Divergent tissue tropism and pathogenicity of novel recombinant GI-19 and GI-22 infectious bronchitis virus strains.

Infectious bronchitis virus (IBV) is a highly contagious pathogen in poultry that causes significant economic losses worldwide. In this study, two novel IBV isolates, CK/CH/MY/24053 of the GI-19 lineage and CK/CH/SC/25605 of the GI-22 lineage, were isolated and analyzed through whole-genome sequencing, recombination mapping, pathogenicity assessment in SPF chickens, and S1 protein structural comparison. Whole-genome analysis revealed that CK/CH/SC/25605 underwent recombination across different genotypes involving multiple parental lineages, whereas CK/CH/MY/24053 exhibited recombination within

Durable Marked Response of Unresectable Esophageal Squamous Cell Carcinoma After Radiotherapy Alone Followed by Combination Immunotherapy.

We report the case of a 70-year-old man with unresectable esophageal squamous cell carcinoma who achieved durable marked response with combined immune checkpoint inhibitor (ICI) therapy following radiotherapy (RT) alone. The patient presented with dysphagia and stridor due to a bulky esophageal tumor and cervical lymph node metastasis compressing the trachea. He was diagnosed with cT3N1M0 (Stage IIIB; 8th edition of the TNM classification) or cT4(101R-Trachea)N1M0 (Stage IVA; 12th edition of the Japanese classification). He underwent RT alone (total dose, 50 Gy), resulting in substantial tumor

Ectopic Cushing Syndrome Due to a Large Mediastinal Neuroendocrine Tumor.

Ectopic Cushing syndrome is an uncommon cause of hypercortisolism that needs rapid recognition and treatment to reduce complications. Here, we present the case of a 33-year-old man with a 1-year history of severe Cushing syndrome. Biochemical findings showed both high ACTH and 24-hour urine free cortisol, and nonsuppressed morning serum cortisol. The 18fluorine-1,4,7-triazacyclononane-1,4,7-triacetate-octreotide positron emission tomography/computed tomography revealed a large mediastinal tumor with high uptake. Initial biopsy reported a grade 1 neuroendocrine tumor with positive ACTH immunost

Anesthetic Management of a Tracheal Deformity in an Elderly Patient With Chronic Obstructive Pulmonary Disease: A Case Report.

Saber sheath trachea is a tracheal deformity characterized by marked transverse narrowing relative to the anteroposterior diameter and is frequently associated with chronic obstructive pulmonary disease (COPD). Although saber sheath trachea has been discussed as a potentially challenging airway in anesthetic practice, perioperative airway complications have been reported mainly in individual case reports, and their true incidence, severity, and clinical implications remain uncertain. We report the anesthetic management of an elderly patient with COPD and saber sheath trachea undergoing laparos

Pri micropeptide functions as a cell-intrinsic timer controlling the transient phase of cell fate induction.

During development, cells sequentially acquire specific fates through temporally ordered regulatory systems. To ensure the harmonious progression, each system must be activated and subsequently inactivated at the appropriate time. In this study, we show that the duration of fate induction is controlled by the transient expression of polished rice (pri), a gene encoding micropeptides, during Drosophila tracheal development. pri is transiently expressed in prospective tracheal placodes and precedes the expression of trachealess (trh), a master transcription factor that initiates tracheal fate. p

Tracheal melanoma in a cat.

Extraocular melanocytic neoplasms are rare in cats and occur mainly in the skin and oral cavity. Tracheal neoplasms are also rare in cats, with lymphoma being the most commonly reported tumor. Here, we describe a tracheal melanoma in an 8-y-old spayed female domestic shorthair cat that died after 8 d of cyanosis, dyspnea, and wheezing. Radiographic and tracheoscopy findings were consistent with a mid-tracheal obstruction. Gross findings consisted of a poorly demarcated, pale-yellow, 28 × 16 × 15-mm mass that effaced and expanded most of the mid trachea and partially occluded the tracheal lumen

Airway Management During Laparoscopic Surgery in a Patient With Saber Sheath Trachea: A Case Report.

Saber-sheath trachea is a U-shaped diffuse narrowing of the thoracic trachea, specifically observed in patients with chronic obstructive pulmonary disease. The airway management of patients with saber-sheath trachea can be challenging because it possesses characteristics both of tracheal stenosis and tracheomalacia. A 78-year-old man was diagnosed with gastric cancer and underwent robot-assisted laparoscopic total gastrectomy with jejunal interposition reconstruction. We provided general anesthesia combined with thoracic epidural anesthesia for this case. For intraoperative airway management,

[Clinical Case: Micro-drainage of subcutaneous emphysema Grade V through a fenestrated peripheral venous catheter in critical ill].

BACKGROUND: Approximately 30% of surgical procedures in the United States require orotracheal intubation. Generally laryngeal and tracheal injuries are rare. However, when they do occur, they can lead to major complications. Subcutaneous emphysema is the most common clinical presentation and is mostly treated conservatively and self-limiting. However, when it is Grade V, control strategies should be considered. There are reports in the literature on micro-drainage of subcutaneous emphysema through a fenestrated peripheral venous catheter with satisfactory results. CLINICAL CASE: We present th

Clinical significance of tracheal deviation in patients with idiopathic pleuroparenchymal fibroelastosis.

BACKGROUND: Idiopathic pleuroparenchymal fibroelastosis (IPPFE) is a rare form of interstitial pneumonia, characterized by upper lobe-predominant fibrosis. Tracheal deviation is frequently observed in this condition; however, its clinical relevance remains unclear. This study aimed to clarify the clinical significance of tracheal deviation in patients with IPPFE. METHODS: We retrospectively analyzed 84 patients diagnosed with IPPFE at our institution between 2005 and 2024. Tracheal deviation on chest radiographs was defined as tracheal displacement beyond the midline. The distance and angle o

High-flow nasal oxygen with flexible channel oxygen insufflation for ankylosing spondylitis and cervical fracture-dislocation: a case report and literature review.

BACKGROUND: Ankylosing spondylitis predisposes patients to cervical fracture and spinal cord injury after minor trauma. Tracheal intubation must therefore be performed with minimal cervical motion to avoid iatrogenic neurological damage, a challenge for the anaesthetist. CASE PRESENTATION: A 41-year-old man with ankylosing spondylitis sustained a traumatic fracture of the seventh cervical spine, with displacement and spinal cord compression. After thorough airway assessment, he received high-flow nasal oxygen to maintain oxygenation, combined with oxygen insufflation through the flexible worki

A field-ready colourimetric LAMP assay for detection of Mycoplasma gallisepticum using rapid DNA extraction.

Mycoplasma gallisepticum (MG) is a major respiratory pathogen of poultry, and rapid point of care tests for routine on farm surveillance are currently unavailable. This study describes a user-friendly Loop Mediated Isothermal Amplification (LAMP) assay for detecting MG, used together with a rapid DNA extraction method that supports field deployment. The performance of the LAMP assay was compared with a conventional PCR assay using three sample sets, including MG cultures, tracheal swabs from vaccinated and unvaccinated chickens, and cloacal swabs from turkeys. DNA was extracted using a commerc

Respiratory sound analysis in a rabbit tracheomalacia model.

Tracheomalacia, which softens the tracheal wall and leads to airway collapse during breathing, can be challenging to detect, especially in children, due to its vague symptoms and the invasive nature of standard diagnostic procedures. In this study, we developed a rabbit model of tracheomalacia by surgically removing a portion of the tracheal cartilage in New Zealand White rabbits. Breath sounds and airway pressure were recorded using a clinical-grade stethoscope setup. From each exhaled breath, 6,373 acoustic features were extracted, and 51 of these, which showed consistent and statistically s

CSE Induces Lung Epithelial Cell-derived Exosomal LINC01133-mediated FOXA1 Activity Through SRSF6 to Affect MRC-5 Cellular Lipid Metabolism and Autophagy.

Smoking is an important risk factor for the pathogenesis of idiopathic pulmonary fibrosis (IPF). Cigarette smoke (CS) damages alveolar epithelial cells and induces them to release exosomes (Exos). Although exosomal LINC01133 has been shown to regulate epithelial–mesenchymal transition (EMT), its effect and mechanism of action in IPF remain unclear. The present study aimed to investigate the effect and molecular mechanism of exosomal LINC01133 in IPF. Bleomycin (BLM) was injected into the trachea of mice to establish an IPF animal model, and the mice were treated with CS or Exos. In addition, 5

Right-Sided Aortic Arch Presenting as Chronic Cough in a 50-Year-Old Patient With DiGeorge Syndrome: A Report of a Rare Case.

A right-sided aortic arch is a rare congenital vascular anomaly that can be seen in individuals with 22q11.2 microdeletion syndromes, such as DiGeorge syndrome. Although most patients are asymptomatic, airway or esophageal compression may occur in a small subset of individuals. In this case, a 50-year-old male patient with DiGeorge syndrome presented with a persistent dry cough for roughly six weeks that was not improving despite medical treatment. Computed tomography (CT) imaging showed a right-sided aortic arch, and over several years, it progressively continued to compress the patient's tra

Bronchial artery embolization before endobronchial resection to avoid severe bleeding in central airway obstruction: a single-center retrospective pilot study.

BACKGROUND: Bronchoscopic resection represents a facilitated alternative option for restoration of airway patency in selected cases. However, the procedure is susceptible to complications arising from severe hemorrhaging. This single-center retrospective pilot study aimed to evaluate the safety and feasibility of bronchial artery embolization (BAE) prior to endobronchial resection in patients with central airway obstruction (CAO). METHOD: Patients with CAO caused by hypervascular tumors with strong enhancement on computed tomography (CT) and/or hemoptysis were included in the study. All neopla

Convex-probe endobronchial ultrasound-guided cryobiopsy for pleural tumors: three case reports.

BACKGROUND: Endobronchial ultrasound-guided cryobiopsy (EBUS-cryo) is an emerging minimally invasive technique that enables acquisition of high-quality tissue specimens, particularly from mediastinal lymphadenopathy. Here, we describe three cases in which its application to pleural tumors helped establish a definitive diagnosis. CASE DESCRIPTION: Case 1 involved a 74-year-old man with multiple pleural lesions detected via computed tomography (CT), the largest of which was encased by major vessels. EBUS-cryo sampling of the largest lesion, performed through the left lower bronchus, led to the

Impact of respiratory motion on dose to the airways in central and ultra-central lung stereotactic ablative body radiotherapy.

BACKGROUND AND PURPOSE: Stereotactic ablative body radiotherapy (SABR) achieves excellent local control in medically inoperable non-small cell lung cancer. However, treating central and ultra-central tumours carries significant risks to adjacent critical structures, notably the central airways. This study evaluated the suitability of average value intensity projection (AvIP) imaging for airway delineation and assessed the adequacy of a 5 mm planning risk volume (PRV), by quantifying the influence of respiratory motion on dose to the airways using four-dimensional (4D) dose accumulation. Veno-venous-extracorporeal membrane oxygenation as a bridge to surgical airway in an intellectual disability patient with a large thyroid goiter: a case report.

INTRODUCTION: Large thyroid goiters are uncommon in Western countries but remain relatively frequent in the Middle East and parts of Africa. These goiters can compress or displace the trachea, causing airway narrowing, respiratory distress, and higher anesthetic risk. Airway management becomes even more challenging in uncooperative or cognitively impaired patients. In such cases, extracorporeal membrane oxygenation (ECMO) can provide a valuable alternative to conventional intubation, though it introduces distinct challenges for surgeons, intensivists, and anesthesiologists. CASE PRESENTATION:
