AIM OF THE STUDY: Delays in diagnostic paracentesis for suspected spontaneous bacterial peritonitis (SBP) contribute to preventable mortality in cirrhosis. This study aimed to develop and internally validate a pragmatic bedside score using routinely available admission variables to stratify the risk of SBP at hospital presentation.
MATERIAL AND METHODS: In this retrospective single-center cohort study, 1,312 consecutive adults with cirrhosis and ascites admitted between January 2020 and January 2025 were screened. A total of 1,000 unique patients met the eligibility criteria and were randomly

Erysipelothrix rhusiopathiae is a pathogenic bacterium infecting both humans and animals and causing erysipeloid, a disease that can leads to osteoarticular conditions, abscesses, and septic arthritis. Here, we report the sequenced ancient genome of E. rhusiopathiae obtained from teeth of an Alan child who lived in the North Caucasus in 6th-7th centuries AD. Differential diagnosis using paleopathological methods suggested that the child suffered from tuberculosis. However, analysis of tooth pulp revealed no signs of M. tuberculosis but a massive presence of ancient E. rhusiopathiae DNA. A comp

BackgroundLocal spread linked to breast carcinoma demonstrates a diverse spectrum of clinical presentations, including zosteriform, telangiectatic, and erysipeloid lesions. Pigmented epidermotropic infiltration resulting from breast carcinoma is seldom observed, occasionally posing diagnostic challenges owing to its resemblance to various other pigmentary disorders, such as malignant melanoma and other melanocytic lesions.Patient PresentationA 47-year-old woman presented with complaints of a hyperpigmented, crusted lesion involving the nipple-areolar region of the left breast, which on histopa

Septicaemic erysipeloid caused by Erysipelothrix rhusiopathiae is a rare zoonosis with high mortality. Hepatic injury as a complication has never been documented, and quantitative monitoring of pathogen clearance dynamics remains unexplored. To investigate the clinical characteristics of liver function damage caused by septicaemic erysipeloid, and explore reasonable diagnosis and treatment strategies. The clinical data of a 46-year-old male patient with liver function damage caused by septicaemic erysipeloid are summarized. Blood agar plate culture, drug sensitivity test, and a PCR method were

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

A diffuse erythematous, edematous tender swelling with a distinct border and local rise in temperature suggest a morphological diagnosis of erysipelas-like eruption. While cellulitis and pseudocellulitis have an ill defined tender erythematous plaque. Erysipelas, and cellulitis are not a straightforward diagnosis as multiple cutaneous eruptions mimic erysipelas. It is important to have comprehensive knowledge about the different causes of erysipelas-like eruptions and pseudocellulitis to reach a diagnosis and treat appropriately. Many infectious diseases such as histoplasmosis, leishmaniasis,

Leishmaniasis is caused by an intracellular parasite transmitted to humans by the bite of a sandfly: Phlebotomus. The disease can present in three ways: visceral, cutaneous, or mucocutaneous forms. Unusual clinical presentations of cutaneous leishmaniasis have been reported: psoriasiform, eczematiform, erysipeloid, and sporotrichoid, depending on host immune status and Leishmania subspecies. We report a case of an unusual presentation of erysipeloid cutaneous leishmaniasis treated with the combination of Metronidazole and Clarithromycin. A 67-year-old woman presented with a 2-month history of

BACKGROUND: Breast cancer has become the second most common cancer after lung cancer. Patients may present with skin manifestations at the time of initial diagnosis, while erysipel-like carcinoma typically appears later, following initial treatment. This delay increases the risk of misdiagnosis.
CASE PRESENTATION: The patient was a 51-year-old female. A modified radical mastectomy for left breast carcinoma (pT2N3M0, stage IIIC; tumor size 4.6 cm × 4.5 cm × 1.6 cm, 14/21 axillary lymph nodes involved), HER2-positive type, was performed on April 21, 2021. In April 2024 (three years post-surgery

Erysipeloid cutaneous leishmaniasis (ECL) is a rare presentation of cutaneous leishmaniasis that classically presents as erythematous, infiltrated lesions on the centrofacial region. We conducted a systematic review on the demographic characteristics, clinical presentations, laboratory findings, and treatment of ECL in the Old World. A systematic literature search was performed using the terms ("erysipeloid" OR "erysipelas") AND "leishmaniasis" across Web of Science, PubMed, Scopus, Embase, and Google Scholar. Only studies reporting cases from the Old World were included. Based on the anatomic

Cutaneous metastases are rare manifestations of malignancies. We report a case of a 50-year-old man with an atypical presentation of erysipelas-like skin lesions, later found to be cutaneous metastases from an unknown primary. 18 F-FDG PET/CT revealed visceral metastases; however, it failed to demonstrate uptake in the skin lesion, while 68 Ga-FAPI PET/CT additionally showed intense uptake in the skin, including the right parotid gland, which is likely the primary site. IHC findings confirmed a primary salivary duct adenocarcinoma. This case underscores FAPI PET/CT's utility in detecting FDG-n

KEY CLINICAL MESSAGE: Bullous pemphigoid did not follow the course of Gastric Carcinoma relapse and remisson, unlike the malignant acanthosis nigricans which was in alignment with the paraneoplastic effect of the Gastric Carcinoma.
ABSTRACT: Acanthosis nigricans (AN) is a dermatosis characterized by the presence of hyperpigmented, velvety cutaneous thickening in the flexural areas, posterior neck, and occasionally the extensor surfaces of hand, face, and oral mucosa. AN is commonly associated with insulin resistance, drugs, and rarely internal malignancy (malignant AN). Bullous pemphigoid (BP

BACKGROUND: The clinical appearance of cutaneous leishmaniasis (CL) lesions located in the nasal area can be confused with many inflammatory or tumoral diseases. In this study, we aimed to determine the frequency of nasal involvement and the morphological variants of CL lesions located in the nasal region.
METHODS: In this retrospective study, we included patients who presented to our Skin and Venereal Diseases Clinic in the province of Şanlıurfa, located in the Southeastern Anatolia region of Turkey between September 2022 and October 2023, and were diagnosed with CL with nasal involvement. F

Carcinoma en cuirasse is a rare form of cutaneous metastasis characterized by the spread of a primary malignant tumor to the skin, most commonly associated with breast cancer in women. It may present as papulonodular lesions, erysipeloid, sclerodermiform infiltration, or en cuirassed, typically appearing months or years after the initial diagnosis of the primary malignancy. Diagnosis of carcinoma en cuirasse can be challenging, but histology can help distinguish it from other skin conditions. Treatment options for carcinoma en cuirasse are not well-defined due to the limited number of reported

Erysipelothrix rhusiopathiae is a facultative anaerobe Gram-positive bacillus, which is considered a zoonotic pathogen. E. rhusiopathiae causes erysipeloid, mainly in occupational groups such as veterinarians, slaughterhouse workers, farmers, and fishermen. Two cutaneous forms (localised and generalised) and a septicaemic form have been described. Here, we report the isolation of a strain of E. rhusiopathiae from a 56-year-old immunocompetent obese male admitted to Fondazione IRCCS Policlinico San Matteo Pavia (Italy). Blood cultures were collected and Gram-positive bacilli were observed. E. r

Zoonotic cutaneous leishmaniasis (ZCL) is a neglected tropical disease caused by Leishmania (L.) major. This zoonosis is characterized by a broad-spectrum clinical polymorphism and may be underestimated and poorly treated since it is a simulator of various dermatoses. The aim of our study was to analyze the clinical polymorphism of patients with ZCL. A total of 142 patients with confirmed CL based on the microscopic examination of skin lesion biopsies were included in this study. Molecular typing of Leishmania species revealed that all patients were infected with L. major. In total, 14 clinica

parts of the world (1,2). CL is characterized by significant clinical variability. An ulcerated nodule on the exposed parts of the body (corresponding to the parasite inoculation site by the vector insect) is the classic presentation. However, other forms of clinical presentations also exist (3,4). CL can be present on unusual locations such as the scalp, the genital region, or palmoplantar areas. Localization in the foot poses a diagnostic challenge due to similarity to disorders which produce ulcerations. The latter include diabetes and leprosy. A 75-year-old Afghani man presented with a lar

Cutaneous leishmaniasis can present in many different clinical forms. Diagnosis of atypical forms is often delayed. It is useful to keep in mind the diagnosis of cutaneous leishmaniasis, a mimicking disease, to reduce unnecessary treatment and patient morbidity. Erysipeloid leishmaniasis should be considered when presented as long-term erysipelas-like lesions that do not respond to antibiotics. We want to present our five patients with erysipeloid leishmaniasis, one of the atypical clinical forms.
