Impaired insulin secretion in response to mixed meal ingestion is associated with hypersomatostatinemia in individuals with cystic fibrosis.

Cystic fibrosis (CF) is a monogenic disease characterized by exocrine pancreas insufficiency, glucose intolerance, and a high prevalence of diabetes. Diminished insulin secretion is a key contributor to glucose intolerance and diabetes in people with CF, yet the pathophysiology of this deficiency is not fully understood. Somatostatin is a hormone secreted by several tissues in the body, including the delta cells of the pancreas, and is known to inhibit insulin secretion; however, its role in postprandial hypoinsulinemia in people with CF remains unclear. This cross-sectional study included 16

Pancreatic hydatid cyst with a characteristic "water-lily" sign.

BACKGROUND: Cystic echinococcosis rarely involves the pancreas and may mimic common cystic neoplasms. Recognition of characteristic imaging signs is central to diagnosis; the "water-lily" sign (floating detached endocyst) is a rare and highly characteristic finding in favor of transitional-stage hydatid cyst and, when recognized, can prevent inappropriate cyst puncture and guide stage- and anatomy-based management. CASE: A 34-year-old woman from Iran, an endemic region for cystic echinococcosis, presented with intermittent epigastric/right upper quadrant pain. At an outside institution, endos

Solid Pseudopapillary Epithelial Neoplasm of the Pancreas: Two Cases and a Review of the Literature.

Solid pseudopapillary epithelial neoplasm (SPEN) of the pancreas is a rare cystic tumor of exocrine origin that predominantly affects young women. It may manifest with nonspecific abdominal symptoms or be discovered incidentally during imaging for unrelated conditions. We present two cases of SPEN involving the pancreatic body in young female patients, both successfully treated with curative surgical resection. Histopathological and immunohistochemical analyses in both cases confirmed the diagnosis, demonstrating characteristic pseudopapillary architecture and nuclear β-catenin positivity. Com

Increased risk of cancer in patients with mixed connective tissue disease: a population-based cohort study in Korea.

INTRODUCTION: We conducted a nationwide, population-based cohort study to evaluate the risk of cancer in Korean patients with mixed connective tissue disease (MCTD) compared with the corresponding risk in the general population. METHODS: Using the Korean National Health Insurance Service data, we identified 2,094 patients with newly diagnosed MCTD between 2004 and 2020 (16,409 person-years). Standardized incidence ratios (SIRs) were calculated to compare the overall and site-specific cancer risks between patients with MCTD and the age- and sex-matched general population. RESULTS: The overall

Risk assessment of multi-focal side-branch IPMNs: Long-term follow-up of one thousand patients.

BACKGROUND: While the natural history of solitary side-branch intraductal papillary mucinous neoplasms (SB-IPMNs) is increasingly well-described, the prevalence, disease course, and oncologic risk of multifocal disease remain poorly defined. It is unknown whether the presence of multiple cysts confers additional oncologic risk compared to solitary lesions. Our objective was to evaluate the natural history and oncologic risk associated with multifocal SB-IPMNs compared with solitary SB-IPMNs during long-term surveillance. METHODS: Patients with side-branch intraductal papillary mucinous neopla

Spatiotemporal metabolic networks in a preclinical model of pancreatic cancer and associated cachexia.

Cachexia or body wasting is a significant contributing factor to the morbidity and mortality of pancreatic cancer. Despite an unmet need, its metabolic underpinnings remain poorly understood. Here, we characterize a mouse model that recaptures the longitudinal progression of human cachexia during pancreatic cancer development. We profile the metabolomes of pre-, early- and late-stage cancer-associated cachexia in male and female pancreas, its interstitial fluid (IF), plasma, liver, adipose tissue and skeletal muscle. We find that each tissue has a unique metabolome and trajectory across stages

Hybrid Insulin Peptides in Type 1 Diabetes: How They Form and Why They Matter.

Type 1 diabetes is a T cell-mediated autoimmune disease that leads to the destruction of the insulin-producing beta cells of the pancreas, yet the antigens that drive this response have been difficult to define. This review traces the identification of hybrid insulin peptides (HIPs), posttranslationally modified autoantigens formed when a fragment of insulin is joined through a peptide bond to a fragment of a second beta cell granule protein, creating a sequence not encoded by any single gene. Beginning with the search for the antigen of the diabetogenic BDC-2.5 clone in the nonobese diabetic

Pembrolizumab plus gemcitabine and cisplatin for advanced biliary tract cancer: longer-term follow-up from the randomised, double-blind, placebo-controlled, phase 3 KEYNOTE-966 trial.

BACKGROUND: At the final analysis of KEYNOTE-966 (median follow-up 25·6 months), pembrolizumab plus gemcitabine and cisplatin (pembrolizumab group) versus placebo plus gemcitabine and cisplatin (placebo group) provided a statistically significant, clinically meaningful improvement in overall survival without any new safety signals in participants with advanced biliary tract cancer. We report results from a median study follow-up of 36·6 months (∼3 years). METHODS: In the randomised, double-blind, placebo-controlled, phase 3 KEYNOTE-966 trial conducted at 175 medical centres worldwide, eligibl

CCKBR signalling controls human definitive endoderm specification and pancreatic lineage differentiation.

AIMS/HYPOTHESIS: Paracrine signalling by hormone-producing endocrine cells potentially influences pancreatic development, but this feedback regulation remains poorly understood. Here, we investigated the roles of gastrin (GAST), cholecystokinin (CCK) and their receptor cholecystokinin B receptor (CCKBR) during human pancreatic development. METHODS: Human iPSCs were differentiated stepwise into definitive endoderm and then the pancreatic endocrine lineage. To assess CCKBR function, CRISPR/Cas9 was used to generate CCKBR-knockout iPSC lines. We profiled transcriptional states by bulk and single

GSDMD palmitoylation induces acinar cell pyroptosis via promoting GSDMD-N oligomerization to aggravate hyperlipidemic acute pancreatitis.

Pancreatic acinar cell pyroptosis played a key role in acute pancreatitis (AP), while the underlying molecular mechanisms were unclear. In this study, hyperlipidemic acute pancreatitis (HLAP) model mice and primary acinar cells were employed for the in vivo and in vitro experiments, respectively. Western blot was employed to assess Gasdermin D-N-terminal pore-forming domain (GSDMD-N) oligomerization; Flow cytometry was employed to assess cell pyroptosis; Immunofluorescence was performed to assess GSDMD-N location in the cytoplasm; HE and TUNEL staining were employed to assess the pathological

Heterotaxy Syndrome Combining Univentricular Heart, Polysplenia and Truncated Pancreas: A Case Report.

BACKGROUND AND OBJECTIVES: Univentricular heart is a complex congenital heart disease with rare survival into adulthood. It could be part of a heterotaxy syndrome when, apart from the cardiac malformations, there are associated abnormal arrangements of extra-cardiac organs like the lungs, spleen and pancreas. The syndrome is said to be rarer than the univentricular heart and to have a worse clinical outcome. METHODS: After clinical assessment, the patient was investigated with Chest X-ray, Electrocardiography, Echocardiography, and contrast-enhanced CT scan of the chest and abdomen to make th

New insights into β-cell vulnerability in type 1 diabetes-Lessons from nature.

Type 1 diabetes results from an autoimmune-mediated destruction of pancreatic β-cells, yet considerable heterogeneity exists regarding disease onset, progression and severity. Emerging evidence suggests that this variability reflects not only differences in immune responses, but also differences in the architecture and immune landscape of the endocrine pancreas. Our recent Steve Morgan Foundation Grand Challenge studies have provided important 'lessons from nature', revealing that the endocrine tissue in the human pancreas exists as a continuum of endocrine objects (EOs), ranging from isolated

Temporal Trends in Six Major Cancers: Implications for Lifestyle-Based Cancer Prevention and Early Detection Across the Life Course.

BACKGROUND/OBJECTIVES: Cancer remains a serious public health challenge in the United States. Six major cancers (female breast, colorectal, liver, lung/bronchus, pancreas, and prostate) account for a substantial proportion of the national cancer burden. Understanding the changes in the incidence of these cancers over time can provide insights for prevention and control. This study examined temporal trends in the cancer incidence according to age at onset and stage at diagnosis. METHODS: This population-based study analyzed data from the United States Cancer Statistics database for 2001-2023,

Belatacept Conversion in Simultaneous Pancreas-Kidney and Liver-Kidney Transplant Recipients: A Single-Center Feasibility Cohort and Narrative Review.

BACKGROUND: Calcineurin inhibitor (CNI) nephrotoxicity drives long-term kidney-function decline after simultaneous pancreas-kidney (SPK) and liver-kidney (LK) transplantation, but belatacept conversion, as an alternative strategy, is poorly studied in these recipients, whose non-kidney allograft rejection risk differs. METHODS: We retrospectively studied six SPK and six LK recipients converted from CNI- to belatacept-based immunosuppression and analyzed the cohorts separately alongside a narrative review of published SPK/LK conversion experiences. Outcomes were estimated glomerular filtration

Rare origin of the right inferior phrenic artery from the anterior inferior pancreaticoduodenal artery in a patient with hepatocellular carcinoma.

The inferior phrenic arteries show considerable variation in their origins. We describe a rare origin of the right inferior phrenic artery (RIPA) from the anterior inferior pancreaticoduodenal artery (AIPDA) in a patient with ruptured hepatocellular carcinoma (HCC), identified by contrast-enhanced computed tomography (CT) and angiography, and discuss its anatomical and clinical significance. A woman in her 80s underwent emergency transcatheter arterial embolization for ruptured HCC. Preprocedural contrast-enhanced CT did not identify the origin of the RIPA at its usual sites but suggested that

Pancreatic steatosis is not associated with impaired insulin secretion among individuals with diabetes.

Aims We aimed to assess pancreatic and hepatic steatosis (PS/HS) association with residual insulin secretion and insulin resistance in individuals with diabetes. Methods Pancreatic/liver fat fractions (PFF/LFF) in magnetic resonance imaging (MRI) were used as a proxy for PS and HS in 170 participants. Insulin secretion was estimated by HOMA2-B, C-peptide response 6 minutes after an i.v. bolus of 0.5 mg glucagon (ΔC-Pe), and fasting proinsulin to C-peptide ratio (PI:C). Insulin resistance was estimated by HOMA2-IR, Adipo-IR and i.v. insulin (0.05 IU/kg) tolerance test. Results PFF did not corre

Pancreatic Invasion in Distal Cholangiocarcinoma Is a Poor Prognostic Factor: A New Approach Integrating Tumor Depth of Invasion.

BACKGROUND: The 8th edition of the American Joint Committee on Cancer (AJCC) staging system for distal cholangiocarcinoma (dCCA) uses depth of invasion (DOI) for T classification; however, it does not consider invasion into adjacent organs such as the pancreas. We investigated the prognostic significance of DOI and pancreatic invasion in patients with dCCA. METHODS: We retrospectively analyzed 130 patients with dCCA who underwent pancreatoduodenectomy between 2003 and 2016. The optimal DOI cutoff for predicting overall survival (OS) was determined using time-dependent receiver operating chara

Case Report: Metastatic duodenal hepatoid adenocarcinoma: a rare pathology with exceptional response to dual immune checkpoint blockade.

Hepatoid adenocarcinoma (HAC) is a rare, malignant, extrahepatic tumor with hepatocellular differentiation. HAC is often associated with elevated serum alpha-fetoprotein (AFP) concentrations, which makes it difficult to distinguish from primary hepatocellular carcinoma (HCC). It is a difficult diagnosis based on histopathology, as the immunohistochemistry (IHC) profile often overlaps with hepatocellular carcinoma. HAC can arise from multiple organs, including the stomach, esophagus, colon, pancreas, duodenum, and peritoneum, with the stomach being the most common site. The treatment of HAC is
