It is difficult to determine the size and localization of rotator cuff tears preoperatively. But with the special arthrographic technique, a diagnosis with about 80% accuracy was possible in 65 surgically confirmed rotator cuff tears. With this technique, 22 massive cuff tears were found in conservatively treated patients. In these patients, the plain roentgenograms obtained at the initial examination were also analyzed. The roentgenographic findings included narrowing of the acromiohumeral interval and degenerative changes of the humeral head, the tuberosities, the acromion, the acromioclavic

The neck, shoulder, and upper trunk serve as donor sites for commonly used flaps in head and neck reconstruction. In order to prevent permanent functional disabilities, a physical therapy program was organized. The anatomy, physiology, and kinesiology of the shoulder muscles that are commonly used in myocutaneous flaps are reviewed. Their functional deficit and compensation are described. Exercise protocols for each group of muscles were developed. Prevention of postoperative "frozen shoulder" and the avoidance of tension and torsion of the transposed muscle pedicle are important during the im

Reports of isolated long thoracic nerve palsy are rare in Japan. We reported a case of isolated long thoracic nerve palsy, resulted from recurrent injury to the nerve. Muscle CT and electrodiagnostic study were useful for confirming diagnosis of this cases. This patient was a student aged 20 years, with nothing of importance in his family or past history. After he started practicing archery, winging of left scapula was gradually developed. Physical examination revealed weakness and atrophy of left serratus anterior muscle. There was no wasting and weakness of other should girdle muscles. Hemat

Analysis of Neandertal and recent human scapular glenoid fossae reveals that the former had long, narrow, and flat glenoid articular surfaces relative to those of modern humans. Comparison of glenoid length, breadth, and curvature to humeral articular dimensions demonstrates that Neandertal glenoid length and curvature scale to proximal and distal humeral articular dimensions in the same manner as those of modern humans. The remaining contrast is in the relatively greater glenoid fossa width seen in modern humans. This difference in morphology implies differences in the habitual degree of dors

Between 1975 and 1979, anterior acromioplasty was performed in 65 patients with 66 involved shoulders. The procedure appears to be safe and reasonably effective. Of the 65 patients in this study, 50 had chronic tendon inflammation with fibrosis, and 16 also had a small supraspinatus tendon tear. These 16 had rotator cuff repairs. Twenty-six patients were also treated with distal clavicle excision, and seven had tenodesis of the long head of the biceps brachii. The average age of the patients was 50 years (range, 23-75 years). All patients were followed for an average of eight years (range, thr

A study of human cadaveric glenohumeral joints was undertaken to investigate the mechanism of external rotation during abduction. Previously explored extra-articular influences such as the coracoacromial arch and the rotator cuff muscles were removed from the specimens. A specially designed device was used to measure abduction in the scapular plane as well as any rotation of the humerus. The findings show that external rotation is a consistent feature of passive abduction in the plane of the scapula and could take place in the absence of extra-articular influences. The mechanism appears to lie

This study reviews the cases of 21 children with 24 Sprengel's disease, operated from 1961 to 1981. They were classified according to the classification of P. Rigault and J.C. Pouliquen (5 grade I, 12 grade II, 4 grade III). Multiple deformities were associated. The average operative age was 8 years old. The surgical procedure was in 3 cases a partial scapular resection; in 15 a unilateral Woodward and in 3 a bilateral Woodward. The authors have observed 7 post-operative complications, including 2 brachial plexus palsies. The average follow-up is 12 years. Cosmetic results are good in 57.2 per

This study reviews the demographic, radiologic, and histologic characteristics of 13 cases of an important primary skeletal neoplasm, giant cell tumor of bone, occurring in an uncommon location, the scapula. That eight of 13 patients presented prior to 20 years of age contrasts significantly with the typical age distribution (between 20-40 years) encountered in giant cell tumors arising in long bones. As it does elsewhere in the skeleton, giant cell tumor of the scapula frequently demonstrates cystic and/or telangiectatic components on histologic examination. The radiologic appearances of gian

The authors present the preliminary results of 50 cases of impingement syndrome treated surgically using an operative technique which at least included an anterior Neer acromioplasty. Surgery was performed after a period of progression ranging from several months to several years. These included 2 Neer stage 1 (4%), 29 stage II (58%) and 19 stage III (38%). In all cases an anterior Neer acromioplasty was carried out. In 18 cases, an additional procedure was also performed (resection of the acromial extremity of the clavicle, suture of a rotator cuff rupture, resection reinsertion of the long h

1266 cases of Periarthropathia humeroscapularis (PHS) were analyzed according to their clinical and sonographic findings. We found in 32.8% a PHS simplex (simple tendinitis), in 12.6% a PHS adhäsiva (stiff and painful shoulder with intact cuff), in 17% a PHS calcarea (calcareous tendinitis) and in 33.3% a PHS destructiva (rotator cuff lesions). Isolated tendinitis of long head of biceps was rare (0.7%). Frozen shoulder (adhaesive capsulitis) was differentiated from PHS adhäsiva and accounted of 1.9%. 33.3% of frozen shoulder patients suffered from type I Diabetes mellitus. Women were more affe

A "winged" scapula is a rare, poorly understood, and potentially disabling curiosity following anesthesia and surgery. It is produced by dysfunction of the long thoracic nerve and consequent paralysis of the serratus anterior muscle. A survey of senior anesthesiologists indicated a consistent lack of familiarity with the entity. This article presents six cases of postoperative long thoracic nerve palsy. In a literature review of 111 instances of long thoracic nerve palsy, 51 were trauma-related, 47 were either idiopathic or of debatable origin, and 13 appeared following a surgical or obstetric

Seventeen patients with complete separation of the acromioclavicular joint were operated on acutely with transposition of the coracoid tip to the clavicle. All the patients were examined after a mean follow-up time of 7.5 years. Twelve patients had good function, 3 had slight pain in extreme positions, and 2 had poor function with pain at rest. Eleven patients had the screw removed, 2 of them because of deep infection. In 3 patients the coracoid tip fragmented during the operation. Two patients had an unsatisfying cosmetic result. As long as nonoperative treatment gives equal or better long-te

A panel of 399 individuals from 24 kindreds with facioscapulohumeral muscular dystrophy (FSHD) has been established for genetic linkage studies. A previous suggestion of linkage on the distal long arm of chromosome 14 to the locus (IGHG) for the constant region of the heavy chain of IgG immunoglobulin was tested from serum Gm allotypes and from DNA analysis using an IGHG DNA probe. After applying an age-dependent weighting for presently unaffected but at risk individuals close linkage between the IGHG and FSHD loci was excluded.

This is a case report of a 15-year-old boy with an unusual fracture of the glenoid. The mechanism of injury was a fall on the partially extended, abducted arm. The fall produced a fracture with displacement of the inferior half of the glenoid. The technique of open reduction and exposure used differed from the standard approach in that the long head of the triceps was sectioned at its glenoid origin to facilitate the reduction. At one-year follow-up, the patient demonstrated a full range of motion and excellent triceps strength.

Elastofibroma dorsi is a pseudotumorous connective tissue mass that characteristically arises between the chest wall and the inferior angle of the scapula. Its long axis is typically craniocaudal. Two patients are illustrated, each presenting with a fixed subscapular mass. Computed tomography showed each mass as elongated, incompletely marginated, and of soft tissue density deep to the inferior angle of a scapula. The latissimus dorsi and an underlying layer of fat were posterolaterally elevated inferior to the tip of the scapula. After incisional biopsy excluded sarcoma, each mass was locally

Based upon an analysis of observations of 93 patients treated for dislocation of the acromial end of the clavicle and a study of long-term results of the treatment of 68 of them, the authors recommend using the method of a one-step closed setting the acromial end of the clavicle followed by a percutaneous transarticular fixation of the clavicular-acromial junction with the Kirschner wire and plaster bandage for 6 weeks.

Major disturbances of shoulder function secondary to axillary burn contracture require some form of surgical release if conservative therapy has been unsuccessful. Any area large enough to need extensive skin grafts should be considered for reconstruction using a parascapular fasciocutaneous flap. This flap, which may be elevated easily, provides an option for single-stage correction of the deformity without the need for long-term rehabilitation or splinting.

Compared to other painful conditions on the shoulder suprascapular nerve entrapment is an obscure and uncommon syndrome causing severe shoulder pain and disability, and is easily cured if only it is recognized. The condition was described by Thompson and Kopell in 1959, Schilf reported a case of isolated suprascapular entrapment in 1952. The nerve passes through the suprascapular notch, the roof of the notch is formed by the transverse scapular ligament. The shape of the suprascapular notch may be guilty for entrapment symptoms. Suprascapular nerve compression may result of acute trauma, of tr

Two cases of aggressive fibromatosis are described, one with tricentric origin and with the first tumor originating in the hand, the other with malignant transformation into a myxoid fibrosarcoma, grade III. The primary treatment of aggressive fibromatosis should be wide excision, and the follow-up should be long, taking into account the possibility of multicentricity and the risk of malignant transformation.
