumatos

Background: Laparoscopic cholecystectomy for acute cholecystitis carries an increased risk of biliovascular injuries. Fluorescence cholangiography (FC) is a valuable diagnostic tool for identifying extrahepatic bile ducts (EHBD). The objective of this study was to evaluate the efficacy of FC in delineating EHBD anatomy, both before and after dissection, based on the critical view of safety (CVS) principles. Methods: Urgently admitted patients were prospectively stratified into two groups, depending on whether they had mild or moderate acute cholecystitis, in accordance with the 2018 Tokyo guid

Neuroendocrine neoplasms (NENs) are heterogeneous tumors with a common phenotype. There are two fundamentally different groups of NENs: well differentiated with a low proliferative index, called neuroendocrine tumors (NETs) and poorly differentiated with a high proliferative index, called neuroendocrine carcinoma (NEC). Extrahepatic bile duct NETs (EBNETs) account for 0.2% to 2% of all gastrointestinal NETs and up to 2% of all malignant neoplasms of the extrahepatic bile ducts. To date, 114 cases of EBNETs have been described, with disease-free survival ranging from 2 to 240 months. In most ca
