Mid-term results of a minimally invasive management of scaphoid non-union: Arthroscopy-assisted osteosynthesis and autologous bone grafting.

OBJECTIVE: To analyze clinical and radiological mid-term outcomes in patients with scaphoid pseudarthrosis treated with arthroscopy-assisted osteosynthesis and autologous bone grafting. MATERIALS AND METHODS: Thirty-one patients (27 males, 4 female) were included in a retrospective study, with one bilateral case. The mean age was 28 years, and mean follow-up was 54 months. Twenty-six non-unions were in the waist, 5 in the proximal third, and one in the distal third. Arthroscopic debridement was performed and autologous bone graft from iliac crest was used. Fixation was performed percutaneousl

Osteological differences in the humerus of loggerhead and green turtles.

Limbs are the principal locomotory structures in vertebrate body, closely related to the habitat use and locomotion strategies of the species. Sea turtles rely almost entirely on their fore flippers for aquatic locomotion. The humerus is the most crucial bone structure connecting the scapulocoracoid and flipper, and studies thereof are used to understand turtles' locomotion mechanisms and swimming strategies. To define the morphological traits of the humerus of loggerhead (Caretta caretta) and green (Chelonia mydas) turtles and analyze interspecific differences, sea turtle carcasses were sampl

Split hamate fractures as part of complex ulnar carpometacarpal injuries: radiographic and functional outcomes.

BACKGROUND: Split hamate fractures, which most often are a complicating part of fracture dislocations of the 5th or 5th and 4th carpometacarpal joints, are rare and challenging injuries that can severely affect hand function. The injury usually requires surgical treatment. However, both short- and long-term data on the effectiveness of these interventions are limited. The objective of this study is to advance understanding of this rare hand surgical condition by investigating its epidemiology, etiopathogenesis, subsequent complications, and functional outcomes. METHODS: Between 2010 and 2024,

Thumb Carpometacarpal Joint Denervation Has Lower Reoperation Rates Than Trapeziectomy for Thumb Base Arthritis: A Retrospective Database Study.

PURPOSE: Various surgical options exist to treat thumb carpometacarpal (CMC) osteoarthritis, including trapeziectomy-based procedures and CMC denervation. Presently, there is limited data on the relative longevity of these 2 procedures, with conflicting data on the rates of secondary conversion after CMC denervation. We sought to investigate the rates of secondary surgery in patients undergoing trapeziectomy or denervation for CMC osteoarthritis. METHODS: The PearlDiver Mariner Database was retrospectively queried for patients with CMC arthritis undergoing trapeziectomy-based procedures or CM

Whole-Epiphysis Trabecular Bone in Tamarin Limbs Suggests Effects of Leaping Distance Alongside Non-Biomechanical Factors.

OBJECTIVES: Beyond non-biomechanical factors, the trabecular architecture of long bone epiphyses variably underlies functional adaptations to locomotor behavior. A recent study, characterizing tamarin trabecular bone using traditional metrics derived from Volumes of Interest (VOIs), identified a preliminary relationship with leaping distance. We hypothesized that quantifying trabecular topological indices from whole epiphyses in the same system would enhance detection of locomotor signals. MATERIALS AND METHODS: We analyzed μCT scans of humeral and tibial whole-epiphyseal trabecular bone in f

Staged Centralisation and Pollicisation for Radial Longitudinal Deficiency: A Case Series Emphasising Pre-pollicisation Index-Middle Finger Pinch Training.

Background: Radial longitudinal deficiency (RLD) is a rare congenital malformation that severely impairs grasp and pinching due to radial ray and thumb hypoplasia or aplasia. Standard management typically involves early centralisation to align and stabilise the wrist, followed by pollicisation when the thumb cannot be reconstructed. However, little attention has been paid to pre-pollicisation-rehabilitation strategies that might facilitate the acquisition of postoperative pinch function. This study aimed to report early functional outcomes after staged reconstruction for severe RLD, highlighti

Patient perspectives on humeral lengthening in achondroplasia: association between treatment timing and acceptance.

PURPOSE: Achondroplasia is associated with upper-limb shortening, which may impair independence in daily activities. While lower-limb lengthening is commonly performed, humeral lengthening is often delayed. This study evaluated the functional impact of upper-limb shortening and patient perspectives regarding humeral lengthening. METHODS: A single-centre cross-sectional questionnaire study was conducted in 27 patients with achondroplasia divided into three groups according to treatment status. Outcomes included independence in daily activities, functional and aesthetic satisfaction, and willin

Bilateral Sprengel deformity with asymmetrical omovertebral connections in a child with Klippel-Feil syndrome: a rare case report.

An 8-year-old male patient with bilateral Sprengel deformity, characterized by a right-sided bony omovertebral bar and a left-sided fibrous band, in the context of Klippel-Feil syndrome is presented in this case report. The patient had limited shoulder movement, a short neck, and considerable cosmetic issues. The radiological evaluation showed the omovertebral structures to be asymmetrical and a cervical fusion at C2-C3. The patient underwent 2 separate surgical procedures: the right side was operated on when the patient was 8 years old, and the left side was operated on 8 months later. The pr

A 4-Anchor Suture Tape Internal Bracing Technique for Ligamentous Perilunate Injuries.

A 31-year-old man presented with an acute, ligamentous perilunate dislocation and paresthesia in the median nerve distribution after a dirt bike accident and underwent closed reduction. The following morning, he was treated with carpal tunnel release and open reduction and internal fixation using a 4-anchor suture tape internal bracing treatment technique through a dorsal carpal approach. The construct consisted of a series of internal braces dunked into four carpal anchors to reinforce/recreate the scapholunate interosseous ligament, lunotriquetral interosseous ligament, and dorsal intercarpa

Development of radiographic references for skeletal maturity in Indian children.

OBJECTIVES: The Tanner-Whitehouse 3 (TW3) method recognized globally for assessing skeletal maturity are based primarily on Western and East Asian populations, limiting their accuracy when applied to Indian children. The objective of present study was to develop population-specific reference values for Skeletal Maturity Scores (SMS) in Indian children aged 1-18 years using the TW radius-ulna-short bones (RUS-13) system, and to validate these data against the in vogue TW3 system. METHODS: This retrospective study analysed 3,549 left-hand radiographs (1,741 boys; 1,808 girls) from healthy India

In vivo virtual investigation of the implant location for scaphoid fractures.

INTRODUCTION: Scaphoid fractures still have high rate of nonunion, mainly due to the tenuous blood supply and only primary bone healing. Rigid fixation facilitates this kind of bone healing and is considered as vital as protection of the blood supply. The optimal implant (screw) location should meet the requirements for researcher's biomechanical findings on fracture stability. MATERIALS AND METHODS: Raw CT-scanned data of eight volunteers' wrists were imported into Mimics. The 3D scaphoid was segmented out and calculated, then opened in Geomagic Studio. Four fracture planes (proximal, obliqu

Anomalous three-headed biceps brachii with attachment to the medial epicondyle.

The biceps brachii (BB) demonstrates considerable anatomical variability, with supernumerary heads among the most frequently reported variations. Although accessory heads typically insert onto the common BB tendon, alternative insertion patterns may have important clinical implications, particularly when they alter the relationship of the muscle to adjacent neurovascular structures. During routine dissection of an 82-year-old male cadaver, a three-headed BB was identified in the right upper limb. A prominent third head originated from muscle fibers of the primary BB in the distal third of the

Boosting early bone healing by freeze-dried bovine leukocyte-platelet rich fibrin and nanohydroxyapatite in rabbit: histopathological, molecular, and Raman spectroscopy analysis.

INTRODUCTION: Globally, millions suffer from bone fractures every year, with substantial clinical consequences. Intervention is crucial when healing mechanisms are ineffective. Leukocyte-platelet rich fibrin (L-PRF), by its rapid, simple, and cost-effective preparation methodology alongside a calcium phosphate-based compound such as nanohydroxyapatite may favorably impact clinical practice. SOURCE OF DATA: A critical-sized radius bone defect was produced in 25 White New Zealand rabbits divided into five groups (n = 5 per group) including untreated, autograft, nanohydroxyapatite scaffold, bovi

Short-Term Clinical Comparison of Scapular Stabilization With and Without Added Kinetic-Chain Exercises on Pain, Scapular Kinematics, and Upper-Extremity Function in Overhead Athletes With Subacromial Pain Syndrome: A Randomized Controlled Trial.

BACKGROUND AND PURPOSE: Scapular stabilization exercises are commonly used in the rehabilitation of athletes with subacromial pain syndrome (SAPS) and scapular dyskinesis. Whether supplementing scapular stabilization with kinetic-chain exercises provides greater short-term benefits remains uncertain. This study compared the short-term effects of a scapular stabilization program alone with those of the same program supplemented with additional kinetic-chain exercises. METHODS: Twenty-four overhead athletes with SAPS and scapular dyskinesis were randomly assigned to a scapular stabilization gro

Minimum 5-year outcomes of anatomic total shoulder arthroplasty with the Arthrosurface aspherical humerus and inlay glenoid.

BACKGROUND: Aseptic glenoid loosening is the most common cause of revision after anatomic total shoulder arthroplasty (aTSA). Inlay glenoid components have been shown to reduce edge loading and opposite-edge lift-off forces compared with onlay glenoids. Early clinical results of inlay glenoids have been promising, although there is a paucity of literature detailing mid-term to long-term follow-up. We report the minimum 5-year clinical and radiographic outcomes after aTSA with the Arthrosurface inlay glenoid component and a stemless, aspherical humeral head in an active, young patient populatio
