Large anterior glenoid rim fractures challenge the shoulder surgeon to restore stability while minimizing surgical morbidity. As arthroscopic fixation techniques continue to evolve, the pursuit of increasingly robust biomechanical constructs must be balanced against technical complexity and the potential for iatrogenic complications. But how much fixation is adequate to result in a stable shoulder and promote bone healing? With respect to fracture biology, primary compression and AO trauma principles may not be necessary in the glenohumeral joint with appropriate immobilization and postoperati

Restoration of joint function and mechanics is the goal of every shoulder stabilization surgery. Achieving this goal in the setting of altered anatomy and bone loss is easier said than done. In restoring a joint, it is important to understand what native tissue can be repaired, what can be augmented, or what needs to be reconstructed. For this, we rely on biomechanical research, patient outcomes, and long-term follow-up of patients. Bone augmentation procedures have provided a crucial role in shoulder instability treatment, but perhaps it is time to consider soft-tissue augmentation to the gle

BACKGROUND: Ballistic trauma to the proximal humerus poses a significant challenge because of extensive bone loss and soft tissue damage, often involving the rotator cuff, deltoid muscle, and axillary nerve. In cases where limb preservation is the priority, scapulohumeral arthrodesis (SHA) remains a viable option, particularly when prosthetic reconstruction is contraindicated because of severe musculotendinous injury. The Masquelet 2-stage induced membrane technique has shown promising results in lower limb reconstruction, yet its application for upper limb arthrodesis in ballistic injuries re

This study investigated relationships between bullet mass, velocity, kinetic energy (KE), and fracture propagation in flat bones. Sixty full metal jacketed 9-mm bullets of different masses were fired at pig (Sus scrofa) scapulae. A chronograph measured bullet velocity prior to impact. Fracture characteristics were documented using standard terminology, counted, and measured at the entrance aspect, exit aspect, and overall per specimen. Few significant relationships between ballistic and fracture variables were found across the ranges tested, suggesting possible threshold effects. Bullet mass i

BACKGROUND: This study evaluated the long-term clinical and radiological outcomes of operatively treated scapular fractures using the tissue-sparing posterior Brodsky approach. We hypothesized that osteosynthesis using this approach would yield favorable functional outcomes.
METHODS: This retrospective study included patients with acute scapular fractures treated operatively using the posterior Brodsky approach from January 2015 to December 2019. Radiologic evaluation included fracture classification and fracture union. Functional outcomes were assessed using the Constant-Murley Score (CMS),

BACKGROUND: The scapula free flap is an unpopular option for osseous reconstruction of the head and neck in part due to the inability to perform simultaneous ablative surgery and harvesting of the free flap. In addition, it was originally described as having minimal bone stock making dental rehabilitation with dental implants very difficult.
METHODS: Retrospective review from 2015 to 2025 of patients that underwent segmental resections involving the maxillofacial complex and received scapula free tissue transfer with dental implants. Contributing surgeons are affiliated with the following ins

BACKGROUND: Scapula fractures are rare injuries (< 1% of fractures), and there remains substantial variability regarding decision-making for the treatment of displaced scapula fractures. Scapula fractures are often underdiagnosed and therefore undertreated. Despite the existence of surgical interventions, there remains a paucity of understanding with regard to the appropriate triaging of displaced scapula fractures, which can lead to significant long-term detriment to patients who sustain these injuries. With a better understanding of surgical indications, surgeons can more efficiently counsel

PURPOSE: Scapula alata (SA) is characterized by scapular winging and limited shoulder function, often as a result of long thoracic nerve impairment. While the physical manifestations are well-documented, the broader implications, particularly the psychological and social consequences, remain underexplored. This study aims to explore the lived experiences of patients with SA.
MATERIALS AND METHODS: A qualitative, cross-sectional design was applied. Semi-structured interviews were conducted with 11 patients from two Danish regional hospitals. Data were analyzed using Malterud's Systematic Text

Scapular fractures (SFs) are severe injuries that cause swelling, pain, and restricted movement in the posterior shoulder area. Surgical intervention is recommended for patients with significant displacement or multiple fractures. While most SF surgeries utilize general anesthesia, the role of nerve blocks is often overlooked. The infraspinatus-teres minor (ITM) interfascial block, a combined suprascapular and axillary nerve block technique, has been shown to be safe and effective. It is theorized that the ITM interfascial block may specifically target analgesia at the subacromial bursa, enhan

PURPOSE: The purpose of this study was to evaluate the Latarjet coracoid graft status using computed tomography (CT) at minimum 5-year follow-up in a population with less than 5% glenoid bone loss preoperatively. We hypothesized that complete graft lysis would occur in accordance with Wolff's law.
METHODS: This retrospective multicenter study included patients treated with an open Latarjet procedure across 8 centers who had less than 5% preoperative glenoid bone loss and available preoperative and follow-up CT scans. Glenoid bone defects were measured using the best-fit circle method on 2D CT

BACKGROUND: Failed shoulder arthroplasty often presents with glenoid bone loss, posing a challenge for both surgeons and patients. This study aimed to evaluate the available revision strategies for failed shoulder arthroplasty in the presence of severe glenoid bone loss. Severe bone loss was defined as a glenoid that is not capable of reconstruction using a standard reverse total shoulder glenoid or augmented baseplate.
METHODS: Following the guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses, PubMed and Google Scholar were systematically searched for (Revisi

The aim of this study was to compare the results of patients who underwent biceps tenotomy or tenodesis alongside rotator cuff rupture repair (RCRR), as well as to assess its relationship with alterations in subacromial volume. The hypothesis was that patients undergoing biceps tenodesis would experience greater improvements in outcome scores 2 years after the surgery and larger subacromial volumes as the long head of the biceps tendon maintains its tension. Methods: This is a retrospective study including patients who underwent an RCRR concomitant with a biceps procedure (88 tenotomy and 63

BACKGROUND: Aseptic glenoid loosening is the most common cause of revision after anatomic total shoulder arthroplasty (aTSA). Inlay glenoid components have been shown to reduce edge loading and opposite-edge lift-off forces compared with onlay glenoids. Early clinical results of inlay glenoids have been promising, although there is a paucity of literature detailing mid-term to long-term follow-up. We report the minimum 5-year clinical and radiographic outcomes after aTSA with the Arthrosurface inlay glenoid component and a stemless, aspherical humeral head in an active, young patient populatio

BACKGROUND: Variants of the shoulder are commonly encountered and may occasionally be confused with pathological conditions. Our goal is to provide an overview of common normal variants of the shoulder's anatomical structures as well as any associated clinical or surgical relevance.
MATERIAL AND METHODS: Following a literature search of common variants of the shoulder and supporting structures, this review provides a summary of variants related to the scapular body, acromion, glenoid, coracoid, labrum, long head biceps tendon, glenohumeral ligamentous complex, and rotator cuff.

Primary lymphoma of the bone is an uncommon malignancy that most frequently involves the long bones while the flat bone involvement is rare. We report the case of a man in his 20s who presented with progressive shoulder pain without systemic symptoms. Imaging demonstrated an intramedullary lytic lesion of the acromion with cortical thinning and minimal soft tissue involvement. Histopathological examination confirmed germinal centre type diffuse large B-cell lymphoma. Fluorine-18 fluorodeoxyglucose positron emission tomography/CT demonstrated a solitary hypermetabolic acromial lesion without no

BACKGROUND: Posterior humeral subluxation (PHS) in B2 and B3 glenoid is a cause of asymmetric long-term stress on the glenoid and the potential reason for glenoid loosening in anatomic total shoulder arthroplasty and painful glenoid erosion in hemiarthroplasty with metallic heads. We hypothesized that corrective and concentric (C2) reaming of the glenoid associated with pyrocarbon hemiarthroplasty (HA-PYC) could improve the centering of the humeral head and decrease the risk of persistent painful glenoid erosion in young and active patients with B2 and B3 glenoid.
METHODS: Between 2014 and 20

BACKGROUND: Postcapsulorrhaphy arthropathy (PCA) may result as a long-term sequela of instability surgery, eventually necessitating shoulder arthroplasty. Alterations to the soft tissue and bony anatomy during the index surgery may lead to unique wear patterns of the glenohumeral joint compared with what is observed in primary glenohumeral osteoarthritis (GHOA). We sought to characterize the three-dimensional glenoid morphology of patients with PCA to understand whether arthritic wear patterns are distinct from those observed in GHOA.
METHODS: Patients diagnosed with GHOA or PCA were retrospe

BACKGROUND: Proximal humeral fractures (PHFs) are a significant healthcare burden, representing the third most common osteoporotic fracture in older adults. Although open reduction and internal fixation (ORIF) with the Proximal Humeral Internal Locking System (PHILOS) provides a reliable rate of osseous union, postoperative complications such as shoulder stiffness and residual pain remain prevalent. Current rehabilitation protocols are predominantly glenohumeral-centric and often overlook fracture-associated scapular dyskinesis. This randomized controlled trial (RCT) aims to evaluate whether i

CASE: We present the case of a 33-year-old man with recurrent anterior and posterior shoulder instability, with bidirectional glenoid bone loss. The patient underwent all-arthroscopic double bone block procedure using iliac crest grafts fixed with suture anchors. At the 6-year follow-up, the patient remained pain-free, with no recurrent dislocations, and achieved a full range of motion with normal muscle strength.
CONCLUSION: The all-arthroscopic double bone block technique using suture anchors is a feasible and durable procedure for complex bidirectional glenoid bone loss. This method provid
