L-Histidine alleviates tacrolimus-induced islet dysfunction via the gut-pancreas axis.

Tacrolimus is widely used after organ transplantation but can impair pancreatic β-cell function and glucose homeostasis. Using rodent models, antibiotic-mediated microbiota depletion, fecal microbiota transplantation, 16S rRNA sequencing, metabolomic profiling, and MIN-6 cells, we examined whether microbiota-associated L-histidine contributes to β-cell protection. Tacrolimus altered gut microbial composition and increased histidine-associated microbial and metabolic signals. L-histidine supplementation improved glucose tolerance and insulin secretion, reduced β-cell apoptosis, and increased PI

External validation of the SNAPP score for hepatocellular carcinoma recurrence after living donor liver transplantation.

BACKGROUND: Binary morphologic criteria cannot capture biologic heterogeneity among living donor liver transplantation (LDLT) recipients. SNAPP score (tumor size, number, AFP, PIVKA-II, PET/CT) yields a graded recurrence-risk estimate but lacks independent validation. We externally validated SNAPP for hepatocellular carcinoma (HCC) recurrence after LDLT. METHODS: SNAPP was applied without refitting in 230 LDLT recipients with HCC at two Korean centers and benchmarked against Milan, French-AFP, Metroticket 2.0, MoRAL, RETREAT, and three LDLT-derived selection systems (Kyoto, Medanta LDLT, and

[18F]DOPA PET Reveals Metabolically Active Transcriptomic States Associated with Clinical Outcome in Pancreatic Neuroendocrine Tumors.

[18F]fluorodihydroxyphenylalanine ([18F]DOPA) uptake is dependent on amino acid transport and decarboxylation pathways, including large neutral amino acid transporter 1, encoded by SLC7A5, which are linked to tumor aggressiveness. This study evaluated whether in vivo uptake of [18F]DOPA can serve as an imaging biomarker for pancreatic neuroendocrine tumors (panNETs), reflecting a tumor's transcriptional status and a patient's clinical course. Methods: This retrospective single-center study was based on a transcriptomic cohort of 74 primary panNETs. A subset of 22 panNETs from patients who unde

TGF-β-Targeting Platinum(IV) Prodrugs Remodel Tumor Microenvironment of Pancreatic Cancer via Antifibrotic and Immunomodulatory Effects.

Pancreatic ductal adenocarcinoma (PDAC) features dense desmoplastic stroma that impedes drug penetration and immune infiltration, and promotes therapeutic resistance. As a central driver of stromal fibrosis and immunosuppression, TGF-β represents an attractive target for overcoming these barriers. Herein, we report novel Pt(IV) prodrugs conjugated with TGF-β inhibitors (hydronidone/4-hydroxyphenylacetic acid), which simultaneously achieve chemotherapy and tumor microenvironment remodeling. Biological screening identified HP-12C and AP as lead compounds with tumor-selective cytotoxicity and sub

Targeted Gastrointestinal Delivery of mRNA Lipid Nanoparticles Enables Systemic Vaccination and Glucagon-like Peptide-1 Metabolic Therapy.

Lipid nanoparticles (LNPs) administered parenterally often show poor localization to multiple tissue types, including the pancreas and gastrointestinal (GI) tract. Furthermore, patients typically prefer orally administered drugs to those that are given parenterally. We therefore investigated whether GI LNP administration to buccal, gastric, small intestinal, colonic, and rectal submucosal tissues could simultaneously circumvent parenteral administration while enhancing LNP delivery to previously inaccessible locations. We found that, in addition to local targeting, gastric LNP administration a

Impact of Donor Substance Use on Outcomes Following Pancreas Transplantation.

BACKGROUND: Donor substance use is considered by many a risk factor for poor outcomes following pancreas transplantation, yet little is known regarding the impact of donor substance use on actual outcomes. This study aimed to determine the impact of substance use, particularly cocaine, on early and late outcomes following pancreas transplantation. METHODS: This was a single center, retrospective analysis of all pancreas transplants from 2003-2023. Clinical outcomes including 30-day peak serum lipase, 7-day, 90-day, and 1-year pancreas allograft survival were assessed by donor exposure to subs

Robotic-assisted transduodenal ampullectomy for a neuroendocrine tumour: A step-by-step surgical video.

BACKGROUND: When endoscopic resection of ampullary neuroendocrine tumors (NETs) is not feasible due to tumour size or location, transduodenal ampullectomy can offer a parenchymal-sparing alternative to a formal pancreaticoduodenectomy (Whipple procedure) [1,2]. We present a robotic-assisted transduodenal ampullectomy (RATA) in a 71-year-old male with an incidental 14 mm ampullary NET found during routine screening. Preoperative 68Ga-DOTATATE PET-CT demonstrated intense somatostatin-receptor avidity (SUVmax 66.8) and absent FDG uptake. The MDT recommended surgical excision due to size and Ki-67

Resection of solid pseudopapillary neoplasms of the pancreas: a 25-year experience at the mayo clinic.

INTRODUCTION: Solid pseudopapillary neoplasms (SPNs) are rare pancreatic tumors with generally favorable outcomes following surgical resection. However, recurrence can occur in a small subset of patients, and clinicopathologic factors associated with recurrence remain incompletely defined. We evaluated the surgical management and long-term outcomes of patients undergoing resection for SPN at the Mayo Clinic. METHODS: A retrospective review of patients undergoing pancreatectomy for SPN between 2000 and 2024 was performed, analyzing demographic, clinical, operative, pathological and survival da

Pancreatic head solid-pseudopapillary neoplasm in a man treated by robot-assisted laparoscopic partial pancreatectomy: a case report.

BACKGROUND: Solid-pseudopapillary neoplasm (SPN) of the pancreas is a rare tumor with low malignant potential, most commonly occurring in the pancreatic body or tail of young women. Atypical presentations, particularly in male patients and cases involving the pancreatic head, may pose diagnostic and therapeutic challenges. This report highlights surgical management considerations in a patient with inconclusive preoperative findings. CASE PRESENTATION: We report the case of a 37-year-old man with a large lesion initially interpreted as a retroperitoneal mass closely associated with the pancrea

Intraoperative Pancreatoscopy Demonstrates Feasibility of Pancreatic Preservation During Robotic Pancreatoduodenectomy for IPMN in Elderly Patients.

BACKGROUND: Main duct intraductal papillary mucinous neoplasm (MD-IPMN) carries a substantial risk of malignant transformation and often requires surgical resection.1,2 Preservation of the remnant pancreas remains an important goal, particularly in elderly patients.3,4 Therefore, determining the optimal extent of resection requires a careful balance between oncological safety and preservation of pancreatic function.5,6 The utility of intraoperative pancreatoscopy during robotic pancreatoduodenectomy (RPD) for this purpose has not been well established.7 METHODS: We present two elderly patients

A methylation-dependent reader couples arginine methylation to S6K2 signaling in vivo.

Arginine methylation regulates diverse cellular processes, yet how methyl-arginine reader proteins couple this modification to kinase signaling in vivo remains poorly defined. Here, we identify staphylococcal nuclease and Tudor domain containing 1 (SND1) as a pancreas-enriched effector that links symmetric dimethylarginine (SDMA) recognition to S6 kinase 2 (S6K2) activation. Disruption of SDMA-mediated binding by a point mutation in the SND1 Tudor domain markedly diminishes ribosomal protein S6 (S6) phosphorylation in the pancreas. Mechanistically, SND1 engages SDMA-modified arginines within t

Comprehensive insight into the anti-dyslipidemic and pancreatic lipase inhibition activities of Clematis flammula L. and Fraxinus angustifolia Vahl. extracts using two animal models.

Clematis flammula and Fraxinus angustifolia leaves are commonly used in Algeria to treat inflammatory diseases, with potential hypolipidemic effects that have not been investigated so far. The present study explored the in vitro, ex vivo, and in vivo hypolipidemic effects of ethanolic leaf extracts from both plants using two animal models (mice and rabbits) with distinct lipid metabolism. Dyslipidemia was induced by oral administration of Triton X-100, once daily for three consecutive days. Immediately after each Triton X-100 administration, animals received plant extracts (100 and 200 mg/kg b

Venous Thromboembolism in Acute Necrotizing Pancreatitis Is Associated With Worse Outcomes, With Subtype-Specific Patterns: A Nationwide Analysis, 2016-2022.

OBJECTIVES: Venous thromboembolism (VTE) complicates acute necrotizing pancreatitis (ANP), but outcomes across extra-splanchnic deep vein thrombosis (DVT), portal vein thrombosis (PVT), and splenic vein thrombosis (SVT) remain poorly defined. We evaluated national trends and in-hospital outcomes in ANP with versus without VTE and across VTE subtypes. METHODS: Using the National Inpatient Sample (2016-2022), adults hospitalized with principal ANP were identified. Secondary diagnosis codes captured VTE subtypes. Primary analyses compared mortality and major complications in ANP with versus with

Endoscopic ultrasound in hepatology: A European Society of Gastrointestinal Endoscopy (ESGE) Technical and Technology Review.

BACKGROUND: Endoscopic ultrasound (EUS)-guided techniques are increasingly applied in hepatology, including for tissue acquisition, vascular access, pressure measurement, and selected therapeutic interventions. However, adoption remains heterogeneous because of variability in technical approaches, devices, outcome definitions, operator expertise, and the quality of available evidence. METHODS: This European Society of Gastrointestinal Endoscopy (ESGE) Technical and Technology Review summarizes the current evidence on selected EUS-guided techniques in liver disease and portal hypertension, inc

MAF Transcription Factors: Emerging Key Players in Cardiovascular Health and Disease.

Cardiovascular diseases are a leading cause of morbidity and mortality worldwide, and their complex pathophysiological mechanisms remain incompletely understood. The regulatory role of transcription factors provides a crucial avenue for investigating the mechanisms underlying these diseases. The MAF transcription factor family is a key member of the AP-1 superfamily, comprising two subfamilies: large MAFs (including MAFA, MAFB, MAF [c-MAF], and NRL) and small MAFs (including MAFK, MAFG, and MAFF). Different members exhibit distinct cellular expression patterns and functions within tissues: sma

Intestinal Macrophage-Mediated Exosomal Repackaging Establishes Bioactive Reservoirs for Long-Acting Oral Peptide Therapy.

Oral peptide delivery systems are conventionally engineered to improve intestinal absorption, but they rarely address the progressive and systemic nature of chronic inflammatory disorders such as type-2 diabetes (T2D), which requires both durable glycemic control and immunometabolic management. Here, we report an oral delivery system based on porous calcium carbonate/sodium alginate (PCC/SA) microparticles loaded with exenatide-chitosan oligosaccharide (EXE-COS). The PCC/SA carrier facilitates the efficient intestinal absorption and transepithelial transport of EXE-COS. Notably, in the lamina

Gjb2 V37M Modifies Metabolic Responses to Chronic High-Fat Feeding in Mice.

GJB2 encodes connexin 26 (Cx26), and pathogenic GJB2 variants are among the most common genetic causes of hereditary hearing loss; because Cx26 is expressed in metabolically relevant tissues including liver, pancreas, and adipose tissue, whether the hearing-loss-associated c.109G>A (p.V37M) variant also modifies systemic metabolic responses to high-fat diet (HFD) exposure remains unclear. Three-month-old male wild-type (WT) and homozygous Gjb2 V37M mice were assigned to low-fat diet (LFD) or high-fat diet (HFD) groups, generating four study groups: WT-LFD, WT-HFD, V37M-LFD, and V37M-HFD.
