Association between age and outcomes of evidence-based feeding protocol versus usual care in critically ill patients: secondary analysis of a multicenter cluster-randomised controlled trial.

BACKGROUND: Nutrition therapy is essential in critically ill patients, yet large randomised trials have shown inconsistent results regarding the effect of an evidence-based feeding protocol. The NEED trial showed a feeding protocol could effectively promote delivery of enteral nutrition (EN) but did not reduce mortality. Age-related physiological differences may modify responses to nutritional therapy. This post hoc analysis of the NEED trial evaluated whether age modifies the association between implementation of an evidence-based feeding protocol and clinical outcomes among critically ill pa

Pathway incompatibility between NF-κB and RAS signaling constrains oncogenicity in B-cell leukemia.

Oncogenic pathways do not always cooperate; in some contexts, their co-activation is antagonistic and suppresses tumorigenesis, a phenomenon we termed pathway incompatibility. However, the mechanisms underlying this antagonism and the role of receptor context in shaping these interactions remain unclear. During normal B-cell development, precursor B-cell receptor (pre-BCR) signaling supports survival and proliferation of early B-cell precursors before transition to expression of the mature B-cell receptor (BCR). B-cell acute lymphoblastic leukemia (B-ALL), the most common childhood cancer, is

Inflammatory and catabolic activity mediate the association of plasma β-hydroxybutyrate and mortality in heart failure.

INTRODUCTION: Ketone body levels are elevated in patients with heart failure (HF). Recently, plasma levels of ketone bodies have been associated with mortality in patients with HF. However, little is known about the biological processes active in patients with high levels of ketone bodies. The objective of this study was to investigate the features and prognosis of patients with HF with elevated β-hydroxybutyrate levels. Furthermore, we aimed to determine the specific biological processes that are active in these patients that may result in elevated levels of β-hydroxybutyrate. METHODS: β-hyd

Understanding physiological adaptations of pregnancy to the chronic hypoxia of high altitude to inform obstetrical and neonatal care.

The chronic hypoxia experienced by high-altitude residents profoundly impacts pregnancy, influencing maternal health, placental function, and the growth and development of the fetus. Exposure to chronic hypoxia during pregnancy increases the risk of complications like preeclampsia and fetal growth restriction and has lasting effects on offspring, extending from infancy to adulthood. Understanding how chronic hypoxia alters physiological adaptations to pregnancy and pregnancy outcomes is essential for high-altitude populations and for gaining broader insights into the mechanisms underlying redu

Effect of Exercise on Coronary Hemodynamics Under the Earth's Gravity and Microgravity Conditions: A CFD Study.

Exposure to microgravity affects the cardiovascular system and fluid distribution in the vasculature system of the human body depending on the short term or long term exposure. Literature suggests that in the initial few days in the microgravity environment, a short term exposure, the cardiac output and plasma volume are altered. Gradually, after about a week of long exposure to the microgravity environment, these parameters return to the same levels as those under the Earth's gravity. In this computational fluid dynamics (CFD) study, we investigate the effect of microgravity environment on co

ABO Blood Group Mismatch in Heart Transplantation Is Not Associated With Elevated Donor-Derived Cell-Free DNA.

Donor-derived cell-free DNA (dd-cfDNA) is a reliable, noninvasive marker for detecting allograft rejection after heart transplantation (HT). We evaluated whether dd-cfDNA levels differ in recipients of ABO-compatible, nonidentical (mismatched) grafts. We performed a retrospective single-center cohort study of 219 adult HT recipients transplanted between January 2019 and May 2023, each with at least one dd-cfDNA sample collected between November 2019 and April 2024. ABO mismatch was defined as a donor blood type A, B, or AB that was compatible with but nonidentical to the recipient's type; only

Navigating failure and trust in clinical training: Balancing trainees' autonomy while safeguarding patients.

Trust is like porcelain-precious, strong in its purpose, but fragile in its nature. When it breaks, we can piece it back together, yet the cracks will always remain. Each line tells a story of rupture and repair, of effort poured into restoration. The question, though, is not simply can it be repaired, but whether we are willing to live with its imperfections and learn from them. Trust sits at the heart of medicine. Between patients and physicians, between learners and their supervisors, between colleagues or peers who depend on each other in the most vulnerable of moments. We know how easily

IL-1β-driven SERPINE1 Upregulation is Associated with CAD-ICM-related Cardiomyocyte Remodeling.

INTRODUCTION: Coronary artery disease (CAD) may progress to ischemic cardiomyopathy (ICM) and heart failure through maladaptive cardiomyocyte remodeling and myocardial fibrosis. This study aimed to identify key molecular mediators linking inflammatory and fibrotic signaling during this pathological transition. METHODS: Single-cell RNA sequencing data from human ventricular tissues, including 2 CAD, 3 ICM, and 3 healthy samples (12,818 cells; 7 cell clusters), were integrated for analysis. Cell-cell communication and pseudotime trajectory analyses were performed. Findings were further validate

Deep Learning Discrimination of Colorectal Cancer using Retinal Fundus Photographs: UK Biobank Case-Control Study.

INTRODUCTION/OBJECTIVE: Retinal fundus photography captures microvascular and neuroretinal features reflecting systemic health. While Deep Learning (DL) has enabled extraction of cardiometabolic and aging biomarkers from retinal images, its application to oncologic disease status discrimination remains largely unexplored. This study evaluated whether DL analysis of retinal fundus photographs can discriminate individuals with colorectal cancer (CRC) from those without CRC (non-CRC) in a curated UK Biobank case-control setting. METHODS: This retrospective imaging study used 1,600 UK Biobank fun

Beyond Glycemic Control: Multi-pathway Cardioprotective Mechanisms of Selective Na+/glucose Co-transporter 2 Inhibitors with Emerging Implications for Atrial Fibrillation.

INTRODUCTION: Atrial fibrillation (AF) is the most common form of cardiac arrhythmia and is associated with electrocardiographic changes resulting from several complex mechanisms. While management of AF relies mainly on ion channel blockers and beta blockers for cardioversion and control of ventricular rate, many patients remain symptomatic despite the use of optimal therapy, highlighting the need for new upstream drug therapies to further improve management. METHODS: In this narrative review, we summarize and discuss key findings related to the use of selective sodium-glucose co-transporter

A human engineered mini-heart platform for mimicking ventricular pump function.

AIM: Engineered cardiac tissue models for in vitro physiological studies often fail to replicate the pump function of the heart. Despite promising advancements, the use of engineered cardiac chambers is often hindered by complex fabrication processes and invasive characterization techniques. Here, we engineered a chamber-like human cardiac model with pumping function, referred to as a 'mini-heart', by employing a novel sacrificial molding approach within a customized bioreactor. METHODS AND RESULTS: Human pluripotent stem cell-derived cardiomyocytes and human cardiac fibroblasts were embedded

Concomitant atrial fibrillation ablation and left atrial appendage occlusion: rationale, outcomes, and future directions.

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia worldwide and remains a major contributor to stroke, heart failure, hospitalization, and cardiovascular mortality. Catheter ablation has become a central strategy for rhythm control, while left atrial appendage occlusion (LAAO) offers an effective mechanical option for stroke prevention in patients with contraindications to oral anticoagulation. Performing AF ablation and LAAO during the same procedure addresses complementary therapeutic goals while potentially improving procedural efficiency, reducing hospital exposure,

Conditions That Limit Outpatient Abortion Care at Clinics in the United States.

OBJECTIVE: While most people in the United States obtain abortion care in outpatient settings, such facilities may limit care for some individuals because of a variety of conditions. The situations when abortion care cannot be obtained in the outpatient setting have not been evaluated, leaving a deficit in understanding the need for hospital-based abortion care. METHODS: We obtained outpatient abortion facility care protocols from the Society of Family Planning #We Count project, which, in January 2024, invited participating organizations with outpatient physical facilities to share their pro

Spatial heterogeneity of APD under long QT conditions: characterisation and relationship to sinus activation sequence.

AIMS: Enhanced spatial variation in ventricular action potential duration (APD) is known to contribute to arrhythmia development under long QT conditions, although the causes of APD variation are unclear. Specifically, between-individual variation in the locations of pro-arrhythmic long APD islands and associated repolarisation gradients is difficult to explain in the context of established regional variation in electrophysiology. This study aimed to explore the mechanisms underlying the spatial heterogeneity of ventricular APD under baseline and long QT conditions. METHODS AND RESULTS: New Z

Role of Interventional Radiology in Management of C-Section Scar Ectopics.

BACKGROUND: Cesarean Scar Ectopic Pregnancy (CSEP) is an uncommon but potentially fatal syndrome defined by the implantation of the gestational sac inside the scar tissue of a prior cesarean surgery. The incidence of cesarean deliveries is rising, currently accounting for up to 6% of all ectopic pregnancies. CSEP causes unique challenges in diagnosis and treatment because it can lead to serious problems like uterine rupture, placenta accreta spectrum, bleeding, and infertility. CASE REPORT: This report describes two patients with Type I CSEP who were treated by using a less invasive method th

The polymeric nanovectors landscape for cancer immunotherapies towards advanced therapy medicinal products (ATMPs).

Despite major advances in oncology, cancer remains one of the leading causes of mortality worldwide, underscoring the need for more effective and personalized therapeutic solutions. Advanced therapy medicinal products (ATMPs), including gene and cell-based therapies, have emerged as a promising frontier for immune cell engineering in oncology. However, current approaches still face several technical/safety issues, mainly associated with the delivery strategies used and their complex manufacturing processes. In this context, polymeric nanovectors (pNVs) have gained attention as a promising tool

P2X3 receptor antagonism improves hindlimb blood flow during exercise in a conscious ovine model of hypertensive heart failure with preserved ejection fraction.

Exercise intolerance in heart failure with preserved ejection fraction (HFpEF) is linked to impaired vascular regulation and exaggerated autonomic responses. Although sensory afferent signalling via P2X3 receptors (P2X3Rs) contributes to exaggerated cardiovascular reflex activity in hypertension, its role in mediating haemodynamic limitation during HFpEF is unknown. We tested whether selective P2X3R antagonism improved blood pressure and exercise haemodynamics in a conscious ovine model of hypertensive HFpEF. Female hypertensive HFpEF sheep and age-matched sham controls were instrumented for c

Depressive Symptoms, sleep Disturbance, and Cardiometabolic Multimorbidity Among Adults With Arthritis: A Cross-Sectional Analysis of NHANES 2013-2020.

While the association between depressive symptoms and cardiometabolic disease is well established, the relationships among depressive symptoms, sleep disturbance, and cardiometabolic multimorbidity (CMM) in adults with arthritis remain insufficiently understood. This cross-sectional study used National Health and Nutrition Examination Survey (NHANES) 2013-2020 data and included 4031 adults with arthritis. Depressive symptoms were assessed using the 9-item Patient Health Questionnaire (PHQ-9), and sleep disturbance was defined by self-reported physician-diagnosed sleep disorder. CMM was defined
