Azithromycin in crisis: antimicrobial resistance, diagnostics gaps, and vaccine-linked stewardship for enteric infections in low- and middle-income countries.

BACKGROUND: Azithromycin remains an important oral treatment option for enteric infections in low- and middle-income countries (LMICs), where rising fluoroquinolone resistance, limited diagnostic capacity, and restricted access to parenteral therapy have encouraged widespread empirical use. Increasing azithromycin resistance among Shigella sp., typhoidal and non-typhoidal Salmonella, and Campylobacter sp., however, threatens its clinical utility. OBJECTIVES: This Perspective examines azithromycin resistance as a health-system challenge rather than solely a microbiological phenomenon. Diagnost

Invasive non-typhoidal Salmonella disease in the vaccine-development era: diagnostic blind spots, antimicrobial resistance, genomic transmission, and prevention.

SUMMARYInvasive non-typhoidal Salmonella (iNTS) disease is a severe bloodstream and focal invasive infection caused by non-typhoidal serovars of Salmonella enterica, especially S. Typhimurium and S. Enteritidis. Its clinical importance is disproportionate to its routine visibility because disease often presents as undifferentiated fever, sepsis, meningitis, severe anaemia-associated deterioration, recurrent bacteraemia, or focal infection, while definitive recognition depends on blood or other sterile-site microbiology. Global estimates indicate substantial mortality, but the true incidence re

Whole-genome sequencing links a Salmonella Newport ST164 outbreak on Fernando de Noronha to prior circulation in the Brazilian poultry supply chain.

Foodborne outbreaks at geographically isolated tourist destinations pose distinctive One Health challenges, combining limited local surveillance capacity, complex intercontinental supply chains, and high visitor turnover. In May 2021, a diarrheal outbreak linked to a gastronomic festival in Fernando de Noronha, that is a remote UNESCO World Heritage island off northeastern Brazil, was attributed to Salmonella enterica serovar Newport ST164. We applied an integrated genomic approach and epidemiological investigation to propose a transmission chain contextualizing and refining case definition of

In situ anchoring of biocompatible ergothioneine-gold nanoantibiotic on Spirulina for oral therapy of bacterial enteritis.

Severe bacterial enteritis causes significant clinical manifestations and has a widespread impact, while conventional antibiotic treatments have exacerbated antimicrobial resistance, underscoring the urgent need for effective therapeutic alternatives. To address this critical gap, we develop a biohybrid therapeutic platform that combines antibacterial ergothioneine-capped gold nanoclusters (EGT-Au NCs) with Spirulina for oral delivery. EGT alone has no antibacterial activity, yet the engineered EGT-Au NCs exhibit antibacterial efficacy approaching that of conventional antibiotics, while also m

Comparative genomics of the monophasic variant of Salmonella Typhimurium: analysis of Colombian genomes and their relationship with international lineages.

The monophasic variant of Salmonella enterica serovar Typhimurium (STVM) represents a growing threat to global public health owing to its wide dissemination, capacity to adapt to multiple hosts, and antimicrobial resistance. In this study, 98 STVM isolates recovered in Colombia (57 from humans and 41 from pig farms and abattoirs) were genomically characterized between 2015 and 2022 and compared with 102 representative genomes of international lineages by whole-genome sequencing (WGS) and phylogenomic analysis. Phylogenomic analysis revealed the existence of two well-defined endemic lineages in

Genomic Detection of Multidrug-Resistant H58 ST1 Salmonella Typhi in a South African Urban Waterway Reveals Epidemiological Links With Clinical Isolates.

Typhoid fever remains a major public health concern in South Africa. However, the role of urban waterways as environmental reservoirs and contributors to the spread of resistant Salmonella Typhi remains poorly understood. This study characterized four S. Typhi isolates recovered from the Jukskei River in Johannesburg, South Africa, using whole-genome sequencing. Genomic DNA was extracted and sequenced on the Illumina NextSeq. 2000 platform, and assembled genomes were analyzed for sequence types (STs), AMR determinants, plasmids, virulence factors, and phylogenetic relatedness using 124 clinica

Lactotransferrin upregulation affects the pathological changes of non-small cell lung cancer by regulating ferroptosis.

BACKGROUND: Recent studies have highlighted the role of ferroptosis, a form of regulated cell death driven by iron-dependent lipid peroxidation, in cancer biology. This study aims to investigate the effect of lactotransferrin (LTF) upregulation on the pathological changes related to non-small cell lung cancer (NSCLC) via the inhibition of ferroptosis. METHODS: LTF's involvement in NSCLC was investigated through cell experiments and clinical samples. Cell models with stable LTF knockdown or overexpression were established by lentiviral transduction. Cell viability and cytotoxicity were evaluat

Exposed yet unmapped: evidence of differential flood exposure in deprived urban areas using citizen science.

BACKGROUND: Due to their immature immune system, neonates are at high risk of infection. This vulnerability when combined with limited resources and health education in developing countries can lead to sepsis, resulting in high global neonatal mortality rates. Many of these deaths are preventable. The BabyGel pilot trial tested the feasibility of conducting the main randomised trial, with the provision of alcohol handgel to postpartum mothers for prevention of neonatal infective morbidity in the rural community. This secondary analysis sought to evaluate the methods of detecting infections in

Establishing a chicken embryo model for studying infection control of a novel lipid nanoparticle.

The rise of antibiotic-resistant infections highlights the urgent need for alternative therapeutic strategies. Monoglyceride-based lipid nanoparticles (LNPs) offer both antimicrobial activity and the potential for controlled drug delivery. In this study, LNPs with diameters of 230-350 nm and ζ-potential of approximately -35 mV demonstrated high colloidal stability and strong antibacterial efficacy in vitro, achieving complete growth inhibition of Staphylococcus aureus (S. aureus) at 0.04 mg/mL and Salmonella enterica subsp. enterica serovar Enteritidis (S. Diagnostic and pathological characterization of senecavirus A-associated epidemic transient neonatal losses in swine.

Senecavirus A (SVA) has emerged as a significant viral pathogen of swine, traditionally associated with idiopathic vesicular disease but more recently linked to epidemic transient neonatal losses (ETNL), a syndrome characterized by increased mortality in piglets without consistent vesicular lesions. The mechanisms underlying SVA-associated neonatal mortality remain unclear due to the lack of specific clinical signs, frequent co-infections, and limited tissue characterization. This retrospective study analyzed 186 neonatal submissions (<24 days old) that were tested for SVA by RT-qPCR at the Io

Too Hot, Too Wet: Bayesian Spatial Modeling of Climate-Driven Salmonella Risk in New South Wales, Australia, 1991-2022.

Salmonella infections contribute significantly to gastrointestinal-related hospitalisations in Australia and remain a major global public health concern. Although seasonal patterns in Salmonella incidence have been documented globally, there is limited evidence on the influence of climatic factors, particularly rainfall, humidity, flooding, and temperature, in the Australian context. This study investigated the relationship between climatic extremes and Salmonella infections across Local Health Districts (LHDs) in New South Wales (NSW), Australia, using a Spatial Bayesian Distributed Lag Non-L
